Objective Aging is a common starting point for many diseases, and homeostatic dysregulation (HD) is an important intervention indicator of aging. This study investigates the association between Tibetan noodle consumption and HD. Methods Using baseline survey data from the China Multi-Ethnic Cohort study (CMEC), HD was constructed on the basis of clinical biomarkers and anthropometric indices. Dietary information was obtained from the Food Frequency Questionnaire (FFQ). Multivariable linear regression analysis was used to explore the association between Tibetan noodle consumption and HD.Stratified analyses were conducted by sex, age, and BMI to explore the heterogeneity of the association effect. Results A total of 4 485 study participants were included. Compared with non-consumption of Tibetan noodles, a weekly consumption frequency of more than 7 times was associated with lower HD (β=-0.304, 95% CI: -0.593~-0.015). Similarly, a weekly intake exceeding 1 050 grams of Tibetan noodles demonstrated reduced HD (β=-0.130, 95% CI: -0.245~-0.014). Additionally, sex might act as a potential effect modifier. Conclusion Tibetan noodle consumption was inversely associated with HD, suggesting that consumption of Tibetan noodles, a specialty diet, has potential value in improving HD and slowing down aging.

目的 衰老是多种疾病的共同起点,稳态失调(homeostatic dysregulation)是衰老的一个重要干预指标。探究食用藏面与稳态失调的关联。方法 利用中国西南区域自然人群队列研究(The China Multi-Ethnic Cohort study,CMEC)基线调查数据,基于临床生物标志物和人体测量指标构建稳态失调。通过食物频率问卷(food frequency questionnaire,FFQ)获得饮食信息。采用多因素线性回归分析探索食用藏面与稳态失调之间的关联。将性别、年龄及身体质量指数作为分层因素进行分层分析以探索关联效应的异质性。结果 共纳入4 485名研究对象。与不食用藏面相比,食用藏面>7次/周与稳态失调存在负向关联(β=-0.304,95%CI:-0.593 ~ -0.015),食用藏面克重>1 050g/周也与稳态失调存在负向关联(β=-0.130,95%CI:-0.245 ~ -0.014)。此外,性别可能是潜在的效应修饰因子。结论 食用藏面与稳态失调呈负向关联,提示特色饮食藏面的食用在改善稳态失调、减缓衰老方面具有潜在价值。

Objective Aging is a common starting point for many diseases, and homeostatic dysregulation (HD) is an important intervention indicator of aging. This study investigates the association between Tibetan noodle consumption and HD. Methods Using baseline survey data from the China Multi-Ethnic Cohort study (CMEC), HD was constructed on the basis of clinical biomarkers and anthropometric indices. Dietary information was obtained from the Food Frequency Questionnaire (FFQ). Multivariable linear regression analysis was used to explore the association between Tibetan noodle consumption and HD.Stratified analyses were conducted by sex, age, and BMI to explore the heterogeneity of the association effect. Results A total of 4 485 study participants were included. Compared with non-consumption of Tibetan noodles, a weekly consumption frequency of more than 7 times was associated with lower HD (β=-0.304, 95% CI: -0.593~-0.015). Similarly, a weekly intake exceeding 1 050 grams of Tibetan noodles demonstrated reduced HD (β=-0.130, 95% CI: -0.245~-0.014). Additionally, sex might act as a potential effect modifier. Conclusion Tibetan noodle consumption was inversely associated with HD, suggesting that consumption of Tibetan noodles, a specialty diet, has potential value in improving HD and slowing down aging.

