Objective

This study aimed to identify the dual trajectories of social isolation and depression among middle-aged and older patients with hypertension, discuss the relationship between the dual trajectories, and investigate common influential factors. Method Using data from 2013 to 2020 of the China Health and Retirement Longitudinal Study (CHARLS), a longitudinal cohort was formed for the study. A group-based trajectory model was used to identify trajectories of social isolation and depression in 3 223 middle-aged and older patients with hypertension and the relationship between the dual trajectories. Multi-class logistic regression was used to analyze common influential factors of the dual trajectories.

Results

Three trajectories of social isolation were identified: low-stable without social isolation, medium-stable without social isolation, and high-increasing with social isolation. Three depression trajectories were observed: low-stable without depression, medium-increasing with depression, and high-stable with depression. Dual-trajectory analysis showed in middle-aged and older patients with hypertension who were in the medium-stable without social isolation and high-increasing with social isolation trajectory groups, the probability of depression trajectories in the medium-increasing with depression and high-stable with depression groups was 61.84% to 87.46%. In middle-aged and older patients with hypertension who belonged to the medium-increasing with depression and high-stable with depression trajectory groups, the probability of social isolation in the medium-stable without social isolation and high-increasing with social isolation trajectory groups was 70.03% to 91.63%. The level of education(social isolation trajectories in middle school RRR=0.756, 95% CI：0.599-0.954, RRR=0.496, 95% CI: 0.326-0.755; social isolation trajectories in high school or above RRR=0.516, 95% CI: 0.375-0.709, RRR=0.262, 95% CI: 0.137-0.501; depression trajectories in middle school RRR=0.773, 95% CI: 0.622-0.959, RRR=0.728, 95% CI: 0.574-0.994, depression trajectories in high school or above RRR=0.421, 95% CI: 0.309-0.571, RRR=0.439, 95% CI: 0.253-0.763) and sleeping 7-9 hours a night (social isolation trajectories RRR=0.824, 95% CI: 0.681-0.997, RRR=0.596, 95% CI: 0.440-0.806; depression trajectories RRR=0.597, 95% CI: 0.501-0.711, RRR=0.486, 95% CI: 0.362-0.653) were common influential factors of the dual trajectories of social isolation and depression.

Conclusion

There is a high degree of consistency and significant correlation between the social isolation trajectory and the depression trajectory. Interventions targeting the dual trajectories and co-factors of social isolation and depression should be considered to improve their effectiveness.

目的

识别中老年高血压患者社会隔离和抑郁的双轨迹,探讨他们之间的相互关系及共同影响因素。

方法

使用中国健康与养老追踪调查(CHARLS) 2013—2020年数据进行分析。采用GBTM双轨迹模型和logistic回归，分析3 223例中老年高血压患者社会隔离与抑郁的双轨迹和共同影响因素。

结果

识别出社会隔离有无社会隔离低稳定、无社会隔离中稳定、社会隔离高上升3种轨迹,抑郁有无抑郁低稳定、抑郁中增加、抑郁高持续3种轨迹。双轨迹分析表明,社会隔离处于无社会隔离中稳定和社会隔离高上升轨迹组的中老年高血压患者,抑郁处于抑郁中增加和抑郁高持续轨迹组的概率在61.84%~87.46%;抑郁处于抑郁中增加和抑郁高持续轨迹组的中老年高血压患者,社会隔离处于无社会隔离中稳定和社会隔离高上升轨迹组的概率在70.03%~91.63%。文化程度(初中社会隔离轨迹组RRR=0.756, 95% CI: 0.599~0.954, RRR=0.496, 95% CI: 0.326~0.755;高中及以上社会隔离轨迹组RRR=0.516, 95% CI: 0.375~0.709, RRR=0.262, 95% CI: 0.137~0.501;初中抑郁轨迹组RRR=0.773, 95% CI: 0.622~0.959, RRR=0.728, 95% CI: 0.574~0.994;高中及以上抑郁轨迹组RRR=0.421, 95% CI: 0.309~0.571, RRR=0.439, 95% CI: 0.253~0.763)、夜间睡眠7~9小时(社会隔离轨迹组RRR=0.824, 95% CI: 0.681~0.997, RRR=0.596, 95% CI: 0.440~0.806;抑郁轨迹组RRR=0.597, 95% CI: 0.501~0.711, RRR=0.486, 95% CI: 0.362~0.653)是社会隔离轨迹和抑郁轨迹的共同影响因素(均P<0.05)。

