Objective

To develop a quality assessment scale for children's musculoskeletal disorders health short videos and evaluate its reliability and validity.

Methods

The initial scale was developed through literature review, semi-structured interviews, and Delphi expert consultation. A total of 240 health short videos on children's musculoskeletal disorders were selected from Douyin as research subjects and evaluated by two raters. The 480 datasets were randomly divided into two groups for item analysis and reliability/validity testing.

Results

The scale consisted of 22 items across five dimensions (presentation literacy, creator authority, information comprehensibility, video production quality, and audience feedback). The item-level content validity index and scale-level content validity index were both 1.00. The scale demonstrated a Cronbach's α of 0.812,inter-rater reliability of 0.872, and test-retest reliability of 0.914. All items had factor loadings above 0.400, and the cumulative variance explanation rate of the five dimensions was 67.464%. Confirmatory factor analysis indicated good model fit.

Conclusion

The quality assessment scale for children's musculoskeletal disorders health short videos exhibits good reliability and validity, effectively evaluating the quality of relevant short videos.

目的

编制儿童肌肉骨骼疾病健康短视频质量评价量表，并检验其信效度。

方法

通过文献回顾、半结构式访谈以及德尔菲专家咨询形成量表初稿，于抖音上选取240份儿童肌肉骨骼疾病健康短视频作为研究对象，由两名评价人员对其进行评价。将480份数据随机分成两组，用于项目分析和信效度检验。

结果

量表包括五个维度，分别为演讲素养、创作者权威性、信息内容的易理解性、短视频制作质量及接收者的反馈，共22个条目。量表的条目级内容效度指数和量表级内容效度指数均为1.00。量表的Cronbachα系数为0.812，评价者间信度为0.872，重测信度为0.914。量表各条目的因子载荷在0.400以上，五个维度的累计方差解释率为67.464%，验证性因子分析显示模型拟合良好。

结论

儿童肌肉骨骼疾病健康短视频质量评价量表具有良好的信度和效度，可以有效地评估相关短视频的质量。

目的

编制儿童肌肉骨骼疾病健康短视频质量评价量表，并检验其信效度。

方法

通过文献回顾、半结构式访谈以及德尔菲专家咨询形成量表初稿，于抖音上选取240份儿童肌肉骨骼疾病健康短视频作为研究对象，由两名评价人员对其进行评价。将480份数据随机分成两组，用于项目分析和信效度检验。

结果

量表包括五个维度，分别为演讲素养、创作者权威性、信息内容的易理解性、短视频制作质量及接收者的反馈，共22个条目。量表的条目级内容效度指数和量表级内容效度指数均为1.00。量表的Cronbachα系数为0.812，评价者间信度为0.872，重测信度为0.914。量表各条目的因子载荷在0.400以上，五个维度的累计方差解释率为67.464%，验证性因子分析显示模型拟合良好。

结论

儿童肌肉骨骼疾病健康短视频质量评价量表具有良好的信度和效度，可以有效地评估相关短视频的质量。

Objective

To develop a quality assessment scale for children's musculoskeletal disorders health short videos and evaluate its reliability and validity.

Methods

The initial scale was developed through literature review, semi-structured interviews, and Delphi expert consultation. A total of 240 health short videos on children's musculoskeletal disorders were selected from Douyin as research subjects and evaluated by two raters. The 480 datasets were randomly divided into two groups for item analysis and reliability/validity testing.

Results

The scale consisted of 22 items across five dimensions (presentation literacy, creator authority, information comprehensibility, video production quality, and audience feedback). The item-level content validity index and scale-level content validity index were both 1.00. The scale demonstrated a Cronbach's α of 0.812,inter-rater reliability of 0.872, and test-retest reliability of 0.914. All items had factor loadings above 0.400, and the cumulative variance explanation rate of the five dimensions was 67.464%. Confirmatory factor analysis indicated good model fit.

Conclusion

The quality assessment scale for children's musculoskeletal disorders health short videos exhibits good reliability and validity, effectively evaluating the quality of relevant short videos.

