Objective

To analyze the treatment enrollment status and influencing factors for rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province, providing a basis for improving prevention and control strategies.

Methods

Based on case data, drug resistance screening records, and information on drug-resistant patients in Guizhou Province from 2016 to 2023, the enrollment rate (%) of rifampicin-resistant patients was described to characterize their distribution. Chi-square tests and binary logistic regression models were employed to analyze influencing factors.

Results

The average annual enrollment rate for rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province from 2016 to 2023 was 56.00%, increasing from 35.71% in 2016 to 88.20% in 2023 (trend χ2=729.839, P<0.001). An upward trend was observed across gender, age groups (<20 years, 20-39 years, 40-59 years, and ≥60 years), and nine municipalities (P<0.05 for all). Analysis of influencing factors indicated that compared to patients who were <20 years old, local, new cases, those with reports from designated multidrug-resistant tuberculosis hospitals, and those without diagnosis delays, the following were identified as risk factors for treatment enrollment: age ≥60 years (OR=0.471, 95%CI: 0.296-0.750), migrant status (OR=0.503, 95%CI: 0.391-0.646), initial treatment positive at 2/3 months (OR=0.676, 95%CI: 0.464-0.983), reports from general tuberculosis hospitals (OR=0.080,95%CI: 0.063-0.102) and from the disease control center (OR=0.048, 95%CI: 0.040-0.057), and diagnosis delays (OR=0.551,95%CI: 0.048-0.648) (P<0.05 for all).

Conclusion

The treatment enrollment status of rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province has gradually improved, but continued attention is necessary. It is recommended to strengthen interventions for key populations, including those aged ≥60 years, migrants, initial treatment positive at 2/3 months, patients with reports from non-multidrug-resistant designated tuberculosis hospitals, and those experiencing diagnosis delays, to further enhance treatment enrollment rates.

目的

分析贵州省利福平耐药肺结核患者的纳入治疗情况及影响因素,为完善防控策略提供依据。

方法

基于2016—2023年贵州省肺结核患者个案、耐药筛查登记和耐药患者个案信息,用率(%)描述利福平耐药患者纳入治疗的分布特征,采用χ2检验和二分类logistic回归模型分析其影响因素。

结果

2016—2023年贵州省利福平耐药肺结核患者年均纳入治疗率为56.00%,从2016年35.71%上升至2023年88.20%(趋势χ2=729.839,P<0.001)。其中,男性、女性,<20岁、20~39岁、40~59岁和≥60岁和9个市(州)均呈上升趋势(P均<0.05)。影响因素分析显示,与<20岁、本地、新患者、检测报告单位为耐多药结核病定点医院和没有发生确诊延迟的患者相比,≥60岁(OR=0.471,95%CI:0.296~0.750)、流动(OR=0.503,95%CI:0.391~0.646)、初治2/3月末阳性(OR=0.676,95%CI:0.464~0.983)、检测报告单位为普通肺结核定点医院(OR=0.080,95%CI:0.063~0.102)与疾控中心(OR=0.048,95%CI:0.040~0.057)以及发生确诊延迟(OR=0.551,95%CI:0.048~0.648)是患者纳入治疗的危险因素(P均<0.05)。

结论

贵州省利福平耐药肺结核患者纳入治疗情况逐步改善,仍需持续重视。建议对≥60岁、流动、初治2/3月末阳性、检测报告单位为非耐多药定点结核以及发生确诊延迟患者等重点人群加强干预,继续提升患者的纳入治疗情况。

目的

分析贵州省利福平耐药肺结核患者的纳入治疗情况及影响因素,为完善防控策略提供依据。

方法

基于2016—2023年贵州省肺结核患者个案、耐药筛查登记和耐药患者个案信息,用率(%)描述利福平耐药患者纳入治疗的分布特征,采用χ2检验和二分类logistic回归模型分析其影响因素。

