Objective

To assess Hantaan virus infection risk across districts in Sichuan Province, providing a scientific basis for prevention and intervention measures for Hemorrhagic Fever with Renal Syndrome (HFRS) and data for evaluating their effectiveness.

Methods

This study utilized open-source databases and mathematical modeling. HFRS case report data were collected to analyze epidemic status and spatial distribution using traditional epidemiological methods, while a Bayesian spatiotemporal model examined temporal effects, spatial effects, spatiotemporal interactions, and the impact of meteorological, socioeconomic, and healthcare factors on regional risk levels.

Results

From 2015 to 2021, HFRS incidence in Sichuan showed temporal variation and seasonality, with Yanyuan County reporting the highest cases. Spatial analysis indicated that most districts had risk values consistent with the provincial average, with high-risk areas in the southern, northeastern and central regions. Relative risk declined overall from 2015 to 2020, with a 41.7% decrease in 2020 compared to 2015, although a slight increase occurred in 2021 (0.22 per 100 000). Yanyuan County had the highest relative risk (2.66 [1.64, 4.57]), while other regions aligned with the provincial average. Urbanization rate and humidity affected HFRS incidence negatively, and precipitation affected HFRS incidence positively.

Conclusion

HFRS incidence in Sichuan Province is generally low, with higher rates in specific districts, highlighting spatial risk variability. Meteorological, socioeconomic, and healthcare factors influence infection risk. Monitoring relevant indicators and implementing timely interventions are essential to prevent increased human infection risk.

目的

评估四川省各区县人群感染汉坦病毒的风险,为制定肾综合征出血热(HFRS)的防控和干预措施提供科学依据,并为评估干预效果提供基础数据。

方法

本研究基于开源数据库,通过数学建模方法进行分析。收集HFRS报告病例数据,采用传统流行病学方法了解疾病流行现状及空间分布,并构建贝叶斯时空模型分析时间效应、空间效应及时空交互效应,以及气象、社会经济和医疗卫生等因素对各地区风险值的影响。

结果

2015—2021年四川省HFRS年发病率随时间变化且具有季节性,盐源县病例报告最多。空间效应分析显示,大部分区县风险值与平均水平持平,高风险区主要集中在南部、东北部和中部;时间效应分析表明,2015—2020年HFRS相对风险整体下降,2020年发病率较2015年下降约41.7%,但2021年略有上升(0.22/10万);时空交互效应结果显示,盐源县的相对风险值最高(RR=2.66,95% CI:1.64~4.57),其他地区风险与四川省平均水平一致。城市化率、湿度对HFRS发病具有负向影响,降水对HFRS发病具有正向影响,影响均具有显著性。

结论

四川省HFRS整体发病率较低,仅部分区县较高,且风险存在空间差异。气象环境、社会经济和医疗卫生因素均影响感染风险。因此,应重点监测相关指标变化,并及时干预以防止人感染风险上升。

目的

评估四川省各区县人群感染汉坦病毒的风险,为制定肾综合征出血热(HFRS)的防控和干预措施提供科学依据,并为评估干预效果提供基础数据。

方法

本研究基于开源数据库,通过数学建模方法进行分析。收集HFRS报告病例数据,采用传统流行病学方法了解疾病流行现状及空间分布,并构建贝叶斯时空模型分析时间效应、空间效应及时空交互效应,以及气象、社会经济和医疗卫生等因素对各地区风险值的影响。

结果

2015—2021年四川省HFRS年发病率随时间变化且具有季节性,盐源县病例报告最多。空间效应分析显示,大部分区县风险值与平均水平持平,高风险区主要集中在南部、东北部和中部;时间效应分析表明,2015—2020年HFRS相对风险整体下降,2020年发病率较2015年下降约41.7%,但2021年略有上升(0.22/10万);时空交互效应结果显示,盐源县的相对风险值最高(RR=2.66,95% CI:1.64~4.57),其他地区风险与四川省平均水平一致。城市化率、湿度对HFRS发病具有负向影响,降水对HFRS发病具有正向影响,影响均具有显著性。

结论

四川省HFRS整体发病率较低,仅部分区县较高,且风险存在空间差异。气象环境、社会经济和医疗卫生因素均影响感染风险。因此,应重点监测相关指标变化,并及时干预以防止人感染风险上升。

Objective

To assess Hantaan virus infection risk across districts in Sichuan Province, providing a scientific basis for prevention and intervention measures for Hemorrhagic Fever with Renal Syndrome (HFRS) and data for evaluating their effectiveness.

