Objective

To investigate the association between the cardiometabolic index (CMI) and diabetes risk among hypertensive patients.

Methods

Utilizing health examination data from the National Basic Public Health Service Project in Jianye District, Nanjing (2019—2023), CMI was calculated basing on triglycerides, high-density lipoprotein cholesterol, waist circumference, and height. Diabetes was diagnosed according to the National Basic Diabetes Prevention and Management Guidelines (2022). Multivariable logistic regression models were applied to evaluate the association between CMI and diabetes risk, with odds ratios(ORs) and 95% confidence intervals(CIs) calculated. Restricted cubic spline analysis was used to explore the dose-response relationship. Subgroup analyses were performed to assess effect modifications.

Results

This study included 56 760 hypertensive patients, with a final effective sample of 28 665 after excluding those with missing CMI-related indicators, of whom 7 535 had diabetes. Compared to those with lower CMI, hypertensive patients with higher CMI had significantly increased diabetes risk (P<0.001). Logistic analysis showed the highest CMI group (Q4) had a 40% higher diabetes risk than the lowest group (Q1) (OR=1.40, 95% CI: 1.30-1.52). Dose-response analysis revealed a nonlinear relationship (nonlinear P<0.001), indicating extreme CMI values might signal higher metabolic risks. Stratified analysis found a stronger association between CMI and diabetes risk in female hypertensive patients (OR=1.21, 95% CI: 1.16-1.27).

Conclusion

In Nanjing Jianye District residents, there is a significant positive correlation between CMI and diabetes risk in hypertensive patients, more pronounced in women. It is recommended to include CMI in metabolic risk assessment for community hypertensive patients, with a focus on personalized intervention for abdominal obesity and dyslipidemia in women.

目的

探讨高血压患者心脏代谢指数(Cardiometabolic Index,CMI)与糖尿病风险之间的关系。

方法

基于南京市建邺区国家基本公共卫生服务项目健康体检数据(2019—2023年),由甘油三酯、高密度脂蛋白胆固醇、腰围和身高四个指标计算高血压患者的CMI,依据《国家基层糖尿病防治管理指南(2022)》诊断标准判定糖尿病。通过多变量logistic回归分析,评估CMI与糖尿病之间的关联,计算比值比(Odds Ratio,OR)及其95%置信区间(Confidence Interval,CI)等指标。使用限制性立方样条曲线探索CMI与糖尿病之间的剂量-反应关系。并进行分层分析,进一步探讨不同亚组中的影响差异。

结果

本研究共纳入56 760名高血压患者,剔除计算CMI相关指标缺失的参与者后,最终有效样本28 665例,其中共有7 535名伴有糖尿病。与CMI较低者相比,CMI水平较高的高血压患者,患糖尿病风险显著增加(P<0.001)。Logistic分析结果显示,与最低四分位组(Q1)相比,高CMI组(Q4)的糖尿病风险增加了40%(OR=1.40,95% CI: 1.30~1.52)。在剂量-反应分析中,CMI与糖尿病风险呈非线性关系(非线性P<0.001),表明过高或过低的CMI值可能预示更高的代谢风险。进一步分层分析表明,女性高血压患者的CMI与糖尿病风险的关联效应更强(OR=1.21,95% CI: 1.16~1.27)。

结论

在南京市建邺区居民中,CMI与高血压患者的糖尿病风险之间存在显著正相关,此关联效应在女性群体中更为显著。建议将CMI纳入社区高血压患者代谢风险评估,重点关注女性及腹型肥胖、血脂异常的个性化干预。

基于南京市建邺区国家基本公共卫生服务项目健康体检数据(2019—2023年),由甘油三酯、高密度脂蛋白胆固醇、腰围和身高四个指标计算高血压患者的CMI,依据《国家基层糖尿病防治管理指南(2022)》诊断标准判定糖尿病。通过多变量logistic回归分析,评估CMI与糖尿病之间的关联,计算比值比(Odds Ratio,OR)及其95%置信区间(Confidence Interval,CI)等指标。使用限制性立方样条曲线探索CMI与糖尿病之间的剂量-反应关系。并进行分层分析,进一步探讨不同亚组中的影响差异。

