Objective

To understand the aging trend and some characteristics of people living with HIV who first received antiretroviral therapy (ART) in Guizhou province from 2005 to 2022, and to provide reference for the development of follow-up management measures for the elderly population.

Methods

The baseline characteristics of HIV/AIDS patients receiving ART in Guizhou province from 2005 to 2022 were collected from the National HIV/AIDS Antiretroviral Treatment Information System. Joinpoint regression was used to analyze the trends of baseline gender, age, transmission route, time from diagnosis to treatment initiation, and CD4+T lymphocyte (CD4+) level.

Results

From 2005 to 2022, a total of 29 366 elderly patients (aged≥50 years) in Guizhou Province received ART treatment for the first time. The proportion of elderly men receiving ART showed a decreasing trend from 2007 to 2022 (APC=-1.30%, P<0.001). The proportion of elderly patients with married status and having a spouse decreased (APC=-2.44%, P<0.001). The proportion of elderly patients with infection through heterosexual transmission reached a turning point in 2007, and it showed a rapid upward trend from 2005 to 2007 (APC=264.70%, P<0.001). The proportion of elderly patients with a one-to-two-year time interval from diagnosis to starting treatment decreased from 2005 to 2007 (APC=-10.22%, P<0.001), and then showed an upward trend from 2007 to 2022 (APC=1.29%, P<0.001). The proportion of elderly patients with baseline CD4+ level≤200 showed a turning point in 2013 [95% CI: 2008-2018], and it decreased from 2005 to 2013 (APC=-8.11%, P<0.001).

Conclusion

From 2005 to 2022, the elderly patients who first received ART in Guizhou province were mainly male, married, heterosexual transmission, 1-2 years from diagnosis to treatment, and baseline CD4+ level ≤200, and the proportion of elderly patients was on the rise. Follow-up management strategies of ART should be developed for the elderly population to improve the quality of ART.

目的

了解2005—2022年贵州省每年首次接受抗病毒治疗(ART)的 HIV 感染者中老龄化情况及部分特征变化趋势,为制定老年人群的艾滋病抗病毒治疗随访管理措施提供参考依据。

方法

从国家艾滋病抗病毒治疗信息系统收集 2005—2022年在贵州省接受ART 的感染者的基线特征,通过 Joinpoint 回归分析基线性别、年龄、感染途径、确诊到开始治疗时间、CD4+T淋巴细胞(简称CD4+)水平的趋势变化。

结果

2005—2022年贵州省共29 366例老年(≥50岁)老年患者首次接受ART治疗。接受ART的老年男性构成比在2007—2022年呈下降趋势(APC=-1.30%,P<0.001)。接受ART的老年患者婚姻状况为已婚有配偶的构成比呈下降趋势(APC=-2.44%,P<0.001)。老年患者感染途径为异性性传播构成比趋势变化转折点出现在2007年,2005—2007年呈快速上升趋势(APC=264.70%,P<0.001)。老年患者从确诊到开始治疗时间为1~2年的构成比在2005—2007年呈下降趋势(APC=-10.22%,P<0.001),2007—2022年呈上升趋势(APC=1.29%,P<0.001)。老年患者基线CD4+水平≤200的构成比趋势变化转折点出现在2013年[95% CI:2008~2018],2005—2013年呈下降趋势(APC=-8.11%,P<0.001)。

结论

2005—2022年贵州省每年首次接受抗病毒治疗的老年患者主要以男性、已婚有配偶、异性性传播、确诊到开始治疗时间为1~2年、基线CD4+水平≤200为主,且老年构成比呈上升趋势,应针对老年人群制定抗病毒治疗随访管理策略,提升抗病毒治疗质量。

目的

了解2005—2022年贵州省每年首次接受抗病毒治疗(ART)的 HIV 感染者中老龄化情况及部分特征变化趋势,为制定老年人群的艾滋病抗病毒治疗随访管理措施提供参考依据。

