Objective

To explore the impact of dynamic changes in depressive states on activity of daily living (ADL) limitations among middle-aged and older adults.

Methods

Based on data from the China Health and Retirement Longitudinal Study from 2011 to 2020, 8 617 individuals aged 45 and above were included. The transition characteristics of depressive states were analyzed using a Markov chain model. Latent class analysis was used to identify patterns of depressive changes, and the impact of different depressive trajectories on ADL limitations was assessed using multivariate logistic regression.

Results

The transition probabilities of depressive states showed that the stability of no depression (0.76) and severe depression (0.37) was the highest. However, the transition probabilities from no depression to moderate and severe depression were 0.32 and 0.2,respectively. Latent class analysis identified three types of depressive trajectories: low-stable (1 007 cases with ADL limitations), gradually increasing (1 526 cases), and highly fluctuating (665 cases). Multivariate logistic regression showed that compared with the low-stable group, the risk of ADL limitations was significantly higher in the highly fluctuating group (OR=7.75, 95%CI: 6.54-9.17, P<0.05), followed by the gradually increasing group (OR=2.86; 95%CI: 2.58-3.18, P<0.05).

Conclusion

Dynamic changes in depressive states significantly affect ADL limitations. The group with highly fluctuating depressive trajectories has the highest risk. Clinically, it is necessary to strengthen the monitoring of depressive trajectories and design enhanced interventions for individuals with fluctuating depression to improve functional outcomes.

目的

探讨中老年人群抑郁状态动态变化对日常生活活动受限(activity of daily living,ADL)的影响。

方法

基于中国健康与养老追踪调查2011—2020年数据,纳入8 617名45岁及以上中老年人。采用马尔可夫链模型分析抑郁状态转移特征,通过潜在类别分析识别抑郁变化模式,并利用多因素logistic回归评估不同抑郁轨迹对ADL受限的影响。

结果

抑郁状态转移概率显示,无抑郁(0.76)与重度抑郁(0.37)稳定性最高,无抑郁向中/重度抑郁的转移概率分别为0.32和0.2。潜在类别分析识别出三类抑郁轨迹:低度平稳型(ADL受限1 007例)、缓升平稳型(1 526例)和波动显著型(665例)。多因素logistic回归显示,与低度平稳型相比,波动显著型ADL受限风险显著升高(OR=7.75,95%CI: 6.54 ~9.17,P<0.05),缓升平稳型次之(OR=2.86,95%CI: 2.58 ~ 3.18,P<0.05)。

结论

目的

探讨中老年人群抑郁状态动态变化对日常生活活动受限(activity of daily living,ADL)的影响。

方法

基于中国健康与养老追踪调查2011—2020年数据,纳入8 617名45岁及以上中老年人。采用马尔可夫链模型分析抑郁状态转移特征,通过潜在类别分析识别抑郁变化模式,并利用多因素logistic回归评估不同抑郁轨迹对ADL受限的影响。

结果

抑郁状态转移概率显示,无抑郁(0.76)与重度抑郁(0.37)稳定性最高,无抑郁向中/重度抑郁的转移概率分别为0.32和0.2。潜在类别分析识别出三类抑郁轨迹:低度平稳型(ADL受限1 007例)、缓升平稳型(1 526例)和波动显著型(665例)。多因素logistic回归显示,与低度平稳型相比,波动显著型ADL受限风险显著升高(OR=7.75,95%CI: 6.54 ~9.17,P<0.05),缓升平稳型次之(OR=2.86,95%CI: 2.58 ~ 3.18,P<0.05)。

结论

抑郁状态动态变化显著影响ADL受限功能,波动显著型群体风险最高。临床需加强抑郁轨迹监测,针对波动人群设计强化干预以改善功能预后。

Objective

To explore the impact of dynamic changes in depressive states on activity of daily living (ADL) limitations among middle-aged and older adults.

