Objective

To investigate the association between different domains of physical activity (PA) and adiposity-related indicators among rural older adults in Chengdu, offering empirical evidence to inform the design of tailored health intervention policies.

Methods

An observational analysis was conducted using data from a cluster randomized controlled trial performed in rural areas of Chengdu. The Physical Activity Scale for the Elderly was used to assess total physical activity, work-related physical activity, housework physical activity, and leisure-time physical activity levels. Body mass index (BMI), body fat percentage, waist-to-hip ratio (WHR), and waist-to-height ratio (WHTR) as adiposity-related indicators were measured. A linear mixed model was employed to analyze the relationship between different domains of PA and adiposity-related indicators. Sensitivity analyses were conducted in control group to assess the robustness of the findings.

Results

Of the 508 enrolled participants (average age: 70.93 years), 282 were female, while the remaining 226 were male. Total PA, work-related PA, and household PA levels all showed significant negative correlations with BMI, body fat percentage, WHR, and WHTR (total PA: β=-0.004 9, P<0.001; β=-0.013 5, P<0.001; β=-0.000 1, P<0.001; β=-0.000 1, P<0.001; work-related PA: β=-0.003 1, P=0.006; β=-0.012 8, P=0.005; β=-0.000 1, P=0.007; β=-0.000 1, P<0.001; household PA: β=-0.008 4, P=0.006; β=-0.017 0, P<0.001; β=-0.000 2, P<0.001; β=-0.000 2, P<0.001). However, leisure-time PA was only significantly negatively associated with WHR and WHTR (β=-0.000 1, P=0.007; β=-0.000 1, P=0.011). Sensitivity analysis further confirmed the robustness of these associations.

Conclusion

Higher levels of total PA, work-related PA, and household PA were associated with lower BMI, body fat percentage, WHR, and WHTR in older adults. Higher leisure-time PA was only associated with lower WHR and WHTR. These findings suggest that future physical activity recommendations for older adults should be tailored based on adiposity-related indicators to develop precise and effective intervention strategies.

目的

探究成都市农村老年人不同类型体力活动与多种体重相关指标的关联，为制定针对性的健康干预策略提供依据。

方法

利用在成都市农村地区进行的群随机对照试验数据进行观察性分析。通过老年人体力活动量表评估总体力活动、工作相关体力活动、家务性体力活动和闲暇时体力活动水平。通过体格测量得到身体质量指数（Body Mass Index，BMI）、体脂率、腰臀比（Waist to Hip Ratio，WHR）和腰高比（Waist-to-Height Ratio，WHTR）体重相关指标。采用线性混合模型分析不同类型体力活动与各体重相关指标的关系，并在对照组中进行敏感性分析以检验结果稳健性。

结果

共纳入508名研究对象，平均年龄为70.93岁，其中女性282人，男性226人。总体力活动、工作相关体力活动及家务性体力活动水平与BMI、体脂率、WHR及WHTR均呈显著负相关（总体力活动：β=-0.004 9，P<0.001；β=-0.013 5，P<0.001；β=-0.000 1，P<0.001；β=-0.000 1，P<0.001；工作相关：β=-0.003 1，P=0.006；β=-0.012 8，P=0.005；β=-0.000 1，P=0.007；β=-0.000 1，P<0.001；家务性：β=-0.008 4，P=0.006；β=-0.017 0，P<0.001；β=-0.000 2，P<0.001；β=-0.000 2，P<0.001）。闲暇时体力活动水平则仅与WHR和WHTR呈显著负相关（β=-0.000 1，P=0.007；β=-0.000 1，P=0.011）。敏感性分析结果进一步证实了以上关联的稳健性。

结论

总体力活动、工作相关体力活动和家务性体力活动水平越高，老年人的BMI、体脂率、WHR及WHTR越低；而闲暇时体力活动水平越高，则WHR及WHTR越低。提示今后应基于老年人体重相关指标制定更具针对性的、精准有效的体力活动推荐方案。

