Objective

To construct an evaluation indicator system for the development of occupational disease clinical specialties, providing a basis for the construction and evaluation of occupational disease clinical specialties.

Methods

An initial expert consultation questionnaire was developed through literature research and group discussions. The Delphi method was applied to conduct two rounds of expert consultations with 34 experts in the field of occupational diseases from 13 provinces (autonomous regions and municipalities) including Beijing, Sichuan, and Henan. The final evaluation indicator system for occupational disease clinical specialties was formed, and the hierarchical weights of each index were determined using the priority diagram method.

Results

The response rates for the two rounds of expert consultations were 97.14% and 94.12%. The expert authority coefficients were 0.856 and 0.860, and the Kendall’s concordance coefficients of the experts were 0.18 and 0.12 (P<0.01). The final occupational disease clinical specialty evaluation indicator system consisted of 5 first-level indicators, 17 second-level indicators, and 67 third-level indicators.

Conclusion

The enthusiasm, authority, and consistency of the two rounds of expert consultations were high. This evaluation indicator system is scientifically sound and reliable, providing a reference for the construction and high-quality development of occupational disease clinical specialties.

目的

构建职业病临床专科建设评价指标体系,为职业病临床专科建设和评价提供依据。

方法

通过文献研究法和小组讨论法构建初始专家咨询问卷,运用德尔菲法对来自北京、四川、河南等13个省(自治区、直辖市)的34名职业病领域专家进行2轮专家咨询,形成职业病临床专科评价指标体系,使用优序图法确定各级指标权重。

结果

两轮专家咨询问卷的回收率分别为97.14%和94.12%,专家权威系数为0.856和0.860,专家肯德尔和谐系数分别为0.18和0.12(P<0.01)。最终构建了由5个一级指标、17个二级指标、67个三级指标构成的职业病临床专科评价指标体系。

目的

构建职业病临床专科建设评价指标体系,为职业病临床专科建设和评价提供依据。

方法

通过文献研究法和小组讨论法构建初始专家咨询问卷,运用德尔菲法对来自北京、四川、河南等13个省(自治区、直辖市)的34名职业病领域专家进行2轮专家咨询,形成职业病临床专科评价指标体系,使用优序图法确定各级指标权重。

结果

两轮专家咨询问卷的回收率分别为97.14%和94.12%,专家权威系数为0.856和0.860,专家肯德尔和谐系数分别为0.18和0.12(P<0.01)。最终构建了由5个一级指标、17个二级指标、67个三级指标构成的职业病临床专科评价指标体系。

结论

两轮专家咨询的积极性、权威性和一致性均较高,该评价指标体系有较强的科学性和可靠性,可为职业病临床专科建设和高质量发展提供参考依据。

Objective

To construct an evaluation indicator system for the development of occupational disease clinical specialties, providing a basis for the construction and evaluation of occupational disease clinical specialties.

Methods

An initial expert consultation questionnaire was developed through literature research and group discussions. The Delphi method was applied to conduct two rounds of expert consultations with 34 experts in the field of occupational diseases from 13 provinces (autonomous regions and municipalities) including Beijing, Sichuan, and Henan. The final evaluation indicator system for occupational disease clinical specialties was formed, and the hierarchical weights of each index were determined using the priority diagram method.

Results

The response rates for the two rounds of expert consultations were 97.14% and 94.12%. The expert authority coefficients were 0.856 and 0.860, and the Kendall’s concordance coefficients of the experts were 0.18 and 0.12 (P<0.01). The final occupational disease clinical specialty evaluation indicator system consisted of 5 first-level indicators, 17 second-level indicators, and 67 third-level indicators.

Conclusion

The enthusiasm, authority, and consistency of the two rounds of expert consultations were high. This evaluation indicator system is scientifically sound and reliable, providing a reference for the construction and high-quality development of occupational disease clinical specialties.

