Objective To investigate the influencing factors of anemia and anemia exacerbation in people living with HIV/AIDS (PLWHA) during antiretroviral therapy (ART), as well as the impact of anemia on the prognosis of PLWHA. Methods This retrospective cohort study collected baseline and follow-up data for PLHIV who initiated ART at a specialized infectious-disease hospital in Guangzhou from February 10, 2004 to March 29, 2019. A generalized linear mixed-effects model was used to analyze the factors influencing anemia during long-term ART in PLWHA without anemia at baseline. The Cox proportional hazards model was used to analyze both the influencing factors of anemia exacerbation in those with baseline anemia and the impact of baseline anemia and follow-up anemia on the prognosis. Results There were 11 688 PLWHA, of whom 3 853(32.97%) had anemia at baseline. Among 7 835 PLWHA without anemia at baseline, 1 695 (21.63%) developed anemia during the follow-up. Among 3 470 PLWHA with mild or moderate anemia at baseline, 845 (24.35%) had anemia exacerbation during the follow-up. Risk factors for follow-up anemia in PLWHA included being female, aged ≥50 years at ART initiation, being married/cohabiting, being divorced/separated/widowed, blood transmission, baseline opportunistic infections, higher number of baseline clinical symptoms, the initial ART regimen containing Zidovudine (AZT), CD4 cell count <200 cells/μL, and abnormal serum creatinine. Risk factors for anemia exacerbation included being female, being married/cohabiting, being divorced/separated/widowed, an interval of ≥6 months from diagnosis to ART initiation, ≥2 categories of baseline clinical symptoms, an initial ART regimen containing AZT, and abnormal serum creatinine. The risk of death in PLWHA without anemia at baseline but developing anemia during the follow-up was 5.02 times (95% CI: 2.45-10.27) than that among those without anemia at both baseline and during the follow-up. The risk of death with anemia at both baseline and during the follow-up was 6.00 times (95%CI: 3.04-11.85) than that among those without anemia at both baseline and during the follow-up. Conclusion Follow-up anemia and anemia exacerbation during ART are affected by multiple factors. Baseline anemia and follow-up anemia are important risk factors for the prognosis of PLWHA. Anemia screening for high-risk groups should be strengthened to strive for early detection, early diagnosis, and early treatment, thereby reducing the risk of death of PLWHA.

目的 探讨HIV/AIDS患者在抗逆转录病毒治疗(antiretroviral therapy,ART)过程中贫血、贫血加重的影响因素,及贫血对患者预后的影响。方法 回顾性收集广州市某传染病专科医院2004年2月10日—2019年3月29日首次接受ART的HIV/AIDS患者基线和随访数据,采用广义线性混合效应模型分析基线无贫血患者长期ART过程中随访贫血的影响因素,采用Cox比例风险回归模型分析基线贫血患者贫血加重的影响因素及基线贫血和随访贫血对患者预后的影响。结果 HIV/AIDS患者11 688例,基线贫血3 853例(32.97%);7 835例基线无贫血患者在随访过程中发生贫血1 695例(21.63%),3 470例轻、中度贫血患者在随访中贫血加重845例(24.35%)。随访贫血的危险因素包括女性、开始ART年龄≥50岁、已婚/同居、离异/分居/丧偶、血液传播、基线机会性感染、基线临床症状种类数越多、初始ART方案含齐多夫定(Zidovudine,AZT)、CD4细胞计数<200个/μl、血肌酐异常。女性、已婚/同居、离异/分居/丧偶、确诊到开始ART间隔时间≥6个月、基线临床症状种类数≥2种、初始ART方案含AZT、血肌酐异常是贫血加重的危险因素。基线无贫血而随访过程中贫血患者的死亡风险是基线、随访过程中均无贫血者的5.02倍(95%CI:2.45~10.27);基线、随访过程中均贫血患者的死亡风险是基线、随访均无贫血者的6.00倍(95%CI:3.04~11.85)。结论 患者在ART过程中随访贫血、贫血加重受多种因素影响,基线贫血和随访贫血是HIV/AIDS患者预后的重要危险因素,应加强高危人群贫血筛查,力求做到早发现、早诊断、早治疗,从而降低患者的死亡风险。