目的

探究成都市农村老年人不同类型体力活动与多种体重相关指标的关联，为制定针对性的健康干预策略提供依据。

方法

利用在成都市农村地区进行的群随机对照试验数据进行观察性分析。通过老年人体力活动量表评估总体力活动、工作相关体力活动、家务性体力活动和闲暇时体力活动水平。通过体格测量得到身体质量指数（Body Mass Index，BMI）、体脂率、腰臀比（Waist to Hip Ratio，WHR）和腰高比（Waist-to-Height Ratio，WHTR）体重相关指标。采用线性混合模型分析不同类型体力活动与各体重相关指标的关系，并在对照组中进行敏感性分析以检验结果稳健性。

结论

总体力活动、工作相关体力活动和家务性体力活动水平越高，老年人的BMI、体脂率、WHR及WHTR越低；而闲暇时体力活动水平越高，则WHR及WHTR越低。提示今后应基于老年人体重相关指标制定更具针对性的、精准有效的体力活动推荐方案。

Objective

To investigate the association between different domains of physical activity (PA) and adiposity-related indicators among rural older adults in Chengdu, offering empirical evidence to inform the design of tailored health intervention policies.

Methods

An observational analysis was conducted using data from a cluster randomized controlled trial performed in rural areas of Chengdu. The Physical Activity Scale for the Elderly was used to assess total physical activity, work-related physical activity, housework physical activity, and leisure-time physical activity levels. Body mass index (BMI), body fat percentage, waist-to-hip ratio (WHR), and waist-to-height ratio (WHTR) as adiposity-related indicators were measured. A linear mixed model was employed to analyze the relationship between different domains of PA and adiposity-related indicators. Sensitivity analyses were conducted in control group to assess the robustness of the findings.

Results

Of the 508 enrolled participants (average age: 70.93 years), 282 were female, while the remaining 226 were male. Total PA, work-related PA, and household PA levels all showed significant negative correlations with BMI, body fat percentage, WHR, and WHTR (total PA: β=-0.004 9, P<0.001; β=-0.013 5, P<0.001; β=-0.000 1, P<0.001; β=-0.000 1, P<0.001; work-related PA: β=-0.003 1, P=0.006; β=-0.012 8, P=0.005; β=-0.000 1, P=0.007; β=-0.000 1, P<0.001; household PA: β=-0.008 4, P=0.006; β=-0.017 0, P<0.001; β=-0.000 2, P<0.001; β=-0.000 2, P<0.001). However, leisure-time PA was only significantly negatively associated with WHR and WHTR (β=-0.000 1, P=0.007; β=-0.000 1, P=0.011). Sensitivity analysis further confirmed the robustness of these associations.

Conclusion

Higher levels of total PA, work-related PA, and household PA were associated with lower BMI, body fat percentage, WHR, and WHTR in older adults. Higher leisure-time PA was only associated with lower WHR and WHTR. These findings suggest that future physical activity recommendations for older adults should be tailored based on adiposity-related indicators to develop precise and effective intervention strategies.