方法

从国家艾滋病抗病毒治疗信息系统收集 2005—2022年在贵州省接受ART 的感染者的基线特征,通过 Joinpoint 回归分析基线性别、年龄、感染途径、确诊到开始治疗时间、CD4+T淋巴细胞(简称CD4+)水平的趋势变化。

结果

2005—2022年贵州省共29 366例老年(≥50岁)老年患者首次接受ART治疗。接受ART的老年男性构成比在2007—2022年呈下降趋势(APC=-1.30%,P<0.001)。接受ART的老年患者婚姻状况为已婚有配偶的构成比呈下降趋势(APC=-2.44%,P<0.001)。老年患者感染途径为异性性传播构成比趋势变化转折点出现在2007年,2005—2007年呈快速上升趋势(APC=264.70%,P<0.001)。老年患者从确诊到开始治疗时间为1~2年的构成比在2005—2007年呈下降趋势(APC=-10.22%,P<0.001),2007—2022年呈上升趋势(APC=1.29%,P<0.001)。老年患者基线CD4+水平≤200的构成比趋势变化转折点出现在2013年[95% CI:2008~2018],2005—2013年呈下降趋势(APC=-8.11%,P<0.001)。

结论

2005—2022年贵州省每年首次接受抗病毒治疗的老年患者主要以男性、已婚有配偶、异性性传播、确诊到开始治疗时间为1~2年、基线CD4+水平≤200为主,且老年构成比呈上升趋势,应针对老年人群制定抗病毒治疗随访管理策略,提升抗病毒治疗质量。

Objective

To understand the aging trend and some characteristics of people living with HIV who first received antiretroviral therapy (ART) in Guizhou province from 2005 to 2022, and to provide reference for the development of follow-up management measures for the elderly population.

Methods

The baseline characteristics of HIV/AIDS patients receiving ART in Guizhou province from 2005 to 2022 were collected from the National HIV/AIDS Antiretroviral Treatment Information System. Joinpoint regression was used to analyze the trends of baseline gender, age, transmission route, time from diagnosis to treatment initiation, and CD4+T lymphocyte (CD4+) level.

Results

From 2005 to 2022, a total of 29 366 elderly patients (aged≥50 years) in Guizhou Province received ART treatment for the first time. The proportion of elderly men receiving ART showed a decreasing trend from 2007 to 2022 (APC=-1.30%, P<0.001). The proportion of elderly patients with married status and having a spouse decreased (APC=-2.44%, P<0.001). The proportion of elderly patients with infection through heterosexual transmission reached a turning point in 2007, and it showed a rapid upward trend from 2005 to 2007 (APC=264.70%, P<0.001). The proportion of elderly patients with a one-to-two-year time interval from diagnosis to starting treatment decreased from 2005 to 2007 (APC=-10.22%, P<0.001), and then showed an upward trend from 2007 to 2022 (APC=1.29%, P<0.001). The proportion of elderly patients with baseline CD4+ level≤200 showed a turning point in 2013 [95% CI: 2008-2018], and it decreased from 2005 to 2013 (APC=-8.11%, P<0.001).

Conclusion

From 2005 to 2022, the elderly patients who first received ART in Guizhou province were mainly male, married, heterosexual transmission, 1-2 years from diagnosis to treatment, and baseline CD4+ level ≤200, and the proportion of elderly patients was on the rise. Follow-up management strategies of ART should be developed for the elderly population to improve the quality of ART.

