Objective To investigate the effects of spicy diet on male semen quality. Methods This study recruited male volunteers from Chongqing and Puyang, analyzed the impact of spicy diets on male semen quality, and further conducted a stratified analysis by region to verify the stability of the conclusions. Logistic regression evaluated spicy diet’s association with abnormal semen quality (normal/abnormal classification). Results A total of 2 128 subjects were included in the study (1 394 from Puyang and 779 from Chongqing). Linear mixed-effects model analysis revealed that duration of chili consumption was positively correlated with semen volume(β=0.05, 95% CI: 0.01-0.09, P=0.012), sperm concentration (β=1.64, 95% CI:0.35-2.93, P=0.013), total sperm count (β=6.89, 95% CI: 0.89-12.89, P=0.024), progressive motility (β=1.22, 95% CI: 0.75-1.69, P<0.001), VCL (β=0.73, 95% CI: 0.36-1.11, P<0.001), BCF (β=0.24, 95% CI: 0.15-0.32, P<0.001), STR (β=1.31, 95%CI: 0.73-1.90, P<0.001), WOB (β=0.91, 95% CI: 0.22-1.61, P=0.010), and normal morphology rate (β=0.34, 95% CI: 0.18-0.50, P<0.001), while negatively correlated with head defect rate (β=-1.11, 95% CI: -1.54~-0.67, P<0.001) and neck defect rate (β=-1.20, 95% CI: -1.62~-0.77, P<0.001). Weekly frequency of chili consumption showed positive correlations with sperm concentration (β=2.48, 95% CI: 0.05-4.92, P=0.046) and progressive motility (β=1.06, 95% CI: 0.18-1.94, P=0.019). Monthly frequency of chili consumption was positively associated with VSL (β=0.82, 95% CI: 0.32-1.31, P=0.001) and VAP (β=0.85, 95% CI: 0.25-1.46, P=0.006). Both duration and weekly frequency of chili consumption exhibited dose-response protective effects on semen quality, showing progressively decreasing risks of abnormal semen quality with increasing consumption duration and frequency (Ptrend<0.001 and 0.042, respectively). Those who had maintained a spicy diet for more than 3 years had a reduced risk of abnormal semen quality. Conclusion Spicy diet is associated with improved semen quality, with long-term consumption potentially reducing abnormality risk.

