Objective To understand the incidence and mortality trends of uterine cancer in tumor registration areas of Shandong Province from 2013 to 2021, and to provide a basis for the prevention and control of uterine cancer. Methods Data were obtained from the Shandong Cancer Surveillance Database. Crude incidence (mortality) rates and age-standardized rates were calculated using Excel 2010 and R 4.4.2. Joinpoint regression was used to calculate the average annual percentage change (AAPC). Results From 2013 to 2021, the crude incidence rate and the Chinese age-standardized incidence rate of uterine cancer among women in tumor registration areas of Shandong Province increased by 4.08% and 2.85% per year, respectively (crude incidence rate AAPC=4.08%, 95% CI: 2.25%-5.95%, P<0.001; Chinese age-standardized incidence rate AAPC=2.85%, 95% CI: 1.02%-4.72%; All P=0.002). Although there was a decrease in crude mortality rate and Chinese age-standardized mortality rate, the difference was not statistically significant (crude mortality rate AAPC=-1.33%, 95% CI: -4.28%-1.71%, P=0.389; Chinese age-standardized mortality rate AAPC=-2.53%, 95% CI: -5.86%-0.91%, P=0.165). The urban incidence rate and mortality were higher than those in rural areas(χ2=513.53, χ2=64.27, both P<0.01). Both the Chinese age-standardized incidence rate in urban areas and the crude incidence rate in rural areas showed upward trends, with annual increases of 3.05% and 3.90%, respectively. Conclusion From 2013 to 2021, uterine cancer among female residents in Shandong Province has shown an upward trend and is a malignant tumor that women should focus on preventing. Early diagnosis and treatment of high-risk groups for uterine cancer should be emphasized, and etiological exploration and research should be strengthened to provide reference for the prevention and treatment of uterine cancer.

目的 了解2013—2021年山东省肿瘤登记地区子宫癌发病和死亡趋势,为防控子宫癌提供依据。方法 数据来源于山东省肿瘤监测数据库,用Excel 2010和R 4.4.2计算发病(死亡)率、标化率等,采用joinpoint计算平均年度变化百分比(average annual percentage change,AAPC)。结果 2013—2021年山东省肿瘤登记地区女性子宫癌发病率和中标发病率平均每年分别上升4.08%、2.85%(发病率AAPC=4.08%,95%CI:2.25%~5.95%,P<0.001;中标发病率AAPC= 2.85%,95%CI:1.02%~4.72%;均P=0.002);死亡率和中标死亡率虽有下降但差异均无统计学意义(死亡率AAPC=-1.33%,95%CI:-4.28%~1.71%,P=0.389;中标死亡率AAPC=-2.53%,95%CI:-5.86%~0.91%,P=0.165)。城市发病率和死亡率均高于农村(χ2=513.53、64.27,均P<0.01),城市中标发病率和农村发病率呈上升趋势,分别每年上升3.05%和3.90%。结论 2013—2021山东省女性居民子宫癌呈上升趋势,是女性重点预防的恶性肿瘤,应重视子宫癌高危人群的早诊早治,同时加强病因学探索研究,为子宫癌防治工作提供参考依据。