结果

2016—2023年贵州省利福平耐药肺结核患者年均纳入治疗率为56.00%,从2016年35.71%上升至2023年88.20%(趋势χ2=729.839,P<0.001)。其中,男性、女性,<20岁、20~39岁、40~59岁和≥60岁和9个市(州)均呈上升趋势(P均<0.05)。影响因素分析显示,与<20岁、本地、新患者、检测报告单位为耐多药结核病定点医院和没有发生确诊延迟的患者相比,≥60岁(OR=0.471,95%CI:0.296~0.750)、流动(OR=0.503,95%CI:0.391~0.646)、初治2/3月末阳性(OR=0.676,95%CI:0.464~0.983)、检测报告单位为普通肺结核定点医院(OR=0.080,95%CI:0.063~0.102)与疾控中心(OR=0.048,95%CI:0.040~0.057)以及发生确诊延迟(OR=0.551,95%CI:0.048~0.648)是患者纳入治疗的危险因素(P均<0.05)。

结论

贵州省利福平耐药肺结核患者纳入治疗情况逐步改善,仍需持续重视。建议对≥60岁、流动、初治2/3月末阳性、检测报告单位为非耐多药定点结核以及发生确诊延迟患者等重点人群加强干预,继续提升患者的纳入治疗情况。

Objective

To analyze the treatment enrollment status and influencing factors for rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province, providing a basis for improving prevention and control strategies.

Methods

Based on case data, drug resistance screening records, and information on drug-resistant patients in Guizhou Province from 2016 to 2023, the enrollment rate (%) of rifampicin-resistant patients was described to characterize their distribution. Chi-square tests and binary logistic regression models were employed to analyze influencing factors.

Results

The average annual enrollment rate for rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province from 2016 to 2023 was 56.00%, increasing from 35.71% in 2016 to 88.20% in 2023 (trend χ2=729.839, P<0.001). An upward trend was observed across gender, age groups (<20 years, 20-39 years, 40-59 years, and ≥60 years), and nine municipalities (P<0.05 for all). Analysis of influencing factors indicated that compared to patients who were <20 years old, local, new cases, those with reports from designated multidrug-resistant tuberculosis hospitals, and those without diagnosis delays, the following were identified as risk factors for treatment enrollment: age ≥60 years (OR=0.471, 95%CI: 0.296-0.750), migrant status (OR=0.503, 95%CI: 0.391-0.646), initial treatment positive at 2/3 months (OR=0.676, 95%CI: 0.464-0.983), reports from general tuberculosis hospitals (OR=0.080,95%CI: 0.063-0.102) and from the disease control center (OR=0.048, 95%CI: 0.040-0.057), and diagnosis delays (OR=0.551,95%CI: 0.048-0.648) (P<0.05 for all).

Conclusion

The treatment enrollment status of rifampicin-resistant pulmonary tuberculosis patients in Guizhou Province has gradually improved, but continued attention is necessary. It is recommended to strengthen interventions for key populations, including those aged ≥60 years, migrants, initial treatment positive at 2/3 months, patients with reports from non-multidrug-resistant designated tuberculosis hospitals, and those experiencing diagnosis delays, to further enhance treatment enrollment rates.

