Objective

To understand the current status of digital health literacy among rural elderly individuals in Shandong Province and analyze the main influencing factors.

Methods

A multi-stage stratified cluster random sampling method was employed to select 720 rural individuals aged 60 years and older from the eastern, central, and western regions of Shandong Province for a questionnaire survey. Multiple linear regression analysis was used to explore the factors influencing digital health literacy among the elderly.

Results

The average digital health literacy score of the 720 rural elderly participants was 58.31±16.34. Multiple linear regression revealed that factors associated with higher digital health literacy included normal BMI (β=8.28, P<0.001), low body weight (β=18.61, P<0.001), self-reported good health status (β=6.90, P<0.001), general health status (β=10.20, P<0.001), being married (β=3.49, P=0.001), monthly income between 3 000-4 999 yuan (β=6.75, P<0.001), income ≥5 000 yuan (β=11.48, P<0.001), and having health insurance coverage (urban employee health insurance/rural resident health insurance) (β=5.58, P=0.001).

Conclusion

The level of digital health literacy among rural elderly individuals in Shandong Province needs improvement. Targeted measures should be implemented from five aspects: personal traits, behavioral characteristics, interpersonal relationships, living and working conditions, and policy environment, in order to enhance digital health literacy in this population.

目的

了解山东省农村老年人数字健康素养现况,分析主要影响因素。

方法

采用多阶段分层整群随机抽样方法,在山东省东、中、西部地区抽取720名60岁及以上农村老年人进行问卷调查。采用多元线性回归分析探究老年人数字健康素养的影响因素。

结果

720名农村老年人的数字健康素养总分为(58.31±16.34)分。多元线性回归结果显示,BMI体重正常(β=8.28,P<0.001),体重偏低(β=18.61,P<0.001)、自评健康状况好(β=6.90,P<0.001),一般(β=10.20,P<0.001)、婚姻状态已婚(β=3.49,P=0.001)、月收入3 000~4 999元(β=6.75,P<0.001),≥5 000元(β=11.48,P<0.001)、有医疗保险类型城镇职工医疗保险/城乡居民医疗保险(β=5.58,P=0.001)等的农村老年人数字健康素养较高。

结论

山东省农村老年人数字健康素养水平有待提升,应从个人特质、行为特征、人际关系、生活和工作条件、政策环境五个层面有针对性地综合施措,提高山东省农村老年人数字健康素养水平。

目的

了解山东省农村老年人数字健康素养现况,分析主要影响因素。

方法

采用多阶段分层整群随机抽样方法,在山东省东、中、西部地区抽取720名60岁及以上农村老年人进行问卷调查。采用多元线性回归分析探究老年人数字健康素养的影响因素。

结果

720名农村老年人的数字健康素养总分为(58.31±16.34)分。多元线性回归结果显示,BMI体重正常(β=8.28,P<0.001),体重偏低(β=18.61,P<0.001)、自评健康状况好(β=6.90,P<0.001),一般(β=10.20,P<0.001)、婚姻状态已婚(β=3.49,P=0.001)、月收入3 000~4 999元(β=6.75,P<0.001),≥5 000元(β=11.48,P<0.001)、有医疗保险类型城镇职工医疗保险/城乡居民医疗保险(β=5.58,P=0.001)等的农村老年人数字健康素养较高。

结论

山东省农村老年人数字健康素养水平有待提升,应从个人特质、行为特征、人际关系、生活和工作条件、政策环境五个层面有针对性地综合施措,提高山东省农村老年人数字健康素养水平。

Objective

To understand the current status of digital health literacy among rural elderly individuals in Shandong Province and analyze the main influencing factors.

Methods

A multi-stage stratified cluster random sampling method was employed to select 720 rural individuals aged 60 years and older from the eastern, central, and western regions of Shandong Province for a questionnaire survey. Multiple linear regression analysis was used to explore the factors influencing digital health literacy among the elderly.

Results

The average digital health literacy score of the 720 rural elderly participants was 58.31±16.34. Multiple linear regression revealed that factors associated with higher digital health literacy included normal BMI (β=8.28, P<0.001), low body weight (β=18.61, P<0.001), self-reported good health status (β=6.90, P<0.001), general health status (β=10.20, P<0.001), being married (β=3.49, P=0.001), monthly income between 3 000-4 999 yuan (β=6.75, P<0.001), income ≥5 000 yuan (β=11.48, P<0.001), and having health insurance coverage (urban employee health insurance/rural resident health insurance) (β=5.58, P=0.001).