近年来,随着老龄化趋势的发展,艾滋病病毒感染者/艾滋病病人(简称HIV/AIDS病人)老龄化的问题也越来越严峻[1]。“十三五”规划将≥50岁人群列为艾滋病防控的重点人群[2]。2005年贵州省正式启动抗病毒治疗(ART)工作,每年新入组治疗患者不断增加[3]。本研究采用Joinpoint回归模型分析2005—2022年贵州省每年首次接受ART的老年HIV感染者的年龄、性别、基线CD4+T淋巴细胞水平等不同特征的构成比变化趋势,为针对老年人群的艾滋病防控及抗病毒治疗随访管理提供科学依据。
2005年1月1日—2022年12月31日在贵州省各级抗病毒治疗定点医疗机构首次接受ART的HIV/AIDS患者,排除基线年龄缺失者。从国家艾滋病抗病毒治疗信息系统收集感染者基线治疗信息,包括年龄、性别、感染途径、CD4+水平等基本特征。年龄≥50岁定义为老年HIV/AIDS病人[4]。本研究经贵州省疾病预防控制中心伦理委员会批准(编号:W2023-22)。
将数据导入Excel建立2005—2022年时间序列数据“.CSV”文件,采用R 4.3.1进行Lilliefors修正K-S(统计量Z)正态性检验,组间比较使用卡方检验。设yi为因变量构成比,用百分数表示,xi为自变量年度,年龄、性别、感染途径、确诊到开始抗病毒治疗时间、CD4+水平为分组变量。建立系列观察值(xiyi),其中,i=1~nxi≤…≤xn;使用JPR软件拟合Joinpoint回归模型,计算回归系数(β0,Slope/β1δk)、95%置信区间(confidence interval,CI)、年度变化百分比(annual percent change,APC)。模型分为两类:
如果数据服从正态分布,则采用线性JPR模型(1),表达式:
如果服从泊松和指数分布,则选用对数线性模型(2),表达式:
式中,β0表示不变参数,β1表示斜率参数(回归系数);δk=δn+1-βn表示分段函数的回归系数;τk为未知转折点,k为转折点个数,当(xi-τk)>0时,(xi-τk+=(xi-τk),否则(xi-τk+=0。若分布不明确,则分别拟合线性模型和对数线性模型,再比较二者的均方差(mean squared errors,MSE)大小,最后选择MSE较小者拟合模型[5]。采用网格搜索法(grid search method,GSM)确定转折点数量、位置及模型参数,采用Monte Carlo置换检验(permutation test,PT)和修正贝叶斯信息准则(modified Bayesian information criterion,MBIC)选择最佳模型,MBIC值最小时的模型即为最优模型[5]。检验水准α=0.05,双侧检验。
本研究纳入老年患者(≥50岁)HIV/AIDS患者29 366例,其中男性19 465例(66.28%),已婚有配偶18 390例(62.62%),异性性传播27 665例(94.21%),确诊到开始治疗时间为1~2年27 105例(92.30%),基线CD4+水平≤200(个/μl)19 228例(65.48%)。
2005—2022年每年首次接受ART的老年患者性别构成比不服从正态分布(Z=0.263,P<0.001)。男性构成比趋势变化转折点出现在2007年(95% CI:2007~2017),回归方程:Eyi|xi]=e-140.38+0.07xi-0.09(xi-2007)+,MSE=0.00。2005—2007年构成比变化趋势不明显(APC=7.48%,P>0.05),2007—2022年呈下降趋势(APC=-1.30%,P<0.001)。女性构成比趋势变化转折点出现在2009年(95% CI:2007~2010)和2012年(95% CI:2011~2019),回归方程:Eyi|xi]=e256.01-0.13xi+0.29(xi-2009)+-0.15(xi-2012)+,MSE=0.02。2005年—2009年女性构成比呈下降趋势(APC=-11.83%,P<0.001),2009年—2012年呈上升趋势(APC=17.30%,P<0.001),2012年—2022年构成比变化趋势不明显(APC=1.14%,P>0.05)。(图1