目的 探索辛辣饮食对男性精液质量的影响。方法 以重庆地区和濮阳地区作为研究现场,招募男性志愿者进行调查研究。分析辛辣饮食对男性精液质量的影响,并进一步按地区进行分层分析以验证结论的稳定性;将精液质量分为“正常”或“异常”,应用logistic回归模型进一步分析辛辣饮食与精液质量损伤间的关系。结果 纳入研究对象2 128例,其中濮阳1 394例,重庆779例。线性混合效应模型分析结果表明,食辣持续时间与精液量(β=0.05,95%CI:0.01~0.09,P=0.012)、精子浓度(β=1.64,95%CI:0.35~2.93,P=0.013)、精子总数(β=6.89,95%CI:0.89~12.89,P=0.024)、前向运动率(β=1.22,95%CI:0.75~1.69,P<0.001)、曲线速度(β=0.73,95%CI:0.36~1.11,P<0.001)、搏动交叉频率(β=0.24,95%CI:0.15~0.32,P<0.001)、直线度(β=1.31,95%CI:0.73~1.90,P<0.001)、摆动(β=0.91,95%CI:0.22~1.61,P=0.010)、正常形态率(β=0.34,95%CI:0.18~0.50,P<0.001)呈正相关,与头部缺陷率(β=-1.11,95%CI:-1.54~-0.67,P<0.001)、颈部缺陷率(β=-1.20,95%CI:-1.62~-0.77,P<0.001)呈负相关。食辣周频次与精子浓度(β=2.48,95%CI:0.05~4.92,P=0.046)、前向运动率(β=1.06,95%CI:0.18~1.94,P=0.019)呈正相关。食辣月频次与直线速度(β=0.82,95%CI:0.32~1.31,P=0.001)、平均路径速度(β=0.85,95%CI:0.25~1.46,P=0.006)呈正相关。食辣持续时间和周频次对精液质量的保护作用呈现剂量-反应关系,随着食辣持续时间和周频次的增加,精液质量异常风险逐渐降低(Ptrend分别为<0.001和0.042)。食辣持续时间大于三年者其精液质量异常风险降低。结论 辛辣饮食与精液质量有关,长期保持辛辣饮食可能降低精液质量异常风险。

Objective To investigate the effects of spicy diet on male semen quality. Methods This study recruited male volunteers from Chongqing and Puyang, analyzed the impact of spicy diets on male semen quality, and further conducted a stratified analysis by region to verify the stability of the conclusions. Logistic regression evaluated spicy diet’s association with abnormal semen quality (normal/abnormal classification). Results A total of 2 128 subjects were included in the study (1 394 from Puyang and 779 from Chongqing). Linear mixed-effects model analysis revealed that duration of chili consumption was positively correlated with semen volume(β=0.05, 95% CI: 0.01-0.09, P=0.012), sperm concentration (β=1.64, 95% CI:0.35-2.93, P=0.013), total sperm count (β=6.89, 95% CI: 0.89-12.89, P=0.024), progressive motility (β=1.22, 95% CI: 0.75-1.69, P<0.001), VCL (β=0.73, 95% CI: 0.36-1.11, P<0.001), BCF (β=0.24, 95% CI: 0.15-0.32, P<0.001), STR (β=1.31, 95%CI: 0.73-1.90, P<0.001), WOB (β=0.91, 95% CI: 0.22-1.61, P=0.010), and normal morphology rate (β=0.34, 95% CI: 0.18-0.50, P<0.001), while negatively correlated with head defect rate (β=-1.11, 95% CI: -1.54~-0.67, P<0.001) and neck defect rate (β=-1.20, 95% CI: -1.62~-0.77, P<0.001). Weekly frequency of chili consumption showed positive correlations with sperm concentration (β=2.48, 95% CI: 0.05-4.92, P=0.046) and progressive motility (β=1.06, 95% CI: 0.18-1.94, P=0.019). Monthly frequency of chili consumption was