Conclusion

The level of digital health literacy among rural elderly individuals in Shandong Province needs improvement. Targeted measures should be implemented from five aspects: personal traits, behavioral characteristics, interpersonal relationships, living and working conditions, and policy environment, in order to enhance digital health literacy in this population.

中国作为是世界上老年人口规模最大的国家[1],老龄化形势极为严峻。在此背景下,数字技术为应对老龄化挑战提供了广泛机遇[2],老年人倾向利用数字技术获取健康与医疗信息,但也面临应用数字媒介及搜寻信息、获取信息能力的挑战[3]。数字健康素是影响老年人从网络获取、理解有效健康信息的重要因素,也是影响老年人自我健康管理能力的关键因素。目前学界对农村老年人数字健康素养研究较少,因此本研究在了解老年人生活状况的基础上,探讨农村老年人数字健康素养现状及其影响因素,为相关部门促进提升老年人数健康素养,推动智慧健康养老发展,满足家庭和个人多层次、多样化的健康养老服务需求提供对策依据。
既往研究多聚焦于老年人数字健康素养的单一影响因素,而健康生态学模型(HEM)则提供了一个系统性、综合性的研究框架[4],包括个人特质因素(生理、心理)、行为特征、人际关系(社会文化)、生活和工作条件、政策环境等因素。运用该模型可克服既往研究的局限性,系统性探讨老年人数字健康素养现状及影响因素[5]
研究对象为山东省农村老年人。采用多阶段分层整群随机抽样的方法,根据经济水平和地理位置,在山东省随机抽取3个地级市,在每个地级市随机抽取3个县,在每个县随机抽取 3个村进行调查。在获得受试知情同意后,从符合纳入与排除标准的老年人中进行筛选,每个村随机调查20~30人。根据横断面调查样本量计算公式计算样本量,设检验水准α为0.05,对应Zα/21.96。P值为根据既往研究或预实验确定的预期人口比例,既往研究中,中国老年人数字健康素养合格率为11.1%[6],故本研究中P取0.11。d为容许误差,本研究中设定为0.03。计算得出本研究所需最少样本量为418例。实际共调查 750 人,回收问卷 732份,剔除无效问卷 12份问卷,最终有效问卷 720份,问卷有效回收率为 96%。研究通山东第二医科大学伦理委员会审批(2024YX292)。
本研究使用课题组自制《老年人数字健康素养现况及影响因素调查问卷》,信效度检验结果显示问卷的 Cronbach α系数为0.969,KMO值为0.960,问卷的信度和结构效度较高。根据健康生态学模型筛选和整理变量;①个人体特质层:性别、年龄、BMI、是否患有慢性疾病;②行为特征层:吸烟、饮酒、自评健康状况、对健康状况关注程度、是否使用互联网查询信息、搜索信息频率、感知有用性、感知准确性等;③人际关系层:婚姻状况、户籍类型、居住情况等;④生活和工作条件层:工作状态、人均年收入、受教育程度等;⑤政策环境层:医保付费方式等调查内容。
采用SPSS 26.0软件进行统计学分析,计量资料以均值、标准差来表示,计数资料以频率、百分比来表示,组间比较方差齐则两组间比较采用t检验、多组间比较采用单因素方差分析,若方差不齐采用Brown-Forsythe检验,使用多元线性回归法探究老年人数字健康素养的影响因素,根据健康生态学模型的五个层级,构建了五个不同的模型:模型1仅包含第一个层级的指标,模型2包含第一个和第二个层级的指标,模型3包含第一个、第二个和第三个层级的指标,模型4包含了第一个、第二个、第三个和第四个层级的指标,而模型5则包含了健康生态学模型中所有五个层级的指标。检验水准α=0.05。
本研究共纳入720名调查对象,其中男性占比较大,有 396 人(55.0%);65~69岁的居民较多,有285人(39.6%);374人(51.9%)为非城镇户口;已婚老年人共519人(82.1%);文化程度为初中有322人(44.7%);与配偶同住老年人有441人(61.3%);患有慢性病的老年人为467人(64.9%);半数以上老年人使用互联网查询健康相关信息(78.5%),接近半数老年人认为网络健康信息一般有用(43.9%),全部研究对象中,数字健康素养总分平均值为58.31±16.34分。
