Objective

To analyze the changing trends of urolithiasis burden among Chinese young and middle-aged adults from 1990 to 2021 and predict disease burden from 2022 to 2030, providing evidence for public health strategies and healthcare resource allocation.

Methods

Utilizing the Global Burden of Disease 2021 database, we calculated incidence and disability-adjusted life years (DALYs) for urolithiasis in Chinese adults aged 20-54. Joinpoint regression identified temporal trends and turning points. Age-period-cohort modeling decomposed age, period, and cohort effects on incidence. Autoregressive integrated moving average (ARIMA) models projected future disease burden.

Results

From 1990 to 2021, urolithiasis cases increased from 9.566 million to 10.197 million (EAPC=0.20%), while age-standardized incidence declined (EAPC= -0.39%). DALYs decreased from 58 700 to 42 200 person-years (EAPC= -1.06%), with DALY rate declining (EAPC= -1.67%). Males consistently exhibited higher standardized incidence and DALY rates. Age-period-cohort analysis showed a linear rise in incidence risk with age, with relative risks (RR) from 0.279 to 2.331. The period effect had a “rising-declining” fluctuation (RR 0.937-1.080). The cohort effect decreased steadily as birth year advanced, with RR values from 0.835 to 1.607. Projections suggest cases will reach 10.396 4 million by 2030, yet standardized incidence and DALY metrics will maintain downward trajectories.

Conclusion

Despite decreasing standardized disease burden and persistent gender disparities since 1990, urolithiasis demonstrates age-progressive risks, period-specific fluctuations, and cohort-driven mitigation. Predicted high absolute burden underscores the urgency for targeted prevention strategies to address this persistent public health challenge.

目的

分析1990—2021年我国中青年尿石症负担变化趋势,并预测2022—2030年的疾病负担,为制定的公共卫生策略和配置医疗资源提供参考。

方法

依托全球疾病负担研究2021数据库,计算我国中青年人群(20~54岁)尿石症发病和伤残调整寿命年(DALY)指标。运用Joinpoint回归分析疾病负担变化趋势和转折点,借助年龄-时期-队列模型估算发病率的年龄、时期和队列效应,并通过自回归积分滑动平均(ARIMA)模型预测2022—2030年尿石症疾病负担。

结果

1990—2021年我国中青年尿石症发病数从956.62万例增至1 019.79万例(EAPC=0.20%),发病率呈下降趋势(EAPC= -0.39%),DALY数从5.87万人年降至4.22万人年(EAPC= -1.06%),DALY率亦呈下降趋势(EAPC= -1.67%)。男性标化发病率和标化DALY率持续高于女性。年龄-时期-队列模型显示,发病风险随年龄增长呈线性递增,各年龄组相对危险度(RR)范围为0.279~2.331;时期效应呈“上升-下降”波动特征,RR值范围为0.937~1.080;队列效应则随出生年份推移稳步下降,RR值范围为0.835~1.607。2022—2030年发病数将波动增至1 039.64万例,但标化发病率持续下降,DALY相关指标保持稳定。

结论

1990—2021年我国中青年尿石症疾病负担呈下降趋势且性别差异显著,发病风险随年龄增长而升高,随时期推移而波动,随出生队列发展而下降。预测显示未来绝对负担仍将维持高位,提示需制定精准防控策略以降低疾病负担。

目的

分析1990—2021年我国中青年尿石症负担变化趋势,并预测2022—2030年的疾病负担,为制定的公共卫生策略和配置医疗资源提供参考。

方法

依托全球疾病负担研究2021数据库,计算我国中青年人群(20~54岁)尿石症发病和伤残调整寿命年(DALY)指标。运用Joinpoint回归分析疾病负担变化趋势和转折点,借助年龄-时期-队列模型估算发病率的年龄、时期和队列效应,并通过自回归积分滑动平均(ARIMA)模型预测2022—2030年尿石症疾病负担。