目的 探讨HIV/AIDS患者在抗逆转录病毒治疗(antiretroviral therapy,ART)过程中贫血、贫血加重的影响因素,及贫血对患者预后的影响。方法 回顾性收集广州市某传染病专科医院2004年2月10日—2019年3月29日首次接受ART的HIV/AIDS患者基线和随访数据,采用广义线性混合效应模型分析基线无贫血患者长期ART过程中随访贫血的影响因素,采用Cox比例风险回归模型分析基线贫血患者贫血加重的影响因素及基线贫血和随访贫血对患者预后的影响。结果 HIV/AIDS患者11 688例,基线贫血3 853例(32.97%);7 835例基线无贫血患者在随访过程中发生贫血1 695例(21.63%),3 470例轻、中度贫血患者在随访中贫血加重845例(24.35%)。随访贫血的危险因素包括女性、开始ART年龄≥50岁、已婚/同居、离异/分居/丧偶、血液传播、基线机会性感染、基线临床症状种类数越多、初始ART方案含齐多夫定(Zidovudine,AZT)、CD4细胞计数<200个/μl、血肌酐异常。女性、已婚/同居、离异/分居/丧偶、确诊到开始ART间隔时间≥6个月、基线临床症状种类数≥2种、初始ART方案含AZT、血肌酐异常是贫血加重的危险因素。基线无贫血而随访过程中贫血患者的死亡风险是基线、随访过程中均无贫血者的5.02倍(95%CI:2.45~10.27);基线、随访过程中均贫血患者的死亡风险是基线、随访均无贫血者的6.00倍(95%CI:3.04~11.85)。结论 患者在ART过程中随访贫血、贫血加重受多种因素影响,基线贫血和随访贫血是HIV/AIDS患者预后的重要危险因素,应加强高危人群贫血筛查,力求做到早发现、早诊断、早治疗,从而降低患者的死亡风险。

Objective To investigate the influencing factors of anemia and anemia exacerbation in people living with HIV/AIDS (PLWHA) during antiretroviral therapy (ART), as well as the impact of anemia on the prognosis of PLWHA. Methods This retrospective cohort study collected baseline and follow-up data for PLHIV who initiated ART at a specialized infectious-disease hospital in Guangzhou from February 10, 2004 to March 29, 2019. A generalized linear mixed-effects model was used to analyze the factors influencing anemia during long-term ART in PLWHA without anemia at baseline. The Cox proportional hazards model was used to analyze both the influencing factors of anemia exacerbation in those with baseline anemia and the impact of baseline anemia and follow-up anemia on the prognosis. Results There were 11 688 PLWHA, of whom 3 853(32.97%) had anemia at baseline. Among 7 835 PLWHA without anemia at baseline, 1 695 (21.63%) developed anemia during the follow-up. Among 3 470 PLWHA with mild or moderate anemia at baseline, 845 (24.35%) had anemia exacerbation during the follow-up. Risk factors for follow-up anemia in PLWHA included being female, aged ≥50 years at ART initiation, being married/cohabiting, being divorced/separated/widowed, blood transmission, baseline opportunistic infections, higher number of baseline clinical symptoms, the initial ART regimen containing Zidovudine (AZT), CD4 cell count <200 cells/μL, and abnormal serum creatinine. Risk factors for anemia exacerbation included being female, being married/cohabiting, being divorced/separated/widowed, an interval of ≥6 months from diagnosis to ART initiation, ≥2 categories of baseline clinical symptoms, an initial ART regimen containing AZT, and abnormal serum creatinine. The risk of death in PLWHA without anemia at baseline but developing anemia during the follow-up was 5.02 times (95% CI: 2.45-10.27) than that among those without anemia at both baseline and during the follow-up. The risk of death with anemia at both baseline and during the follow-up was 6.00 times (95%CI: 3.04-11.85) than that among those without anemia at both baseline and during the follow-up. Conclusion Follow-up anemia and anemia exacerbation during ART are affected by multiple factors. Baseline anemia and follow-up anemia are important risk factors for the prognosis of PLWHA. Anemia screening for high-risk groups should be strengthened to strive for early detection, early diagnosis, and early treatment, thereby reducing the risk of death of PLWHA.