结论

社会隔离轨迹和抑郁轨迹有高度一致性和显著相关性。应考虑针对社会隔离、抑郁的双重轨迹和共同影响因素进行干预。

目的

识别中老年高血压患者社会隔离和抑郁的双轨迹,探讨他们之间的相互关系及共同影响因素。

方法

使用中国健康与养老追踪调查(CHARLS) 2013—2020年数据进行分析。采用GBTM双轨迹模型和logistic回归，分析3 223例中老年高血压患者社会隔离与抑郁的双轨迹和共同影响因素。

结果

识别出社会隔离有无社会隔离低稳定、无社会隔离中稳定、社会隔离高上升3种轨迹,抑郁有无抑郁低稳定、抑郁中增加、抑郁高持续3种轨迹。双轨迹分析表明,社会隔离处于无社会隔离中稳定和社会隔离高上升轨迹组的中老年高血压患者,抑郁处于抑郁中增加和抑郁高持续轨迹组的概率在61.84%~87.46%;抑郁处于抑郁中增加和抑郁高持续轨迹组的中老年高血压患者,社会隔离处于无社会隔离中稳定和社会隔离高上升轨迹组的概率在70.03%~91.63%。文化程度(初中社会隔离轨迹组RRR=0.756, 95% CI: 0.599~0.954, RRR=0.496, 95% CI: 0.326~0.755;高中及以上社会隔离轨迹组RRR=0.516, 95% CI: 0.375~0.709, RRR=0.262, 95% CI: 0.137~0.501;初中抑郁轨迹组RRR=0.773, 95% CI: 0.622~0.959, RRR=0.728, 95% CI: 0.574~0.994;高中及以上抑郁轨迹组RRR=0.421, 95% CI: 0.309~0.571, RRR=0.439, 95% CI: 0.253~0.763)、夜间睡眠7~9小时(社会隔离轨迹组RRR=0.824, 95% CI: 0.681~0.997, RRR=0.596, 95% CI: 0.440~0.806;抑郁轨迹组RRR=0.597, 95% CI: 0.501~0.711, RRR=0.486, 95% CI: 0.362~0.653)是社会隔离轨迹和抑郁轨迹的共同影响因素(均P<0.05)。

结论

社会隔离轨迹和抑郁轨迹有高度一致性和显著相关性。应考虑针对社会隔离、抑郁的双重轨迹和共同影响因素进行干预。

Objective

This study aimed to identify the dual trajectories of social isolation and depression among middle-aged and older patients with hypertension, discuss the relationship between the dual trajectories, and investigate common influential factors. Method Using data from 2013 to 2020 of the China Health and Retirement Longitudinal Study (CHARLS), a longitudinal cohort was formed for the study. A group-based trajectory model was used to identify trajectories of social isolation and depression in 3 223 middle-aged and older patients with hypertension and the relationship between the dual trajectories. Multi-class logistic regression was used to analyze common influential factors of the dual trajectories.

Results

Three trajectories of social isolation were identified: low-stable without social isolation, medium-stable without social isolation, and high-increasing with social isolation. Three depression trajectories were observed: low-stable without depression, medium-increasing with depression, and high-stable with depression. Dual-trajectory analysis showed in middle-aged and older patients with hypertension who were in the medium-stable without social isolation and high-increasing with social isolation trajectory groups, the probability of depression trajectories in the medium-increasing with depression and high-stable with depression groups was 61.84% to 87.46%. In middle-aged and older patients with hypertension who belonged to the medium-increasing with depression and high-stable with depression trajectory groups, the probability of social isolation in the medium-stable without social isolation and high-increasing with social isolation trajectory groups was 70.03% to 91.63%. The level of education(social isolation trajectories in middle school RRR=0.756, 95% CI：0.599-0.954, RRR=0.496, 95% CI: 0.326-0.755; social isolation trajectories in high school or above RRR=0.516, 95% CI: 0.375-0.709, RRR=0.262, 95% CI: 0.137-0.501; depression trajectories in middle school RRR=0.773, 95% CI: 0.622-0.959, RRR=0.728, 95% CI: 0.574-0.994, depression trajectories in high school or above RRR=0.421, 95% CI: 0.309-0.571, RRR=0.439, 95% CI: 0.253-0.763) and sleeping 7-9 hours a night (social isolation trajectories RRR=0.824, 95% CI: 0.681-0.997, RRR=0.596, 95% CI: 0.440-0.806; depression trajectories RRR=0.597, 95% CI: 0.501-0.711, RRR=0.486, 95% CI: 0.362-0.653) were common influential factors of the dual trajectories of social isolation and depression.