结果

1990—2021年我国中青年尿石症发病数从956.62万例增至1 019.79万例(EAPC=0.20%),发病率呈下降趋势(EAPC= -0.39%),DALY数从5.87万人年降至4.22万人年(EAPC= -1.06%),DALY率亦呈下降趋势(EAPC= -1.67%)。男性标化发病率和标化DALY率持续高于女性。年龄-时期-队列模型显示,发病风险随年龄增长呈线性递增,各年龄组相对危险度(RR)范围为0.279~2.331;时期效应呈“上升-下降”波动特征,RR值范围为0.937~1.080;队列效应则随出生年份推移稳步下降,RR值范围为0.835~1.607。2022—2030年发病数将波动增至1 039.64万例,但标化发病率持续下降,DALY相关指标保持稳定。

结论

1990—2021年我国中青年尿石症疾病负担呈下降趋势且性别差异显著,发病风险随年龄增长而升高,随时期推移而波动,随出生队列发展而下降。预测显示未来绝对负担仍将维持高位,提示需制定精准防控策略以降低疾病负担。

Objective

To analyze the changing trends of urolithiasis burden among Chinese young and middle-aged adults from 1990 to 2021 and predict disease burden from 2022 to 2030, providing evidence for public health strategies and healthcare resource allocation.

Methods

Utilizing the Global Burden of Disease 2021 database, we calculated incidence and disability-adjusted life years (DALYs) for urolithiasis in Chinese adults aged 20-54. Joinpoint regression identified temporal trends and turning points. Age-period-cohort modeling decomposed age, period, and cohort effects on incidence. Autoregressive integrated moving average (ARIMA) models projected future disease burden.

Results

From 1990 to 2021, urolithiasis cases increased from 9.566 million to 10.197 million (EAPC=0.20%), while age-standardized incidence declined (EAPC= -0.39%). DALYs decreased from 58 700 to 42 200 person-years (EAPC= -1.06%), with DALY rate declining (EAPC= -1.67%). Males consistently exhibited higher standardized incidence and DALY rates. Age-period-cohort analysis showed a linear rise in incidence risk with age, with relative risks (RR) from 0.279 to 2.331. The period effect had a “rising-declining” fluctuation (RR 0.937-1.080). The cohort effect decreased steadily as birth year advanced, with RR values from 0.835 to 1.607. Projections suggest cases will reach 10.396 4 million by 2030, yet standardized incidence and DALY metrics will maintain downward trajectories.

Conclusion

Despite decreasing standardized disease burden and persistent gender disparities since 1990, urolithiasis demonstrates age-progressive risks, period-specific fluctuations, and cohort-driven mitigation. Predicted high absolute burden underscores the urgency for targeted prevention strategies to address this persistent public health challenge.