随着抗逆转录病毒治疗(antiretroviral therapy,ART)的广泛应用以及AIDS防控机制的日益完善,HIV感染率得到有效控制,AIDS相关死亡率也显著降低,HIV/AIDS患者预期寿命显著延长[1]。然而贫血仍是HIV/AIDS患者较为常见的并发症,研究表明,大于或等于15岁的HIV/AIDS患者贫血患病率高达46.6%[2]。随着贫血程度加重,可出现乏力、头晕、心悸等症状,严重影响患者的生活质量和预后[3]
国内外多数研究仅关注HIV/AIDS患者基线贫血患病情况及其对预后的影响[2-4],但在长期治疗过程中患者可能多次发生贫血,这类重复测量的观察值之间可能存在相关,目前缺乏关于长期ART过程中随访期间患者贫血情况及其对预后影响的纵向研究。本研究采用回顾性队列研究,采用广义线性混合效应模型,探讨HIV/AIDS患者随访期间贫血、贫血加重的影响因素及其对死亡的影响,为改善患者生活质量、评估疾病进展和预后、指导临床治疗以及促进公共卫生管理提供有力支持。
数据来源于广州市某传染病专科医院HIV/AIDS患者ART数据库。纳入标准:(1)2004年2月10日—2019年3月29日启动ART;(2)开始ART时年龄≥15岁;(3)有完整的基线记录及至少进行2次随访,且时间间隔不超过2年。排除标准:(1)随访时间<6个月;(2)既往有ART史;(3)怀孕者。最终筛选出11 688例纳入本研究。患者在开始抗病毒治疗前签署知情同意书。本研究经广州医科大学附属市八医院医学伦理委员会批准(编号:AF/SC-01/01.1)。
队列起点为2004年2月10日—2019年3月29日首次开始ART的时间。针对贫血加重的研究,结局事件为基线合并贫血的HIV/AIDS患者在随访过程中发生贫血加重(轻度转中、重度贫血或中度贫血转重度贫血)。针对预后研究,结局事件为随访过程中发生死亡,包括AIDS相关死亡和非AIDS相关死亡。
删失:研究对象在随访期间未发生贫血加重(或死亡),包括在治、失访、转诊、停药或最后一次随访时患者仍然存活等情况。其研究终点为末次随访时间或随访截止时间。
队列中主要包括:(1)人口学信息:患者性别、开始ART年龄、婚姻状况等;(2)临床特征:确诊HIV感染日期、开始ART日期、感染途径、WHO临床分期、机会性感染情况、临床症状;(3)临床用药:抗结核治疗史、复方新诺明服用史、ART方案;(4)实验室指标:基线CD4+T淋巴细胞计数、CD8+T淋巴细胞计数、血肌酐水平等。
(1)贫血:根据世界卫生组织(WHO)发布的贫血分级标准:男性血红蛋白110~129 g/L、女性血红蛋白110~119 g/L为轻度贫血;血红蛋白80~109 g/L为中度贫血;血红蛋白<80 g/L为重度贫血[2]
(2)身体质量指数(body mass index,BMI):偏瘦为BMI<18.5 kg/m2,正常为BMI≥18.5 kg/m2且<24 kg/m2,超重为BMI≥24 kg/m2且<28 kg/m2,肥胖为BMI≥28 kg/m2
(3)基线机会性感染:根据《国家免费艾滋病抗病毒药物治疗手册(第4版)》[5],HIV/AIDS患者在开始ART治疗前患有耶氏肺孢子菌肺炎、结核病、播散性非结核分枝杆菌感染等疾病。
(4)基线临床症状种类数:HIV/AIDS患者出现发热、咳嗽、咳痰、呼吸困难、胸痛、腹泻、淋巴结肿大等症状的数量。
(5)血肌酐正常参考范围:男性为53~106 μmol/L,女性为44~97 μmol/L。
采用SAS 9.4软件进行统计分析,计数资料采用n(%)描述。采用广义线性混合效应模型分析基线无贫血HIV/AIDS患者长期ART随访过程中贫血的影响因素,采用Cox比例风险回归模型分析基线贫血患者贫血加重的影响因素及贫血对预后的影响。检验水准α=0.05。