2005—2022年每年首次接受ART的老年患者婚姻状况构成比不服从正态分布(Z=0.191,P<0.001)。婚姻状况为未婚的构成比趋势变化转折点出现在2010年(95% CI:2007~2017),回归方程:Eyi|xi]=e610.46-0.30xi+0.48(xi-2010)+,MSE=0.34。2005—2010年未婚老年感染者构成比呈下降趋势(APC=-26.22%,P<0.001),2010—2022年呈上升趋势(APC=19.79%,P<0.001)。婚姻状况为已婚有配偶的构成比呈下降趋势(APC=-2.44%,P<0.001),回归方程:Eyi|xi]=e53.95-0.02xi,MSE=0.01。婚姻状况为离异或丧偶及不详的构成比趋势变化转折点出现在2007年(95% CI:2007~2019),回归方程:Eyi|xi]=e-1 618.62+0.81xi-0.76(xi-2007)+,MSE=0.34。2005—2007年呈快速上升趋势(APC=124.34%,P<0.001),2007—2022年呈缓慢上升趋势(APC=4.56%,P<0.001)。(图2
2005—2022年每年首次接受ART的老年患者感染途径构成比不服从正态分布(Z=0.297,P<0.001)。异性性传播构成比趋势变化转折点出现在2007年(95% CI:2007~2008),回归方程:Eyi|xi]=e-2 592.54+1.29xi-1.23(xi-2007)+,MSE=0.11。2005—2007年异性性传播构成比呈快速上升趋势(APC=264.70%,P<0.001),2007—2022年呈缓慢上变换趋势无统计学意义(APC=1.98%,P>0.05)。同性性传播构成比趋势变化转折点出现在2007年(95% CI:2007~2007)、2010年(95% CI:2010~2011)和2017年(95% CI:2013~2020),回归方程:Eyi|xi]=e1002.88-0.50xi+1.17(xi-2007)+-0.99(xi-2010)++0.25(xi-2017)+,MSE=0.09。2005—2007年同性性传播构成比呈下降趋势(APC=-39.33%,P<0.001),2007—2010年呈上升趋势(APC=94.55%,P<0.001),2010—2017年呈下降趋势(APC=-27.42%,P<0.001),2017—2022年变化趋势无统计学意义(APC=-6.89%,P>0.05)。注射吸毒及其他构成比趋势变化转折点出现在2011年(95% CI:2007~2020),回归方程:Eyi|xi]=e673.98-0.33xi+0.21(xi-2011)+,MSE=0.13。但2005—2011年、2011—2022年2个区段的趋势变化均无统计学意义(P>0.05)。(图3
2005—2022年每年首次接受ART的老年患者确诊到治疗时间的构成比不服从正态分布(Z=0.333,P<0.001)。确诊到开始治疗时间<1年的构成比趋势变化转折点出现在2008年(95% CI:2007~2012),回归方程:Eyi|xi]=e-870.87+0.44xi-0.56(xi-2008)+,MSE=0.10。2005—2008年呈上升趋势(APC=54.52%,P<0.001),2008—2022年呈下降趋势(APC=-12.11%,P<0.001)。确诊到开始治疗时间为1~2年的构成比趋势变化转折点出现在2007年(95% CI:2007~2008),回归方程:Eyi|xi]=e-220.68-0.11xi+0.12(xi-2007)+,MSE=0.00。2005—2007年呈下降趋势(APC=-10.22%,P<0.001),2007—2022年呈上升趋势(APC=1.29%,P<0.001)。确诊到开始治疗时间>2年的构成比在2005—2022变化趋势无统计学意义(APC=-1.85%,P>0.05),回归方程:Eyi|xi]=e38.64-0.02xi,MSE=10.43。(图4
