Objective

To conduct a quantitative evaluation of China’s Huimin insurance policy texts, provide references for the formulation and improvement of future Huimin insurance policies, and promote the further improvement of the multi-level medical security system.

Methods

Based on the text mining method, 14 Huimin insurance policies issued at the national and provincial levels were processed. A PMC index model was constructed to conduct an overall and surface chart analysis of the 14 Huimin insurance policies, and a comparison was made between Policy P10 and Policy P7.

Results

The results of the PMC index model showed that the average PMC index score of the 14 Huimin insurance policies was 6.98, all at an acceptable level or above. Among them, 6 policies were rated as excellent, and 8 policies were rated as acceptable. The policies scored relatively high in policy content (0.87) and policy nature (0.81), and relatively low in policy perspective (0.61), incentive and restraint (0.60), and policy timeliness (0.33).

Conclusion

China’s Huimin insurance policies are generally at an acceptable level. They can be improved by strengthening the top - level design of Huimin insurance policies, improving the incentive and restraint mechanism, and optimizing policy timeliness.

目的

对我国惠民保政策文本进行量化评价,为未来惠民保政策的制定和完善提供参考,推动多层次医疗保障体系的进一步完善。

方法

基于文本挖掘法对国家和省级层面出台的14项惠民保政策进行处理,构建政策建模一致性(PMC)指数模型,对14项惠民保政策进行整体性和曲面图分析,并对政策P10和政策P7进行对比。

结果

PMC指数模型结果表明,14项惠民保政策PMC指数得分均值为6.98分,均为可接受及以上水平,其中有6项政策为优秀等级,8项政策为可接受等级。在政策内容(0.87)、政策性质(0.81)上得分较高,在政策视角(0.61)、激励约束(0.60)、政策时效(0.33)上得分较低。

结论

我国惠民保政策总体处于可接受水平,可通过强化惠民保政策顶层设计、完善激励约束机制及优化政策时效等措施进行完善。

基于文本挖掘法对国家和省级层面出台的14项惠民保政策进行处理,构建政策建模一致性(PMC)指数模型,对14项惠民保政策进行整体性和曲面图分析,并对政策P10和政策P7进行对比。

结果

PMC指数模型结果表明,14项惠民保政策PMC指数得分均值为6.98分,均为可接受及以上水平,其中有6项政策为优秀等级,8项政策为可接受等级。在政策内容(0.87)、政策性质(0.81)上得分较高,在政策视角(0.61)、激励约束(0.60)、政策时效(0.33)上得分较低。

结论

我国惠民保政策总体处于可接受水平,可通过强化惠民保政策顶层设计、完善激励约束机制及优化政策时效等措施进行完善。

Objective

To conduct a quantitative evaluation of China’s Huimin insurance policy texts, provide references for the formulation and improvement of future Huimin insurance policies, and promote the further improvement of the multi-level medical security system.

Methods

Based on the text mining method, 14 Huimin insurance policies issued at the national and provincial levels were processed. A PMC index model was constructed to conduct an overall and surface chart analysis of the 14 Huimin insurance policies, and a comparison was made between Policy P10 and Policy P7.

Results

The results of the PMC index model showed that the average PMC index score of the 14 Huimin insurance policies was 6.98, all at an acceptable level or above. Among them, 6 policies were rated as excellent, and 8 policies were rated as acceptable. The policies scored relatively high in policy content (0.87) and policy nature (0.81), and relatively low in policy perspective (0.61), incentive and restraint (0.60), and policy timeliness (0.33).

Conclusion

China’s Huimin insurance policies are generally at an acceptable level. They can be improved by strengthening the top - level design of Huimin insurance policies, improving the incentive and restraint mechanism, and optimizing policy timeliness.

