Objective

To establish a health emergency competency evaluation index system for public health personnel, so as to provide a theoretical basis for building a health emergency team.

Methods

The competency index was screened and the evaluation index frame was established by literature research method, expert association commercial method and Delphi method.The analytic hierarchy process was used to calculate the weight value of the importance of indicators at all levels, and the evaluation index system was formed.

Results

A total of 21 experts were invited to conduct letter inquiries, and the recovery rate of the two rounds of expert letter inquiries was 100%.The Cronbach’s α coefficients of the expert letter consultation form were 0.968 and 0.963,expert authority level 0.865,Kendall’s W coordination coefficient in 2 rounds of correspondence suggested that experts had a high consistency (P<0.05). The evaluation index system of public health personnel’s competency in health emergency work included 3 first-level indicators,11 second-level indicators and 68 third-level indicators.The weights of the first-level indicators are respectively health emergency literacy (0.517 4), professional literacy (0.336 1) and personal literacy (0.146 6).

Conclusion

Good health emergency literacy is the basis for public health personnel to respond to public health emergencies.The weight of each index in this index system is determined by Delphi method and analytic hierarchy process, and the results are scientific and reliable.

目的

建立公共卫生人员卫生应急工作胜任力评价指标体系,为打造卫生应急队伍提供理论依据。

方法

利用文献研究法、专家会商法、德尔菲法筛选胜任力指标并建立评价指标框架;利用层次分析法计算各级指标重要性的权重值,形成评价指标体系。

结果

共邀请21名专家进行函询,2轮专家函询表的回收率均为100%,专家函询表的 Cronbach α系数分别为0.968 和0.963,专家权威程度0.865,2轮函询Kendall’s W协调系数提示专家一致性较高(P<0.05),最终确定公共卫生人员卫生应急工作胜任力评价指标体系包括3个一级指标、11个二级指标和68个三级指标,一级指标权重从大到小分别是卫生应急素养(0.517 4)、职业素养(0.336 1)、个人素养(0.146 6)。

结论

良好的卫生应急素养是公共卫生人员应对突发公共卫生事件的基础,利用德尔菲法和层次分析法确定的该指标体系各指标的权重,结果科学可靠。

目的

建立公共卫生人员卫生应急工作胜任力评价指标体系,为打造卫生应急队伍提供理论依据。

方法

利用文献研究法、专家会商法、德尔菲法筛选胜任力指标并建立评价指标框架;利用层次分析法计算各级指标重要性的权重值,形成评价指标体系。

结果

共邀请21名专家进行函询,2轮专家函询表的回收率均为100%,专家函询表的 Cronbach α系数分别为0.968 和0.963,专家权威程度0.865,2轮函询Kendall’s W协调系数提示专家一致性较高(P<0.05),最终确定公共卫生人员卫生应急工作胜任力评价指标体系包括3个一级指标、11个二级指标和68个三级指标,一级指标权重从大到小分别是卫生应急素养(0.517 4)、职业素养(0.336 1)、个人素养(0.146 6)。

结论

良好的卫生应急素养是公共卫生人员应对突发公共卫生事件的基础,利用德尔菲法和层次分析法确定的该指标体系各指标的权重,结果科学可靠。

Objective

To establish a health emergency competency evaluation index system for public health personnel, so as to provide a theoretical basis for building a health emergency team.

Methods

The competency index was screened and the evaluation index frame was established by literature research method, expert association commercial method and Delphi method.The analytic hierarchy process was used to calculate the weight value of the importance of indicators at all levels, and the evaluation index system was formed.

Results

A total of 21 experts were invited to conduct letter inquiries, and the recovery rate of the two rounds of expert letter inquiries was 100%.The Cronbach’s α coefficients of the expert letter consultation form were 0.968 and 0.963,expert authority level 0.865,Kendall’s W coordination coefficient in 2 rounds of correspondence suggested that experts had a high consistency (P<0.05). The evaluation index system of public health personnel’s competency in health emergency work included 3 first-level indicators,11 second-level indicators and 68 third-level indicators.The weights of the first-level indicators are respectively health emergency literacy (0.517 4), professional literacy (0.336 1) and personal literacy (0.146 6).

Conclusion

Good health emergency literacy is the basis for public health personnel to respond to public health emergencies.The weight of each index in this index system is determined by Delphi method and analytic hierarchy process, and the results are scientific and reliable.