尿石症作为全球性公共卫生问题,在我国呈现显著增长态势。流行病学研究揭示,我国尿石症疾病负担持续加重,2019年患病人数达1 768万例,标化发病率高达889.9/10万,且发病率呈每十年增长一倍的快速增长趋势[1]。我国大陆地区肾结石患病率已达7.54%[2],显著高于多数慢性非传染性疾病。在人口老龄化加速、饮食结构西化及环境暴露等多因素协同作用下,该疾病呈现出5年复发率超50%的顽固性特征[3],已成为我国慢性病综合防控体系亟待突破的公共卫生问题。监测数据显示,我国中青年人群的发病率呈现上升趋势[4],大约三分之二的患者在60岁前发病[5]
全球疾病负担研究(Global Burden of Disease Study, GBD)凭借其系统性、连续性和多维度的数据优势,为尿石症疾病负担的动态评估提供了新的研究范式。本研究基于GBD 2021数据,构建多维分析框架系统解析中国中青年人群尿石症疾病负担的动态演变特征。通过Joinpoint回归模型识别疾病负担变化的关键时间节点,量化不同阶段的变化百分比;运用年龄-时期-队列模型解构年龄效应、时期效应和出生队列效应对疾病发展的独立影响;建立自回归积分滑动平均(Autoregressive Integrated Moving Average, ARIMA)模型预测未来疾病负担的中长期演变趋势。研究通过揭示我国中青年尿石症流行病学特征的时空异质性,为精准制定泌尿系统疾病分级防控策略、优化区域医疗资源配置提供循证依据。
本研究所用的全部数据均来自GBD 2021数据[6]。该数据库采用标准化的疾病统计指标和疾病负担评估方法,系统评估了全球204个国家和地区的369种疾病或伤害,以及87种风险因素所致的疾病负担,为全球卫生政策制定提供了重要的循证依据。
本研究选取GBD 2021数据库中1990—2021年我国20~54岁人群尿石症疾病负担数据,采用发病数/率和伤残调整寿命年(Disability-adjusted life years,DALY)数/率及其年龄标准化率量化我国中青年人群的尿石症疾病负担情况。在本研究中,将20~54岁人群按照5岁为组距进行分组,共划分为7个年龄组。在1990—2021年这一全部观察时期,为避免相邻出生队列产生时间重叠问题,采用时间点值展开分析。通过相应时期的年份减去年龄,计算出生队列。
本研究运用Excel 2021对数据进行整理,采用发病数/率和DALY数/率,描述我国不同年份、性别、年龄的尿石症疾病负担。运用Joinpoint Regression Program 4.9.1.0软件进行Joinpoint回归模型分析,以探究中青年尿石症发展趋势的局部变化特征。该模型能识别疾病发展转折点,并依此对整体趋势进行分段,通过对数线性回归计算年度百分比变化(Annual Percent Change,APC)、估计年度百分比变化(Estimated APC,EAPC)和平均年度百分比变化(Average APC,AAPC),用以阐述1990—2021年我国中青年尿石症疾病负担的变化趋势。采用内生因子估计法构建年龄-时期-队列模型,该方法限定年龄、时期、队列各组系数之和为零,可排除3个因素之间的混杂影响。此模型通过剖析这3个效应,助力研究者理解发病率随时间的变化趋势,更好地评估尿石症受上述因素影响的变动情况。其主要参数为年龄、时期和队列的相对危险度(Relative Risk,RR)值,RR值可描述发病风险水平。将年龄、时期和出生队列的总体水平设置为参照,当RR>1时,表明风险增加;当RR<1时,意味着风险降低,数值越高风险越高。运用Stata 14.0软件构建年龄-时期-队列模型。运用ARIMA模型开展时间序列预测,重点估算2022—2030年我国中青年人群的尿石症疾病负担变化趋势。通过R 4.4.1 软件“forecast”包中的auto.arima函数来拟合时间序列数据。检验水准设定为α=0.05。
