Objective

To investigate the expression and clinical significance of protein-O-fucosyltransferase 1 in non-small cell lung cancer (NSCLC), so as to provide a reference for the diagnosis and treatment of NSCLC.

Methods

The expression of POFUT1in NSCLC was determined by TCGA, UALCAN database and immunohistochemistry. Based on the clinical data form TCGA database were extracted for survival analysis by Kaplan-Meier method, and analysis of the effect of POFUT1 mRNA expression level on prognosis of patients with NSCLC.

Results

POFUT1 was highly expression in lung adenocarcinoma (LUAD) and lung squamous cell carcinoma (LUSC) tissues (all P<0.05). The expression level of POFUT1 mRNA in LUAD was correlated with clinical stage (P<0.001) and that in LUSC was correlated with tumor size (P<0.001). COX regression analysis showed that high mRNA expression of POFUT1 (P=0.012), tumor size, clinical stage and lymph node metastasis were associated with the prognosis of LUAD patients (all P<0.001), and clinical stage III-IV was an independent risk factor for LUAD patients. Tumor size (P=0.001), clinical stage III-IV (P=0.006) and high expression of POFUT1 mRNA (P=0.049) were associated with the prognosis of LUSC patients, and tumor size greater than 5cm and high expression of POFUT1 were independent risk factors for LUSC patients. The survival analysis result showed that the survival rate of patients with high expression of POFUT1 was significantly lower than that of patients with low expression.

Conclusion

POFUT1 is highly expressed in NSCLC tissues, and high POFUT1 expression is correlated with poor prognosis in patients. POFUT1 can serve as a biomarker for prognostic evaluation of NSCLC.

目的

探讨非小细胞肺癌中蛋白-O-岩藻糖基转移酶1的表达及其临床意义,为非小细胞肺癌的诊断和治疗提供参考依据。

方法

通过TCGA、UALCAN数据库和免疫组化技术探讨POFUT1 mRNA和POFUT1蛋白在非小细胞肺癌的表达情况。提取TCGA数据库的临床数据,采用Kaplan-Meier法进行生存分析,采用COX比例风险回归模型分析POFUT1 mRNA表达水平对非小细胞肺癌患者预后的影响。

结果

POFUT1 mRNA和POFUT1蛋白在肺腺癌和肺鳞癌组织中均高表达(均P<0.05)。肺腺癌中POFUT1 mRNA的表达水平与临床分期相关(P<0.001),肺鳞癌中POFUT1 mRNA的表达与肿瘤直径相关(P<0.001)。COX回归分析结果显示,POFUT1 mRNA高表达(P=0.012)、肿瘤直径、临床分期、淋巴结转移与肺腺癌患者预后相关(均P<0.001),临床III-IV期是肺腺癌患者独立危险因素。肿瘤直径(P=0.001)、临床III-IV期(P=0.006)和POFUT1 mRNA高表达(P=0.049)与肺鳞癌患者预后相关,肿瘤直径大于5cm和POFUT1高表达是肺鳞癌患者独立危险因素。生存分析结果显示POFUT1高表达的非小细胞肺癌患者生存率显著低于低表达患者。

结论

POFUT1在非小细胞肺癌组织中高表达,且其高表达与患者不良预后相关,可作为非小细胞肺癌预后评估的生物标志物。

目的

探讨非小细胞肺癌中蛋白-O-岩藻糖基转移酶1的表达及其临床意义,为非小细胞肺癌的诊断和治疗提供参考依据。

方法

通过TCGA、UALCAN数据库和免疫组化技术探讨POFUT1 mRNA和POFUT1蛋白在非小细胞肺癌的表达情况。提取TCGA数据库的临床数据,采用Kaplan-Meier法进行生存分析,采用COX比例风险回归模型分析POFUT1 mRNA表达水平对非小细胞肺癌患者预后的影响。

