Objective

The aim of this study is to test the reliability and validity of the Chinese version of CP-Q among college students.

Methods

The English version of the CP-Q was translated, back-translated following the Brislin model, and cultural adaptation was performed to develop the Chinese-version CP-Q.The reliability, validity and feasibility of the questionnaire were evaluated by investigation of questionnaires offline and electronic questionnaires online.

Results

A total of 658 participants were investigated with 593 valid questionnaires obtained, the total effective rate was 90.1%.Subjects finished the Chinese-version CP-Q and Pittsburgh Sleep Quality Index (PSQI); Munich Chronotype Questionnaire (MCTQ) and 24h dietary recalls. Moderate to strong correlations were observed in the sleep/wake schedules between the Chinese-version CP-Q, PSQI and MCTQ, both for work/study days and free days, the highest was the free day sleep time r=0.922(P<0.001).The correlations between sleep, wake, first eating event, lunch time and last eating event in work/study days and free days with the same variables in the 24h recall showed r=0.441-0.659 (P<0.05), in addition to the lunchtime in free days (r=287, P=0.076). A total of 7 items among the 17 variables in the Chinese-version CP-Q evaluated in the test and retest phases presented intraclass correlation coefficients ≥0.75.

Conclusion

The Chinese-version CP-Q has good reliability and validity and could reflect the real situation of eating time and sleeping time. The questionnaire can be used to assess the status of one’s chrononutrition.

目的

汉化时序营养概况问卷,并检验中文版问卷的信度和效度。

方法

按照Brislin翻译模式将英文版时序营养概况问卷翻译为中文,并进行文化调适。通过线下发放纸质问卷和线上收集电子问卷以评价问卷的信效度和可行性。

结果

本研究共收集了658份问卷,其中有效问卷共593份(问卷有效率为90.1%)。研究对象回答了中文版时序营养概况问卷、慕尼黑睡眠时型量表、匹兹堡睡眠质量指数量表和24 h膳食回顾。其中中文版时序营养概况问卷与匹兹堡睡眠质量指数量表和慕尼黑睡眠时型量表的相似条目(休息日/工作日的睡觉/觉醒时间)进行比对,其Pearson相关系数较高,中等至强相关,最高为第一阶段纸质问卷的休息日睡觉时间r=0.922(P<0.001)。将中文版时序营养概况问卷与24 h膳食回顾比较,有工作/休息日的睡觉、觉醒、首次膳食、中餐及末次膳食时间,除休息日午餐时间r为0.287(P=0.076),其余在0.441~0.659(P<0.05)之间。17个时序营养概况问卷条目的重测信度中共有7个条目的组内相关系数大于等于0.75。

结论

中文版时序营养概况问卷具有良好信效度,且能较好反映填写者膳食时间、睡觉时间真实情况,可用于评估人群的时序营养情况。

目的

汉化时序营养概况问卷,并检验中文版问卷的信度和效度。

方法

按照Brislin翻译模式将英文版时序营养概况问卷翻译为中文,并进行文化调适。通过线下发放纸质问卷和线上收集电子问卷以评价问卷的信效度和可行性。

结果

本研究共收集了658份问卷,其中有效问卷共593份(问卷有效率为90.1%)。研究对象回答了中文版时序营养概况问卷、慕尼黑睡眠时型量表、匹兹堡睡眠质量指数量表和24 h膳食回顾。其中中文版时序营养概况问卷与匹兹堡睡眠质量指数量表和慕尼黑睡眠时型量表的相似条目(休息日/工作日的睡觉/觉醒时间)进行比对,其Pearson相关系数较高,中等至强相关,最高为第一阶段纸质问卷的休息日睡觉时间r=0.922(P<0.001)。将中文版时序营养概况问卷与24 h膳食回顾比较,有工作/休息日的睡觉、觉醒、首次膳食、中餐及末次膳食时间,除休息日午餐时间r为0.287(P=0.076),其余在0.441~0.659(P<0.05)之间。17个时序营养概况问卷条目的重测信度中共有7个条目的组内相关系数大于等于0.75。

结论

中文版时序营养概况问卷具有良好信效度,且能较好反映填写者膳食时间、睡觉时间真实情况,可用于评估人群的时序营养情况。

Objective

The aim of this study is to test the reliability and validity of the Chinese version of CP-Q among college students.

Methods

The English version of the CP-Q was translated, back-translated following the Brislin model, and cultural adaptation was performed to develop the Chinese-version CP-Q.The reliability, validity and feasibility of the questionnaire were evaluated by investigation of questionnaires offline and electronic questionnaires online.