职业健康是健康中国建设的重要基础和组成部分,近年来我国职业病防治体系不断完善,但职业病医疗机构还不完全适应高质量发展的新要求[1],职业病防治支撑服务和保障能力亟待加强[2]。由于职业病防治工作具有较强专业性和公益性,其主要由公立医院承担[3]。临床专科建设是公立医院高质量发展的重要保障[4],2021年国务院办公厅印发《关于推动公立医院高质量发展的意见》中提到,引领公立医院高质量发展要加强临床专科建设[5],早在2010年职业病专业就被纳入国家临床重点专科建设项目中[6]。因此,职业病临床专科建设是职业病医疗机构高质量发展的重要内容,也是健康中国建设的重要部分。但目前职业病临床专科建设尚缺乏指标体系,而且职业病防治工作需覆盖预防、诊疗及康复全过程[2],并承担一定行政工作及法律责任[6],不能完全照搬现有的各类临床专科评价指标体系。在职业健康高质量发展、职业病疾病谱变化和防治工作转变为以“健康”为中心的背景下,构建一套职业病临床专科建设评价指标体系尤为重要。
德尔菲法(Delphi Method)通过向专家发放多轮问卷进行调查,经过反复归纳和修订,达成较为一致的意见,其广泛应用于评价指标体系的构建[7-8]
1.1.1 组建专家咨询组专家的纳入标准为中国职业病防治院联盟中全国各地职业病医疗机构的管理人员或专业技术人员,或高校、疾病预防控制机构从事职业病防治相关专业的教师、技术人员和管理人员,以及在卫生健康主管部门中从事职业病防治工作的管理人员,从事相关工作5年及以上。排除标准为:中级职称以下,本科学历或学士学位以下。
1.1.2 指标筛选与初步构建检索国务院、国家卫生健康委以及各省级卫生健康主管部门针对医院考核、评审、专科建设等方面的政策文献,同时借鉴社会评价体系,构建备选评价指标池。经前期研究小组讨论和专家咨询,初步拟定包含5个一级指标、16个二级指标和76个三级指标的职业病临床专科建设评价指标体系,其中5个一级指标为医疗服务、教学水平、科研能力、人才队伍和社会服务与评价。
1.1.3 专家咨询建立职业病临床专科建设评价指标体系德尔菲问卷,采用电子问卷方式开展2轮专家咨询。问卷内容包括专家基本信息、职业病临床专科建设指标重要性评分和专家熟悉程度及判断依据三个部分。指标体系部分通过Likert 5级评分法请专家对所有一、二、三级指标进行重要性打分[9],1分为“非常不重要”,2分为“不重要”,3分为“一般重要”,4分为“重要”,5分为“非常重要”。问卷专家权威程度和熟悉程度分值见表1表2[10]。对重要性均数< 4.0或者变异系数(CV)>0.25的指标进行调整或删除[11]
使用SPSSAU构建优序图权重计算表,计算指标得分(TTL)和各指标权重值[12]。对多个指标的大小进行两两比较,来确定不同指标的重要性次序。在两两比较时,通过构建棋盘图,将指标重要性较大者赋值为“1”,表明其更加重要,将重要性较小者赋值为“0”;若两个指标重要性相等,则赋值为“0.5”。将每项指标的赋值横向求和,得到指标的相对分值,每项指标的相对分值除以各指标相对分值之和,即为每项指标的权重。
采用Excel 2021 和SPSS 25.0对两轮专家函询结果进行整理和描述性数据分析。专家积极程度、权威程度分别通过问卷回收率、专家权威系数(Cr)表示[13],专家意见协调程度采用Kendall-W法作统计检验,检验水准α=0.05。使用优序图法计算各指标的权重。
本研究共纳入来自北京、四川、河南、云南、内蒙古等13个省(自治区、直辖市)的34名专家。年龄40岁以上者29人(85.29%),男性16人(47.06%),女性18人(52.94%),专家的工作机构主要为职业病医疗机构,有29人(85.29%),高级职称专家28人(82.35%),硕士及以上学历者21人(61.76%),工作年限超过10年的专家有33人(97.06%)。在具体工作内容方面,有10名专家(29.41%)同时承担医疗卫生技术、行政管理和科研教学中的两项至三项工作。详见表3
2.2.1 专家积极程度本研究在第一轮时邀请35位专家填写问卷,实际参与咨询专家35人,回收问卷34份,专家积极系数为97.14%。在第二轮问卷咨询中,向此前参与研究的34名专家发放问卷,共回收32份专家问卷,专家积极系数为94.12%。