positively associated with VSL (β=0.82, 95% CI: 0.32-1.31, P=0.001) and VAP (β=0.85, 95% CI: 0.25-1.46, P=0.006). Both duration and weekly frequency of chili consumption exhibited dose-response protective effects on semen quality, showing progressively decreasing risks of abnormal semen quality with increasing consumption duration and frequency (Ptrend<0.001 and 0.042, respectively). Those who had maintained a spicy diet for more than 3 years had a reduced risk of abnormal semen quality. Conclusion Spicy diet is associated with improved semen quality, with long-term consumption potentially reducing abnormality risk.

近年来,精子数量一直以平均每年超过1.0%的速度降低[1]。随着饮食模式发生改变,不同饮食模式对精液质量的影响备受关注[2-3]。研究发现,辣椒的食用与全因死亡率呈负相关,对癌症、呼吸系统和心血管疾病等的发生具有保护作用[4],可能因为辣椒含有辣椒素、辣椒素样物质以及抗氧化剂维生素C[5-6]。辣椒素样物质是辣椒素受体(辣椒素TRPV1)的潜在激动剂,具有清除自由基和抑制过度活跃过氧化物酶系统的作用[7]。精子中含有大量TRPV1[8],提示辣椒的食用可能对精液质量有影响。本研究选择辛辣饮食较多的重庆地区和辛辣饮食较少的濮阳地区作为研究现场,探索辛辣饮食对男性精液质量的影响。
本研究以重庆地区和濮阳地区作为研究现场,招募男性志愿者进行问卷调查,并采集精液进行检测。纳入标准:(1)18岁≤年龄≤60岁;(2)禁欲2~7 d。排除标准:有睾丸损伤史、泌尿科诊断的泌尿生殖系统炎症史、不完全睾丸脱位史、附睾炎史、精索静脉曲张治疗史。本研究得到新乡医学院伦理审查委员会批准,伦理审查批号为XYLL—20170311,且在所有志愿者签署知情同意书后进行调查。
问卷调查内容包括一般人口学特征、吸烟、饮酒、辛辣食物食用等饮食习惯。辛辣食物是指含有新鲜辣椒和/或干辣椒和/或辣椒添加剂的食物。辛辣饮食习惯调查内容包括饮食辣度、食辣持续时间、周频次、月频次。饮食辣度是指一般喜好的食物辣度,用受试者感受来描述,分为:无辣、微辣、中辣、重辣;食辣持续时间是指开始规律地食用辣味食物到现在,总共有多长时间;周频次是指最近三个月每周食用辣味食物(包括辣味零食)的频次;月频次是指最近三个月每月食用以辣为特色的地方菜(如火锅、麻辣烫、烧烤、泡椒等)的频次。体格检查测量了研究对象的身高和体重。
本研究通过手淫法采集精液样本。采集后,样本置于37℃恒温箱直至液化,液化后立即进行精液参数检测。
采用精子质量检测分析仪cft-9201(江苏瑞奇生命科学有限公司)测量精液参数,包括精液量(ml)、精子浓度(百万/ml)、总精子数(百万)、前向运动率(%)、非前向运动率(%)、总精子运动率(%)及运动参数。运动参数包括曲线速度(curvilinear velocity,μm/s)、直线速度(straight-line velocity,μm/s)、平均路径速度(average path velocity,VAP,μm/s)、侧摆幅度(amplitude of lateral head displacement,μm)、摆动(wobble,%)、线性(linearity,%)、直线度(straightness,%)、搏动交叉频率(beat-cross frequency,BCF,Hz)。精子形态检测采用Diff-Quik法染色后镜检。
利用SPSS 22.0软件对数据进行整理和分析,分类变量资料采用计数和构成比进行统计描述;计量资料使用()或[MP25P75)]进行描述。利用t检验或χ2检验对不同特征研究对象的辛辣饮食习惯及精液质量行差异性分析;利用方差分析对辛辣饮食与精液质量的关系进行单因素分析,并利用线性回归模型进行了趋势检验;采用线性混合效应模型进行辛辣饮食对精液质量影响的多因素分析,并进一步按地区进行分层分析以验证结论的稳定性。根据世界卫生组织颁布的《人类精液检查与处理实验室手册(第6版)》,将精液质量分为“正常”或“异常”,应用logistic回归模型分析辛辣饮食与精液质量损伤间的关系。检验水准α=0.05。
在濮阳招募志愿者1 349名,重庆招募志愿者779名,合计纳入研究对象2 128名。研究对象平均年龄(27.91±6.13)岁,身体质量指数(body mass index, BMI)为(24.10±3.95)kg/m2,43.61%的研究对象吸烟,54.95%的研究对象饮酒。