Methods

Based on data from the China Health and Retirement Longitudinal Study from 2011 to 2020, 8 617 individuals aged 45 and above were included. The transition characteristics of depressive states were analyzed using a Markov chain model. Latent class analysis was used to identify patterns of depressive changes, and the impact of different depressive trajectories on ADL limitations was assessed using multivariate logistic regression.

Results

The transition probabilities of depressive states showed that the stability of no depression (0.76) and severe depression (0.37) was the highest. However, the transition probabilities from no depression to moderate and severe depression were 0.32 and 0.2,respectively. Latent class analysis identified three types of depressive trajectories: low-stable (1 007 cases with ADL limitations), gradually increasing (1 526 cases), and highly fluctuating (665 cases). Multivariate logistic regression showed that compared with the low-stable group, the risk of ADL limitations was significantly higher in the highly fluctuating group (OR=7.75, 95%CI: 6.54-9.17, P<0.05), followed by the gradually increasing group (OR=2.86; 95%CI: 2.58-3.18, P<0.05).

Conclusion

Dynamic changes in depressive states significantly affect ADL limitations. The group with highly fluctuating depressive trajectories has the highest risk. Clinically, it is necessary to strengthen the monitoring of depressive trajectories and design enhanced interventions for individuals with fluctuating depression to improve functional outcomes.