老年人数字健康素养在年龄、BMI、文化程度、户口类型、居住状态、家庭人均月收入、自评健康状况、电子产品使用频率、是否使用互联网查询健康相关信息、向家人寻求帮助频率、感知风险性、信任程度、医疗保险类型等因素,差异有统计学意义(P<0.05);吸烟情况,差异无统计学意义(P>0.05),详见表1表2
以老年人数字健康素养总分作为因变量,根据健康生态学模型分层条件,从上述单因素分析中对老年人数字健康素养有统计学意义的因素进行选取作为自变量进行多元线性回归分析。最终结果显示,模型1~5的R2值分别为0.233、0.487、0.494、0.554、0.563,表明模型模型拟合优度最好,模型5的结果显示BMI、体检次数、电子产品使用频率、感知有用性、感知风险性、信任程度、婚姻状态、家庭人均月收入、受教育程度、医疗保险类型等是老年人数字健康素养总分的影响因素(P<0.05)。详见表3
本研究的调查结果显示山东省农村老年人数字健康素养总分(58.31±16.34)分,各维度得分均低于标准中位数,健康素养整体水平较低,与刘思齐等[7]研究的中国西南地区老年人数字健康素养现状相似;与Tennant[8]、Levy[9]等人研究相比,中国老年人数字健康素养水平低于美国老年人数字健康素养水平,应重视提高老年人的数字健康素养。本研究利用健康生态学模型对影响因素进行分类,采用多元线性回归分析影响因素。健康生态学模型的五层影响因因素中个人特质层属于下游因素,行为特征层与人际关系层为中游因素,生活和工作条件层与政策环境层属于上游因素。
个人特质直接影响老年人数字健康素养水平,研究显示,年龄、BMI、是否患有慢性疾病能显著影响老年人数字健康素养水平。首先,年龄越小,数字健康素养水平要高,这与Arcury[10]、Paige[11]等众多学者研究一致,年龄增长导致认知能力和记忆力下降,影响数字技术学习能力[12]。BMI指数偏低的老年人的数字健康素养水平更好,既往研究显示肥胖与注意力减退、认知障碍相关,影响专注力及应用数字工具能力[13]。未患有慢性疾病的老年人的数字健康素养水平更好,可能原因是患有慢性疾病的老年人,例如患有如关节炎、心血管疾病、呼吸系统疾病等,可能导致老年人身体疲劳和疼痛感,从而影响了他们的数字健康素养水平[14-16]
本研究显示中游因素对老年人数字健康素养水平有显著影响。体检次数多、自评健康状况好的老年人的数字健康素养更高,与Ghazi[17]、Liu[18]等人研究结果相似,生活方式越健康,更能有效使用数字技术。电子产品使用频率、搜索健康信息频率与老年人数字健康素养得分呈正相关,这与Hoogland[19]、段怡雯[20]等人研究结论相似,通过健康信息的获取提升健康素养。对网络健康信息感知有用性高、信任度高、感知风险性低的老年人,信息筛选能力更强、态度更为积极,数字健康素养更高。此外,已婚及接受家人指导频率的老年人数字健康素养水平更高,可能原因是该类型的老年人通常有配偶或家庭成员的支持与陪伴,Li等[21]研究表明,家庭支持促进个体健康,赋予老年人获得归属感与学习自信,进而提升数字健康素养水平。
上游因素间接影响老年人数字健康素养。本研究显示,家庭人均月收入高、受教育程度高的老年人的数字健康素养水平更高,可能原因是高学历的老年人阅读、分析、理解能力强[22],更易理解和掌握数字健康知识。从事脑力劳动为主的老年人的数字健康素养水平更高,根据Jorm等[23]的研究表明,务农、无正式工作是晚年认知功能下降的危险因素,而认知能力下降可能会导致老年人理解能力下降,影响健康信息理解与数字技术适应。拥有城镇职工医疗保险/城乡居民医疗保险的老年人的数字健康素养水平更高,健康保障较好,能够更频繁地接触健康管理服务,数字健康素养水平也更高[24]。以上因素从社会物质环境及宏观层面间接作用于老年人数字健康素养。
综上所述,本研究利用健康生态学模型,由近端到远端,综合多层面、多因素对老年人数字健康素养的影响因素进行分析。可从从上游、中游和下游因素综合施策。在上游层面,通过社区健康教育和信息技术培训提升低教育水平群体的数字技能,提供政策补贴帮助经济困难的老年人获得数字设备。在中游层面,鼓励老年人定期体检和增加电子产品使用频率,增强健康意识;家属的主动引导和支持也是关键,能够帮助老年人更好地利用数字健康资源。在下游层面,可以通过个性化教育、适老化工具设计、健康生活方式干预和慢性病管理数字工具提供,帮助老年人克服使用障碍,提升数字健康素养。本研究采用了多阶段分层整群随机抽样方法,保证了样本代表性,但限于横断面设计,无法揭示因果关系。未来可扩大样本量,开展跨区域的纵向研究,以进一步探讨长期影响因素。