Methods

This study utilized open-source databases and mathematical modeling. HFRS case report data were collected to analyze epidemic status and spatial distribution using traditional epidemiological methods, while a Bayesian spatiotemporal model examined temporal effects, spatial effects, spatiotemporal interactions, and the impact of meteorological, socioeconomic, and healthcare factors on regional risk levels.

Results

From 2015 to 2021, HFRS incidence in Sichuan showed temporal variation and seasonality, with Yanyuan County reporting the highest cases. Spatial analysis indicated that most districts had risk values consistent with the provincial average, with high-risk areas in the southern, northeastern and central regions. Relative risk declined overall from 2015 to 2020, with a 41.7% decrease in 2020 compared to 2015, although a slight increase occurred in 2021 (0.22 per 100 000). Yanyuan County had the highest relative risk (2.66 [1.64, 4.57]), while other regions aligned with the provincial average. Urbanization rate and humidity affected HFRS incidence negatively, and precipitation affected HFRS incidence positively.

Conclusion

HFRS incidence in Sichuan Province is generally low, with higher rates in specific districts, highlighting spatial risk variability. Meteorological, socioeconomic, and healthcare factors influence infection risk. Monitoring relevant indicators and implementing timely interventions are essential to prevent increased human infection risk.

肾综合征出血热(HFRS),也称流行性出血热(EHF),是由汉坦病毒感染引起的自然疫源性疾病[1]。HFRS在亚欧大陆广泛流行,中国病例最为严重,约占全球的90%,成为一个重要的公共卫生问题[2-3]。该病的传染源主要是感染汉坦病毒的啮齿类动物,人类通过气溶胶或直接接触带毒宿主的排泄物、污染食物或水而感染[4]。我国主要宿主包括黑线姬鼠、褐家鼠和小家鼠,这些动物适应性强、分布广,与人类接触密切,是HFRS持续流行的原因之一[3,5]。感染汉坦病毒后,患者常表现出“三痛、三红”等症状,严重时可导致休克、意识障碍和肾衰竭[6-7],目前尚无特效治疗,仅能进行对症治疗,因此研究感染风险尤为重要。此外,传染病的传播受自然和社会因素的影响,本研究将气象、社会经济及医疗卫生等因素综合考虑,分析四川省人群感染HFRS的相对风险,为疫情防控提供参考。
HFRS病例报告数据来源于中国疾病预防控制信息系统子系统传染病报告信息管理系统,从四川省疾病预防控制中心获得,涵盖2015—2021年;气象资料取自欧盟及欧洲中期天气预报中心的ERA5-Land数据集,处理为年度数据;四川省社会经济来自四川省统计年鉴,卫生数据来自于四川卫生健康统计年鉴;可视化结果使用的四川省行政区矢量图则来自国家基础地理信息中心网站。