结果

本研究共纳入56 760名高血压患者,剔除计算CMI相关指标缺失的参与者后,最终有效样本28 665例,其中共有7 535名伴有糖尿病。与CMI较低者相比,CMI水平较高的高血压患者,患糖尿病风险显著增加(P<0.001)。Logistic分析结果显示,与最低四分位组(Q1)相比,高CMI组(Q4)的糖尿病风险增加了40%(OR=1.40,95% CI: 1.30~1.52)。在剂量-反应分析中,CMI与糖尿病风险呈非线性关系(非线性P<0.001),表明过高或过低的CMI值可能预示更高的代谢风险。进一步分层分析表明,女性高血压患者的CMI与糖尿病风险的关联效应更强(OR=1.21,95% CI: 1.16~1.27)。

结论

在南京市建邺区居民中,CMI与高血压患者的糖尿病风险之间存在显著正相关,此关联效应在女性群体中更为显著。建议将CMI纳入社区高血压患者代谢风险评估,重点关注女性及腹型肥胖、血脂异常的个性化干预。

Objective

To investigate the association between the cardiometabolic index (CMI) and diabetes risk among hypertensive patients.

Methods

Utilizing health examination data from the National Basic Public Health Service Project in Jianye District, Nanjing (2019—2023), CMI was calculated basing on triglycerides, high-density lipoprotein cholesterol, waist circumference, and height. Diabetes was diagnosed according to the National Basic Diabetes Prevention and Management Guidelines (2022). Multivariable logistic regression models were applied to evaluate the association between CMI and diabetes risk, with odds ratios(ORs) and 95% confidence intervals(CIs) calculated. Restricted cubic spline analysis was used to explore the dose-response relationship. Subgroup analyses were performed to assess effect modifications.

Results

This study included 56 760 hypertensive patients, with a final effective sample of 28 665 after excluding those with missing CMI-related indicators, of whom 7 535 had diabetes. Compared to those with lower CMI, hypertensive patients with higher CMI had significantly increased diabetes risk (P<0.001). Logistic analysis showed the highest CMI group (Q4) had a 40% higher diabetes risk than the lowest group (Q1) (OR=1.40, 95% CI: 1.30-1.52). Dose-response analysis revealed a nonlinear relationship (nonlinear P<0.001), indicating extreme CMI values might signal higher metabolic risks. Stratified analysis found a stronger association between CMI and diabetes risk in female hypertensive patients (OR=1.21, 95% CI: 1.16-1.27).

Conclusion

In Nanjing Jianye District residents, there is a significant positive correlation between CMI and diabetes risk in hypertensive patients, more pronounced in women. It is recommended to include CMI in metabolic risk assessment for community hypertensive patients, with a focus on personalized intervention for abdominal obesity and dyslipidemia in women.