我国对公共卫生领域胜任力的研究开始于2006年,相关研究文献较少,其中针对胜任力评价体系的研究仅占三分之一,研究对象也仅限于疾控机构人员、基层公共卫生人员,或者针对公共卫生医师或岗位开展研究,未将各级各类医疗卫生机构从事公共卫生的人员作为整体研究,且未针对卫生应急工作的特点和重点,构建不同的评价体系[1-7]。突发公共卫生事件的应急处置工作不仅仅局限于疾控机构,各级各类医疗卫生机构工作人员才是事件处置的基石,而长期以来医疗体系中的“重医轻防”,导致医疗机构忽视了卫生应急方面的职能,也导致事件处置工作中,公共卫生人员工作能力欠缺、事件处置滞后。本研究旨在运用德尔菲法和层次分析法[8-9]建立一套公共卫生人员卫生应急工作的胜任力评价指标体系,便于今后开展相关评估,为政府资源配置、公共卫生人员卫生应急能力建设参考提供辅助决策。
通过文献回顾法、专家会商法、小组讨论法初步建立评价指标体系。采用德尔菲法进行专家函询,构建出一套科学系统的指标体系;采用层次分析法,由专家对指标之间进行两两相对重要性评分,以此计算各级指标的权重。
利用知网、万方、PubMed等数据库,主题词为“卫生应急”“公共卫生”“胜任力”“层次分析法”“德尔菲法”等,时间设定在2012—2022年,检索有关公共卫生人员卫生应急胜任力评价指标的文献资料,去除重复文献、与本研究主题不符的文献,最终得重要文献54篇,结合目前卫生应急工作特点,梳理出适合于各级各类医疗卫生机构公共卫生人员卫生应急相关工作胜任力指标,经课题小组讨论及专家会商初步筛选备选指标。
经小组讨论,专家遴选标准为从事突发公共卫生事件相关工作5年及以上;本科及以上学历;中级及以上职称;熟悉并愿意参加本研究,且保证至少参与2轮德尔菲法[8,10]。按照此标准从全省范围内邀请了21名专家,分别来自省、市、区疾控机构,医疗机构,卫生行政部门和高等院校。
函询表分为3部分,第一部分为研究的目的、填写要求和注意事项;第二部分为专家的基本信息;第三部分为胜任力指标,指标的重要程度采用Likert 5点评分法进行评分[2,6],并收集专家对指标的意见和建议,以及判定依据和熟悉程度自评表。
采用邮件方式开展2轮次专家函询,每次于10日后回收。专家对指标重要性进行评分,并对函询表中的重复、缺失、不完善的内容提出修改意见。通过专家权威程度、积极程度、意见集中程度和意见协调程度来评估指标的合理性和可靠性[8,10]。课题组对指标重要性均值>3.5分且变异系数<25%的指标予以保留,并根据专家们的意见进行修订,确定最终评价体系。
课题组根据指标间关系确定该指标体系的层次模型,并制成判断矩阵评分表,由专家们按照Satty标度,分别对不同层次之间的各指标按照两两相互比较的方式,进行重要性打分,并开展一致性检验[10-11];利用专家们对各项指标评分的几何平均数,计算指标权重。
采用WPS 2023版双录入专家函询数据;通过SPSS 22.0进行德尔菲法相关指标计算,包括指标重要性评分的算术均值和标准差、变异系数(%),对专家的基本情况、专家权威系数、专家积极系数和专家意见集中程度进行统计分析[8,10];通过非参数检验进行专家意见一致性的Kendall’s W检验,检验水准α=0.05;对函询表的信度进行Cronbach α系数检验,Cronbach α系数>0.7代表信度良好;通过Yaahp 12.2 软件进行层次分析,开展权重计算和随机一致性比率(Random conformance Rate,CR),CR<0.05表示一致性较好[3]
共邀请21名专家,专家平均年龄为(39.57±4.27)岁,硕士及以上学历者12人(57.14%),高级职称专家17人(80.95%);卫生应急相关工作年限为(12.19±6.17)年,≥10年的专家为15名(71.43%),详见表1
参与本课题德尔菲法2轮专家函询的专家均为21名,函询表有效回收率均为100%,可认为专家积极程度高。
专家权威程度(Cr)由专家熟悉程度(Cs)和专家判断依据(Ca)确定,公式为Cr =(Ca + Cs)/2。Ca =0.88,Cs =0.85,故Cr=(0.88+0.85)/2=0.865。结果大于0.85,说明专家具有较高的权威性。详见表2表3
第一轮德尔菲法专家意见Kendall’s W协调系数为0.247~0.427,第二轮为 0.301~0.638。两轮协调系数检验提示专家具有一致性(P<0.01),表明两轮德尔菲专家咨询的专家意见较协调、可靠。详见表4