通过2015—2021年四川省HFRS病例数据,对HFRS发病总体流行情况及三间分布进行概述,对影响因素与报告病例之间的相关关系进行探索,同时排除共线性问题。
为避免因指标间量纲及数值差异对模型准确性造成影响,本研究采用Z-score标准化法对纳入分析的指标进行处理,以统一数据量纲,便于后续建模分析。
贝叶斯方法将未知信息视为随机变量,并以概率形式表达。该方法涉及先验和后验信息,前者基于历史数据的理解,后者通过整合先验知识和样本数据推导新分布[8],可用于研究疾病发生与传播风险,并全面分析时空、环境和社会经济因素对疾病风险的影响[9],其具有适应性强、在样本量小的情况下仍能提供可靠结果,以及有效处理数据不确定性和噪声等优点。在本研究中,依据贝叶斯时空模型公式,逐步纳入空间项、时间项和时空交互项,构建不同贝叶斯模型,选择最优模型进行相对风险估计及影响因素分析,最终选定Model 5进行后续分析,参数设置参考既往研究及默认值,详见表1
φi,t代表区县i与全省HFRS的相对风险水平,α0为截距项,代表总体风险水平;xi,t,n为在t时刻对区县i的相对风险影响的第n个指标;βn是对应影响因素回归系数,量化其素对HFRS风险效应;μi为区县i的空间结构效应,反映不同区县之间的相关风险效应,服从条件自回归分布;νi为空间非结构效应,服从独立同分布。空间结构与非结构效应采用Besag-York-Mollié(BYM)模型来估计区域发病风险,该区域发病风险的RR表达为exp(μii)。γt为时间结构效应,采用二阶随机游走反映时间趋势,ηt为时间非结构效应。RR即本文相对风险,其表示为在不同影响因素作用下,该地区该年人群相对于全省平均风险,增加或减少的感染风险。
数据预处理使用Excel完成;描述性分析和绘图采用Origin,发病率空间可视化和空间聚集性分析在ArcGIS 10.7进行。此外,本研究利用R 4.2.0中的INLA包构建和分析贝叶斯时空模型,同时使用ggspatial和ggplot 2等包绘制HFRS相对风险值地图,实现风险可视化。
在研究期间,四川省共报告HFRS病例1 082例,发病情况呈波动趋势。空间分布上,病例主要集中在四川东部和南部,其中盐源县的年均发病率最高。整体来看,HFRS报告病例数和发病率先升后降,季节性特征明显。2017、2018和2021年,盐源县及全省整体在春季(3—6月)和冬季(10—12月)报告病例增多,其余月份病例较少,见图12
通过Spearman秩相关性分析,大部分影响因素的相关系数均小于0.8,表明不存在共线性。在考虑相关系数并排除共线性后,纳入后续分析的变量包括降水量、平均气温、相对湿度、城市化率、每千人口卫生技术人员数和归一化植被指数,见表2
由于2015—2021年HFRS发病数据中零值较多,因此考虑建立零膨胀模型。通过HFRS报告发病数据建立两种不同模型,并对比其DIC值以判断拟合效果。结果显示,零膨胀泊松时空模型的DIC=1 227.74,低于零膨胀负二项模型的1 271.56,因此最优模型为零膨胀泊松时空交互模型,详见表3
通过空间效应分析,大多数区县的HFRS相对风险接近平均水平。相对风险值较高的区县主要集中在南部和东北部,中部也有高风险地区,如凉山彝族自治州的盐源县和木里县。时间效应方面,研究期间HFRS的相对风险值总体呈下降趋势,但各年均高于全省平均水平。具体变化趋势为2015—2018年相对风险值下降,2019、2020年略有上升,2021年再次下降,详见表45
在时空交互效应方面,相对风险值大于1的地区主要包括凉山彝族自治州、广元市及南充市的部分区县,而甘孜、阿坝等地区的相对风险值则小于1。在考虑交互效应时,同一年不同发病地区的相对风险值存在差异,且同一地区不同年份的相对风险值也会变化。相对风险值最高的地区为凉山州的盐源县,2018年相对风险值为2.66(95% CI:1.64~4.57),而2019年降至0.48(0.28~0.80),低于1。其余地区各年份的HFRS发病风险均与平均风险持平,详见图3
基于贝叶斯时空交互模型分析影响因素对四川省HFRS人群感染风险的影响,结果显示城市化率、湿度和降水对HFRS有统计学意义。回归系数均值分别为-0.69、-0.37和0.28,相应的相对风险值为0.50(0.31~0.80)、0.69(0.52~0.90)和1.32(1.03~1.68)。其中,降水的正向效应显著,每增加一个单位,感染风险增至原来的1.32倍;而城市化率和湿度则呈负影响,增加一个单位时,HFRS的相对风险分别减少50%和31%。其他变量未表现出统计学意义,详见表6