研究对象中有辛辣饮食习惯者1 894人,占比89.00%;无辛辣饮食者234人,占比11.00%。与无辛辣饮食者相比,有辛辣饮食者年龄略低,吸烟和饮酒者占比较高(P<0.001)。见表1
结果显示,随着辛辣饮食的辣度增加,精子浓度(Ptrend=0.001)和曲线速度(Ptrend=0.033)呈增加趋势;随着食辣周频次的增加,精子浓度(Ptrend=0.002)、精子总数(Ptrend=0.040)、前向运动率(Ptrend<0.001)、正常形态率(Ptrend=0.016)、曲线速度(Ptrend=0.002)和BCF(Ptrend=0.003)也增加;随着食辣月频次的增加,精子前向运动率(Ptrend=0.002)、曲线速度(Ptrend=0.001)、直线速度(Ptrend=0.001)、VAP(Ptrend=0.002)和BCF(Ptrend=0.023)也有增加趋势;辛辣饮食持续时间与总精子数(Ptrend<0.001)、前向运动率(Ptrend=0.003)、正常形态率(Ptrend<0.001)、曲线速度(Ptrend<0.001)、直线速度(Ptrend<0.001)、VAP(Ptrend<0.001)、线性(Ptrend<0.001)和摆动(Ptrend<0.001)呈正相关。见表2

按地区进行分层分析结果与未分层结果较相似。只是可能因重庆地区无辛辣饮食者较少,导致部分关联未发现统计学意义,重庆人群食辣持续时间与头部缺陷和颈部缺陷的关联分析P值分别为0.059和0.068,食辣周频次与精子浓度、侧摆幅度和颈部缺陷的关联分析P值分别为0.079、0.073和0.082。见表45
根据世界卫生组织第六版精液参数标准,将精液质量分为正常和异常进行分析。其中模型一是辛辣饮食降低精液质量异常的优势比,模型二是调整了年龄、BMI、吸烟和饮酒因素后,辛辣饮食降低精液质量异常的校正优势比。模型一结果表明,喜重辣者、食辣持续时间大于3年者与周频次2~4次、5~7次和≥15次者的精液质量异常风险均降低。模型二结果表明,喜微辣、中辣、重辣与无辣相比,精液质量异常的风险分别降至0.70(95%CI:0.50~0.98)、0.68(95%CI:0.46~0.99)和0.45(95%CI:0.23~0.85)(Ptrend=0.057);周频次2~4次、5~7次和≥15次与≤ 1次相比,精液质量异常的风险分别降至0.71(95%CI:0.53~0.96)、0.69(95%CI:0.49~0.96)和0.50(95%CI:0.32~0.81)(Ptrend= 0.042);持续时间≤5年、≤10年和>10年与不吃辣组相比,精液质量异常的风险分别降至0.61(95%CI:0.39~0.95)、0.50(95%CI:0.32~0.78)和0.54(95%CI:0.36~0.79),食辣持续时间对精液质量的保护作用呈较强的剂量-反应关系(Ptrend<0.001);月频次≥15次与≤ 1次相比,精液质量异常的风险降至0.55(95%CI:0.31~0.98)。模型一与模型二结果均显示食辣持续时间和周频次对精液质量的保护作用呈现剂量-反应关系,随着食辣持续时间和周频次的增加,精液质量异常风险逐渐降低(Ptrend分别为<0.001和0.042),食辣持续时间大于3年者其精液质量异常风险降低。见图1
男性生殖健康问题已成为重要公共卫生问题。肥胖[9]、吸烟[10]、饮酒[11]、饮食[12]和辐射暴露[13]等生活方式因素可能以不同的方式影响男性生殖健康。探索损害男性生殖健康的危险因素固然重要,另一方面寻求男性生殖健康的保护因素亦不可忽视。
辣椒是中国食用最广泛的食物之一,其在中国的产量和消费量约占世界的一半[14]。本研究结果显示,本研究中89.0%的男性经常食用辛辣食物,高于李立明团队[15]的调查结果(78.56%)。这可能是由于本研究纳入了以食辣闻名的重庆地区。
本研究发现辛辣饮食与精子浓度、精子总数、前向运动及其他运动性参数都有正向关联,且辛辣饮食可降低精液质量异常的检出。辣度、食辣持续时间、周频次和月频次都对精液质量有影响,喜重辣者、食辣持续时间大于3年者、周频次2~4次、5~7次和≥15次者以及月频次≥15次者的精液质量异常风险均降低。周频次与月频次结果都显示≥15次者可降低精液质量异常的检出。可能由于周频次与月频次相比分组更均匀,暴露量的同质性更高,更容易捕捉到真实关联,从而显示出周频次更明确的效果。同时月频次的调查因涉及较长时间周期,志愿者在回忆时可能出现模糊性,削弱真实暴露与精液参数间的关联强度。辣椒含有丰富的生物活性成分,如辣椒素、辣椒素样物质以及抗氧化剂维生素C[5-6],研究发现辣椒素样物质具有清除自由基和抑制过度活跃过氧化物酶系统的作用[7]。美国国家健康与营养检查调查(NHANES)III的一项队列结果[16]显示,食用辣椒的参与者的总死亡率为21.6%,而不食用辣椒的参与者的总死亡率为33.6%(绝对风险降低12%,相对危险度为0.64)。精子中含有大量辣椒素受体。通过摄入辣椒素可以激活伤害感受器增强脾脏生发中心反应并提升抗流感免疫力[17]
本研究存在的局限性。一是,未对辣椒食用量进行定量统计。二是,辣度是研究对象自我感觉,可能存在个体间的差异。
综上所述,辛辣饮食与精液质量之间存在正相关关系,长期的辛辣饮食可以有效防止精液质量损伤。由于相关研究较少,需要进一步的研究来探索辣椒对男性生殖健康的影响。