Results

A total of 658 participants were investigated with 593 valid questionnaires obtained, the total effective rate was 90.1%.Subjects finished the Chinese-version CP-Q and Pittsburgh Sleep Quality Index (PSQI); Munich Chronotype Questionnaire (MCTQ) and 24h dietary recalls. Moderate to strong correlations were observed in the sleep/wake schedules between the Chinese-version CP-Q, PSQI and MCTQ, both for work/study days and free days, the highest was the free day sleep time r=0.922(P<0.001).The correlations between sleep, wake, first eating event, lunch time and last eating event in work/study days and free days with the same variables in the 24h recall showed r=0.441-0.659 (P<0.05), in addition to the lunchtime in free days (r=287, P=0.076). A total of 7 items among the 17 variables in the Chinese-version CP-Q evaluated in the test and retest phases presented intraclass correlation coefficients ≥0.75.

Conclusion

The Chinese-version CP-Q has good reliability and validity and could reflect the real situation of eating time and sleeping time. The questionnaire can be used to assess the status of one’s chrononutrition.

传统认为,“吃多少”和“吃什么”容易引起肥胖等代谢性疾病,目前多项研究表明进食的时机在能量代谢的过程中起着重要作用,夜间进食较多、进食窗口过长容易导致昼夜节律失调,进而影响健康,是肥胖、高血压、二型糖尿病的影响因素[1-2]。时序营养学(chrono-nutrition)是近年来较新的热门研究领域,一般指24小时周期内,食物摄入时机可影响人体生理代谢过程和疾病的发生发展,食物摄入的时机(膳食行为节律)需与人体消化吸收和代谢的内在生物节律协调,否则易导致代谢紊乱,诱发疾病[3]。膳食行为是一种复杂的行为,食物摄入的时间、频次和摄入量等受到多种因素影响,如个人习惯、民族习俗、轮班工作等[4]。在进食时间方面,一般认为有三种情况:(1)进食习惯规律性,例如工作日与休息日之间存在膳食时差,即工作日和休息日进食窗口中点的差异;(2)每日膳食频率(每日进食次数);(3)膳食时间,例如深夜进食或不吃早餐[5]。膳食时差(Eating Jet Lag),与社会时差类似,定义为进食时间的可变性,指工作日和休息日进行膳食活动的时间差[6]。已有研究表明,不合理的膳食时间会影响生物钟节律和新陈代谢过程[7];动物研究也发现,相对于活动期的摄食,休息期的食物摄入更容易增加体重[3]。目前,评估膳食模式的工具较少,可评估进食时间的工具主要有24 h膳食回顾[8]、膳食模式问卷[9]、夜食综合征问卷[10]、基于智能手机拍照记录评估[11]以及基于加速度计监测[12]。这些评估工具有的较为繁杂,需要研究人员实时记录,有的需要借助仪器,花费较高的成本且海量图片分析难度较高,应用于大样本膳食模式的调查存在一定局限性。2020年,美国学者Allison C. Veronda及其团队编制的时序营养问卷(the Chrononutrition Profile-Questionnaire, CP-Q)可以通过18个条目评估研究对象的时序营养偏好和时序营养行为,可完整地调查工作/休息日的膳食模式,具有较好信效度[13],2023年已被翻译葡萄牙语版,呈现出较好有效性和可重复性[14]。本研究旨在对英文版时序营养问卷进行跨文化调试,并评价汉化后问卷的信度和效度,为评价我国人群膳食时差提供可信的问卷工具。