研究期间四川省HFRS的年发病率在0.11/10万~0.35/10万之间,呈现出先上升再下降的趋势。同时在整个研究期间,盐源县的发病数及发病率(10/10万~71.75/10万)远高于其余地区以及四川省平均水平。因此,应该加强对部分地区的重视,分析呈现高发的原因,从而控制并降低该地区的发病率。
四川省HFRS发病率在2015—2016年波动平稳,2018年迅速上升并达到高峰后逐渐下降,具有一定季节性。研究表明,家鼠型病例主要在春夏流行,而姬鼠型病例则在秋冬流行[10],盐源县为混合型疫区,呈现出较明显双峰分布,这与国内其他地区的HFRS季节性特征一致[11-12]。其他地区的季节性不明显,但整体在10月后出现上升趋势,可能因农忙季节导致田间劳作增加与宿主接触机会增多,从而提高感染率。空间分布上,四川省HFRS分布不均,高发地区主要集中在南部,这与该地区亚热带半湿润气候有关,气温和湿度有利于鼠类繁殖,从而增加感染风险[13-15]
模型分析显示,时空效应下仅个别地区在不同年份有较大差异,其余地区HFRS相对风险与平均风险持平,呈现低流行。盐源县的风险值在2017和2018年均大于1,但2019年降至1以下,这与邓明菊等人研究的趋势一致[5],风险值下降可能与2018年病例急剧增加后盐源县开展的第二轮HFRS疫苗接种有关,重点人群接种率达到89%[13]。HFRS疫苗接种通常针对16至60岁人群,但研究发现低于16岁和高于60岁的人群仍占一定比例,这类人群免疫力较低,易受感染,因此调整疫苗接种年龄范围可能有助于降低高风险及其他地区HFRS病例的发生。
本研究发现,降水、湿度和城市化率对HFRS发生有一定影响,这与其他研究一致[16-18]。降水增加感染风险,因为适宜的降雨有利于植被生长,为宿主动物提供良好的生存环境和足够的食物,导致密度升高,从而增加病毒传播风险,此外过多降水可能引发洪涝,宿主尸体污染水源,促使疾病传播。因此,降水较多时应警惕HFRS发生,并进行预防宣传和水源保护。城市化率与HFRS风险呈负相关,可能因为城市化反映了地区经济水平,随着城市化发展,居民生活条件改善,科学除鼠方法得以推广,同时公众对啮齿动物和HFRS防治的认识提高。城市化还可能导致植被覆盖减少,从而降低HFRS发病率。湿度与感染风险呈负相关,因高湿度使空气中细小颗粒和气溶胶沉降更快,带病毒的气溶胶在空气中停留时间缩短,且高湿度可能影响病原体在环境中的存活能力,降低宿主动物和人群的感染风险[12]图4)。以相对风险较高的盐源县为例,盐源县属亚热带季风气候,具备降水丰富、雨量集中、植被覆盖丰富和温差较小等特点,且因其地势平坦适合农业种植,为宿主动物提供丰富食物来源且平坦地势易积水泛滥,使得宿主动物密度增加从而提高了病毒传播风险,因此应加强居住地排水系统建设,防止降水过多时积水泛滥创造鼠类栖息和传播的条件;调整农业种植模式,通过轮作、间作等方式避免单一作物种植导致鼠类栖息过于集中;定期开展鼠类监测以了解鼠类的分布和密度变化,在农田、乡村和城市边缘区等栖息地进行精准的灭鼠工作。同时其城市化率相对较低,公共卫生基础设施相对薄弱,鼠类防治和疾病控制的工作存在一定困难,且当地农民因防范意识不足,储粮方式大多为露天堆放或简易储存间储存,使得宿主动物及其排泄物等与人类接触机会增加[5],因此应有针对性对盐源县当地群众进行健康教育,提倡更为科学合理的储粮方式,同时在雨季来临之前,通过媒体、社区等渠道向居民和工作场所传播防控知识,增强公众防范意识。
综上所述,四川省HFRS发病整体呈低流行趋势,但发病风险存在空间差异,部分高风险地区仍需关注。日常监测中,应特别关注相对风险较高以及近几年发病率较高的地区,并加强疫情监控,对于这些地区,应重点推进疫苗接种工作,并根据区域特点采取针对性防控措施。例如,在降水较多的地区,特别是雨季时,应加强水源保护和鼠类防治;而城市化率较低的地区,应加大公共卫生基础设施建设,提升疾病防控能力。在相对风险和发病率较低的地区,应做好常规的鼠类监控和防治工作,并利用自然湿度的抑制作用,进一步加强环境卫生管理和公众防护意识。同时,应建立结合气象数据、鼠类密度和疫情的联合监测预警体系,实时追踪气候变化和鼠类密度波动,预测疫情高风险期,提前发布预警信息。通过及时的预防措施,如开展宣传教育和灭鼠行动,可以有效降低人群接触带病宿主的感染风险。本研究的局限性在于缺乏宿主生物学信息、疫苗接种等干预策略数据,以及区域间带毒外溢信息,无法建立全面影响因素模型,限制了动态研究的开展。此外,由于四川省HFRS发病率较低,存在较多零值,可能影响模型估计并增加误差。因此,后续研究应深入探讨宿主动物对病毒的易感性、病毒在宿主间的传播及外溢情况,以及人群免疫力变化对风险的影响,以完善流行风险评估。