肌肉骨骼疾病是导致全球残疾生命年和疼痛的首要原因[1]，大约每100 000名儿童中有3 105名存在肌肉骨骼问题[2]。这类疾病不仅影响儿童的身体健康，还可能对其心理健康和长期发展产生负面影响[3]，甚至增加未来患慢性疾病的风险[4]。面对子女的健康问题，父母常通过互联网搜索相关信息[5]。在这一背景下，在线健康信息的质量影响父母的决策，进而关系到儿童的健康状况。近年来，短视频已成为父母获取健康信息的重要渠道之一。当前健康短视频的质量参差不齐[6-7]，而父母往往缺乏辨别信息质量的能力[8]。虽然国内外学者常用DISCERN、JAMA等工具来评估健康短视频的质量，但这些工具并非为短视频设计的，目前仍缺乏健康短视频质量评价工具[9]。因此，开发儿童肌肉骨骼疾病健康短视频质量评价量表具有重要意义。这不仅能有效评估健康短视频的质量，还能帮助父母获取更符合他们需求的健康信息，从而更好地保障儿童健康。
1　资料与方法

1.1　量表条目池的构建
1.1.1　文献检索
使用“网络”“健康信息”“质量”“评估工具”“web”“health information”“quality”“evaluation tools”等作为检索词，在中国知网、万方数据知识服务平台、维普中文科技期刊全文数据库、中国生物医学文献数据库、PubMed、Web of Science、CINAHL、 Embase、 the Cochrane Library、 PsycINFO、Scopus进行系统的检索，检索时间为数据库建库至2023年10月。文献检索共获得36 672篇文献，最终纳入3篇关于健康短视频质量影响因素的研究、384篇关于健康短视频质量评价的研究以及19篇关于健康信息质量评估工具开发与验证的研究。