抑郁症是一种在全球范围内广泛存在的心理疾病,其危害之大,已被世界卫生组织(WHO)列为造成全球残疾的最大单一因素[1]。抑郁症患者通常会经历持续的情绪低落,对以往感兴趣的事物或活动兴趣减退,同时伴有睡眠障碍等多种症状。这些症状在严重时会极大地影响患者的日常生活活动(activity of daily living,ADL),包括工作、学习、社交以及自我照顾等基本功能[2-3]。数据显示,1990—2017年,全球抑郁症的发病人数从1.72亿增加到2.58亿,增长幅度达到了49.86%[4],预计到 2030年,与之相关的全球经济成本将翻倍,给世界卫生体系带来沉重的医疗与经济压力[3]
抑郁症的发病具有明显的生命周期特征,其风险因素在不同年龄阶段呈现动态变化。研究表明,早期生命经历对抑郁易感性具有持久影响,特别是童年期遭遇的不良经历(如持续性欺凌、家庭功能失调或情感忽视)可显著提高抑郁风险[5];在青少年阶段,由于生物、心理、社会层面的剧烈变化,个体抑郁发病率呈现爆发式增长,抑郁症发病率约为儿童的 4倍[6]。值得注意的是,女性在围绝经期至绝经后阶段面临独特的抑郁易感窗口,这一时期的抑郁风险主要源于激素的波动与社会角色的转变[7]。此外,抑郁症不仅对患者的心理健康造成损害,还与多种身体疾病存在关联。研究表明,抑郁症与糖尿病[8]、心血管疾病[9]以及认知功能异常[10]等慢性疾病的发生和发展密切相关,进一步加重了患者的健康负担。作为一种具有复发倾向的慢性精神疾病,抑郁症的病程多呈波动性进展,患者在缓解期可能保持相对正常的功能状态,但随着病程的演进,抑郁症状的反复发作往往导致患者社会功能的渐进性损害[11]。然而,目前关于抑郁状态的动态变化对ADL的具体影响的相关研究仍较为匮乏,需要进一步深入探讨。
因此,本研究利用中国健康与养老追踪调查(China Health and Retirement Longitudinal Study,CHARLS)2011、2013、2015、2018和2020年的数据,对10年间抑郁状态变化对ADL的影响进行了系统研究。通过长期的追踪调查,本研究旨在填补现有研究的不足,为深入了解抑郁症的病程特征及其对患者日常生活的影响提供实证依。
该研究利用了CHARLS的数据,CHARLS是一项全面的、全国性的纵向追踪研究计划,旨在评估中国成年人的健康和营养状况[12]。CHARLS针对45岁及以上的中老年人建立一个高质量、可公开访问的微型数据库。该数据库包括广泛的社会经济因素以及个人的详细健康信息。CHARLS项目于2011—2012年启动,其全国基线调查涵盖了中国150个县区的450个村庄和居民区的不同样本。欲了解更多详细信息,请访问CHARLS官方网站https://charls.pku.edu.cn/
本研究根据2011—2020年数据。最初,共17 705人参与了相关研究,经过筛选剔除无关参与者,最终8 617名参与者被纳入本研究中。排除标准:(1)相关数据缺失;(2)无法判断是否存在ADL受限;(3)没有完整参与五次抑郁问卷调查。经过与原始人群的对比分析,筛选后的人群在年龄和性别分布上表现出良好的一致性(标准化差异<0.1)。在地理分布方面,筛选人群覆盖了原始人群所在的28个省份中的27个(仅新疆维吾尔自治区未纳入)。图1显示了详细的选择过程。在参与之前,每位参与者都获得了书面知情同意,CHARLS研究的方案得到了北京大学伦理审查委员会的批准。
CHARLS项目利用流调中心抑郁量表(CES-D-10)评估研究对象的抑郁状况[13],共10个条目,总分范围为0~30分。在本研究中,依据四分类法将抑郁状态划分为四个等级:无抑郁(≤9分)、轻度抑郁(10~14分)、中度抑郁(15~19分)和重度抑郁(≥20分)。参考先前研究对缺失值的处理[14-15],对于缺失的条目数据,本研究采用多重插补法进行处理。
本研究的结局因素为ADL是否受限。根据2020年CHARLS相关问卷项目,本研究中所收集的ADL数据分为两类:(1)基本ADL:进食、穿衣、上下床、洗澡、如厕和二便控制;(2)工具性ADL:做家务、做饭、购物、吃药和管钱。每项活动能力按照受限程度分为四个等级(1=不受限,2=轻度受限,3=中度受限,4=重度受限)。计算各项ADL得分,当基本ADL和工具性ADL的合计总分大于二者的项目合计总数时,即判定为ADL受限[16]。结局事件的发生时间以完成问卷调查的时间为准。