结果

POFUT1 mRNA和POFUT1蛋白在肺腺癌和肺鳞癌组织中均高表达(均P<0.05)。肺腺癌中POFUT1 mRNA的表达水平与临床分期相关(P<0.001),肺鳞癌中POFUT1 mRNA的表达与肿瘤直径相关(P<0.001)。COX回归分析结果显示,POFUT1 mRNA高表达(P=0.012)、肿瘤直径、临床分期、淋巴结转移与肺腺癌患者预后相关(均P<0.001),临床III-IV期是肺腺癌患者独立危险因素。肿瘤直径(P=0.001)、临床III-IV期(P=0.006)和POFUT1 mRNA高表达(P=0.049)与肺鳞癌患者预后相关,肿瘤直径大于5cm和POFUT1高表达是肺鳞癌患者独立危险因素。生存分析结果显示POFUT1高表达的非小细胞肺癌患者生存率显著低于低表达患者。

结论

POFUT1在非小细胞肺癌组织中高表达,且其高表达与患者不良预后相关,可作为非小细胞肺癌预后评估的生物标志物。

Objective

To investigate the expression and clinical significance of protein-O-fucosyltransferase 1 in non-small cell lung cancer (NSCLC), so as to provide a reference for the diagnosis and treatment of NSCLC.

Methods

The expression of POFUT1in NSCLC was determined by TCGA, UALCAN database and immunohistochemistry. Based on the clinical data form TCGA database were extracted for survival analysis by Kaplan-Meier method, and analysis of the effect of POFUT1 mRNA expression level on prognosis of patients with NSCLC.

Results

POFUT1 was highly expression in lung adenocarcinoma (LUAD) and lung squamous cell carcinoma (LUSC) tissues (all P<0.05). The expression level of POFUT1 mRNA in LUAD was correlated with clinical stage (P<0.001) and that in LUSC was correlated with tumor size (P<0.001). COX regression analysis showed that high mRNA expression of POFUT1 (P=0.012), tumor size, clinical stage and lymph node metastasis were associated with the prognosis of LUAD patients (all P<0.001), and clinical stage III-IV was an independent risk factor for LUAD patients. Tumor size (P=0.001), clinical stage III-IV (P=0.006) and high expression of POFUT1 mRNA (P=0.049) were associated with the prognosis of LUSC patients, and tumor size greater than 5cm and high expression of POFUT1 were independent risk factors for LUSC patients. The survival analysis result showed that the survival rate of patients with high expression of POFUT1 was significantly lower than that of patients with low expression.

Conclusion

POFUT1 is highly expressed in NSCLC tissues, and high POFUT1 expression is correlated with poor prognosis in patients. POFUT1 can serve as a biomarker for prognostic evaluation of NSCLC.

