Objective

This study aims to explore the association between the Life’s Essential 8(LE8) and the prevalence of Metabolic-associated fatty liver disease(MAFLD) and its different subtypes.

Methods

Based on the baseline data of Chinese Cohort of Working Adults(CCWA), study subjects were selected through systematic random sampling. Sociodemographic characteristics and lifestyle behaviors were obtained via questionnaires, and physical examination data were collected. MAFLD was diagnosed based on the physical examination information, and the LE8 score was used to assess LE8. A total of 21 614 participants with complete data were included. A multivariate logistic regression model was employed to analyze the relationship between LE8 scores and the risk of MAFLD.

Results

In the study population, the prevalence of MAFLD was 25.50%. The total population’s LE8 score was 72.81±13.78. Individuals with MAFLD exhibited a significantly lower mean LE8 score (64.59±12.89)compared to those without MAFLD (75.63±12.92). Notably, the lowest LE8 score was observed among patients with diabetic MAFLD (54.60±12.00). Each one-point increment in LE8 was associated with a 7% reduction in the risk of MAFLD occurrence(OR=0.93, 95% CI: 0.92-0.93). Specifically, the risk decreased by 5% for overweight/obese MAFLD (OR=0.95, 95% CI: 0.94-0.95), 2% for metabolic dysfunction/lean MAFLD (OR=0.98, 95% CI: 0.97-0.99), and 10% for diabetic MAFLD (OR=0.90, 95% CI: 0.90-0.91). Furthermore, compared to individuals with a high LE8 score, those with a medium LE8 score had a 4.44-fold increased risk of developing MAFLD (OR=4.44, 95% CI: 4.03-4.89), while those with a low LE8 score had a 13.63-fold increased risk (OR=13.63, 95% CI: 11.67-15.94).

Conclusion

The LE8 score is associated with MAFLD and different subtypes of MAFLD.A lower LE8 score significantly increases the risk of MAFLD and different subtypes of MAFLD. Improving LE8 may serve as an effective strategy for MAFLD prevention.

目的

本研究旨在探索生命健康8要素（the life's essential 8, LE8）与代谢相关脂肪性肝病（metabolic-associated fatty liver disease，MAFLD）及不同亚型MAFLD患病的关联。

方法

基于西南职业人群队列的基线数据，采用系统随机抽样选择研究对象，通过问卷调查获取社会人口学特征、生活行为方式信息，并获取体检数据信息。基于体检信息判断MAFLD，采用LE8得分评估LE8，纳入具有完整信息的21 614名研究对象。采用多因素logistic回归模型分析LE8得分与MAFLD患病风险之间的关系。

结果

在研究人群中，MAFLD患病率为25.50%。总人群LE8得分为（72.81±13.78），MAFLD组的LE8得分为（64.59±12.89），显著低于非MAFLD组的（75.63±12.92），糖尿病型MAFLD患者得分最低为（54.60±12.00）。LE8得分每提高一分，MAFLD发生的风险降低7%（OR=0.93，95% CI：0.92～0.93）；超重/肥胖型MAFLD、代谢异常/瘦型MAFLD和糖尿病型MAFLD发生的风险分别降低5%（OR=0.95，95% CI：0.94～0.95）、2%（OR=0.98，95% CI：0.97～0.99）和10%（OR= 0.90，95% CI：0.90～0.91）。中、低水平LE8得分与高水平LE8得分相比，患MAFLD的风险分别增加约4.44（OR=4.44，95% CI：4.03～4.89）和13.63倍（OR=13.63，95% CI：11.67～15.94）。

结论

LE8得分与MAFLD及不同亚型MAFLD存在关联，较低的LE8得分显著增加MAFLD或不同亚型MAFLD患病风险。提高LE8可能是预防MAFLD的潜在手段。

目的

本研究旨在探索生命健康8要素（the life's essential 8, LE8）与代谢相关脂肪性肝病（metabolic-associated fatty liver disease，MAFLD）及不同亚型MAFLD患病的关联。

方法

基于西南职业人群队列的基线数据，采用系统随机抽样选择研究对象，通过问卷调查获取社会人口学特征、生活行为方式信息，并获取体检数据信息。基于体检信息判断MAFLD，采用LE8得分评估LE8，纳入具有完整信息的21 614名研究对象。采用多因素logistic回归模型分析LE8得分与MAFLD患病风险之间的关系。