对比敏感结核病,耐药结核病治疗复杂、治疗费用更高、治愈率更低等原因,是当前结核病防控工作的难点和重点[1-2]。据世界卫生组织2024年报告,2023年全球利福平耐药结核病患者约40万例,其中我国患者约为2.9万例,高居全球第四位[3]。耐药肺结核若不能及时发现和治疗,将导致更多原发性耐药肺结核的发生,严重影响“终止结核病策略”目标的实现和进程。贵州省作为全国结核病高疫情地区,拟通过分析全省2016—2023年利福平耐药患者的治疗现状,探讨其影响因素,为进一步探索优化本地防控策略和措施提供科学依据,为推动“终止结核病策略”目标的实现奠定基础。
2016—2023年贵州省利福平耐药肺结核患者相关信息资料来源于中国疾病预防控制信息系统子系统-结核病信息管理系统。
从结核病信息管理系统导出2016—2023年贵州省肺结核患者个案、耐药筛查登记和耐药患者个案等信息,收集其社会人口学特征、诊断结果和治疗等信息,用率(%)描述利福平耐药患者纳入治疗的分布特征,采用χ2检验和二分类logistic回归模型分析其影响因素。
根据《中国结核病防治工作技术指南(2021版)》[4]将下列指标定义为:(1)利福平耐药:结核分枝杆菌对利福平耐药,无论对其他抗结核药物是否耐药。(2)纳入治疗率:指某一地区,一定期间内,发现的利福平耐药患者中接受规范治疗方案患者的比例。
确诊延迟[5-6]:指患者首次就诊至确诊为利福平耐药肺结核的间隔时间>14 d。
纳入治疗率(%)的年度变化采用趋势χ2检验;单因素分析采用χ2检验,多因素分析采用二分类logistic回归分析,以患者是否纳入治疗为因变量,人口学特征、就诊情况等因素为自变量。运用SPSS 22.0进行统计检验,检验水准α=0.05。
2016—2023年贵州省共登记肺结核患者300 658例,其中病原学阳性患者140 704例,开展耐药筛查共112 586例,筛查率为80.02%,从中发现耐药患者5 068例,纳入治疗分析的患者共5 068例。见表1
5 068例利福平耐药患者中,纳入规范治疗2 838例,年均纳入治疗率为56.00%,纳入治疗率从2016年35.71%上升至2023年88.20%(趋势χ2=729.839,P<0.001)。见表2图1
5 068例利福平耐药肺结核患者中,男性和女性年均纳入治疗率分别为55.77%、56.58%(χ2=0.271,P=0.603),2016—2023年两性别的纳入治疗率均呈上升趋势(趋势χ2=504.486、227.145,P<0.001)。<20岁、20~39岁、40~59岁和≥60岁4个年龄组的纳入治疗率差异有统计学意义(χ2=108.732,P<0.001),2016—2023年4个组纳入治疗率均呈上升趋势(趋势χ2=29.635、213.580、296.623、17.979,P<0.001)。见表34
2016—2023年贵州省5 068例利福平耐药肺结核患者分布在9个市(州),年均纳入治疗率在40.70%~68.18%之间(χ2=155.160,P<0.001),2016—2023年9个市(州)纳入治疗率均呈上升趋势(趋势χ2=106.018、52.078、118.202、55.126、196.204、108.939、52.450、15.478、65.227,P均<0.001)。见表5
年龄、民族、职业、登记分类、有无合并症、有无合并其他结核、HIV检测结果、诊断单位和是否确诊延迟间的纳入治疗率差异具有统计学意义(P均<0.05)。不同性别和户籍患者的纳入治疗率差异无统计学意义。见表6
以患者是否纳入治疗为因变量(否=0,是=1),将上述因素纳入多因素logistic回归模型,各变量赋值情况见表7。结果显示,与<20岁年龄组、本地患者、新患者、检测报告单位为耐多药结核病定点医院和没有发生确诊延迟的患者相比,≥60岁年龄组(OR=0.471,95%CI:0.296~0.750)、流动患者(OR=0.503,95%CI:0.391~0.646)、初治2/3月末阳性(OR=0.676,95%CI:0.464~0.983)、检测报告单位为普通肺结核定点医院(OR=0.080,95%CI:0.063~0.102)与疾控中心(OR=0.048,95%CI:0.040~0.057)、以及发生确诊延迟(OR=0.551,95%CI:0.468~0.648)是影响患者纳入治疗的危险因素(P均<0.05)。见表6