本研究纳入HIV/AIDS患者11 688例。男性9 603例(82.16%);<50岁10 092例(86.34%);未婚者5 375例(45.99%);BMI正常7 503例(64.19%)。见表1。基线7 835例(67.03%)HIV/AIDS患者无贫血,3 853例(32.97%)合并贫血;其中1 790例(15.32%)轻度贫血,1 680例(14.37%)中度贫血,383例(3.28%)重度贫血。
分析结果显示,基线无贫血HIV/AIDS患者ART期间第一、二个月贫血患病率分别为9.03%、9.23%,此后逐渐下降至4.26%,治疗第二年到第七年期间缓慢上升为7.44%。男性第一、二个月贫血患病率分别为6.75%、7.54%,贫血变化趋势与总体一致;女性第一个月贫血患病率为25.11%,随后呈下降趋势,六个月后呈缓慢上升趋势,整个随访过程中女性的贫血患病率高于男性。见图1
以每次随访时患者是否贫血为因变量,性别、开始ART年龄、婚姻状况、BMI分级、开始ART年份等为自变量,构建广义线性混合效应模型,设截距为固定效应,ART治疗时间为随机效应。分析结果显示,女性、开始ART年龄≥50岁、已婚/同居、离异/分居/丧偶、血液传播、基线机会性感染、基线临床症状种类数越多、初始ART方案含AZT、CD4细胞计数<200个/μl、血肌酐异常是HIV/AIDS患者随访贫血的危险因素;ART治疗时间、超重、2012—2015年和2016—2019年开始ART是随访贫血的保护因素。见表2
多因素Cox回归模型结果显示,女性、已婚/同居、离异/分居/丧偶、确诊到开始ART间隔时间≥6个月、基线临床症状种类数≥2种、初始ART方案含AZT、血肌酐异常者有更高的贫血加重风险,HR及95%CI表3
以患者是否死亡为因变量,采用Cox比例风险回归模型分析基线、随访期间是否贫血对HIV/AIDS患者预后的影响。多因素分析结果显示,基线无贫血而随访过程中贫血患者的死亡风险是基线、随访过程中均未贫血者的5.02倍(95%CI:2.45~10.27);基线、随访过程中均贫血患者的死亡风险是基线、随访均无贫血者的6.00倍(95%CI:3.04~11.85)。见表4
本研究分析结果显示,广州市HIV/AIDS患者基线贫血患病率为32.97%。高于云南德宏州的21.2%[6]、低于新疆的38.9%[7],亦低于全球的46.6%[2]。基线无贫血患者在随访中贫血发病密度为8.71/100人年,与台州市(8.2/100人年)相近[8]。随访期间轻、中度贫血患者贫血加重发病密度为8.00/100人年,低于台州市贫血加重发病密度(9.1/100人年)[8]
研究发现女性是基线无贫血HIV/AIDS患者长期ART治疗过程中患贫血的危险因素,也是患者贫血加重的危险因素。多项研究显示,女性患者在ART过程中血红蛋白水平比男生低,更易并发贫血[9-11]。这可能与女性月经周期及铁、锌等微量营养素摄入较男性低有关[12]。开始ART年龄≥50岁患者在ART过程中患贫血的风险高于<50岁患者,与既往研究结果一致[4,9-10]。已婚/同居、离异/分居/丧偶是患者发生贫血或贫血加重的危险因素,与霍岩[13]研究结果一致,已婚/同居群体可能面临更高的家庭照料责任或经济压力,导致其治疗依从性下降,进而影响病毒抑制效果,间接增加免疫功能损伤与贫血风险。离异/分居/丧偶者可能因社会支持网络断裂,面临独居、经济困难或抑郁等心理健康问题,导致忽视贫血症状的早期识别,延迟贫血的诊断与干预,从而增加随访期间的贫血发生率。本研究显示,超重者贫血的风险是BMI正常者的0.77倍,即超重是贫血的保护因素,与Lang R等[9]研究结果一致,提示可以通过适当增重来预防贫血发生。