在全球老龄化进程加快的背景下,减缓衰老与延长健康期望寿命已成为关键的研究问题。世界卫生组织(WHO)数据提示,预计从2020到2050年,60岁及以上老年人口将增加一倍(达到21亿),占世界总人口的22%[1]。这一老龄化趋势在西藏自治区(简称西藏)呈现出独特的区域差异。人口普查数据显示,在1982—2010年,西藏老年人口从13.89万人增长至23.04万人,年均增长率为1.82%,高于总人口年均增长率1.66%[2]。而在2010—2018年,西藏自治区65岁及以上老年人口占比从5.09%增至5.68%[3-4],增长速率反而相对平缓。这可能与西藏独特的高原地理环境(平均海拔>4 000m)和区别于中国其他民族和全球其他人群的特色饮食(主食藏面、糌粑、红肉、酥油茶和青稞茶等)有关[5]。低压低氧环境、高原特色膳食与衰老相关生理机制的复杂作用下,可能形成区别于其他地域的老龄化特征。稳态失调(homeostatic dysregulation)衡量了与衰老相关的多系统生理失调[6-10]。并可基于临床血生化指标构建,具有高可及性和高成本效益的优势,适于常规筛查。在保持一定衰老测量的准确性基础上,尤其适于医疗欠发达的高原地区和大规模人群研究[6]。为早期识别和干预衰老提供了有效的方法[6-7]
饮食对生物衰老过程中的影响尤为重要[8-9]。健康的饮食习惯有助于改善能量代谢、维持代谢平衡、减少氧化应激,并预防异常炎症反应,从而延缓细胞水平的衰老过程[10-12],改善个体的整体健康状况。西藏地区的特色食品如青稞富含抗氧化成分和丰富的营养物质,有助于降低胆固醇和延缓氧化应激,维持机体的稳态平衡[13-14]。藏面作为藏族传统饮食中的经典面食,是日常饮食中常见的主食之一。然而,目前关于藏面是否能够延缓生物衰老尚缺乏相关研究证据。探索藏面摄入与稳态失调之间的关联,有助于深入理解藏族传统饮食的健康效益,为民族特色饮食提供新的研究视角,并为区域性抗衰老策略提供科学依据。因此,本研究基于中国西南区域自然人群队列研究(The China Multi-Ethnic Cohort study,CMEC,简称西南队列)中西藏地区的基线调查数据,探索了拉萨藏族人群特色饮食藏面摄入与稳态失调之间的关联。
西南队列是一项基于社区的大规模前瞻性队列研究,包括中国西南部欠发达的五个省份(四川、云南、贵州、重庆和西藏),纳入了汉族和其他六个少数民族(藏族、彝族、苗族、白族、布依族和侗族)。基线调查于2018年5月—2019年9月进行,收集了99 556名15~86岁参与者的数据。西南队列研究采用多维度数据收集方法,主要包括:(1)问卷调查:收集人口统计学特征、社会经济状况、生活方式因素及健康相关史等信息;(2)医学检查:包括人体测量和体格检查等;(3)临床实验室检测:采集血液、唾液、尿液和粪便样本进行检测。所有程序均由四川大学伦理审查委员会(K2016038,K2020022)和各参与研究中心的当地伦理委员会批准,且从所有受试者处获得书面知情同意书。本研究纳入了基线调查中西藏拉萨监测点的研究对象(n=7 737),并排除了缺乏构建稳态失调指标信息的参与者(n=2 766)、缺乏藏面食用信息(n=12)及协变量信息(n=474)的人群,最终纳入4 485名研究对象。
藏面食用的相关信息通过问卷收集。藏面食用频率问题的回答选项包括次数/d、次数/周、次数/月和次数/年,每次食用量(熟重)按克计量。本研究将食用频率结果统一换算为次数/周(每天食用频率乘7,每月食用频率除以4,每年食用频率除以48),并将藏面食用频率视为等级变量,分为四组:不吃、0~3次/周、3~7次/周、>7次/周。通过每周食用频率与每次食用克重相乘获得每周食用克重,并按照25%和75%百分位数分为四组:不吃、0~150g/周、150~1 050g/周、>1 050g/周。
稳态失调通过测量多种分子或细胞生物标志物与其正常值的偏差来评估年龄相关的生理功能障碍[6]。稳态失调评分的大小与参考人群密切相关,参考人群应选择健康成人。本研究在99 556名参与基线调查的参与者中,选择年龄在30~39岁且自报无任何基础疾病的参与者作为参考人群。