本研究控制了以下协变量:(1)人口学特征:年龄、性别;(2)社会经济因素:受教育程度、居住地类型、婚姻状况;(3)生活方式:睡眠时长、吸烟状况、饮酒状况;(4)生理指标:身体质量指数(body mass index,BMI)、腰围、收缩压、舒张压。
本研究采用R 4.4.1软件进行数据清洗、插补和统计分析。首先,对于缺失的抑郁数据,通过R软件mice包创建五个插补数据集,插补模型纳入人口学变量、健康行为指标及其他相关变量,最终结果通过Rubin规则进行合并。其次,我们构建离散时间齐次马尔可夫链模型分析2011—2020年研究对象抑郁状态的动态变迁特征。状态空间根据抑郁状态的等级进行划分,模型假设转移概率在十年间保持稳定,且下一状态仅依赖当前状态。同时,为了为解决2015—2018间隔三年可能引入的时间异质性,采用加权调整后的概率,两年间隔系数为1,三年间隔系数为0.67。之后采用潜在类别分析(latent class analysis, LCA)方法,基于五次调查的抑郁状态数据,识别研究对象可能存在的潜在亚组。最后,根据潜在类别分析结果进行分组,运用单因素logistic分析筛选对ADL受限相关的协变量,再采用多因素logistic回归模型,构建多模型策略探讨不同抑郁状态变迁模式对ADL受限的影响。在统计描述方面,分类变量以频数(百分比)表示,组间比较采用χ2检验或Fisher精确概率法;连续变量以(均值±标准差)表示,组间比较采用t检验。所有统计检验均采用双侧检验,显著性水平α=0.05。
本研究采用多重插补法处理抑郁问卷条目缺失数据,缺失率范围为0.6%~12.1%。插补后数据分布与原始非缺失数据呈现良好的一致性。基于五个插补数据集,我们构建了马尔可夫链模型分析抑郁状态转移概率。模型齐性检验显示假设成立(χ2=2.97,P=0.96),表明转移概率具有时间稳定性。转移概率矩阵结果见图2。研究发现,状态维持概率最高的三个转移路径分别为:无抑郁状态的自我维持(0.76)、无抑郁向轻度抑郁的转变(0.48)以及重度抑郁状态的自我维持(0.37)。这一结果表明,研究对象的抑郁状态呈现出明显的“状态依赖性”,即个体倾向于维持其当前的抑郁状态水平。同时,由无抑郁到中度(0.32)、重度(0.2),由轻度到中度(0.28)、重度(0.19)的可能性不小,这表明即使个体当前没有抑郁或抑郁程度很低,他们仍有相当的概率发展较为严重的抑郁。此外,抑郁状态的渐进性加重趋势明显,表现为轻度向中度(0.28)以及中度向重度(0.24)的转变概率相对较高。
在研究中发现多重插补数据集间的潜在类别分析结果高度一致,最大类别概率波动小于1.2%,因此从五个插补数据集中选择了第一个数据集进行后续的研究和分析。
本研究通过潜在类别分析,将人群分为三类:低度平稳型、缓升平稳型和波动显著型。每类人群内部,每年的抑郁状态构成占比如图3所示,显示了每类人群抑郁状态变化的拟合曲线。
低度平稳型群体的抑郁状态在整个时间段内保持在较低水平,几乎没有显著变化,且抑郁程度较低,主要集中在无和轻度之间。波动显著型群体的抑郁状态在2011—2013年有所下降,随后在2013—2015年显著上升,并在2015年后逐渐下降。中重度抑郁人群占多数。缓升平稳型群体的抑郁状态在2011—2015年略有下降,随后在2015—2020年逐渐上升,尽管整体趋势是上升的,但上升幅度较为平缓,抑郁状态在2011—2020年逐渐从无和轻度向中重度转移。
共纳入8 617例受试者,平均年龄(58.18±8.55)岁,女性占54.31%。截至2020年共发现3 200(37.14%)名受试者出现ADL受限,其中低度平稳型1 007例,波动显著型665例,缓升平稳型1 526例。如表1所示,接近69%的人受教育程度在小学及以下;85.95%正与伴侣同居;93.78%居住在乡村;吸烟率和饮酒率分别为37.98%和32.85%。组间比较显示,ADL受限人群呈现独特的风险特征组合,女性比例(63.28%)、未受教育比例(36.56%)、独居比例(17.03%)及乡村居住比例(95.59%)均高于ADL不受限人群。同时ADL受限人群中波动显著型(20.84%)和缓升平稳型(47.69%)抑郁状态人群占比更高。除BMI和舒张压外,年龄、性别、受教育程度、与伴侣同居情况等变量在组间均存在显著差异(P<0.05)。