城市定制型商业医疗保险也称普惠型商业补充医疗保险,简称惠民保[1],具有“门槛低、保费低、保额高”等特性[2]。2015年深圳推出的重特大疾病补充医疗保险是惠民保的雏形[3]。2020年《关于深化医疗保障制度改革的意见》指出,要推进补充医疗保险、商业健康保险的发展。2021年《关于规范保险公司城市定制型商业医疗保险业务的通知》指出要规范开展惠民保业务。近年来,各地开始发布指导意见,对惠民保的保障内容等方面进行规定,但仍存在赔付风险增加、权责不清晰、逆向选择与“死亡螺旋”等问题,如何有效地制定和完善政策是促进惠民保可持续发展的重要基础。
目前,关于惠民保已有的研究多从现状、问题、发展趋势以及对策等视角进行分析[3-6],也有学者从多层次医疗保障体系角度对惠民保展开研究[7-9],邵珺铄等基于扎根理论对惠民保政策文件进行文本分析[2],而对惠民保政策的研究相对缺乏。政策建模一致性(policy modeling consistency,PMC)指数模型作为一种政策评价的工具,弥补了定量研究方法中政策评价视角单一等局限,已应用于各领域政策评价,尚未用于惠民保政策研究。本研究基于文本挖掘视角,获取高频词和社会网络知识图谱,引入PMC指数模型对我国惠民保政策进行量化评价,以期为未来惠民保政策的制定和完善提供参考。
以“惠民保”“城市定制型商业医疗保险”“普惠型商业健康保险”“普惠型商业补充医疗保险”等为关键词在北大法宝数据库进行检索,以国家及各省级政府官方网站等作为补充,检索时间为2023年8月以前发布的政策文件。政策纳入排除标准:国家或省级层面出台文件,排除其它层面出台文件;政策内容和标题均体现惠民保主题,排除与主题关联性较低的政策;文件类型纳入意见、通知等,排除政策解读、报道等。最终纳入14份有效政策文件。见表1
PMC指数模型是Ruiz Estrada等[10]提出的政策量化模型。它的功能是分析单项政策的内部一致性和优劣势,反映政策整体与各方面的差异,进而对政策进行科学的量化评价[11]。我国惠民保政策PMC指数模型构建的具体步骤见图1
惠民保政策PMC指数的计算具体步骤为:(1)将所构建的10个一级变量和43个二级变量放入多投入产出表;(2)按照公式1和2对二级变量赋值;(3)根据公式3计算每个一级变量的值;(4)根据公式4计算PMC指数。结合Ruiz Estrada等[10]的研究,惠民保政策评价等级划分标准见表2
其中,t为一级变量,j为二级变量,n或T(Xtj)代表某一级变量下二级变量的数量。
绘制PMC曲面图能直观立体地分析不同政策和变量的优劣势。它是由每项政策一级变量值转化的三阶矩阵。因矩阵具有对称性,政策公开对结果无影响,故剔除。根据公式5绘制PMC曲面图。
利用ROSTCM 6.0软件对惠民保政策进行文本处理,通过人工筛查生成反映政策要点的40个高频词,见表3。利用Net Draw工具生成惠民保政策社会网络图谱,见图2。根据惠民保政策自身特性,结合高频词和社会网络图,参照以往相关政策评价研究,建立了10个一级变量、43个二级变量。其中政策性质、政策时效、政策受体、政策视角、激励约束、政策评价和政策公开的选取是参考以往研究建立,发布机构、政策目标、政策内容是基于政策文本挖掘和高频词建立。
多投入产出表是一种能够用二级变量来度量一级变量的数据分析体系。一级变量代表政策的不同评价维度,每个一级变量包含了n个二级变量。二级变量没有重要性之分,利用二进制公式赋值,政策包含该变量赋值为1,否则为0。见表4
根据表5显示,14项惠民保政策的均值为6.98,排名为P10>P13>P9>P1>P3>P8>P14>P4=P5>P12>P2>P6>P11>P7。惠民保政策都达到了可接受及以上水平,其中处于优秀等级的政策有P1、P3、P8、P9、P10、P13,共6项;可接受的政策有P2、P4、P5、P6、P7、P11、P12、P14,共8项;无完美和不良政策。通过对14项惠民保政策的一级指标得分取平均值,绘制PMC曲面图,见图3。除X10政策公开外,X5政策内容(0.87)、X1政策性质(0.81)、X6政策受体(0.76)和X4政策目标(0.73)得分较高,表明惠民保政策内容包括服务能力和监督管理等方面,政策体现了引导、建议和监管等性质,政策目标较明确,政策受体涉及政府机构和商业保险机构等相关受体,但仍存在政策性质缺少预测、政策受体较少涉及社会组织等问题。X9政策评价均值为0.64,表明政策相对较好,但在内容详实等方面还需完善。X3发布机构均值为0.63,多为医疗保障局和银保监局发布,其它机构涉及相对较少。X7政策视角均值为0.61,大部分政策以宏观视角为主,少数微观和宏观视角相结合。X8激励约束均值为0.60,表明政策较少涉及人才培养和法律法规等内容。X2政策时效均值为0.33,大多是短期规划,中期和长期政策较少。
P10的PMC指数为8.38,排名第一。P7的PMC指数为5.33,排名最末。通过雷达图、曲面图对P10和P7进行对比,见图46。发现两项政策在政策评价、发布机构、政策视角、政策性质、政策受体、政策内容和激励约束上差异较大,在政策目标、政策时效、政策公开上相同。政策P10由浙江省医保局、财政厅、监管局和税务局等多部门联合发文,从宏观和微观视角制定惠民保政策,政策目标明确,责任分工清晰,推进具有浙江特色的共富型多层次医疗保障体系更加成熟。政策P7由贵州省医保局单独发文,没有从微观视角制定惠民保政策的实施细则,责任分工不够明晰,对于推动贵州省惠民保政策的发展有待进一步加强。
当前,我国惠民保缺乏国家层面的专项政策,且有些省份尚未出台公开的惠民保政策,其中大部分政策以宏观视角为主,少数微观和宏观视角相结合。因此,应当强化顶层设计,在制定惠民保政策时,既要向上看看又要向下看看,向上看看为的是全面准确把握国家要求,向下看看为的是摸清群众对惠民保的需求。此外,要重视政策的宏观视角和微观视角相结合,从宏观上把握惠民保政策的整体框架,从微观上完善惠民保政策的具体实施步骤,推动惠民保政策的高质量发展。
目前惠民保政策的激励约束主要以风险管理、制度衔接和信息支持等方面为主,而在人才培养和法律法规等方面还有待进一步完善。为了完善激励约束机制,避免“死亡螺旋”问题,要注重培养专业的医药学人才和信息技术人才,惠民保需要大量的精算数据与风险监管,依赖于专业人才的输送。同时,政府应当建立健全法律法规和政策支持,引导承保机构合理合法的设计保险方案,明确利益相关者的权利与义务,对于骗保等行为严厉打击。
目前我国惠民保政策主要是短期政策,中期和长期规划相对缺乏,可能会造成惠民保政策前沿性和可持续性不够等问题。因此在未来的惠民保政策制定中,应当增加中期和长期政策的制定,适应群众需求,注意政策前瞻性。此外,要加强短期、中期和长期政策相结合,可分阶段在各省市落实惠民保政策,增加试点省份和城市,总结试点经验,及时完善惠民保政策,促进惠民保政策的可持续性。