我国老年人口基数大，人口老龄化进程持续加快，目前已进入中度老龄化社会。国家统计局最新数据显示，截至2024年末，全国60岁及以上人口数已超3.1亿，占我国总人口的22.0%[1]。到2035年，我国老年人口占总人口的比重预计将突破30%，总数超过4亿。这种结构性的转变预示着重度老龄化阶段的全面到来[2]。在此背景下，如何实现健康老龄化已成为亟待解决的重大公共卫生课题。
老年人体重异常问题日益凸显。既往研究表明，超重与肥胖是心血管疾病和Ⅱ型糖尿病等慢性疾病的风险因素，严重影响老年人的健康水平和生活质量[3-4]。然而，老年期的体重管理具有复杂性和特殊性，需在控制脂肪过度积累的同时警惕营养不良的风险[5]。因此，采用身体质量指数（Body Mass Index, BMI）、体脂率、腰臀比（Waist to Hip Ratio, WHR）、腰高比（Waist-to-Height Ratio, WHTR）等多维指标来综合评估体重状况，比单纯依赖体重更能精准反映老年人潜在的健康风险。
体力活动是指由骨骼肌产生的任何消耗能量的身体运动[6]，在维持健康体重和身体成分方面有重要作用[7-8]。老年人群的体力活动主要涵盖闲暇时间体力活动、工作相关体力活动及家务性体力活动等，不同类型体力活动产生的健康效应也存在差异[9-10]。既往研究发现参与体力活动可以帮助维持正常体重、保持较低BMI水平[11]。然而，现有研究多关注总体力活动水平或者单一类型体力活动，且当前证据多以体重或BMI为单一结局[12]，缺乏针对老年人不同类型体力活动与多维体重指标关联的系统研究。
因此，本研究拟以成都市农村60岁及以上的老年人为研究对象，系统分析闲暇时间、工作相关及家务性体力活动与BMI、体脂率、WHR和WHTR之间的关系，旨在为老年人群体重管理与健康促进策略的制定提供依据。
1　对象与方法

1.1　研究设计
本研究是一项基于成都市农村地区老年人体力活动干预项目（Stay Active While Aging, SAWA）开展的观察性分析[13]。SAWA是一项在四川省成都市农村地区实施的整群随机对照试验。本试验已获得四川大学医学伦理委员会的官方批准（编号：K2019073），且课题相关信息已在中国临床试验注册中心公开备案（ChiCTR2100045653）。SAWA采用多阶段随机抽样方法选取成都市简阳市，再进一步抽取简阳市8个村，按照1：1比例将8个村随机分配至干预组或对照组。为最大限度降低沾染风险，各村庄之间距离保持至少4公里。
1.2　研究对象
项目研究对象为60岁及以上的老年人。纳入标准为：（1）能接听电话；（2）能在15分钟内行走400米；（3）可不在使用拐杖或他人帮助下行走；（4）可通过Time Up & Go测试。排除标准为：（1）罹患中风、关节炎、帕金森、严重肺部疾病或心脏病；（2）有严重的听力或认知障碍；（3）近三年进行过大型手术；（4）糖尿病或高血压控制不佳；（5）正在接受癌症治疗；（6）过去一年发生过跌倒。所有参与对象均签署或按捺过知情同意书。
1.3　研究方法
本项目于2021年5月至2023年5月开展，社会人口学特征、健康及相关行为数据于基线期通过问卷收集。体力活动水平、体重相关指标分别于基线、基线后第4周、8周、6个月、12个月及24个月收集。调查过程中均是由经过统一标准化培训的调查员对参与对象进行面对面问卷访谈。
1.3.1　社会人口学特征
社会人口学特征包括：年龄、性别、婚姻状况、家庭年收入、工作状况和受教育程度。
1.3.2　健康及健康相关行为
健康及健康相关行为包括：是否患有高血压、是否患有糖尿病、吸烟情况及饮酒情况。
1.3.3　体力活动水平
体力活动水平采用老年人体力活动量表（Physical Activity Scale for the Elderly, PASE）进行测量。既往研究表明，该量表在中国老年人群中评估体力活动水平具有良好的效度和信度[14-15]。PASE量表将体力活动分为三种类型：闲暇时间体力活动、工作相关体力活动和家务性体力活动。
1.3.4　体重相关指标
体重相关指标包括BMI、体脂率、WHR和WHTR。使用Tanita BC-601分析仪测量参与对象体重与体脂率。身高通过便携式身高仪测量。腰围在受试者站立状态下，于肋骨下缘与髂骨上缘的中点处测定。臀围测量于耻骨联合及臀大肌最凸点处。BMI定义为BMI=体重（kg）/身高（m）2。WHR通过腰围除以臀围计算得出。WHTR通过腰围除以身高计算得出。
1.4　统计方法
分类变量以人数和构成比形式呈现，连续变量则以均值±标准差表示。基于6个时期的数据，采用线性混合模型（Linear Mixed Model）评估不同类型体力活动与多种体重相关指标之间的关联，以不同类型体力活动（总体力活动、闲暇时间、工作相关、家务性）作为固定效应，以个体差异和时间的作为随机效应。模型1不调整协变量，模型2调整社会人口学特征（包括年龄、性别、婚姻状况、家庭年收入、工作状况和受教育程度），健康及相关行为（包括是否患有高血压、是否患有糖尿病、吸烟情况和饮酒情况），以及组别情况（对照组/干预组）。此外，选取对照组的参与对象重复上述统计分析，排除体力活动干预对本观察性分析的影响，以探讨研究结果的稳健性。所有统计分析均使用R 4.4.3完成，统计学显著性判定采用双侧P值<0.05。
2　结果