采用方便抽样法于2023年5月~11月选取高校大学生作为研究对象。主要通过线下招募填写及线上填写问卷进行数据收集。共回收问卷658份,其中回收纸质197份,电子461份,其中有效问卷共593份,问卷总有效率为90.1%。信效度检验时要求样本量为测评工具条目数5~10倍,本研究样本量与变量比接近1:33,满足分析需求。
时序营养问卷由美国学者Allison C. Veronda编制开发,共有18个条目,采用了开放式与单项选择的组合,可用于评价时序营养偏好和时序营养行为。时序营养偏好主包括研究对象何时进食的情况,包括了早餐、最丰盛一餐、晚餐、夜间潜伏期、夜间膳食和膳食窗口六个部分;时序营养行为评估了工作日、休息日以及期望情况下的睡眠、膳食情况,并可分析工作日与休息日之间存在的膳食时差。其可全面评估膳食行为的节律性,目前已被翻译为葡萄牙语,并有较好的有效性和可重复性。本研究前期按照Brislin翻译模式[15]将英文版时序营养概况问卷翻译为中文,并由未阅读过原问卷的医学博士进行回译、修改、文化调适。在2023年5月初进行预调查,并对部分问题进行修改。最后由专家组对中文版时序营养概况问卷初稿进行审查、讨论,达成一致意见,制定最终的中文版时序营养概况问卷。本研究的目的是将英文版时序营养概况问卷(CP-Q)翻译成中文,并在中国人群中进行文化适应和分析其有效性和可重复性。
采用评价睡眠相关情况及昼夜节律且已汉化、信效度高的问卷和24 h膳食回顾作为校标问卷。(1)匹兹堡睡眠指数量表(Pittsburgh Sleep Quality Index, PSQI)[16],可用于评估一个月内的睡眠习惯与睡眠质量,目前已有汉化版并且信效度高,可作为校标工具。(2)慕尼黑睡眠时型量表(the Munich Chronotype Questionnaire, MCTQ)[17],可用于评估休息日和学习日的睡眠及起床时间。(3)24 h膳食回顾[4](24 h dietary recalls),收集研究对象某天的每一次的膳食内容及用餐时间,并根据需要收集了入睡/觉醒时间。
主要分为两个阶段进行。第一个阶段为线下进行,共招募197名研究对象,要求填写基本信息、时序营养概况中文版问卷和慕尼黑睡眠时型量表,对部分研究对象进行了追踪调查,收集研究对象工作日(n=75)或休息日(n=39)某天的24 h膳食回顾,以验证问卷中条目与真实情况的相关性;并在1~2周后,邀请部分(n=90)研究对象重新填写中文版时序营养问卷,以检测问卷的重测信度。第二个阶段为线上进行,向湖南两所高校发放了电子问卷,问卷内容主要包括基本信息、匹兹堡睡眠质量指数量表、慕尼黑睡眠时型量表及中文版时序营养概况问卷,最初收集到461份问卷数据,排除不合要求的数据后,共得到396份问卷数据。两阶段数据分别分析。本研究经湖南师范大学伦理委员会批准(2023-254),研究对象均自愿参与并知情同意。
填写问卷要求参与者保证问卷完整性,全程遵循保密、知情同意及自愿原则。问卷排除标准:(1)规律作答,包括直线作答、特定风格作答等;(2)答案填写前后矛盾、不符合时间逻辑的问卷;(3)作答时间超过1小时或不到5分钟。
采用EpiData 3.1进行数据资料的录入,采用IBM SPSS 27.0进行数据资料的统计分析。对收集到的数据进行统计描述和统计分析。采用均数±标准差描述连续变量且符合正态分布的一般资料;对不符合正态分布的资料采用中位数和上、下百分位数进行描述。中文版时序营养概况问卷中存在大量采用时间作答的题目,参考既往文献,在计算时间均数及标准差时采用HH:MM的格式[13-14,18-19]。在内容信度上将中文版时序营养概况问卷各条目和匹兹堡睡眠质量指数量表、慕尼黑睡眠时型量表相同变量进行比对,以及比较中文版时序营养概况问卷和24h膳食回顾,采用Pearson相关进行分析。相关系数为0~0.3时即使变量之间存在显著性,可以忽略;0.31~0.5时为弱相关;在0.51~0.7时中等相关;0.71~0.9之间为强相关;>0.9为非常强的相关[20]。在重测信度检验时,采用配对样本t检验和组内相关系数(Intra-class Correlation Coefficient,ICC)检验两次测试结果的一致性。其中,ICC值的解释如下:≥0.75表示信度好;0.60~0.74信度较好;0.40~0.59为信度一般;<0.40表示信度差[21]。检验水准为α=0.05。