第一轮专家咨询表条目数为84,Cronbach α系数为0.968,第二轮条目数82,Cronbach α系数为 0.963,2 轮专家咨询表的Cronbach α系数均在0.9以上,内部一致性检验较好,说明专家的意见基本趋于一致。
第1轮德尔菲法专家函询表包括一级指标3个,二级指标12个,和三级指标69个;指标的均值在3.43~5.00之间,变异系数在0%到40.80%之间,均数<3.5或变异系数>25%的指标有4项;经收回整理的专家函询表,综合专家意见,经课题组集体讨论,指标新增3项,删除4项,完善描述9项,将内容相关的2个二级指标进行合并;形成由一级指标3个、二级指标11个、和三级指标68个构成的第二轮专家函询表。经第二轮专家函询,指标的均值在3.62~5.00之间,变异系数在0%到22.24%之间,根据专家的意见对3项指标的表述进行修改完善,无删除或新增条目。最终确定3项一级指标,11项二级指标和68项三级指标作为评价指标。
采用层次分析法,共创建包括总目标决策矩阵在内的15个判断矩阵,计算各指标权重得出,一级指标权重从大到小分别是卫生应急素养(0.517 4)、职业素养(0.336 1)、个人素养(0.146 6),一致性检验CR值均小于0.05,表明各层级指标明确,无逻辑错误,一致性程度高。各指标权重计算结果详见表5
根据德尔菲法,专家人数一般需大于15人[11-13]。本研究邀请21名专家进行了2轮函询,专家均符合遴选标准,代表性较好。专家的权威程度为0.865,表明专家基于自身的实践经验和理论分析,对研究内容的熟悉程度较高,可靠性较高。第2轮Kendall’s W协调系数较第一轮均有提高,结果可信,说明专家对指标的认可程度趋于一致。
各专家对本研究十分重视,积极程度高,2轮函询表的有效回收率均为100%。第一轮函询后,专家提出二级指标中的“沟通能力”属于“领导能力”的一部分,因此将二级指标进行合并;部分专家提出突发公共卫生事件应对中的“自我心理调节能力”和“通常期盼最好结果”同属于“心理应激的基本知识及个人应对策略”,因此删除这2项;另有专家提出,突发公共卫生事件处置中应具备“持续改进的能力”“对他人快速培训的能力”“语音表达的能力”等,因此增加了相关指标。第二轮函询后,专家们仅对3项三级指标的描述进行了修改,无新增和删除意见,最终经课题组的充分讨论,形成3项一级指标,11项二级指标和68项三级指标构建的公共卫生专业人员卫生应急工作胜任力评价指标体系,具有较高的科学性和可靠性。
本研究结果显示,一级指标权重从大到小分别是卫生应急素养(0.517 4)、职业素养(0.336 1)、个人素养(0.146 6),说明良好的卫生应急素养是公共卫生人员应对突发公共卫生事件的基石。但是,目前国内外对于公共卫生人员的卫生应急素养,尚未形成一致的定义或涵义。在参考相关研究的基础,本研究将公共卫生人员卫生应急素养定义为公共卫生人员通过学习卫生应急知识,掌握并保持卫生应急技能,能够有效在各类突发公共卫生事件中开展应对处置的能力。二级指标中,卫生应急技能(0.310 5)、卫生应急知识(0.206 9)、职业技能(0.089 6)、专业知识(0.081 0)权重位于前四,这表明知识与技能的是从事卫生应急工作的核心,而知识与技能可以通过培训得以提升,这与相关的研究结果一致[10,14]。而在实际的卫生应急工作中,也需要公共卫生人员具备良好的沟通协调能力,遵守职业道德规范,同时具有较强的责任感、抗压能力和应变能力。
2020年新型冠状病毒感染的暴发引起了世界的关注,面对着重大突发公共卫生事件的应急处置,广大的公共卫生人员,包括疾控机构、基层医疗卫生机构和综合医疗机构等各级各类医疗卫生机构的公共卫生人员积极投入到任务繁重的卫生应急工作中,这对卫生应急人才需求的急剧增加。后疫情时代,全球形势不断变化,贸易商旅逐渐回恢复,新发或再发传染病多点触发,如何提升公共卫生人员卫生应急工作的胜任力也成为当务之急[15]。构建系统、全面、科学的公共卫生人员卫生应急工作胜任力评价指标体系,有助于掌握卫生应急工作的知识、技能和相关能力要求,能够为各级机构有针对性的人员招聘、选拔、培训和评估提供参考依据,建立以胜任力评价为基础的“人岗匹配”机制,构建科学的人才培养体系,为打造复合型、高效型、专业型的卫生应急人才队伍提供新的思路[3,16]