Conclusion

There is a high degree of consistency and significant correlation between the social isolation trajectory and the depression trajectory. Interventions targeting the dual trajectories and co-factors of social isolation and depression should be considered to improve their effectiveness.

我国成人居民高血压患病率为31.6%[1],高血压患者受疾病及年龄影响,其身体机能逐渐退化、人际关系局限、社会参与减少[2],易出现社会隔离及抑郁、焦虑[3]等多种不良情绪。社会隔离和抑郁会增加高血压和其它心血管疾病发生风险,加大老年高血压的控制难度,严重影响疾病的预后和患者的生活质量[4-6]。社会隔离和抑郁可能是相互影响、相互作用的[7]。以往对社会隔离和抑郁关系的研究大多基于单向关系及横断面研究,忽略了二者的双向动态关系及其异质性发展。本研究基于群组轨迹模型[8] (Group Based Trajectory Modelling, GBTM)建立中老年高血压患者社会隔离与抑郁的双轨迹模型,探讨社会隔离与抑郁的纵向发展轨迹及动态变化的多维机制,确定二者之间的关系及共同影响因素,为减少中老年高血压患者社会隔离和抑郁的发生提供参考依据。
1　资料与方法

1.1　数据来源
本研究数据来源于中国健康与养老追踪调查(CHARLS)数据库,采用2013年、2015年、2018年、2020年四期调查数据。选取2013年年龄≥45岁且患有高血压、同时参加2015年、2018年、2020年三次随访的研究对象,最终纳入有效样本量3 223例。具体筛选流程见图1。
1.2　主要变量
（1）抑郁　采用10个条目的简版抑郁水平评定量表(CES-D10)测量抑郁症状。每个条目按Likert从0~3分计分,总分30分,得分越高,抑郁症状越严重[9]。该量表Cronbach alpha为0.805~0.815之间。
（2）社会隔离　社会隔离包括四方面内容:独居(独居者计1分,其余计0分)、未婚(未婚者计1分,包括分居、离婚、丧偶或从未结婚,已婚计0分)、与子女接触(每周与子女面对面接触、通过电话联系少于1次计1分,其余计0分)、社会参与(在过去的一个月里没有参加以下任何社会活动计1分: ①与朋友互动,②打麻将、下棋、打牌、去社区俱乐部③参加体育、社交④参加社区组织⑤从事志愿、慈善工作⑥参加教育或培训课程;参与过任何一项活动计0分)。社会隔离为0~4分,得分越高,表示社会隔离程度越高[10]。
（3）2013年基线协变量　包括性别、年龄、居住地、文化程度、疼痛、慢性病数量、现在是否饮酒及吸烟、基本生活活动能力(BADL)、夜间睡眠时间等。
1.3　统计学分析
1.3.1　GBTM双轨迹分析原理
GBTM可以识别纵向数据中人群异质性发展轨迹分析轨迹与影响因素或结局之间的关系;在单轨迹模型基础上,GBTM双轨迹通过条件概率和联合概率形式分析两个相关结果发展轨迹之间的联系[8]。采用Stata 18.0软件进行数据处理与统计学分析。
1.3.2　分析步骤
（1）GBTM单轨迹建模:确定社会隔离和抑郁的单轨迹模型参数。①确定最佳轨迹数:分别拟合2~6组轨迹②确定多项式次数分别拟合0~3阶函数,根据贝叶斯信息准则(BIC)绝对值最低、平均后验概率(average posterior probability, AvePP)≥0.7、每个轨迹组成员比例≥5%选择最优模型。
（2）GBTM双轨迹建模:使用社会隔离和抑郁最优单轨迹模型参数拟合双轨迹模型,分析双轨迹的条件概率和联合概率。
（3）连续性变量用描述;分类变量用频数、百分比进行描述。采用logistic回归分析社会隔离轨迹及抑郁轨迹的影响因素。检验水准双侧α=0.05。
2　结果