糖尿病作为一种常见的慢性非传染性疾病,在全球范围内呈现出高发态势。我国作为糖尿病大国,2024年成人糖尿病患者约为1.48亿,位居全球首位。随着人口老龄化趋势不断加剧,预计到2050年,该人数将增至1.68亿,给社会和家庭带来了沉重的疾病负担[1]。在众多与糖尿病相关的危险因素中,高血压与糖尿病的共患现象尤为突出。研究发现,在平均4.5年随访期间,有10.9%的高血压患者新诊断出糖尿病[2];与单纯高血压患者相比,合并糖尿病患者发生心血管事件的危险比(Relative Risk,RR)为2.07(95% CI: 1.86~2.30)[3],极大地增加了疾病负担。胰岛素抵抗(Insulin Resistance,IR)作为糖脂代谢异常的核心机制,既可以导致代偿性高胰岛素血症及β细胞功能衰竭,还可以引发血管收缩增强和钠潴留,在糖尿病与高血压的共病发展中起关键作用[4]。目前评价IR的金标准是高胰岛素-正葡萄糖钳夹技术,但是该方法昂贵费时,不适用于广泛的人群筛查和治疗监测[5]。在上述背景下,心脏代谢指数(Cardiometabolic Index,CMI)作为一种整合代谢健康状况和心血管疾病风险的新指标受到广泛关注。CMI结合了甘油三酯(Triglycerides,TG)、高密度脂蛋白胆固醇(High-Density Lipoprotein Cholesterol,HDL-C)、腰围和身高之比(Waist-to-Height Ratio,WHtR),不仅能通过TG/HDL-C值反映脂质代谢状态,还能通过WHtR精准评估内脏脂肪堆积[6]。相较于传统单一指标(如体重指数或腰围),CMI能更全面地反映胰岛素敏感性和潜在的代谢风险,且具有无创、低成本、易标准化测量的显著优势[7]。然而,现有研究多集中于CMI与心血管疾病的关联,缺少在高血压患者群体中预测糖尿病风险的系统研究,特别是通过CMI表征的IR状态与糖尿病共病发生之间的剂量-反应关系尚未明确。因此,为了探索高血压患者中CMI与糖尿病风险的关联,本研究基于南京市建邺区国家基本公共卫生服务项目高血压患者健康体检数据,分析CMI与糖尿病风险之间的关系,以及深入研究二者之间的剂量-反应关系,为高血压患者的糖尿病风险评估提供更为精准、全面的指标及理论依据,有利于优化临床及社区对高血压患者代谢风险的管理策略。
数据源于2019—2023年南京市建邺区国家基本公共卫生服务项目高血压患者的健康体检资料,该项目适用于老年人、高血压、糖尿病患者等的年度健康检查,属于按照机构职能开展的常规工作范畴,因此无需经伦理审查。本研究初始纳入高血压患者共56 760名。在数据筛选过程中,依据计算CMI所需的指标完整性进行剔除(缺失腰围1 600名、身高1 384名、TG值27 755名、HDL值28 045名),经过严格的数据清洗与筛选,最终确定有效样本总计28 665例,确保了研究数据的完整性和可靠性。见图1
经过培训的调查员使用问卷开展面对面访问,收集的资料包括:①社会人口学特征:年龄、性别、文化程度、婚姻状况等;②生活方式:饮酒状况、吸烟状况、饮食状况、身体锻炼状况等;③常见慢性病史:糖尿病、高血压等。所有体格检查项目(身高、体重、血压等)由调查员按照标准化操作手册进行测量。
①CMI指数:,并根据CMI值,将参与者依据四分位数分为四个组[8];②高血压:参照《中国高血压防治指南(2024年修订版)》诊断标准[9],在未使用降压药的情况下,收缩压≥140 mmHg和(或)舒张压≥90 mmHg,或患者既往有高血压史,本研究高血压患者均经乡镇/社区及以上级别医院确诊;③吸烟状况:现在吸烟、已戒烟和从不吸烟;④饮酒状况:每天饮酒、经常饮酒、偶尔饮酒和从不饮酒;⑤饮食习惯:根据参与者的饮食偏好,将饮食习惯分为荤素均衡、荤食为主、素食为主;⑥锻炼频率:每天锻炼、每周锻炼(每周一次以上)、偶尔锻炼和从不锻炼。
本研究的结局为在横断面调查时是否患有糖尿病。糖尿病的诊断标准为:典型糖尿病症状加上随机血糖水平≥11.1 mmol/L,或加上空腹血糖水平≥7.0 mmol/L,或自我报告有糖尿病病史[10]