构建稳态失调评分的过程如下:(1)本研究选择与实足年龄相关的临床生物标志物和人体测量值,通过Box-Cox转换以实现正态分布;(2)只保留与实足年龄显著相关的生物标志物(|r|>0.1);(3)在进一步排除可能的冗余标志物后,最终纳入了15项指标:收缩压、腰臀比、呼气峰流速、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、糖化血红蛋白、甘油三酯、天冬氨酸转氨酶、γ-谷氨酰转肽酶、白蛋白、碱性磷酸酶、肌酐、尿素、平均红细胞体积和血小板计数。
利用马氏距离来计算每个个体的指标与参考人群的距离得分,从而评估个体的生理功能与参考人群的偏离程度。公式如下:
x表示每个生物标志物的观察值,而μS表示基于参考人群计算的每个生物标志物的总体平均值和方差-协方差矩阵。对计算所得稳态失调评分进行对数转换。稳态失调评分表示个体生理失调相较于参考人群的正常或异常程度,评分越高表示个体生理失调程度越高,并可能经历了更大程度的生物衰老。
协变量数据来自基线问卷和体格检查数据。协变量选择基于既往文献[15]构建有向无环图,包括年龄(岁)、性别(女、男)、受教育程度(未正规上过学、小学、初中及高中、大专、大学及以上)、职业状况(农林牧渔劳动者、工人、家务、行政、服务人员等、离/退休)、家庭年收入(<1.2万元、1.2~1.99万元、2~5.99万元、6~9.99万元、10~20万元、>20万元)、吸烟状况(从不吸烟、戒烟、现在吸烟)、婚姻状态(已婚、未婚)、失眠状况(是、否)、抑郁症状(是、否)、焦虑症状(是、否)、饮料消费(不喝,曾经喝,现在喝)、心脏代谢性疾病家族史(有、无高血压、糖尿病)、总能量摄入(Kcal/周)、身体活动(METs-h/周)、膳食补充剂(是、否)、身体质量指数(body mass index,BMI, kg/m2)、酒精摄入状况。在饮食评估方面,本研究使用停止高血压饮食(dietary approaches to stop hypertension, DASH)评分来评估整体膳食质量,该评分在我们先前的研究中已报告了与稳态失调的负向关联,且能够降低心脏代谢风险[15-16]。另外,考虑到民族特色饮食对结果的可能影响,我们进一步对四种民族特色饮食进行了调整,包括糌粑(吃、不吃)、酥油茶(喝、不喝)、清茶(喝、不喝)、生牛肉(吃、不吃)。
本研究基于藏面食用频次对研究人群进行亚组划分,系统描述各组的基线特征。连续型变量采用中位数(第25百分位数,第75百分位数)进行统计描述,分类变量以频数(百分比)进行描述。采用多因素线性回归分析分别探索藏面食用频次、食用克重与稳态失调之间的关联,以不吃藏面的人群作为参考组。鉴于稳态失调指标呈现偏态分布特征,本研究对其进行对数转换并实施标准化处理,以满足线性回归的正态性假设。为探究潜在的效应修饰因子,本研究将性别、年龄及BMI作为分层因素,用异质性检验来识别关联效应是否有异质性。
为评估结果的稳健性,本研究进行了敏感性分析。排除了基线时自报患有慢性疾病(糖尿病、高血压、高脂血症、非酒精性脂肪肝)的参与者,再进行多因素回归分析,以减弱潜在的反向因果。双侧检验,检验水准α=0.05。本研究采用R4.4.1软件进行统计分析。
本研究共纳入4 485名研究对象,基线特征见表1。随着藏面食用频次的增加,研究对象呈现出以下特点:更年轻,男性比例增高,吸烟率上升,同时体力活动水平较为活跃。此外,随着受教育程度的增加,藏面食用频次逐渐减少,大专及以上学历群体的高频摄入比例明显低于高中及以下学历群体。值得注意的是,稳态失调随着藏面食用频次的增加呈现逐渐下降的趋势。
食用藏面与稳态失调的关联分析结果见表2。模型1为未调整协变量的模型;模型2调整了年龄和性别;模型3调整了其他重要的协变量,包括年龄、性别、受教育程度、职业状况、年收入、吸烟状况、婚姻状况、睡眠状况、抑郁症状、焦虑症状、饮料摄入、心脏代谢性疾病家族史、总能量摄入、身体活动、膳食补充剂、BMI、DASH膳食模式得分、食用四种特色饮食(糌粑、酥油茶、清茶、生牛肉)情况、酒精摄入状况。在模型1、模型2和模型3中,食用藏面与稳态失调均呈负向关联,在模型3中,食用频次为3~7次/周(β=-0.105, P<0.05)和>7次/周(β=-0.304, P<0.05)与稳态失调的负向关联均具有统计学意义。