2.1　2013年基线特征
共调查中老年高血压患者3 223例,年龄61.55±8.79岁,女性1 818例(56.41%),男性1 405例(43.59%)。其余见表1。
2.2　社会隔离轨迹
GBTM轨迹模型显示,社会隔离3组轨迹BIC绝对值最小,阶次为1 1 1分别为无社会隔离低稳定组(21.12%)、无社会隔离中稳定组(65.35%)、社会隔离高上升组(13.53%),各轨迹组成员的比例均大于5%、AvePP均>0.7,模型拟合较好。模型拟合参数如表2所示,轨迹如图2所示。
2.3　社会隔离轨迹组的logistic回归
以社会隔离轨迹组为因变量,以无社会隔离低稳定组为参照,将年龄、性别、居住地等协变量进行无序多项logistic回归。结果显示,年龄在≥76岁、男性、2种及以上慢性病属于社会隔离中稳定组和社会隔离高上升组可能性更大(均P<0.05);初中及以上文化程度、夜间睡眠在7~9h属于无社会隔离低稳定组可能性更大(均P<0.05)。见表3。
2.4　抑郁轨迹
GBTM轨迹模型显示,抑郁3组轨迹BIC绝对值最小,阶次为1 1 2,分别为无抑郁低稳定组(51.34%)、抑郁中增加组(37.13%)、抑郁高持续组(11.53%),各轨迹组成员的比例均大于5%、AvePP均大于0.7,模型拟合程度较好。模型拟合参数部分结果如表4所示,轨迹如图3所示。
2.5　抑郁轨迹组的logistic回归
以中老年高血压患者抑郁轨迹组为因变量,以无抑郁低稳定组为参照,将年龄、性别、居住地等协变量进行无序多项logistic回归。结果显示,男性、初中及以上文化程度、夜间睡眠时间≥7 h属于无抑郁低稳定组可能性更大(均P<0.05);居住在农村、有疼痛、有2种及以上慢性病、有BADL障碍属于抑郁中增加组和抑郁高持续组可能性更大(均P<0.05)。 见表5。
2.6　社会隔离和抑郁的双轨迹
表6为社会隔离轨迹组条件下每个抑郁轨迹组的概率,每列合计为100%。结果显示,无社会隔离低稳定组属于抑郁中增加组和抑郁高持续组的概率分别是29.83%、4.65%(合计34.48%);无社会隔离中稳定组属于抑郁中增加组和抑郁高持续组的概率分别是50.91%、10.93%(合计61.84%);社会隔离高上升组属于抑郁中增加组和抑郁高持续组的概率分别是22.18%、65.28%(合计87.46%)。
表7为抑郁轨迹组条件下社会隔离轨迹组的概率,每列合计为100%。结果显示无抑郁低稳定组属于无社会隔离中稳定组和社会隔离高上升组的概率分别是39.94%、8.05%(合计47.99%);抑郁中增加组属于无社会隔离中稳定组和社会隔离高上升组的概率分别是54.31%、15.72%(合计70.03%);抑郁高持续组属于无社会隔离中稳定组和社会隔离高上升组的概率分别是36.08%、55.55%(合计91.63%)。
表8为社会隔离轨迹组和抑郁轨迹组的联合概率(9个联合概率之和为100%)。24.82%的患者同时属于社会隔离低稳定组和抑郁低稳定组;22.02%的患者属于社会隔离中稳定组和抑郁中增加组,13.61%的患者属于社会隔离高上升组和抑郁高持续组。
3　讨论