单因素logistic回归最终将年龄、性别、受教育程度、与伴侣同居情况、居住地、吸烟状况、饮酒状况、睡眠时长、收缩压和腰围纳入模型调整中,见表2,多模型回归分析具体的调整策略见表3。针对不同抑郁分组人群与ADL受限关系的多因素logistic回归分析显示,在所有模型中,与低度平稳型群体相比,波动显著型群体和缓升平稳型群体都与活动受限存在显著统计学差异(P<0.05)。其中,在模型3调整多个协变量后,与低度平稳型群体相比,波动显著型群体显示出ADL受限的风险显著增加(OR=7.75,95%CI: 6.54~9.17,P<0.05)。缓升平稳型群体(OR=2.86,95%CI:2.58~3.18,P<0.05)尽管其OR值低于波动显著型群体,在调整协变量后的模型中也显示出显著的统计学差。
本研究通过整合CHARLS数据库2011—2020年纵向数据,系统分析了抑郁状态变化对中老年人ADL的影响。研究结果不仅填补了现有研究在长期抑郁状态变化对ADL影响方面的空白,还深化了对抑郁症病程特征及其社会功能影响的理解。
本研究通过马尔科夫链分析发现,个体在10年间抑郁状态具有较高的稳定性,无论是无抑郁状态还是重度抑郁状态,个体倾向于保持其初始状态。这一发现与既往研究中关于抑郁症复发性和慢性化的特征相一致。抑郁症的长期稳定性可能与大脑功能网络的改变有关,比如默认模式网、认知控制网和前扣带网的功能失调[17]。而抑郁症的稳定性,或称为抑郁症的复发,并非普遍现象。研究指出,在一般人群中许多首次发作的个体可能不会再次复发,这与首次发作时识别复发风险重要相关[18]
然而,研究同时发现,从无抑郁到中度或重度抑郁的转移概率不容忽视,这提示抑郁症的发病并非一成不变,即使是当前无抑郁症状的个体,也可能随着时间推移发展为中重度抑郁。这突显了早期识别和干预的重要性,尤其是在中老年人群中,抑郁症的早期症状可能被忽视或误认为是正常的老化过程[17]
研究还发现抑郁状态的演变具有显著异质性,波动显著型群体的ADL受限风险是低度平稳型的7倍以上,并且高于缓升平稳型。这个发现符合抑郁严重程度与功能损伤呈非线性关联的假设[19-20],提示抑郁状态的波动性可能比绝对严重程度对功能损伤更具预测价值。先前的研究指出反复的抑郁波动会导致神经可塑性损伤累积,进而加剧前额叶-边缘系统的功能失调[21],这种动态病理机制或许解释了为何波动型群体在调整协变量后仍保持最高风险水平。
在探讨抑郁症与中老年人ADL受限的关系时,同样需要考虑多种潜在的影响因素。本研究中发现女性、独居和受教育程度低的人群在ADL受限人群中占比较高,而先前的研究指出了这些因素与抑郁风险的相关性[22-24]。女性可能因长期的社会角色和性别差异,在面对生活压力和健康问题时,更易出现情绪困扰和功能障碍[24]。独居者可能缺乏足够的社会互动和情感支持,从而在应对抑郁症状时面临更大挑战[22]。而受教育程度较低的个体可能在获取健康信息、利用医疗资源和进行自我管理方面存在困难,进而影响其对抑郁症状的应对和康复[23]。这些因素产生的挑战会削弱他们在日常生活中的自理能力,进而导致ADL受限。此外,本研究中大部分的参与者居住于乡村,农村地区有限的医疗资源和心理健康污名化可能加剧抑郁状态的不稳定性。这种现象在老龄化加速的中国农村尤为值得关注[25],因为传统家庭支持系统随人口流动正在弱化,而正式社会支持体系尚未完善。
本研究方法学上的创新为抑郁研究提供了新视角。通过整合马尔科夫链与LCA,既捕捉到状态转移的即时概率,又识别出长期演变模式,这种双维度分析框架克服了传统纵向研究将抑郁视为静态分类变量的局限。但是,CHARLS数据中抑郁评估采用CES-D-10量表,虽具良好效度[26],但无法替代临床诊断,可能低估双相障碍等具有情绪波动特征的疾病影响。此外,未纳入抗抑郁药物使用数据可能产生残留混杂,未来研究需结合相关数据进行更精确的暴露评估。
总而言之,本研究认为抑郁状态具有较高稳定性,但无抑郁者仍有向中重度发展的风险。抑郁动态变化对于抑郁人群的日常生活具有丰富的研究价值,抑郁状态波动更明显的群体活动受限风险更高。建议临床实践需重视抑郁状态的动态监测,针对波动显著型患者设计强化干预方案。
