Objective

To investigate the associations between visceral obesity indices and the co-morbidities of hypertension, diabetes mellitus, and dyslipidemia in steel workers.

Methods

This study included 7 318 steelworkers, using multivariate logistic regression to analyze the connection between visceral adiposity-related indices and the co-morbidity of "three highs".

Results

A total of 7 318 steel workers were included, with 1 759 diagnosed with all three conditions, accounting for a 24.04% prevalence rate. After adjusting for age and gender, significant correlations were found between LAP, TyG, and their combined indices with various types of co-morbidities (P<0.001). No interactions were observed between age and these indices in the context of co-morbidities (Pinteraction>0.05), except for the TyG-WC index in cases of two diseases co-occurring (Pinteraction<0.05), and the LAP index, TyG-WHtR index, and LAP-BMI index when all three diseases co-occurred (Pinteraction<0.05).

Conclusion

The LAP index, TyG index, and their combined indices are closely associated with the co-morbidities of hypertension, hyperglycemia, and hyperlipidemia in steelworkers.

背景

肥胖尤其是内脏型肥胖与高血压、糖尿病和血脂异常分别具有较强的关联性,然而内脏型肥胖与“三高”共病的关联尚不明确。

目的

探讨脂质蓄积指数(LAP)、甘油三酯-葡萄糖指数(TyG)以及相关联合指数与钢铁工人高血压、糖尿病和血脂异常共病之间的关系。

方法

本研究以7 318名钢铁工人为研究对象。采用多因素logistic回归分析内脏型肥胖相关指数与“三高”共病之间的相关性。

结果

共有1 759人存在“三高”共病,检出率为24.04%,在调整年龄、性别等因素后,评价内脏型肥胖的LAP、TyG以及联合指数与各种三高共病类型均相关(P<0.001),且各个指数水平与“三高”共病之间存在正相关关系。此外,年龄与各指数在“三高”共病中不存在交互作用(P交互>0.05);年龄与TyG-WC在2种疾病共患中存在交互作用(P交互<0.05);年龄与LAP、TyG-WHtR、LAP-BMI在3种疾病共患中存在交互作用(P交互<0.05)。

结论

基于评价内脏型肥胖的LAP、TyG以及联合指数均与钢铁工人“三高”共病密切相关。

背景

肥胖尤其是内脏型肥胖与高血压、糖尿病和血脂异常分别具有较强的关联性,然而内脏型肥胖与“三高”共病的关联尚不明确。

目的

探讨脂质蓄积指数(LAP)、甘油三酯-葡萄糖指数(TyG)以及相关联合指数与钢铁工人高血压、糖尿病和血脂异常共病之间的关系。

方法

本研究以7 318名钢铁工人为研究对象。采用多因素logistic回归分析内脏型肥胖相关指数与“三高”共病之间的相关性。

结果

共有1 759人存在“三高”共病,检出率为24.04%,在调整年龄、性别等因素后,评价内脏型肥胖的LAP、TyG以及联合指数与各种三高共病类型均相关(P<0.001),且各个指数水平与“三高”共病之间存在正相关关系。此外,年龄与各指数在“三高”共病中不存在交互作用(P交互>0.05);年龄与TyG-WC在2种疾病共患中存在交互作用(P交互<0.05);年龄与LAP、TyG-WHtR、LAP-BMI在3种疾病共患中存在交互作用(P交互<0.05)。

结论

基于评价内脏型肥胖的LAP、TyG以及联合指数均与钢铁工人“三高”共病密切相关。

Objective

To investigate the associations between visceral obesity indices and the co-morbidities of hypertension, diabetes mellitus, and dyslipidemia in steel workers.

Methods

This study included 7 318 steelworkers, using multivariate logistic regression to analyze the connection between visceral adiposity-related indices and the co-morbidity of "three highs".