本研究使用R 4.2.0软件进行统计学分析。正态分布的连续变量采用表示,组间差异比较采用t检验;非正态分布的连续性变量采用中位数(四分位间距)表示,组间差异比较采用秩和检验;分类变量采用频数(构成比)表示,组间差异比较采用χ2检验或秩和检验。此外,采用χ2检验对不同CMI四分位组之间糖尿病患病率的差异进行单因素分析。为了评估CMI与高血压合并糖尿病的关联,采用logistic回归模型进行分析。模型1:调整年龄(连续)和性别(男性、女性);模型2:在模型1基础上调整文化程度(小学以下、小学或初中、高中及以上)、婚姻状况(已婚、其他)、吸烟状况(从不吸、曾经吸、现在吸)、饮酒状况(从不饮、偶尔饮、经常饮、每天饮)、饮食习惯(荤素均衡、荤食为主、素食为主)、锻炼频率(从不、偶尔、每周、每天)、体重;模型3在模型2基础上调整总胆固醇(Total Cholesterol,TC)和低密度脂蛋白胆固醇(Low-Density Lipoprotein,LDL)水平。各模型通过计算比值比(Odds Ratio,OR)及其95%置信区间(Confidence Interval,CI)来评估CMI对糖尿病的影响,并利用广义线性模型对CMI四分位组进行趋势分析。使用四分位数将CMI进行分组,采用logistic回归模型分析不同组间的关联,计算OR和95% CI。使用限制性立方样条(Restricted Cubic Spline,RCS)绘制CMI与糖尿病的剂量-反应关系曲线。分层分析基于模型3,进一步分析CMI对糖尿病的影响是否在不同亚组(如年龄、性别、饮酒状况、吸烟状况等)中有所不同。本研究采用双侧检验,显著性水平设定为α=0.05。
共纳入28 665名研究对象,其中女性占55.41%,参与者的平均年龄为68.14±8.70岁。结果显示,不同CMI水平组研究对象的年龄、性别、受教育水平、婚姻状况、吸烟状况、饮酒状况、锻炼频率、体重、TC、LDL的差异有统计学意义(均P<0.05)。与CMI水平较低组的研究对象相比,CMI水平越高,男性占比越多,女性占比越少,吸烟、饮酒、少锻炼者占比越多,体重、TC和LDL平均水平也越高。见表1。进一步分析发现,随着CMI水平的升高,糖尿病患病率呈现出逐渐增加的趋势,组间差异经χ2检验显示具有统计学意义(χ2=35.306,P<0.001)。见表2
使用三种不同的多变量logistic回归模型评估患高血压人群中,CMI与糖尿病风险之间的关系。模型3发现糖尿病风险随着CMI四分位数的增加而增加,与Q1组相比(趋势P值<0.001),Q2、Q3和Q4组的OR值分别为1.22(95% CI: 1.13~1.32)、1.28(95% CI: 1.18~1.38)和1.40(95% CI: 1.30~1.52),表明高血压患者中,较高CMI者合并糖尿病的比率高。见表3
经截断处理(去除头尾各0.5%极端值)后,RCS分析显示,在99%的高血压患者中,CMI与糖尿病风险呈现显著的非线性关联(非线性P<0.001),呈现先快速上升后平缓的剂量-反应特征(附RCS曲线及人群分布直方图,见图2)。
基于模型3发现,在高血压患者中,CMI与糖尿病风险之间存在着性别分组下的影响关系(交互作用P<0.001)。男性组中的OR为1.04(95% CI: 0.997~1.08),而女性组中的OR为1.21(95% CI: 1.16~1.27)。见表4
本研究基于南京市建邺区国家基本公共卫生服务项目高血压患者健康体检数据,系统探讨了CMI与糖尿病风险的关系。结果显示,CMI水平较高的高血压患者,其罹患糖尿病的风险增加,CMI与糖尿病风险之间存在非线性关系。分层分析结果显示,CMI与糖尿病风险的关联在女性高血压患者中更为明显。
高血压与糖尿病共病的现象极为普遍,大约50%的糖尿病患者患有高血压,大约20%的高血压患者患有糖尿病[11],在本研究中28 665名高血压患者就有26.29%患有糖尿病。从发病风险角度来看,相较于健康人群,已经患有高血压或糖尿病的个体,其后续再患上另一种疾病(糖尿病或高血压)的风险会增加1.5到2.0倍[11]。此外,合并高血压的糖尿病患者相较于血压正常的糖尿病患者,脑卒中等心血管事件的发生风险以及死亡风险均显著上升[12]。深入探究其原因,高血压与糖尿病的高共病率主要归因于两者存在IR这一共同病理机制。一是通过抑制磷脂酰肌醇3-激酶/蛋白激酶B信号通路减少外周组织葡萄糖摄取,导致代偿性高胰岛素血症及β细胞功能衰竭[13];二是激活肾素-血管紧张素系统与损伤内皮一氧化氮合酶功能,引发血管收缩增强和钠潴留,从而同步促进糖尿病发生与血压升高,在糖尿病与高血压的共病发展中起关键作用[4]。CMI作为一种整合WHtR和TG/HDL-C的综合代谢指标,能够更全面地反映高血压患者IR、脂肪分布异常以及相关代谢紊乱的程度[6]。WHtR作为反映腹型肥胖的敏感指标,与IR密切相关。有研究表明在预测糖尿病风险方面,WHtR(RR=1.68,95% CI: 1.52~1.84)优于腰围(RR=1.63,95% CI: 1.52~1.72)、腰臀比(RR=1.53,95% CI: 1.40~1.65)和体重指数(RR=1.54,95% CI: 1.49~1.60)[14]。TG/HDL-C值也是糖尿病发展的重要预测因子,国内一项纵向研究发现,高比值组的糖尿病风险是低比值组的2倍以上(HR=2.59,95% CI: 2.20~3.05)[15]。因此,CMI不仅能够反映单一代谢异常,还能捕捉这些异常的交互作用,从而更敏感地预测糖尿病风险,CMI预测糖尿病风险的潜力正在逐渐被证实[16]