目的 了解2013—2021年山东省肿瘤登记地区子宫癌发病和死亡趋势,为防控子宫癌提供依据。方法 数据来源于山东省肿瘤监测数据库,用Excel 2010和R 4.4.2计算发病(死亡)率、标化率等,采用joinpoint计算平均年度变化百分比(average annual percentage change,AAPC)。结果 2013—2021年山东省肿瘤登记地区女性子宫癌发病率和中标发病率平均每年分别上升4.08%、2.85%(发病率AAPC=4.08%,95%CI:2.25%~5.95%,P<0.001;中标发病率AAPC= 2.85%,95%CI:1.02%~4.72%;均P=0.002);死亡率和中标死亡率虽有下降但差异均无统计学意义(死亡率AAPC=-1.33%,95%CI:-4.28%~1.71%,P=0.389;中标死亡率AAPC=-2.53%,95%CI:-5.86%~0.91%,P=0.165)。城市发病率和死亡率均高于农村(χ2=513.53、64.27,均P<0.01),城市中标发病率和农村发病率呈上升趋势,分别每年上升3.05%和3.90%。结论 2013—2021山东省女性居民子宫癌呈上升趋势,是女性重点预防的恶性肿瘤,应重视子宫癌高危人群的早诊早治,同时加强病因学探索研究,为子宫癌防治工作提供参考依据。

Objective To understand the incidence and mortality trends of uterine cancer in tumor registration areas of Shandong Province from 2013 to 2021, and to provide a basis for the prevention and control of uterine cancer. Methods Data were obtained from the Shandong Cancer Surveillance Database. Crude incidence (mortality) rates and age-standardized rates were calculated using Excel 2010 and R 4.4.2. Joinpoint regression was used to calculate the average annual percentage change (AAPC). Results From 2013 to 2021, the crude incidence rate and the Chinese age-standardized incidence rate of uterine cancer among women in tumor registration areas of Shandong Province increased by 4.08% and 2.85% per year, respectively (crude incidence rate AAPC=4.08%, 95% CI: 2.25%-5.95%, P<0.001; Chinese age-standardized incidence rate AAPC=2.85%, 95% CI: 1.02%-4.72%; All P=0.002). Although there was a decrease in crude mortality rate and Chinese age-standardized mortality rate, the difference was not statistically significant (crude mortality rate AAPC=-1.33%, 95% CI: -4.28%-1.71%, P=0.389; Chinese age-standardized mortality rate AAPC=-2.53%, 95% CI: -5.86%-0.91%, P=0.165). The urban incidence rate and mortality were higher than those in rural areas(χ2=513.53, χ2=64.27, both P<0.01). Both the Chinese age-standardized incidence rate in urban areas and the crude incidence rate in rural areas showed upward trends, with annual increases of 3.05% and 3.90%, respectively. Conclusion From 2013 to 2021, uterine cancer among female residents in Shandong Province has shown an upward trend and is a malignant tumor that women should focus on preventing. Early diagnosis and treatment of high-risk groups for uterine cancer should be emphasized, and etiological exploration and research should be strengthened to provide reference for the prevention and treatment of uterine cancer.

子宫癌是女性常见的恶性肿瘤,2022年全球子宫癌新发42.02万例,占全球癌症的2.1%,死亡9.77万例,死亡占全球癌症的1%[1];2022年中国子宫癌发病7.77万例,发病中标率为7.03/10万,子宫癌死亡1.35万例,死亡中标率为1.06/10万[2]。了解子宫癌发病和死亡分布特征及变化趋势,对于子宫癌防治具有重要意义。本研究采用joinpoint回归对2013—2021年山东省肿瘤登记地区子宫癌发病和死亡情况进行分析。
2013—2021年肿瘤数据资料来源于山东省各肿瘤登记处总数据库,本研究纳入了23个农村点、10个城市点肿瘤登记处的数据。
肿瘤病例分类编码采用国际疾病分类第10版[3],提取编码为C54~C55的子宫癌病例资料进行分析,人口资料来源于统计年鉴。