中文版时序营养概况问卷评估的变量描述见表1。在第一阶段纸质问卷第一次填写中,有79.2%的研究对象认为自己最丰盛的一餐是午餐;在第二阶段电子问卷填写中,有72.7%的研究对象认为自己最丰盛的一餐是午餐。
两阶段人群均填写时序营养概况问卷和慕尼黑睡眠时型量表,两问卷中均涵盖了休息日和工作日的起床、睡觉时间。对两种问卷的相似问题进行了比较,两阶段工作日的起床时间的r分别为0.649(P<0.001)和0.653(P<0.001),以及第二阶段电子问卷的休息日睡觉时间r为0.680(P<0.001),中等相关,其他变量均强相关r=0.741~0.922(P<0.001),其中第一阶段纸质问卷的休息日睡觉时间最高,为0.922 (P<0.001)。
第二阶段人群填写时序营养概况问卷和匹兹堡睡眠质量指数量表,两种问卷均涵盖了睡觉时间和起床时间。在匹兹堡睡眠质量指数量表中,只涉及了平均每天睡觉和起床时间,中文版时序营养概况问卷也应转换成相应平均值。其睡觉时间和起床时间的Pearson相关系数分别为0.734(P<0.001)和0.623(P<0.001)。
与已发布的汉化版评价睡眠时间和起床时间的问卷相比,中文版时序营养概况问卷的相似问题与其中等到强相关,具有良好的效度。
比对中文版时序营养概况问卷的各个条目与24 h膳食回顾的各个时间点情况。其Pearson相关系数除休息日午餐时间为0.287(P=0.076),其余在0.441~0.659之间(P均<0.05),呈弱相关至中等强度相关。其中工作日的首次饮食行为的Pearson相关系数最高,为0.659(P<0.001)。
在重测阶段评估的17个时序营养概况问卷变量中,共有7个变量的组内相关系数大于等于0.75(见表5),这7个变量重测信度好。在重测信度检验中,工作日入睡时间的组内相关系数最高,为0.81(P<0.001),可以认为该条目的可再现性高。其中期望入睡时间、休息日午餐时间和晚上醒来吃东西次数的组内相关系数低于0.4,信度较差。这些指标还通过了配对样本t检验来评价第二次测试与基线测试的再现性,除了休息日的第一次进食时间,其他均P>0.05。
本研究旨在将英文版时序营养问卷(CP-Q)翻译成中文版并进行文化调试,并验证其重复性和可行性。结果表明问卷在参与者人群中具有良好的适用性。
在分析中文版时序营养概况问卷的效度时,我们将部分条目与匹兹堡睡眠质量指数量表和慕尼黑睡眠时型量表的相似条目进行比较,睡觉时间和觉醒时间具有较好的效度,相关性中等到强相关。值得注意的是,与巴西版时序营养概况问卷(CPQ-Brazil)情况相似[14],而当初版的CP-Q问卷提出的是用匹兹堡睡眠质量指数量表(PSQI)与相同变量比较,显然慕尼黑睡眠时型量表(MCTQ)的相同变量与中文版时序营养概况问卷的相关性更高。可能源于同样的评估方式,均分别评价了工作日和休息日的相关条目。
为了验证中文版时序营养概况问卷在实际情况中的效度,我们在第一阶段的研究对象人群中同时追踪了部分研究对象填写问卷后的24 h膳食回顾,其中有评估时间为工作日的有75人,休息日的有39人。除休息日午餐时间r为0.287(P=0.076),其各条目与相应时间的相关性弱到中等相关,在0.441~0.659之间(P均<0.05)。如需进一步验证中文版时序营养概况问卷与实际情况的差别,应连续观察研究对象7天,计算他们工作日及休息日的平均睡眠、饮食活动时间。
同时我们也注意到,在休息日和工作日之间可能存在社会时差[22](Social Jet Lag,指当人们将睡眠时间从工作日转移到休息日时发生的昼夜节律失调)、膳食时差,膳食时差较大,容易导致饮食失调,有研究表明,较高的身体质量指数(BMI)还与在非工作日吃得较晚有关[23]。而可行的中文版时序营养概况问卷可以帮助我们更小成本地计算膳食时差,探讨膳食时差对人群的代谢健康的影响。
在重测信度方面,除了期望入睡时间、休息日午餐时间和晚上醒来吃东西次数的组内相关系数低于0.4,大多数条目的稳定性较高,重测信度高,可用于代表短期内的时序营养情况。值得注意的是,我们仅验证了短期内的重测信度,生活方式的改变、季节的改变等,或是突发的疫情,可能会导致膳食节律(膳食时差、膳食规律性、膳食窗口等)发生改变[24],如需分析中文版时序营养概况问卷对膳食节律描述的稳定性,可在3~6个月后进行重测以验证。
综上,中文版时序营养概况问卷的信效度较好,且能较好反映填写者膳食时间、睡觉时间的真实情况。通过问卷这种更简单、低成本的方式,能较准确地描述人群24 h的膳食、睡眠情况,在研究膳食节律(膳食时差、膳食规律性、膳食窗口等)的影响上更具公共卫生学意义。