肺癌(lung cancer)是常见的恶性肿瘤之一,在我国和全球范围内其发病率和死亡率均占首位。2022年我国新发肺癌病例106.06万,因肺癌死亡病例73.33万[1]。非小细胞肺癌(non-small cell lung cancer,NSCLC)患者约占肺癌患者总数的85%[2],其中主要包括肺腺癌(lung adenocarcinoma,LUAD)和肺鳞癌(lung squamous cell carcinoma,LUSC)。临床中NSCLC的诊治虽取得一定进展,但其死亡率仍居高不下。因此,探明NSCLC的分子发病机理,对NSCLC的诊断、治疗和预后均有重要作用。
糖基化是蛋白质的一种翻译后修饰,在细胞生长、分化、黏附和肿瘤免疫监视等过程中发挥作用[3]。据报道,异常的糖基化修饰与恶性肿瘤的发生发展密切相关[4]。岩藻糖基化是糖基化修饰的重要方式,主要依靠岩藻糖化转移酶、岩藻糖化合成酶和岩藻糖转运蛋白的作用将岩藻糖添加到底物中形成寡糖。岩藻糖基转移酶是通过蛋白质的岩藻糖基化作用参与人类癌症分化、发展和恶性化的关键酶[5]
蛋白-O-岩藻糖基转移酶1(protein O-fucosyltransferase 1,POFUT1)是O-岩藻糖基转移酶的重要成员之一。研究发现,POFUT1在神经胶质瘤、结直肠癌、胃癌等多种实体瘤中高表达,并促进肿瘤细胞的恶性行为[6-8]。但是POFUT1在NSCLC中的表达情况及对患者预后的影响鲜有报道。本研究通过生物信息学分析结合免疫组织化学技术探讨POFUT1在NSCLC中的表达和临床意义,为明确NSCLC诊断和治疗的新靶点提供参考依据。
通过TCGA数据库(https://portal.gdc.cancer.gov/)提取肺腺癌和肺鳞癌RNA-Seq转录组数据和相关临床特征信息。剔除无POFUT1 mRNA表达量和临床特征信息的样本9例。TCGA-LUAD数据集包括58例正常样本,513例LUAD病例;TCGA-LUSC数据集包括51例正常样本,496例LUSC病例。
根据约登指数(敏感度+特异度-1)最大值定义POFUT1 mRNA表达量的最佳临界值。根据TCGA-LUAD中POFUT1 mRNA表达量的最佳临界值4.946 4,将513例LUAD样本分为POFUT1 mRNA高表达组(309例)和POFUT1 mRNA低表达组(204例);根据TCGA-LUSC中POFUT1 mRNA表达量的最佳临界值6.4301,将496例LUSC样本分为POFUT1 mRNA高表达组(67例)和POFUT1 mRNA低表达组(429例)。利用χ2检验分析POFUT1 mRNA表达量与患者临床特征的关系;应用COX回归分析影响NSCLC患者预后的危险因素。使用R包“ggplot2”绘制POFUT1 mRNA差异表达箱形图,R包“survival”绘制生存曲线。应用UALCAN数据库(http://ualcan.path.uab.edu/analysis-prot.html)CPTAC板块在线分析POFUT1蛋白在NSCLC组织与正常肺组织的差异表达。
肺组织标本取自山西省肿瘤医院2013年6月至2014年6月呼吸科经组织病理学确诊的NSCLC组织样本128例(71例LUAD和57例LUSC),正常肺组织样本47例。所有病例术前均未接受任何治疗。该研究经山西省肿瘤医院伦理委员会评审通过(伦理编号:201845)。
取4 μm石蜡切片。65℃烤片4 h;用二甲苯去蜡,乙醇脱水处理;高压抗原修复;3%过氧化氢孵育10 min;5%山羊血清37℃封闭30 min;POFUT1一抗(1:200;武汉三鹰公司)4℃过夜孵育;加50 μL二抗,室温孵育1 h;50 μL辣根酶标记链霉卵白素工作液,37℃孵育1 h;DAB呈色;脱水透明处理后,中性树胶封片。
用NanoZoomer-SQ数字切片扫描仪器对组织切片进行扫描,在NDPView 2软件截取5张代表性图片,用ImageJ软件IHC profiler插件分析。根据公式H-score=[1×(低阳性细胞占比)+2×(阳性细胞占比)+3×(高阳性细胞占比)]×100计算每个样本POFUT1蛋白的表达量,分数范围为0到300分[9]
数据分析使用SPSS 26.0软件完成。计量资料采用均数±标准差表示,独立样本t检验对两组间表达量进行分析。多组样本均数的比较采用单因素方差分析。用χ2检验分析POFUT1的表达与临床特征的关系,其中计数资料用频数(%)表示。检验水准α=0.05。