Results

A total of 7 318 steel workers were included, with 1 759 diagnosed with all three conditions, accounting for a 24.04% prevalence rate. After adjusting for age and gender, significant correlations were found between LAP, TyG, and their combined indices with various types of co-morbidities (P<0.001). No interactions were observed between age and these indices in the context of co-morbidities (Pinteraction>0.05), except for the TyG-WC index in cases of two diseases co-occurring (Pinteraction<0.05), and the LAP index, TyG-WHtR index, and LAP-BMI index when all three diseases co-occurred (Pinteraction<0.05).

Conclusion

The LAP index, TyG index, and their combined indices are closely associated with the co-morbidities of hypertension, hyperglycemia, and hyperlipidemia in steelworkers.

近年来,我国高血压、糖尿病、血脂异常发病率与患病率攀升[1],疾病负担加重,高血压、糖尿病、血脂异常被统称为“三高”,“三高”常聚集性以共病形式发生[2],相互影响。研究表明,“三高”共病患者心血管疾病风险高于不患者和单一患病者[1,3-4]。肥胖是“三高”患病的最主要因素之一[5],肥胖者的“三高”风险是体重正常者的2~4倍[6],尤其是内脏型肥胖与代谢异常、心血管疾病尤其是“三高”密切相关[7-8],内脏脂肪含量每增加一个标准差,高血压、糖尿病的患病风险也相应增加[9-10]。脂质蓄积指数(LAP)、甘油三酯葡萄糖指数(TyG)及其联合指数是评估内脏脂肪的有力指标[11],但与“三高”共病关联尚不明确。因此本研究以钢铁工人为研究对象,探讨不同内脏型肥胖评价指标与“三高”共病之间的相关性,为慢性病防控和“三高”共管防治提供依据。
本研究依托《京津冀地区职业人群健康效应队列》,选取2017年6-9月进行基线调查的7 622名工人,排除304名信息不全者,最终纳入7 318名工人。本研究获华北理工大学伦理委员会批准(批准号:16040),研究对象均签知情同意书。
本研究由课题组自编问卷收集工人个人及家庭基本特征、疾病史、生活方式、工作情况,问卷经前期验证显示在该人群信度良好(内在一致性系数为0.74)[12]。职业暴露情况如噪声作业[13](8 h/d或40 h/w噪声暴露A效声级≥80 dB)、高温暴露[14](WBGT≥25 ℃且伴有生产性热源)和粉尘暴露[15]均根据中华人民共和国国家职业卫生标准进行定义。采用标准方法现场测量研究对象身高、体重、腰围和血压指标,被测者空腹12 h后于指定医院采肘正中静脉血,检测空腹血糖、甘油三酯等指标。
本研究中高血压诊断标准为收缩压≥140 mmHg和(或)舒张压≥90 mmHg,或既往有高血压史,目前正在服用降压药[16]。糖尿病诊断标准[4]为6.1 mmol/L≤空腹血糖(FPG)<7.0 mmol/L(糖尿病前期);FPG≥7.0 mmol/L,或既往有糖尿病史,目前正在接受治疗。血脂异常诊断标准[17]为血清中TC浓度≥6.22 mmol/L(240 mg/dl),和(或)LDL-C浓度≥4.14 mmol/L(160 mg/dl),和(或)TG浓度≥2.26 mmol/L(200 mg/dl),和(或)HDL-C浓度<1.04 mmol/L(40 mg/dl),或既往有血脂异常病史,目前正在服用降脂药。高血压、糖尿病和血脂异常同时患有两种及以上者为“三高”共病。