3.1　中老年高血压患者社会隔离轨迹和抑郁轨迹
本研究基于GBTM模型,识别出中老年高血压患者社会隔离有3种轨迹,与目前研究结果一致[11]。本研究结果显示,基线不存在社会隔离的患者(86.47%),在随访期间比较稳定,没有出现社会隔离状态。基线存在社会隔离的患者(13.53%),随访期间一直处于社会隔离高水平状态,并且在持续上升。研究显示个体长期处于社会隔离状态会通过下丘脑-垂体-肾上腺轴和交感神经系统,引起炎症、慢性病、高血压、动脉粥样硬化[12]等,甚至增加心血管疾病再住院及死亡风险[11]。因此应对中老年高血压患者社会隔离进行筛查,关注已存在社会隔离的患者,防止社会隔离状态加剧。
本研究识别出中老年高血压患者有3种抑郁轨迹,与以往文献研究一致[13-14],可见中老年高血压患者社会隔离和抑郁的发展轨迹存在群体异质性。本研究中51.34%的中老年高血压患者一直处于抑郁水平以下,出现抑郁的可能性很小。37.13%的患者基线时虽无抑郁,但抑郁得分处于临界值且得分持续上升,并在随访期间出现抑郁症状。11.53%的患者在基线时即有抑郁症状且持续存在,可能发展为病理性抑郁。抑郁情绪能够通过神经内分泌机制使交感神经兴奋,引起血压升高,加速血管动脉硬化进程,加大心血管事件发生风险[15];同时还会削弱个体对疾病的正确认知和应对能力。故采取措施对中老年高血压患者抑郁进行及时干预,缓解不良情绪,有助于高血压患者健康管理。
3.2　中老年高血压患者社会隔离轨迹与抑郁轨迹相互关系
本研究探讨了社会隔离轨迹与抑郁轨迹的双向纵向关系,共确定9个亚组。条件概率、联合概率结果表明无社会隔离的中老年高血压患者更有可能无抑郁症状,而处于社会隔离高上升组的患者更有可能属于抑郁高持续组,表明患者社会隔离轨迹和抑郁轨迹有时间动态重叠或共生,存在显著正相关性,与Zhu等[7]研究结果一致。一方面中老年高血压患者社会隔离与抑郁高风险正相关,高水平的社会隔离与中老年人抑郁症的高发病率显著相关[10]。有社会隔离的患者,社会参与行为减少,限制了个体获取情感支持和社会资源的机会等,使个体在面对生活压力时缺乏缓冲机制;患者因为慢性疾病需要经常就医,与他人交往减少,易产生无助感,增加抑郁发生风险。另一方面，抑郁是社会隔离发生的危险因素,与尹艳茹等[16]研究结果一致。有抑郁症状的患者常伴有精力减退、兴趣缺失，不愿与他人交往,社交活动减少,容易发生社会隔离。因此,需要关注社会隔离与抑郁在中老年高血压患者中多维动态的相互作用,可以针对特定亚群制定个性化干预措施、优化干预的有效性,提高高血压健康管理效果。
3.3　中老年高血压患者社会隔离轨迹和抑郁轨迹的基线共同影响因素
本研究logistic回归结果表明文化程度可能是社会隔离和抑郁轨迹的共同保护性因素,与以往研究一致[17-18]。文化程度越高的个体认知能力越强,有更好的信息处理能力、应对策略、社会网络及经济条件等,这些共同构成了一个强大的心理防御体系,可以有效抵抗社会隔离与抑郁的侵袭。应对文化程度较低的中老年高血压患者采取个性化健康教育策略,同时鼓励患者通过多种途径学习提高自己的文化水平,增强应对能力。本文中慢性病数量可能是社会隔离和抑郁轨迹的共同危险因素。夜间7~9 h的睡眠时长的患者更倾向于无社会隔离低稳定轨迹组和无抑郁低稳定组。有研究表明[19]睡眠不足影响大脑的认知功能,个体易出现疲劳、反应迟钝等现象,使个体在社交活动中处于不利地位,加深了社会隔离。充足的睡眠可以降低夜间血压,降低交感神经兴奋、减少应激激素的分泌[20],促进个体情绪稳定,有助于预防抑郁。临床工作者应对中老年高血压患者加强睡眠指导,鼓励患者保持合适的睡眠时间,提高质量睡眠,减少社会隔离与抑郁的发生。
本研究探讨了中老年高血压患者社会隔离与抑郁的双重轨迹,这些轨迹是有共同影响因素及相互关系。依据社会隔离和抑郁不同的轨迹亚组采取有针对性的预防和干预方案,帮助中老年高血压患者走出社会隔离状态有助于预防抑郁的发生或防止其恶化;积极干预抑郁可以帮助患者恢复正常的社交,远离社会隔离。但本研究也存在一定局限性,首先只纳入基线影响因素,忽略了时变协变量对结果的影响;其次没有包含影响双轨迹的所有因素。进一步研究应探究两者相互作用的深层机制及存在的未知叠加效应,以便全面掌握其发展规律。