数据的质量控制采用国际癌症研究中心/国际癌症登记协会[4-5]及《中国肿瘤登记工作指导手册(2016)》[6]相关评价标准。 2013—2021年女性子宫癌合计死亡发病比(M/I)为0.18~0.28,形态学诊断比(MV%)为88.32%~90.34%,仅有死亡医学证明书比例(DCO%)为 0.16%~0.19% 。
应用joinpoint回归计算平均年度变化百分比(average annual percentage change,AAPC)及其95%CI,对2013—2021年山东省子宫癌发病和死亡进行趋势分析。应用χ2检验进行率的比较,双侧检验,检验水准α=0.05[7]。Excel 2010和R 4.4.2计算子宫癌发病率(中标发病率)、死亡率(中标死亡率),中标率采用2010年全国第六次人口普查构成。世标率采用Segi世界标准人口年龄构成计算。
2021年山东省女性子宫癌的发病世标率为8.63/10万,中标发病率为8.24/10万,发病率为13.87/10万。2013—2021子宫癌发病率和中标发病率呈上升趋势,发病率AAPC=4.08%(95%CI:2.25%~5.95%,P<0.001);中标发病率AAPC=2.85%(95%CI:1.02%~4.72%,P=0.002)。
2021年女性子宫癌死亡率2.44/10万,中标死亡率1.33/10万,世标率1.19/10万;2013—2021年女性子宫癌死亡率和中标死亡率虽呈下降趋势,但差异无统计学意义(死亡率AAPC=-1.33%,95%CI:-4.28%~1.71%,P=0.389;中标死亡率AAPC=-2.53%,95%CI:-5.86%~0.91%,P= 0.165)。见表1
2021年城市子宫癌发病率为14.29/10万,农村发病率13.53/10万,城市发病率高于农村(χ2=513.53,P<0.01)。2013—2021年城市女性子宫癌发病率虽有上升但差异无统计学意义(AAPC=3.70, 95%CI:-0.94%~8.57%,P=0.120),中标发病率呈上升趋势(AAPC= 3.05%,95%CI:0.88%~5.26%,P=0.006);农村女性子宫癌发病率呈上升趋势(AAPC=3.90%,95%CI:2.63%~5.18%,P<0.001);中标发病率虽上升但差异无统计学意义(AAPC=2.48%,95%CI:-0.18%~5.21%,P=0.067)。
2021年城市子宫癌死亡率2.70/10万,农村死亡率2.23/10万,城市死亡率高于农村(χ2= 64.27,P<0.01)。城市居民女性子宫癌死亡率和中标死亡率虽有下降,但无统计学意义(死亡率AAPC=-0.52,95%CI:-7.68%~7.20%,P= 0.892;中标死亡率AAPC=-2.43,95%CI:-10.19%~6.01%,P=0.561);农村女性子宫癌死亡率和中标死亡率虽有下降,但无统计学意义(死亡率AAPC=-2.03%,95%CI:-7.27%~3.48%,P=0.462;中标死亡率AAPC=-4.40%,95%CI:-13.32%~5.43%,P=0.367)。见表2
山东省2013—2021年<20岁人群子宫癌发病率均较低,≥20岁人群子宫癌发病率随年龄增长呈上升趋势,其中55~59岁发病率最高。2013—2021年55~59岁、50~54岁、45~49岁人群发病呈上升趋势(AAPC=3.79%、4.33%、4.26%,均P<0.05),其他年龄组人群差异均无统计意义(均P>0.05)。见表3图1
山东省2013—2021年<30岁人群子宫癌死亡率均较低,≥30岁人群子宫癌死亡率随年龄增长呈上升趋势,70~74岁年龄组人群子宫癌死亡率最高,60~64岁、65~69岁年龄组人群死亡均呈下降趋势(AAPC=-5.69%、-2.18%,均P<0.05),其他年龄组人群死亡率差异均无统计意义(均P>0.05)。见表4图2
2021年山东省子宫癌发病率、中标发病率、世标率分别为13.87/10万、8.24/10万、8.63/10万;同期子宫癌死亡率、中标死亡率、世标率分别为2.44/10万、1.33/10万、1.19/10万,均高于2022年中国女性子宫癌(中标发病率7.03/10万、世标率6.84/10万,死亡中标率1.06/10万、世标率1.05/10万)[2]。高于2022年全球子宫癌发病标化率(8.4/10万),但低于2022年全球子宫癌死亡标化率(1.7/10万)[1]
2013—2021年山东省肿瘤登记地区女性子宫癌55~59岁年龄组发病率最高,≥20岁人群发病率随年龄增长呈上升趋势。《2021中国肿瘤登记年报》子宫癌50~54岁发病率最高[8];提示不论城市还是农村50~59岁年龄组是女性子宫癌重点防治人群。
2013—2021年山东省女性子宫癌70~74岁年龄组死亡率最高,≥30岁死亡率随年龄增长呈上升趋势,《2021中国肿瘤登记年报》显示子宫癌年龄别发病率30岁以后快速上升,山东省与全国年龄别死亡模式基本相同,山东省肿瘤登记地区城市女性子宫癌发病率高于农村,城市和农村居民女性子宫癌发病率、中标发病率均呈上升趋势,子宫癌死亡率及中标死亡率呈下降趋势。可能与近年来山东省大力实施“两癌筛查”癌症能够早期发现和治疗有关[39]
有研究提示,肥胖或超重、糖尿病、高血压是子宫癌重要的危险因素[10-13],加强高危人群健康教育,通过合理膳食和运动,降低和预防肥胖、超重、高血压等危险因素对于降低子宫癌的发生具有重要意义。《健康中国行动(2019-2030年)》提出倡导和推进文明健康生活方式,提升全民体重管理意识和技能,预防和控制超重肥胖,切实推动慢性病防治关口前移。2024年国家层面提出“体重管理年”活动,鼓励二级及以上综合医疗机构建设体重管理门诊和肥胖防治中心,开展人群营养干预、诊疗和宣传工作,这些政策的实施和落地为危险因素的降低提供了有力的保障。
综上所述,山东省女性子宫癌的发病率和死亡率仍然较高,防控形势依然严峻,除了推行防癌宣传教育,让高危人群关注和认识到子宫癌的危害;其次加强对女性生殖系统癌症的筛查,早期发现癌前病变,提高早期治疗率;加强人群的一级预防,积极结合“三减控三高行动”,改善人群不良饮食习惯,降低高血压、糖尿病等基础性疾病的发生,这将对子宫癌的防治有很大意义。