2005—2022年每年首次接受ART的老年患者基线CD4+水平构成比不服从正态分布(Z=0.217,P<0.001)。基线CD4+水平>200的构成比趋势变化转折点出现在2011年[95% CI:2008~2015],回归方程:Eyi|xi]=e-1132.31+0.56xi-0.59(xi-2011)+,MSE=0.39。2005—2011年呈上升趋势(APC=75.93%,P<0.001),2011—2022年变化趋势无统计学意义(APC=-2.81%,P>0.05)。基线CD4+水平≤200的构成比趋势变化转折点出现在2013年[95% CI:2008~2018] ,回归方程:Eyi|xi]=e174.26+0.08xi+0.11(xi-2013)+,MSE=0.02。2005—2013年呈下降趋势(APC=-8.11%,P<0.001),2013—2022年变化趋势无统计学意义(APC=2.99%,P>0.05)。基线CD4+水平为不详的构成比变化趋势无统计学意义(P>0.05),回归方程:Eyi|xi]=e143.10-0.07xi,MSE=1.66。(图5
近年来,全球老年人艾滋病所占比例持续上升,由2000年的8%上升到2019年的21%[6],我国老年HIV感染人数也在不断增加[7]。有研究显示,贵州省新报告50岁及以上HIV/AIDS呈逐年增加的趋势,病例报告中≥50岁病例的构成比从2010年的12.6%上升到2018年的48.1%[8]。贵州省每年首次接受ART的老年患者总体呈持续上升趋势,这可能与每年老年HIV感染者新报告人数的增加以及国家不断放宽抗病毒治疗CD4+水平的限制有关[9]。2005年贵州省正式启动抗病毒治疗工作,此后根据国家治疗标准进一步扩大治疗,2003年主要纳入治疗范围的人群为CD4+水平<200的患者,2012年CD4+水平<350即开始治疗,2014年的标准中CD4+水平<500即纳入治疗,2015年的修订建议部分>500的患者也应接受治疗,2016年后采取“发现即治疗”的策略[10],预期未来接受ART的老年患者将会持续增加。
本研究发现,2005-2022年贵州省每年开始ART的老年患者以男性为主,但在2007年后接受治疗的老年男性占比呈下降趋势,而在2007年以后接受治疗的老年女性占比呈上升趋势,这可能与近年来随着检测力度的加强,更多的老年女性患者被发现纳入治疗有关。2005-2022年贵州省接受治疗的老年患者婚姻状况主要以已婚有配偶为主,但占比呈下降趋势,而离异或丧偶及不详的老年患者占比呈上升趋势。感染途径以异性性传播为主,且占比呈上升趋势。有研究表明,在老年患者中主要以婚外异性性行为感染为主,并且这一构成始终保持较高水平[11]。提示应提升老年人群对艾滋病的认识,增强发生性行为时的自我防护意识,提高安全套使用率,降低HIV传播的风险。
越早启动治疗,病毒抑制成功率越高[12],这可能是因为HIV感染者在确诊后启动治疗越快,越能抑制病毒复制、阻止疾病的发展[13]。本研究发现,贵州省每年首次接受ART的老年患者从确诊到开始治疗时间主要以1~2年为主,且占比呈上升趋势,而<1年的占比呈下降趋势,提示需加强老年感染者的抗病毒治疗宣传教育,使确诊的HIV感染者认识到延迟治疗对个体带来的危害,了解尽快启动治疗对身体健康的益处,并尽量动员患者尽快启动治疗。每年开始ART的老年感染者中,首次CD4+水平主要以≤200为主,且构成比在2013—2022年呈上升趋势。2003年我国开始启动ART策略,早期感染者确诊时多为AIDS患者,身体健康状况较差,基线CD4+水平≤200的患者构成比较高。晚发现是指感染者首次HIV检测阳性时CD4+水平≤200,这是当前我国艾滋病防治工作面临的难题[14]。老年患者对艾滋病的知晓率普遍较低[15],自愿咨询检测意愿不强,大部分被发现时CD4+水平处于低水平,患者刚开始接受ART时免疫状态更差、病情更严重[16]。ART是当前对抗病毒感染的重要手段,但若延迟发现并延误治疗,就会错过最佳治疗时机,病毒清除效果差,甚至导致治疗失败的增加[17]。为了确保抗病毒治疗的效果,需积极推进老年HIV常规检测工作,加强老年患者的抗病毒治疗健康教育,提高管理的规范性,控制病情进展。
综上所述,近年来贵州省每年首次接受ART的感染者中,老年患者构成比呈持续上升趋势,以男性、已婚有配偶、异性性传播感染为主,确诊到开始治疗时间为1~2年、基线CD4+水平≤200的构成比呈上升趋势。因此,建议积极开展老年人艾滋病防治的宣传教育,提高老年人群的自我防范意识;增加老年人获得咨询检测服务的可及性,实施早发现、早治疗,控制艾滋病在老年人群中的蔓延。