耐药结核病严重威胁公众身心健康,发现和治疗耐药结核病患者是减少耐药结核病传播的一种主要措施[7]。因此,加强和完善耐药结核病的监测和治疗对于降低结核病疫情具有重要意义。本文分析近年全省利福平耐药肺结核患者纳入治疗情况,拟为下一步结核病防控工作提供依据,以期达到“终止结核病策略”的目标和任务。
本研究对2016—2023年贵州省利福平耐药肺结核患者的纳入治疗情况进行分析发现,近年来全省耐药患者纳入治疗率呈上升趋势。这与全省采取的一系列政策和措施密切相关。一是全省长期贯彻落实国家结核病规划,不断提升对耐药结核病的重视,如同国家传染病医学中心、中国健康管理协会合作开展了贵州省消除耐药结核病危害促进项目,省卫生健康委联合省民政厅、省财政厅和省医保局四部委联合下发《关于印发贵州省遏制耐药结核病推进方案(2022—2025年)的通知》等措施。二是全省逐步建立健全定点医院、疾控中心和基层医疗机构“三位一体”模式,不断提升肺结核诊治和管理规范[8]。三是从2022年起,全省创新建立了“耐药患者转诊平台”,实现耐药患者在普通结核病定点医院和耐多药定点医院之间的发现、转诊、接收诊疗等环节无缝衔接;同时在耐多药定点医院建立“以耐药患者为中心”的绿色收治通道,实现患者“三优先”绿色诊疗服务,提高收治效率。四是全省定期统计耐药患者未纳入治疗情况,开展省、市、县、乡、村逐级追踪工作,通过电话/上门等方式督促动员患者纳入规范治疗。五是推动出台《关于进一步做好活动性肺结核门诊医疗保障的通知》等医疗报销政策,将耐药结核病纳入慢特病门诊报销,并提高报销比例和范畴;同时拨付一定的结核病中央转移支付资金到各耐多药定点医院,为贫困患者提供医疗减免等措施,共同减轻患者经济负担,提高患者医疗可及性。但在全国近年的年报中,贵州省耐药患者的纳入治疗情况跟全国以及其他经济较好地区仍有一定差距,需持续加强重视。
研究发现,不同性别、年龄组和地区间的耐药患者的纳入治疗率呈上升趋势,与全省耐药患者总体变化趋势基本一致。在各地区间的纳入治疗情况有差异,可能与不同地区的经济水平、人口构成、公众结核病知晓情况及相关政策落实力度等差异相关[9-10],有待进一步研究考证。提示在全省纳入治疗情况全面改善的背景下,仍需加强对重点地区的关注。
多因素分析发现,≥60岁、流动、初治2/3月末阳性、检测报告单位为非耐多药定点医院以及发生确诊延迟是利福平耐药肺结核患者纳入治疗的危险因素。≥60岁的患者在贵州省内多数为农民,其经济条件不佳,同时不便前往耐多药定点医院诊治,加上老年人病情复杂和严重,预后较差等原因,都不利于其纳入规范治疗[11-13]。相关学者认为,流动人口不便于结核病的管理[14]。初治2/3月末阳性的患者,多为不规律治疗,依从性较差,同时其对治疗的信心大大降低,严重影响规范诊疗[15]。与耐药检测报告单位在耐多药定点医院的患者相比,其他非耐多药定点医院报告检测耐药的患者的纳入治疗率更低;自“三位一体”防治模式推广以来,明确了市州级耐多药定点医院为耐药患者提供诊断、治疗和关怀等服务,而普通结核病定点医院更多负责普通结核的诊疗,疾控中心主要负责作出疾病控制规划管理条例。因此,在非耐多药定点医院报告检测为耐药的患者,其诊疗和管理流程的规范必定不及耐多药定点医院,提示各卫生机构严格落实“三位一体”防治模式中的职责要求和责任分工的重要性。本研究中,在耐多药定点医院检测报告的耐药患者的占比是逐年增多的,反映了全省“三位一体”防治模式的逐步完善,与全省纳入治疗率呈上升趋势的结果相符。确诊延迟的患者可能由于其确诊前就诊次数越多,确诊前所花时间越多,影响其治疗依从性导致纳入治疗率偏低[16]
本研究首次针对贵州省利福平耐药肺结核患者开展纳入治疗及影响因素分析,发现全省2016—2023年利福平耐药肺结核患者的纳入治疗情况逐步改善,仍需进一步持续重视。其中≥60岁、流动、初治2/3月末阳性、检测报告单位为非耐多药定点医院以及发生确诊延迟是利福平耐药肺结核患者纳入治疗的危险因素。建议进一步加强对以上重点人群的干预,继续提升患者的纳入治疗情况。