本研究发现,调整混杂因素后,CMI与高血压患者糖尿病风险之间存在显著正相关,CMI水平较高的高血压患者(Q4组)的糖尿病风险较最低组(Q1组)增加了40%(OR=1.40,95% CI: 1.30~1.52)。该发现与一项基于中国健康与养老追踪调查数据的前瞻性纵向研究的结果相似,较高的CMI水平与新发糖尿病(HR=1.08,95% CI: 1.06~1.09,P<0.001)相关;与Q1组参与者相比,Q4组参与者患糖尿病的风险明显更高(HR=1.02,95% CI: 1.00~1.05,P=0.054)[17]。国外一项横断面研究,分析国家健康与营养检查调查的数据也有同样的发现,CMI和2型糖尿病之间的关联是正相关的(OR=2.01,95% CI: 1.82~2.22),即使根据四分位数对CMI进行分组时,无论协变量是否调整,这种明显的相关性仍然存在[18]。以上研究结果均可以表明,CMI水平与患糖尿病的风险呈正相关。
本研究的RCS分析结果显示,高血压患者中CMI与糖尿病风险的剂量-反应关系呈非线性关系(非线性P<0.001)。一项美国人群横断面研究同样观察到类似的非线性关系,该研究不仅观察到CMI与2型糖尿病的关系呈反L形(非线性P<0.05),同时还观察到CMI与IR的关系呈反L形(非线性P<0.05),并且预测到的转折点分别为1.30和1.35[18]。日本一项研究得到同样的结论,CMI低于1.01时,CMI与糖尿病发病率之间存在很强的正相关(HR=2.96,95% CI: 1.96~4.46,P<0.0001);当CMI高于1.01时,它们的相关性不显著(HR=1.27,95% CI: 0.98~1.64,P=0.0702)[16]。以上研究结果表明,CMI与糖尿病风险之间存在明显的阈值效应。高血压患者由于普遍存在慢性低度炎症[19],当CMI略有升高时,内脏脂肪堆积和脂质代谢紊乱会进一步激活炎症通路,促使炎症因子的释放增加,导致胰岛素信号通路受损,机体IR程度明显加重。随着CMI持续升高,相关的代谢损害及炎症反应趋于饱和,胰岛β细胞功能进一步恶化的空间有限[20],从而导致糖尿病风险增幅趋于平缓,形成“陡升-平台型”剂量-反应关系。因此,深入理解CMI与糖尿病之间的阈值效应,对于制定有效的预防和干预策略具有重要意义,就比如高血压人群对早期风险预测和干预具有更高的敏感性和临床价值。
此外我们的研究还发现,女性高血压患者中CMI与糖尿病风险的关联更显著(OR=1.21,95% CI: 1.16~1.27),这一结果可能与女性的代谢特点和激素水平变化有关。绝经后女性由于雌激素水平下降,内脏脂肪堆积和脂质代谢异常加剧,这些变化会显著加重IR[21]。有研究还表明,女性的腹部脂肪和炎症因子水平更容易受到内分泌变化的影响[22],从而加剧糖尿病风险。此外,日本一研究也指出,女性患者(HR=2.27,95% CI: 1.61~3.22)的CMI升高与糖尿病发生风险的关联比男性(HR=1.58,95% CI: 1.35~1.85)更强(交互作用P<0.001)[16]
本研究的优势在于基于社区人群大规模体检数据,能够较为全面地评估高血压患者CMI与糖尿病风险的关联。此外,通过使用RCS探讨剂量-反应关系,较好地揭示了CMI的复杂影响。当然,本研究也存在一定的局限性。首先,作为一项横断面研究,本研究无法明确因果关系。其次,CMI的测量数据可能存在测量偏倚,糖尿病的自报数据可能存在信息偏倚。此外,由于研究所用体检数据未包含糖尿病家族史信息,遗传因素未能纳入模型调整,可能影响部分个体的风险评估结果。饮食状况虽然被收集,但仅限于饮食习惯,未能反映更细化的营养摄入水平,对生活方式的控制存在一定局限。以上残留混杂因素均可能在一定程度上影响结果的解释力,后续研究中应通过更全面的数据采集加以改进。
综上所述,本研究显示在高血压患者中,CMI与糖尿病风险呈正相关,并且CMI与糖尿病风险呈非线性,CMI与糖尿病的关联在女性患者中更明显。因此,在高血压患者中,应加强对CMI的监测与控制,着重关注女性患者,并采取针对性干预措施,包括减轻腹型肥胖、优化血脂水平及健康生活方式的促进。