为让量表更适合肌肉骨骼疾病患儿父母使用，本研究于2023年12月—2024年2月对其进行半结构式访谈，以了解他们对健康短视频的看法，并将可能的指标纳入量表条目池。样本量以访谈资料达到饱和为标准，最终纳入14名父母，父母的平均年龄为35.64岁，男女各为7名，学历跨度为初中至硕士。采用内容分析法[10]对访谈资料进行编码、分析、总结、归纳。
本研究结合文献回顾和半结构式访谈的结果，形成5个一级维度，12个二级维度以及59个条目的条目池。
1.2　德尔菲专家咨询
德尔菲法是指通过多轮匿名函询的方式收集并征求专家意见[11]。通过设计德尔菲专家咨询问卷，于2024年6—8月邀请专家采用Likert 5级评分法对维度和条目的重要性进行打分，同时邀请专家提供有关修改、删除或添加条目的反馈。专家遴选标准如下：本科及以上学历；中级及以上职称；在健康教育/健康科普方面有五年以上的工作经验；愿意参加多轮咨询。
删除重要性赋值均数< 4.00或满分比< 20.00%或变异系数>0.25的条目，并结合专家意见对量表的维度和条目进行删除、修改和补充。第一轮专家咨询邀请22名专家进行填写问卷，其中20名专家进行有效回应，专家判断系数Ca=0.95，专家熟悉系数Cs=0.81，专家权威系数Cr=0.88。他们从事临床护理、临床医学、健康科普等领域9～31年。第一轮专家函询后形成5个一级维度，9个二级维度和42个条目。在第二轮专家咨询中，仅邀请在第一轮中提供有效答复的专家进行咨询，17名专家进行有效回复。在第二轮专家函询中有三个条目予以删除，两个条目合并成一个条目，并结合专家意见对量表维度和条目进行修改，最终形成5个一级维度，9个二级维度和38个条目的初始量表。
1.3　预调查
采取目的抽样对20名肌肉骨骼疾病患儿父母进行预调查，要求调查对象对量表的可理解性、适用性、全面性进行评价。20名父母的年龄在（33.85±4.53）（28～44）岁之间，其中13名女性，7名男性，50%的父母具有高中以上文化水平。结果表明，问卷条目清晰、明确且易于理解，无需修改。
本研究获得南方医科大学第三附属医院伦理委员会批准（批号：2023-伦审-099），所有研究对象均知情同意。
1.4　正式调查
1.4.1　研究工具
儿童肌肉骨骼疾病健康短视频质量评价量表初稿包含38个条目，每个条目采用5分评分法（1=非常不同意，5=非常同意）以及一个额外的“不适用（NA）”，其中条目13为反向计分项目。
1.4.2　研究对象
在抖音搜索“儿童脊柱侧弯”“儿童髋关节发育不良”“儿童马蹄足”“儿童斜颈”，按照综合排序的方式，每个疾病纳入符合标准的60份短视频。纳入标准：与疾病健康知识相关的视频；短视频中至少包括一个操作建议。排除标准：无配音且无字幕的视频；重复视频。
1.4.3　资料收集方法
由两名评价人员独立对240份短视频进行评价。在评价之前，对两名评价人员进行量表使用的统一培训。将480份数据随机分成两组，一组用于项目分析，另一组用于验证性因子分析（confirmative factor analysis, CFA）和信度检验。在初始测试约2周后，再次评价60个“儿童脊柱侧弯”短视频，以评估重测信度。
1.4.4　项目分析
（1）临界比值法：将量表总分从高到低排序，提取最高的27%为第一组，最低的27%为第二组。对这两组进行独立样本t检验，删除两组之间没有统计学差异的条目（P>0.05）[12]。
（2）项-总相关系数法：删除项目与总分之间的相关系数（r）< 0.20或没有统计学差异的条目（P>0.05）[13]。
（3）Cronbach α系数法：删除后导致Cronbach α系数增加的条目被删除[14]。
（4）因子分析：Kaiser-Meyer-Olkin（KMO）值 >0.600，Bartlett球型检验具有统计学意义（P<0.05），表明可进行探索性因子分析（exploratory factor analysis,EFA）[15]。使用主成分分析进行因子提取，最大方差法进行旋转，结合特征值（>1）和碎石图来提取公因子数，删除旋转后的因子载荷 <0.400、在多个因子上的因子载荷 ≥0.400以及分类不当的条目[16]，每个因子下至少要保留三个条目[17]。
1.4.5　效度分析
内容效度由条目级内容效度指数（I-CVI）和量表级内容效度指数（S-CVI/Ave）计算。I-CVI>0.78，S-CVI/Ave>0.90认为内容效度可接受[14]。
使用CFA来验证EFA得出的结果。当χ2/v< 3.000，近似均方根误差（root mean square error of approximation, RMSEA）和均方根残差（root mean square residual, RMR）≤ 0.080，增量拟合指数（incremental fit index, IFI）、Tucker-Lewis指数（TLI）和比较拟合指数（comparative fit index, CFI）≥0.900认为模型拟合较好[18]。当平均方差抽取量（average variance extracted, AVE）>0.500且组合信度（composite reliability, CR）>0.700认为具有良好的聚合效度；各因子之间相关系数的绝对值小于AVE的平方根，认为具有良好的区分效度[18]。
1.4.6　量表的信度分析
使用内部一致性、评价者间信度和重测信度来检验评估工具的信度。计算Cronbach α系数来评估内部一致性，组内相关系数（intraclass correlation coeffcient, ICC）来评估评价者间信度和重测信度。Cronbach α和ICC>0.700认为可以接受[19]。
1.4.7　数据分析
使用IBM SPSS Statistics 25.0版进行项目分析、信度检验，使用IBM SPSS Amos 28.0版进行CFA。
2　结果