食用藏面克重与稳态失调的关联分析结果见表3。在模型1、模型2和模型3中,食用藏面克重与稳态失调均呈负向关联,在模型3中,食用克重>1 050g/周(β=-0.130, P<0.05)与稳态失调的负向关联具有统计学意义。
本研究根据性别、年龄及BMI进行了分层分析,在不同分组中食用藏面与稳态失调之间的关联大小和方向与主要分析基本一致,结果见表4。总的来说,食用藏面与稳态失调呈负向关联,且随着每周食用频次的增加,关联强度逐渐增大。在男性分层中,每个频次分组与稳态失调的关联均具有统计学意义。
排除基线自报患有慢性疾病(糖尿病、高血压、高脂血症、非酒精性脂肪肝)的参与者后,食用藏面频率、克重与稳态失调之间的负向关联均表现出稳健性。每周食用藏面3~7次(β=-0.126,P<0.05)和>7次(β=-0.390, P<0.05)与稳态失调的负向关联均具有统计学意义;每周食用藏面150~1 050 g(β=-0.145,P<0.05)和>1 050 g(β=-0.187,P<0.05)与稳态失调的负向关联也具有统计学意义。
本研究基于西南队列基线调查的西藏子样本,探讨了食用藏面与稳态失调之间的关联,结果提示食用藏面与稳态失调之间呈负向关联,且关联强度随着藏面食用频次和食用量的增加而增强。此外,分层分析的结果提示性别可能是潜在的效应修饰因子。
本研究结果表明,相比于不食用藏面的人群,食用藏面与衰老呈负相关。藏面主要由高海拔种植小麦制作而成,富含多种营养素,如膳食纤维、碳水化合物、酚酸、类黄酮、类胡萝卜素和生育酚等[17],为机体生理活动正常运行提供必要的能量和物质基础,具有保护健康的作用。相关研究表明富含碳水化合物的饮食与减缓衰老和改善大脑健康有关,表现为表型年龄的降低和大脑灰质体积的增加[18];膳食纤维的摄入也对衰老具有潜在的保护作用[19],大量膳食纤维的摄入相较于低水平的摄入量表现出更长的端粒长度和更少的生物衰老[20];酚酸对于衰老相关疾病具有益效应,其抗糖尿病作用已被广泛认可[21],有助于控制血糖水平、胰岛素敏感性和高胰岛素血症[22]。此外,小麦中的各种植物化学物对心血管疾病也表现出保护作用,改善高血脂患者的氧化应激和炎症状态[23]。
然而,也有研究提示食用藏面与机体健康状态之间并无关联[24],与本研究结果之间的差异提示藏面可能存在特定饮食文化背景下的健康保护作用。西藏地区具有特殊的高原气候,为了适应这种气候,西藏人民有着独特的地域饮食特色[25],研究表明藏族人群经常食用的食物约有25%是传统的藏族食物,如糌粑、清茶、酥油茶[26]。由富含β-葡聚糖的烤西藏大麦制成的糌粑[27-28]可能预防内脏脂肪性肥胖[29],也与降低糖尿病的患病率有关[30];清茶中的多酚和矿物质也发挥着有益于人体健康的抗氧化和细胞保护作用[31],提示藏面与西藏其他特色饮食之间可能存在的协同效应。
本研究还发现,食用藏面对衰老的有益作用在男性中更强。类似地,一项研究也提示藏族传统饮食的保护作用在男性中才具有统计学意义[32],意味着激素水平可能起作用[33]。这一结果提示在制定营养干预措施时应考虑性别差异的重要性。
本研究较为系统地收集了西藏特色饮食的相关信息,并首次揭示了西藏特色饮食藏面在调节衰老相关代谢稳态中的潜在价值,为藏族特色饮食文化的健康效应评估提供了新视角,有助于针对性营养干预措施的实施;并进行了敏感性分析保证研究结果的稳健性。然而,本研究也存在一些局限性。首先,关于藏面食用的相关信息是来访者自我报告的,可能存在回忆偏倚;其次,本研究为横断面设计的观察性研究,尽管已经尽可能地控制已测量混杂,未测量混杂和反向因果仍可能存在,因此,研究结果应谨慎解读;最后,样本数据仅包括基线调查的西藏子人群,样本量较小且结果外推存在限制。
综上,本研究探索了食用藏面与稳态失调之间的关联,提示二者之间存在的负向关联,揭示了特色饮食对于衰老相关稳态调节的潜在应用,有助于了解藏族传统饮食对健康的影响,为改善高海拔地区居民的饮食和营养状况提供科学依据,为建立基于传统饮食的高海拔营养干预方案提供理论支持。