在临床特征方面,本研究发现确诊到开始ART间隔时间≥6个月的患者贫血加重的风险较高。HIV具有降低造血干细胞增殖分化为成熟血细胞的能力,确诊后较长时间未接受ART治疗,可导致患者免疫功能下降,并发感染的风险增加,这些因素都可能增加贫血加重风险[4]。分析发现,通过血液传播感染的患者发生贫血风险是性传播的1.80倍。Wei等[10]对首次开始ART的436 658名HIV/AIDS患者进行贫血影响因素研究,发现注射吸毒者患贫血的风险较同性性传播高。原因可能是经血液传播途径的患者耐药发生率和病毒学抑制失败率均高于性传播途径患者。血液传播途径的患者疾病进展速度也比性传播途径患者快,这些因素在一定程度上对患者贫血的发病产生影响。本研究发现基线合并机会性感染与贫血发生有关,与既往研究结果研究一致[13-14],这可能与机会性感染破坏患者的骨髓系统从而引起贫血有关。此外,基线临床症状种类数越多,患者越可能发生贫血或贫血加重。霍岩[13]研究发现,持续腹泻是HIV/AIDS患者发生贫血的独立危险因素,患者出现呕吐、腹泻等症状易造成叶酸、B12、铁等营养物质减少,当这些关键营养物质供给持续减少时,会导致人体内用于血液生成的必要原材料在组织中储备量不足,继而引发巨幼细胞性贫血或缺铁性贫血。
在临床用药方面,本研究显示,ART治疗时间长是HIV/AIDS患者合并贫血的保护因素,与既往研究结果一致[9-10,15],这可能与病毒复制抑制、免疫功能恢复有关。研究发现初始ART方案含齐多夫定是患者随访期间贫血或贫血加重的危险因素。多项贫血相关因素研究显示,齐多夫定可导致骨髓抑制,从而导致感染者发生药物相关性贫血[4,8,10,14],应慎重使用该类药物,使用齐多夫定时,应密切关注患者血红蛋白水平,预防和早期发现贫血的发生或加重。
在实验室指标方面,CD4细胞计数<200个/μl是患者随访期间贫血的危险因素,与国内外多项研究结果一致[9-10],尽可能提高患者CD4细胞计数水平,可有效预防患者发生贫血。血肌酐异常是患者随访期间贫血或贫血加重的危险因素。埃塞俄比亚一项回顾性队列研究显示,血清肌酐大于1.1 IU/L是贫血的预测因素[15]。血肌酐异常意味着患者肾功能减弱,会导致促红细胞生成素的绝对或相对生成不足,从而引发贫血[16]。因此对血肌酐异常患者,应进行血红蛋白水平定期监测。
本研究发现,基线无贫血而随访过程中贫血患者的死亡风险是基线、随访过程中均无贫血者的5.02倍;基线、随访过程中贫血患者的死亡风险是基线、随访均无贫血者的6.00倍。Abioye等[17]对坦桑尼亚70 442名接受ART治疗的HIV/AIDS患者进行队列研究,结果显示,轻、中、重度贫血患者的死亡风险分别是无贫血患者的1.29(95%CI:1.14~1.61)、1.88(95%CI:1.62~2.17)和3.33倍(95%CI:2.75~3.99)。一项中国的队列研究显示,HIV/AIDS患者贫血状态与死亡密切相关[10]。贫血已被多项研究确定为HIV感染者死亡的预测指标[4]
本研究存在一定的局限性,队列中缺乏患者文化程度、职业、饮食等变量,无法探究这些因素对HIV/AIDS患者贫血的影响,另外,研究数据仅限于广州市某传染病专科医院,来源单一,可能存在一定的选择偏倚。
HIV/AIDS患者在长期治疗期间往往会多次出现贫血,本研究采用大样本的队列研究,不仅着眼于患者基线贫血状况,还追踪随访期间患者的贫血情况及其对预后的影响。建议在临床治疗过程中,应针对上述高危人群进行定期监测和健康教育,以实现早预防、早发现、早诊断、早治疗,从而降低患者的死亡风险。