2.1　研究对象基线特征
如表1所示，本研究共纳入508名研究对象，其中女性282人，男性226人，平均年龄为70.93岁。大多数研究对象为同居状态（70.9%），且教育程度较低（未正规上过学占69.7%）。基线时参与者的平均总体力活动水平160.48分，平均体脂率为27.55%，BMI平均值为24.17kg/m2，平均WHR为0.93，平均WHTR为0.56。
2.2　不同类型体力活动与体重相关指标的关系
表2展示了不同类型体力活动与多种体重相关指标的关系。
BMI：在模型1中，总体力活动水平和工作相关体力活动与BMI呈负相关（β=-0.004 1，P<0.001; β=-0.004 2，P<0.001）；在模型2中，家务性体力活动也与BMI呈负相关（β=-0.008 4，P=0.006），总体力活动（β=-0.004 9，P<0.001）与工作相关体力活动（β=-0.003 1，P=0.006）的负相关仍然显著。
体脂率：在模型1中，总体力活动水平、工作相关体力活动与体脂率呈负相关（β=-0.010 8，P<0.001；β=-0.019 4，P<0.001）；在模型2中，总体力活动水平、工作相关体力活动和家务性体力活动均与体脂率呈显著负相关（β=-0.013 5，P<0.001; β=-0.012 8，P=0.005；β=-0.017 0，P<0.001）。
WHR：总体力活动、工作相关体力活动以及家务性体力活动在模型1均与WHR呈负相关（β=-0.000 1，P<0.001；β=-0.000 1，P<0.001；β=-0.000 1，P=0.002）；在模型2中总体力活动、闲暇时体力活动、工作相关体力活动以及家务性体力活动与WHR呈负相关（β=-0.000 1，P<0.001；β=-0.000 1，P=0.007；β=-0.000 1，P<0.001；β=-0.000 2，P<0.001）。
WHTR：模型1中，总体力活动和工作相关体力活动与WHTR负相关（β=-0.000 1，P<0.001；β=-0.000 2，P<0.001）；模型2中，闲暇时体力活动和家务性体力活动也显示负相关（β=-0.000 1，P=0.011；β=-0.000 2，P<0.001），总体力活动（β=-0.000 1，P<0.001）和工作中体力活动（β=-0.000 1，P<0.001）的关联仍然显著。
2.3　敏感性分析
在对照组中进一步验证了主要结果的稳健性，见表3。例如：在模型2中，总体力活动与工作相关体力活动仍与BMI（β=-0.004 6，P<0.001；β=-0.003 2，P<0.001）、体脂率（β=-0.013 7，P<0.001；β=-0.015 8，P=0.007）、WHR（β=-0.000 1，P<0.001；β=-0.000 1，P=0.002）和WHTR（β=-0.000 1，P<0.001；β=-0.000 1，P=0.002）呈显著负相关。同时，家务性体力活动也与BMI（β=-0.006 6，P=0.006）、体脂率（β=-0.014 1，P=0.007）和WHTR（β=-0.000 1，P=0.005）呈负相关，但与WHR无显著关联。这些结果与主要分析基本一致。
3　讨论