结果

在研究人群中，MAFLD患病率为25.50%。总人群LE8得分为（72.81±13.78），MAFLD组的LE8得分为（64.59±12.89），显著低于非MAFLD组的（75.63±12.92），糖尿病型MAFLD患者得分最低为（54.60±12.00）。LE8得分每提高一分，MAFLD发生的风险降低7%（OR=0.93，95% CI：0.92～0.93）；超重/肥胖型MAFLD、代谢异常/瘦型MAFLD和糖尿病型MAFLD发生的风险分别降低5%（OR=0.95，95% CI：0.94～0.95）、2%（OR=0.98，95% CI：0.97～0.99）和10%（OR= 0.90，95% CI：0.90～0.91）。中、低水平LE8得分与高水平LE8得分相比，患MAFLD的风险分别增加约4.44（OR=4.44，95% CI：4.03～4.89）和13.63倍（OR=13.63，95% CI：11.67～15.94）。

结论

LE8得分与MAFLD及不同亚型MAFLD存在关联，较低的LE8得分显著增加MAFLD或不同亚型MAFLD患病风险。提高LE8可能是预防MAFLD的潜在手段。

Objective

This study aims to explore the association between the Life’s Essential 8(LE8) and the prevalence of Metabolic-associated fatty liver disease(MAFLD) and its different subtypes.

Methods

Based on the baseline data of Chinese Cohort of Working Adults(CCWA), study subjects were selected through systematic random sampling. Sociodemographic characteristics and lifestyle behaviors were obtained via questionnaires, and physical examination data were collected. MAFLD was diagnosed based on the physical examination information, and the LE8 score was used to assess LE8. A total of 21 614 participants with complete data were included. A multivariate logistic regression model was employed to analyze the relationship between LE8 scores and the risk of MAFLD.

Results

In the study population, the prevalence of MAFLD was 25.50%. The total population’s LE8 score was 72.81±13.78. Individuals with MAFLD exhibited a significantly lower mean LE8 score (64.59±12.89)compared to those without MAFLD (75.63±12.92). Notably, the lowest LE8 score was observed among patients with diabetic MAFLD (54.60±12.00). Each one-point increment in LE8 was associated with a 7% reduction in the risk of MAFLD occurrence(OR=0.93, 95% CI: 0.92-0.93). Specifically, the risk decreased by 5% for overweight/obese MAFLD (OR=0.95, 95% CI: 0.94-0.95), 2% for metabolic dysfunction/lean MAFLD (OR=0.98, 95% CI: 0.97-0.99), and 10% for diabetic MAFLD (OR=0.90, 95% CI: 0.90-0.91). Furthermore, compared to individuals with a high LE8 score, those with a medium LE8 score had a 4.44-fold increased risk of developing MAFLD (OR=4.44, 95% CI: 4.03-4.89), while those with a low LE8 score had a 13.63-fold increased risk (OR=13.63, 95% CI: 11.67-15.94).

Conclusion

The LE8 score is associated with MAFLD and different subtypes of MAFLD.A lower LE8 score significantly increases the risk of MAFLD and different subtypes of MAFLD. Improving LE8 may serve as an effective strategy for MAFLD prevention.

代谢相关脂肪性肝病（metabolic-associated fatty fiver disease，MAFLD）是一种以代谢紊乱为特征的慢性肝病，近年来MAFLD的发病率在全球范围内显著上升，已成为最常见的慢性肝病之一，MAFLD是心血管疾病的主要危险因素，也是导致MAFLD患者发病和死亡的主要原因[1]。不同MAFLD亚型的心血管风险存在差异，瘦型MAFLD和糖尿病型MAFLD患者的心血管风险高于超重/肥胖型MAFLD患者[2]。既往研究表明，MAFLD患病与生活方式及代谢相关生化指标相关[3]。美国心脏协会（American Heart Association,AHA）提出的生命健康8要素（life's essential 8， LE8）评分系统[4]，由Life's Simple 7发展而来，包含与健康相关的主要行为生活方式和生化指标，并增加睡眠健康作为第八个指标，形成了一套更为全面的评估体系[5]，可应用于不同人群和健康结局，如心血管健康风险[6]，认知功能[7]以及生物衰老[8]。LE8强调通过改善健康行为（如饮食、运动、睡眠）和健康因素（如血压、血糖）以降低慢性非传染性疾病（NCDs）的负担[9]，对早期识别疾病风险具有重要意义。
前期在欧美人群的研究发现，LE8评分与MAFLD发病呈显著负相关，较高的LE8评分可降低MAFLD的发生及进展为晚期肝纤维化的风险[10]。然而，中国人的生活方式与欧美人存在明显差异，目前有关中国人群中LE8与MAFLD及其不同亚型的关系尚未进行探讨。因此，本研究基于中国西南职业人群队列（CCWA）的基线数据[6,11]，探讨LE8评分与MAFLD患病风险之间的关系，并进一步分析其与不同MAFLD亚型之间的关系，为MAFLD的预防提供新的思路和依据。
1　对象与方法