结果显示,我国中青年尿石症总发病数从1990年的956.62万例上升至2021年的1 019.79万例(EAPC=0.20%),而发病率则从1 627.18/10万降至1 437.36/10万(EAPC= -0.39%),标化发病率由1 793.16/10万降至964.70/10万(EAPC= -1.98%)。DALY数从5.87万人年降至4.22万人年(EAPC= -1.06%),DALY率从9.98/10万人年降至5.95/10万人年(EAPC= -1.67%),标化DALY率从15.70/10万人年降至5.63/10万人年(EAPC= -3.27%)(P均<0.05)。男性发病和DALY高于女性。见表1图1
1990—2021年我国中青年尿石症标化发病率及标化DALY率均呈下降趋势,男性和女性的标化发病率AAPC分别为-2.16%和-1.52%(均P<0.05),男性平均下降速率高于女性;男性和女性标化DALY率AAPC分别为-3.23%和-3.45%(均P<0.05),男性平均下降速率略低于女性。标化发病率和标化DALY率各阶段变化见图2
年龄效应显示,发病风险随年龄增长线性上升,呈现单调递增态势,全体人群20~24岁RR值为0.279,50~54岁升至2.331;男性20~24岁RR为0.263,50~54岁达2.285;女性20~24岁RR为0.321,50~54岁为2.466。时期效应则呈现“先上升后下降”的变化特征,全体人群1990—1994至2000—2004时期RR值呈上升趋势(1.015~1.080),2010—2014后RR值持续低于1(0.939~0.952),发病风险降低;男性和女性的时期效应变化趋势与全体人群类似。队列效应表明,发病风险随出生队列发展稳步下降,1940—1944年出生队列发病风险最高,2000—2004年风险最低,全体人群、男性和女性RR值变化范围分别为0.835~1.607、0.839~1.650和0.795~1.468(均P<0.05)。见图3
将1990—2021年我国中青年尿石症负担数据转化为时间序列。经检验,原始序列均为非平稳序列,对其进行差分处理后实现平稳化。对残差序列开展Ljung-Box检验,结果表明均为白噪声序列(P>0.05),观测值不存在自相关性,所构建模型拟合能力良好,能够较准确地预测2022—2030年我国中青年尿石症负担。预测结果显示,我国中青年尿石症发病数呈波动上升趋势,从2022年的1 011.27万例增至2030年的1 039.64万例,发病率从2022年的1 446.96/10万增至2030年的1 454.35/10万;标化发病率从2022年的970.52/10万降至2030年的815.33/10万,DALY数从2022年的4.17万人年降至2030年的4.10万人年;DALY率稳定在6.0/10万人年左右;标化DALY率稳定于(5.63~5.64)/10万人年。见图4
本研究基于GBD 2021数据,系统剖析了1990—2021年我国中青年的尿石症疾病负担情况。结果表明,在此研究期间该疾病负担呈现“绝对负担加重而相对负担减轻”的趋势,发病数增加而DALY下降。男性尿石症疾病负担显著重于女性。年龄-时期-队列分析指出,年龄效应显著,尿石症发病风险随年龄增长线性递增;时期效应显示发病风险随时期推移而波动;队列效应则体现为越晚出生的队列,发病风险越低。ARIMA模型预测结果显示,2022—2030年我国中青年尿石症发病数和发病率将持续上升(年均增幅0.3%~0.5%),标化率和DALY相关指标将维持下降或稳定趋势。
我国中青年男性尿石症疾病负担重于女性,且该差异在31年观察期内持续存在,如2021年男性发病数和标化发病率分别为女性的2.43倍和2.31倍,这种差异与既往研究揭示的2:1性别患病率格局高度吻合[7]。从解剖学基础来看,男性尿道较长且存在生理性狭窄,伴随年龄增长出现的前列腺增生进一步阻碍尿液排出,导致尿液中钙盐、草酸盐等成石物质更易结晶沉积。同时,雄激素通过调控草酸代谢通路可能促进晶体形成,而雌激素对结石生成具有抑制作用,形成天然的生理保护屏障[8]。此外,男性普遍存在高蛋白、高嘌呤的饮食偏好,过量摄入动物性蛋白和海鲜类食物导致尿酸排泄量增加,同时钠盐摄入超标加剧尿钙排泄[9]。职业特征方面,男性从事高温作业、重体力劳动的比例较高,持续性脱水状态使尿液浓缩度显著提升,加速成石物质的过饱和析出。社会文化因素亦不可忽视,男性对泌尿系统症状的认知度较低且就医延迟现象普遍,导致疾病进展至更严重阶段[10]