TCGA数据库分析结果显示,POFUT1 mRNA在LUAD(图1A)和LUSC(图1B)中的表达量均显著高于正常肺组织(均P<0.001)。
UALCAN数据库分析显示,与正常肺组织相比,POFUT1蛋白在LUAD和LUSC组织中均呈高表达(均P<0.05)(图2A图2B)。
免疫组化检测结果显示,与正常肺组织相比,POFUT1蛋白在LUAD和LUSC组织中的阳性表达增加(图3A)。对免疫组化结果进行定量分析,POFUT1蛋白在LUAD的表达水平为(100.89±21.67),LUSC为(107.75±31.55),正常肺组织为(71.41±21.62)。单因素方差分析结果显示,POFUT1在LUAD和LUSC中的蛋白表达水平显著高于正常肺组织(F=24.90,P<0.001)(图3B)。
NSCLC患者POFUT1 mRNA表达量与临床特征相关分析结果显示,LUAD患者POFUT1 mRNA的表达与临床分期有关(P<0.001),与性别、肿瘤直径、吸烟史、年龄和淋巴结转移无关(均P>0.05);LUSC患者POFUT1 mRNA的表达与肿瘤直径相关(P<0.001),与性别、年龄、吸烟史,分期以及淋巴结转移均无关(均P>0.05)(表1)。
生存分析结果显示,POFUT1 mRNA高表达的LUAD和LUSC患者生存率显著低于POFUT1低表达患者(图4A图4B),差异有统计学意义(均P<0.05)。单因素COX回归分析结果显示,POFUT1 mRNA高表达(P=0.012)、肿瘤直径、临床分期、淋巴结转移与肺腺癌患者预后相关(均P<0.001);多因素COX回归结果显示,临床III-IV期肺腺癌患者死亡风险比临床I期患者高1.956倍(95%CI:1.000~3.824,P=0.050)(表2)。单因素回归结果显示,肿瘤直径(P=0.001)、临床III-IV期(P=0.006)和POFUT1 mRNA高表达(P=0.049)与肺鳞癌患者预后相关;多因素结果显示,肿瘤直径大于5cm的肺鳞癌患者死亡风险是肿瘤直径小于5cm患者的1.771倍(95%CI:1.272~2.465,P=0.001),且POFUT1高表达的肺鳞癌患者的死亡风险比POFUT1低表达患者高1.491倍(95%CI:1.041~2.137,P=0.029)(表3)。
随着基因检测的深入和诊疗技术的提高,肺癌的死亡率逐步下降,但三分之一肺癌患者被确诊时已是中晚期,导致五年生存率偏低,因此肺癌的早期诊断至关重要。目前,蛋白糖基化修饰作为癌症潜在诊断标志物被广泛研究[10]。异常蛋白糖基化修饰与癌症的发展有关[4]。岩藻糖基转移酶的高表达是恶性细胞转化的标志,被认为是实体瘤的潜在治疗靶点[5]。POFUT1作为O-岩藻糖转移酶之一,定位于20q11.21,可通过O-糖苷键将岩藻糖添加到许多细胞表面和分泌蛋白的表皮生长因子样结构域的共识序列中的丝氨酸或苏氨酸残基上,在O-岩藻糖基化中发挥作用[11]。研究发现POFUT1在多种实体瘤组织(如结直肠癌、胃癌等)中的表达水平均显著高于正常组织,提示POFUT1与肿瘤进展相关[6-8]
Leng团队[12]研究发现与对照组相比,肺癌患者中POFUT1的水平更高,对POFUT1的检测分析有助于肺癌的诊断。本研究的实验结果表明POFUT1 mRNA和蛋白在非小细胞肺癌组织的表达显著高于正常肺组织,提示POFUT1高表达可能与非小细胞肺癌的发生发展有关。研究发现POFUT1高表达与口腔癌组织肿瘤直径正相关[13],与结直肠癌病理分期密切相关[7]。本研究结果显示,POFUT1高表达与LUAD患者临床分期正相关(P=0.000),与LUSC患者肿瘤直径正相关(P=0.000),提示POFUT1有望作为判定非小细胞肺癌病理分期的一种新的参考指标。
本研究的COX回归结果显示POFUT1高表达和肿瘤直径大于5cm是LUSC患者的独立危险因素;POFUT1高表达和临床分期是影响LUAD患者预后的重要临床指标。多项研究表明POFUT1高表达的肿瘤患者比 POFUT1低表达的患者总生存率更低[6-8]。本研究结果也发现POFUT1表达高的非小细胞肺癌患者的生存期更短(P<0.05)。上述研究结果提示POFUT1可能参与非小细胞肺癌的发生发展,可作为判断患者预后的生物标志物。
本研究通过生物信息学结合免疫组化方法发现POFUT1在非小细胞肺癌中高表达,其高表达与LUAD患者临床分期和LUSC患者肿瘤直径相关,是肺鳞癌患者的独立预后因素,提示POFUT1有助于非小细胞肺癌患者的辅助诊断和预后评估,进一步研究POFUT1为阐明非小细胞肺癌发生发展提供新思路。