脂质蓄积指数[18]LAP男性=[WC-65]×TG;LAP女性=[WC-58]×TG;甘油三酯葡萄糖指数[19](TyG)=ln(TG [mg/dL]×FPG [mg/dL]/2);甘油三酯葡萄糖腰围指数[20](TyG-WC)=TyG×WC;甘油三酯葡萄糖腰高比指数[21](TyG-WHtR)=TyG×WHtR;甘油三酯葡萄糖体重指数[20](TyG-BMI)=TyG×BMI;脂质蓄积体重指数(LAP-BMI)=LAP×BMI;本研究按照中位数将各个指数分为Q1和Q2组。
数据分析采用SPSS 23.0和R 4.3.2进行统计分析。进行正态性检验,正态的连续变量使用,非正态的连续变量使用或MP25P75)表示,分类变量使用n(%)的形式表示。符合正态分布的连续变量不同组间差异比较采用单因素方差分析,不符合正态的连续变量不同组间比较采用Wilcoxon秩和检验,分类变量不同组间差异比较采用卡方检验。采用逐步调整的logistic回归模型来分析内脏型肥胖相关指标与“三高”共病的相关性,并分析年龄与内脏型肥胖相关指标之间的交互作用。
该研究最终有7 318名研究对象纳入分析,共有1 759人存在“三高”共病,检出率为24.04%,其中1 531例(20.92%)共患2种疾病,228例(3.12%)共患3种疾病,检出率依次为高血压合并血脂异常(87.72%)、3种疾病共患(3.12%)、高血压合并糖尿病(1.43%)和糖尿病合并血脂异常(1.13%),见表1
在逐步调整协变量后,当各指数为连续变量时,各指数与“三高”共病、2种疾病共患和3种疾病共患之间呈正相关;当各指数为分类变量时,各指数的高水平组与“三高”共病、2种疾病共患和3种疾病共患仍然存在正相关关系(P<0.001);此外,年龄与各指数在“三高”共病中不存在交互作用(P交互>0.05),年龄与TyG-WC在2种疾病共患中存在交互作用(P交互<0.05),年龄与LAP、TyG-WHtR、LAP-BMI在“三高”共病中存在交互作用(P交互<0.05),详见表2~4
本研究结果显示,钢铁人群的高血压、糖尿病和血脂异常共病检出率为24.04%,高于河北保定某社区40岁以上居民的“三高”共病率(18.20%)[22],其中1531例(20.92%)共患 2 种疾病,228例(3.12%)共患 3 种疾病。两病共患模式中,以高血压合并血脂异常共患率最高(18.35%),与南京市成人“三高”共病现状的调查结果一致[23]
本研究发现,各个内脏型肥胖相关指数以及各指数的高水平组与“三高”共病、2种疾病共患以及3种疾病共患之间均呈正相关关系。秦真真等人[24]和CHARLS的一项研究表明[25],高血压和糖尿病患病风险会随TyG指数水平的升高而增加;何平等人[26]对35~70岁中国社区居民的研究结果也表明高血压、糖尿病患病与LAP水平呈正相关,与LAP Q1组相比,女性和男性LAP Q3组高血压患病的OR(95% CI)值分别为2.291(1.935,2.714)、2.381(1.998,2.837),糖尿病患病OR(95% CI)分别为1.555(1.301,1.859)、1.716(1.424,2.069)。也有研究证实,BMI、WC和WHtR的水平与血脂异常患病密切相关[27]。另外本研究还得到,年龄与TyG-WC在2种疾病共患中存在交互作用,年龄与LAP、TyG-WHtR、LAP-BMI在3种疾病共患中存在交互作用(P交互<0.05)。南京一项对于成人“三高”共病的调查也表明,随着年龄的增加,“三高”患病的风险也会增加(χ2趋势=1 3101.67)[23],这可能是年龄与部分指标存在交互作用的原因。
本研究优势在于大样本量(n=7 318),并综合单一及联合内脏型肥胖指数(LAP、TyG及其联合)来深入探讨其与“三高”共病的关联。通过多因素调整,LAP、TyG以及联合指数与“三高”共病的关联仍然存在。但研究也存在局限:一是横断面设计无法确立因果关系;二是结果仅适用于钢铁工人,其他人群适用性未知;三是性别比例失衡,女性样本少,难以充分分析性别差异对内脏脂肪代谢及患病情况的影响。
综上所述,基于评价内脏型肥胖的LAP、TyG以及联合指数均与钢铁工人“三高”共病密切相关。因此,我们可以通过降低LAP、TyG以及联合指数的水平,延缓钢铁工人群体心血管疾病的发生发展,实现“三高”共管。