年龄效应显示,我国中青年尿石症发病风险随年龄增长直线上升,与既往研究结果相同[11]。从35~39岁年龄段开始发病风险RR值大于1,标志着中青年人群进入疾病风险加速上升的关键转折期,是尿石症一级预防的关键窗口期,发病风险的上升,是多种因素共同作用的结果。首先,年龄相关的解剖结构改变是基础性诱因。男性随年龄增长出现的前列腺增生显著增加尿路梗阻风险,导致尿液潴留和成石物质沉积;女性绝经后雌激素水平下降则减弱了对结石形成的抑制作用[12]。其次,代谢功能衰退加剧尿石症疾病进程。随年龄增长,肾脏浓缩功能下降、尿酸排泄效率降低,同时钙磷代谢失衡现象普遍,促使尿液中草酸钙、磷酸盐等成石物质浓度升高。最重要的是,慢性病理因素和行为状态的累积效应。长期高血压、糖尿病等基础疾病在中年群体中高发,这些疾病通过影响肾脏血流动力学和肾小管功能,直接促进结石形成[13]。此外,数十年高盐、高蛋白饮食的累积效应,以及长期脱水状态,使尿液长期处于过饱和状态。年龄效应提示,构建分年龄段精准防控体系势在必行,从而打破“年龄增长-风险累积-结局恶化”的恶性循环。
时期效应表明,我国中青年尿石症发病风险存在显著的时期波动特征,呈现"上升-下降-回升"的非线性演变轨迹。2000—2004年发病风险达到峰值(RR=1.080),风险的增长与快速城市化进程同步,此阶段动物蛋白摄入量激增,含糖饮料消费量翻倍,工业化区域饮用水总硬度升高,多重因素导致尿液过饱和指数骤增[14]。2010—2014年发病风险处于低谷期(RR=0.939),凸显了政策干预成效。如2012年《中国居民膳食指南》将钠盐摄入标准从每日10克降至6克,推动尿钠排泄量下降。2015年后风险回升,RR值从0.937升至0.952,此趋势揭示出新挑战:外卖餐饮渗透率飙升,加工食品日均钠盐摄入量反弹,含果糖饮料消费量增长,重新推高代谢负荷[15]。值得注意的是,职业结构演变带来的久坐问题同样不容忽视,白领群体日均久坐时间增加,可通过尿液浓缩机制促进晶体沉积[16]。时期效应波动轨迹表明,中青年尿石症流行趋势对可干预因素高度敏感,尤其需要关注营养转型中的双刃剑效应:适量蛋白摄入可降低营养不良性结石风险,但过量则会诱发代谢性结石[17]。面对社会转型期的新型风险,建立动态监测机制并实施前瞻性防控策略,将成为遏制疾病负担增长的关键路径。
队列效应显示,我国中青年尿石症发病风险随出生队列推移呈显著下降趋势。与1940—1944年出生的队列相比,2000—2004年出生队列发病风险降低了92.46%。饮食结构的优化成为关键驱动因素,动物蛋白摄入从单一红肉向禽类、鱼类的多元转化,配合食盐加碘等营养干预政策,有效调节了尿液酸碱平衡与代谢稳态[18]。同时,农村安全饮水工程覆盖率不断提高,显著降低高钙区域人群的暴露风险。群体健康行为的代际跃迁重塑了风险格局,年轻一代吸烟率的断崖式下降与运动习惯的普及,直接改善了尿液微环境的理化特性[19]。然而,代际健康增益正面临新挑战——含糖饮料消费激增、外卖饮食隐性钠盐超标与电子屏幕依赖导致的慢性脱水,形成新型代谢负荷[20]。ARIMA模型预测结果也显示,2022—2030年中青年尿石症负担未明显减轻。因此,应对这些挑战需创新防控策略,如开发智能穿戴设备实时监测饮水行为,并针对“屏幕一代”设计碎片化运动方案,以科技赋能守护代际健康转型的成果。
本研究存在以下局限性:首先,GBD 2021数据库的疾病负担指标基于模型估算,存在与真实值偏离风险,可能影响结果准确性;其次,研究局限于国家层面的疾病负担分析,缺乏省级或区域层面的流行病学数据细分;最后,基于年龄-时期-队列模型的生态学研究方法,可能引入群体层面的推论偏倚。
综上,1990—2021年我国中青年尿石症疾病负担呈下降趋势,且性别差异显著。发病风险随年龄增长显著升高,随时期推移波动明显,随出生队列发展呈下降趋势。预测显示未来该人群绝对负担仍将维持较高水平。本研究为制定精准防控策略提供科学支撑,对优化医疗资源配置、完善高危人群筛查体系及推进性别差异化干预具有参考价值。