2.2.2 专家权威程度专家权威系数(Cr)的结果为熟悉程度(Cs)和指标判断依据(Ca)的均值[14]。第一轮专家CsCa的值分别为0.765和0.947,Cr值为0.856。第二轮专家CsCa的值分别为0.769和0.950,Cr值为0.860。
2.2.3 专家协调程度使用肯德尔和谐系数(W)评价专家对于指标体系意见的一致性。第一轮5个一级指标的W值为0.12;16个一级指标的W值为0.18;77个三级指标的W值为0.16,所有指标的总体W值为0.18(P<0.01)。第二轮问卷咨询中5个一级指标的W值为0.12;17个一级指标的W值为0.18;69个三级指标的W值为0.16,所有指标的总体W值为0.12(P<0.01)。详见表4
开展第一轮咨询后,对专家的意见进行整理并结合对各指标的定量分析结果,新增1个二级指标“健康教育开展”和1个三级指标,删除了3个三级指标,将11个三级指标优化整合为5个,并重命名2个三级指标。形成第二轮专家咨询问卷。
根据第二轮专家咨询结果,“职业病出院人数”“大型医用设备职业病检查阳性率”两项三级指标的重要性均数,将其删除。最终形成了5个一级指标,17个二级指标,67个三级指标的职业病临床专科建设指标体系。所有一级指标均同时满足CV<0.25。
采用优序图法计算各指标权重得出,5个一级指标为医疗服务、教学水平、科研能力、人才队伍和社会服务与评价的权重分别为0.36、0.12、0.20、0.28和0.04。一致性检验CR值均小于0.05,表明各层级指标明确,无逻辑错误,一致性程度高。各指标权重计算结果详见表5
本研究选取了来自13个省(自治区、直辖市)的34名专家。专家基于自身实践经验和理论知识为职业病临床专科建设评价指标体系提供了建议和支持,经过两轮的修改和完善形成了包含5个一级指标、17个二级指标和67个三级指标的职业病临床专科建设评价指标体系。
两轮咨询中,专家的积极系数分别为97.14%和94.12%,表明参与专家对本研究的高度重视和积极投入。两轮咨询的专家权威系数均大于0.70[15],并且专家所属工作机构多为职业病医疗机构,工作年限为10年及以上,表明参与专家对职业病领域的熟悉程度较高,对于职业病临床专科建设及发展有着深刻认知和切实体会。此外,前后两轮咨询肯德尔和谐系数(W)分别为0.18和0.12(P<0.01),显示了专家在指标体系构建上的高度一致性[16]。因此本研究具备较高的科学性和可靠性。
职业病临床专科建设评价指标体系包含了医疗服务、教学水平、科研能力、人才队伍和社会服务五个方面的核心要素[3,17]。在医疗服务和人才队伍方面,指标涵盖了医疗质量、效率以及卫生技术人员配置等关键内容;在教学水平和科研能力上,体系突出了教育和科研产出的持续性与创新性;而在社会服务与评价中,体系反映了职业病防控和公众健康教育的重要性。这些指标不仅为职业病专科的建设提供了科学依据,还为政策制定者和管理者在资源分配与绩效评估中提供了量化标准。
具体来看,一级指标中权重较高的为医疗服务和人才队伍,这与有关政策文件中要求加强人才队伍建设,提高技术水平,提高服务质量的目标不谋而合[5,18],体现了医疗服务和人才是临床专科建设的重要部分[19]。二三级指标中,除医疗服务和人才队伍方面,权重较高的为科研项目,通过科研解决临床实际能力,对临床专科能力具有巨大作用,而目前在职业卫生领域研究资助体系还不全面且以高等院校承担为主[20]。在临床专科建设过程中,有必要增加职业病领域各类项目的资助,并积极与高等院校合作,将科研与职业病防控工作相结合。其次学历教育的权重也较高,高层次职业病专科人才离不开高水平人才教育[21],目前职业病医师主要是由临床医师经过职业病培训后从事职业病诊治工作,在前期的学历教育中重点是针对一般疾病的诊断和治疗,对于职业病这种特殊的、与工作环境密切相关的疾病体系,在后续培训中可能无法全面、深入地掌握相关知识。如果能在学历教育中加强职业病防治教育,职业病专科人才队伍将会得到一定程度的改良。
本研究也存在一定的局限性。首先,职业病医疗效率指标有待完善,医疗效率二级指标的重要性评分较高,但其下属三级指标的设立仍然存在争议,由于职业病病种多,治疗复杂,公益性强,现有医疗效率指标均不适用。在后续研究中应增加能够体现职业病临床专科医疗效率的指标,将整个评价指标体系更加完善。此外,本评价指标体系尚未建立评分细则,在后续研究中应与职业病技术、管理方面专家进一步讨论各项指标的适用范围与评分细则。