本研究探索了成都市农村老年人不同类型体力活动与多种体重相关指标的关系。研究结果显示体力活动水平与老年人体重相关指标之间存在显著且稳健的负向关联。具体而言，总体力活动、工作相关体力活动和家务性体力活动水平越高，老年人的BMI、体脂率、WHR及WHTR越低；而闲暇时体力活动水平越高，WHR和WHTR越低。这些关系在敏感性分析中得到进一步验证。
本研究发现总体力活动、工作相关体力活动及家务性体力活动与体重相关指标间存在负向关联。国内研究显示，工作相关体力活动有助于控制男性肥胖[16]；Sarah等人通过对从事体力劳动的移民母亲进行调查，同样发现工作相关体力活动具有预防肥胖的作用[17]。此外，一项针对中国老年男性的研究表明，参与家务与全因及癌症死亡率降低相关，其中体重管理是可能的机制之一[18]。体力活动通过调节机体代谢状态，从而促进脂肪氧化代谢[19]，对于老年人，规律活动还有助于维持肌肉质量与基础代谢率[20]，形成对体重长期稳定的保护效应。上述研究从不同体力活动维度为本研究的结果提供支持，印证了提高老年人体力活动水平对体重控制的潜在益处。
本研究还发现总体力活动、工作相关体力活动以及家务性体力活动与多种体重相关指标均呈显著负相关，而闲暇时体力活动仅与腰臀比和腰高比呈负相关，这一差异反映了农村老年人体力活动的结构性特点：其日常体力活动类型以生产性活动（包括工作相关与家务性体力活动）为主。如表1，研究对象的闲暇时体力活动水平（27.54）远低于工作相关（54.94）和家务性体力活动水平（78.30）。已有研究指出，为有效预防体重增加与脂肪堆积，运动水平需达到一定强度[21]，因此本研究中闲暇时活动水平相对较低，可能未达到所需效应阈值，这或是其与多个体重相关指标未呈现相同效应的原因之一。这说明在评估与促进老年人的体力活动时，应考虑其实际体力活动结构，以提升体重管理的整体效果。
此外，本研究的部分效应值较小，如总体力活动与BMI（β=-0.004 9，P<0.001）、WHR（β=-0.000 1，P<0.001）和WHTR（β=-0.000 1，P<0.001）的关联。Geda等对5 159名中老年人群的前瞻性队列研究报道体力活动与BMI的负向关联，但在该研究中，与平均水平体力活动者相比，较高水平体力活动者的BMI下降速度减缓了0.006 kg/m2/年[22]，这一效应值同样较小，此外，有研究指出老年人群在随访期间BMI变化本身较为有限，而肌肉功能指标的下降更为显著[23]，提示未来研究可关注体力活动对体成分及身体功能的精细化影响。
为排除干预措施可能带来的偏倚，本研究采用对照组进行敏感性分析，进一步增强了研究结论的可信度。在本研究中，对照组分析结果与主要结果基本一致，特别是总体力活动、工作相关体力活动和家务性体力活动与各项指标的关联关系保持稳定，这表明本研究的主要发现具有较好的稳健性和可重复性。
本研究仍然存在一定局限性。第一，本研究是一项观察性分析，虽然发现不同类型体力活动与多种体重相关指标的关联，但证据强度相较于随机对照试验仍较弱。第二，本研究的样本来源于一项体力活动干预项目，可能影响结果的外推性。尽管干预措施本身可能改变参与者的日常活动模式，但对照组的敏感性分析结果与主要结果基本一致，在一定程度上支持了结论的稳健性，未来仍需在自然人群中进一步验证。第三，体力活动数据基于问卷自报，可能会存在回忆偏倚，但已有证据显示PASE量表在我国老年群体中具备良好的信效度[14-15]。第四，饮食行为是体重相关指标的重要影响因素，但由于问卷设计的初始目标限制，本研究未能收集饮食相关变量。第五，鉴于调查对象主要集中在成都市农村老人，研究结果在推广至其他地域、文化背景的人群时仍需谨慎。