2.1　研究对象的特征
240份健康短视频的时长从11～315 s不等，点赞数从4～92 000不等。纳入的健康短视频特征见表1。
2.2　项目分析结果
临界比值法显示，条目10无法计算，条目11、条目21以及条目28没有统计学意义。项-总相关系数法显示，条目10无法计算，条目11、条目13、条目24、条目25、条目27以及条目28与量表总分的相关系数（r）<0.20。删除条目24、条目25、条目27和条目29后，样本的Cronbach α系数有所升高。故剩余29个条目进行EFA。
2.2.1　探索性因子分析结果
29个项目的KMO值为0.712，Bartlett球型检验具有统计学意义，表明适合进行因子分析。条目14、条目19、条目20以及条目26的因子载荷<0.400，故删除。条目12与其他条目不能归类到同一维度下，故删除。公因子6下只有条目22和条目23，故删除这一维度。最终的因子分析中，22个项目的KMO值为0.743，Bartlett球型检验具有统计学意义，提取出5个公因子，累计方差解释为67.464%，每个条目的因子载荷>0.400，EFA结果见表2。
2.3　效度分析
2.3.1　内容效度
六名专家采用Likert 4级评分法对量表条目与维度的相关性进行评价，其评价均为“3”或“4”，故I-CVI为1.00，S-CVI/Ave为1.00。
2.3.2　验证性因子分析
对22个条目进行CFA。根据修饰指数对模型进行调整，χ2/v=2.220，RMSEA=0.071，RMR=0.023，IFI=0.929，TLI=0.916，CFI=0.928。
2.3.3　聚合效度和区分效度
演讲素养、创作者权威性、信息内容的易理解性、短视频制作质量及接收者的反馈五个维度的AVE值分别为0.597、0.460、0.621、0.692、0.581，CR值分别为0.880、0.722、0.860、0.930、0.802，AVE的平方根分别为0.773、0.678、0.788、0.832、0.762，维度之间相关系数的绝对值<AVE的平方根。
2.3.4　信度
22个项目量表的总体Cronbach α的值为0.812，演讲素养、创作者权威性、信息内容的易理解性、短视频制作质量及接收者的反馈五个维度的Cronbach α分别为0.878、0.688、0.872、0.930、0.775。评价者间信度为0.872，重测信度为0.914。
3　讨论

3.1　量表编制过程的科学性
本研究遵循量表开发的原则[20]，结合文献回顾和访谈的结果，开发儿童肌肉骨骼疾病健康短视频质量评价量表。在条目池的构建阶段，纳入父母和专家的意见是有必要的，因为公众评估健康信息的方式与专业人员有所不同。公众主要依赖可导航性、美学和易理解性等指标来评估在线健康信息的质量，较少依赖内容全面性等指标[21]。而专业人员会基于医学指南或教科书构建的工具来评估内容的准确性和完整性[22]。若仅基于专家意见来构建量表，可能无法满足公众的需求，故本研究纳入父母和专家的意见，同时考虑量表的适用性和科学性。本研究纳入的专家来自护理、临床、健康科普等领域，在其专业上有一定的代表性，保证量表条目的可靠性。
3.2　量表信效度的可靠性
信度是用于衡量量表测量结果的一致性，稳定性和可靠性的指标[23]。本研究使用内部一致性，评价者间信度以及重测信度来衡量儿童肌肉骨骼疾病健康短视频质量评价量表的信度。其Cronbach α和ICC均 >0.700，表示其一致性和稳定性较好，且不同评价者之间的结果一致性较高[19]。效度是指评估工具能够准确测量目标结构的程度[23]。本研究从内容效度、结构效度、聚合效度和区分效度四个方面对量表的效度进行检验。I-CVI和S-CVI/Ave均为1.00，表明量表具有良好的内容效度。量表的结构效度通过EFA和CFA来检验。EFA旨在识别最少数量的维度，以简洁地解释变量之间的关系，是心理学理论和测量开发与验证的基本方法[24]。CFA则是验证假设的测量模型与实际观测数据的拟合程度[25]。各条目的因子载荷在0.400以上，五个维度的累计方差解释率为67.464%，CFA的拟合指数均在可接受的范围内，模型拟合度较好，证明量表具有良好的结构效度。此外，量表的聚合效度和区分效度可接受，故儿童肌肉骨骼疾病健康短视频质量评价量表的效度较好。
本研究经文献回顾、半结构式访谈、专家咨询、项目分析及信效度检验后，最终形成儿童肌肉骨骼疾病健康短视频质量评价量表，包括五个维度（演讲素养、创作者权威性、信息内容的易理解性、短视频制作质量及接收者的反馈），共22个条目，以评估相关短视频的质量，为健康短视频质量评价提供参考。但本研究也存在一定的局限性：首先该量表是基于肌肉骨骼疾病患儿父母和相关健康短视频建立的，能否适用于其他领域还有待验证；其次本研究只纳入父母和健康教育领域专家的意见，并未纳入其他领域的专家，未来需与其他领域的专家共同探索健康短视频的质量研究，以充分评估其质量；最后，该评估工具无法评估健康短视频内容的正确性，后续应结合医学指南来评估。