1.1　研究对象
本研究采用横断面研究设计，基于2021年西南职业人群队列（CCWA）基线数据[6]，纳入了四川、贵州、重庆三省市的31 105名在职职工，通过在线问卷收集社会人口学、健康行为、健康要素作答情况，问卷中使用三个常识性问题检验问卷填写是否合格，若存在任一问题回答错误，该问卷将被视为无效；并获取其体检数据信息。
本研究的纳入标准为：（1）年龄≥18岁；（2）调查时在岗；（3）认知能力良好且能正常沟通、知情同意且自愿参加；（4）有完整体检腹部超声检查的体检信息。排除标准：未完整填写问卷者。排除基线无体检结果和问卷主要结果的对象12 291名，共纳入完成了自填问卷主要板块并存在体检信息者27 707名，对问卷填写结果以及体检结果进行筛选过滤，剔除关键问卷变量或体检结果缺失的个体6 093名，最终有21 614名研究对象被纳入本次研究。本研究已通过四川大学华西第四医院/华西公共卫生学院伦理委员会伦理审查（Gwll2021064），所有研究对象均自愿参加并签署知情同意书。
1.2　研究变量
LE8评分：参考AHA提出的LE8评分方法[4]基础上调整赋分标准，LE8评分包括四项健康行为指标：膳食、体力活动、吸烟情况和睡眠情况，以及四项健康要素指标：BMI、血脂、血糖和血压。八项指标分别按照0～100评分，计算八项指标得分的未加权平均值作为最终得分，并将人群的LE8得分情况分为高、中、低水平三组，分别对应总分为：80～100分，50～79分和0～49分；
结局变量：参考最新版MAFLD专家共识[12]，结局变量MAFLD依据腹部超声结果诊断。并进一步根据亚组评判标准[11]将MAFLD细分为：
（1）超重/肥胖型MAFLD：影像学MAFLD确诊，且存在身体质量指数（body mass index，BMI）≥23 kg/m2。
（2）代谢异常/瘦型MAFLD：在影像学MAFLD确诊且体脂较瘦或体脂量正常（BMI<23 kg/m2）基础上，存在下组代谢异常风险因素至少2项：①血压≥130/85 mm Hg或接受特异性药物治疗；②血浆甘油三酯≥1.70 mmol/L或接受特异性药物治疗；③血浆高密度脂蛋白胆固醇：男性<40 mg/dl，女性<50 mg/dl，或接受特异性药物治疗；④糖尿病前期，定义为空腹血糖水平为5.6～6.9 mmol/L，或餐后2 h血糖水平为7.8～11 mmol/L或糖化血红蛋白为5.7%～6.4%；
（3）2型糖尿病型MAFLD：影像学结果存在MAFLD且具有糖尿病病史或空腹血糖>7.0 mmol/L。患有2型糖尿病的超重/肥胖型或代谢异常/瘦型MAFLD被判定为糖尿病型MAFLD。
1.3　协变量
基于前期研究[11]，将年龄、性别、受教育水平、婚姻状况、民族、家庭月收入、饮酒状况等作为协变量。
1.4　统计分析
根据MAFLD是否患病进行分组，连续性变量使用（）进行描述并使用t检验比较组间差异，分类变量使用构成比或率（%）进行描述并用χ2检验比较组间差异（检验水准α=0.05）。构建多元logistic回归模型探究分析LE8连续性得分和MAFLD以及不同亚型MAFLD患病之间的关联性。基于LE8连续性得分，将得分分为高、中、低水平三组，以高得分组为参考，采用多元logistic回归模型分析LE8中等水平得分组和低水平得分组与MAFLD及不同亚型MAFLD的关联，结果采用OR值及95%置信区间（confidence interval，CI）表示，双侧检验，检验水准为α=0.05。基于LE8连续性得分，分别构建以MAFLD及其三种亚型为结局的多元logistic回归预测模型，分析LE8连续性得分对MAFLD及三种亚型的预测概率。所有模型均调整了协变量。采用统计软件R 4.3.2进行分析。

2.1　基本特征
在21 614名研究对象中，平均年龄为（39.78±10.29）岁，86.55%为男性，MAFLD患病率为25.50%。年龄、性别、民族、教育水平、婚姻状况、月收入水平和生活方式均在MAFLD和非MAFLD组间存在统计学差异（P<0.05）。总人群LE8得分为（72.81±13.78）分。除体力活动外，不同LE8水平在不同人口学特征和生活方式中，MAFLD与非MAFLD群体均存在显著的统计学差异。见表1。
2.2　LE8在MAFLD亚型中的得分情况
超重/肥胖型MAFLD是主要亚型，占82.73%，其次是糖尿病型，占14.17%，瘦型/代谢异常型MAFLD人数最少，为3.11%。所有亚型中，糖尿病型MAFLD的LE8均分最低，为（54.60±12.00）分，高水平LE8在超重/肥胖型MAFLD占比最高，为15.90%。见表2。
2.3　生命健康8要素得分与MAFLD患病的关联性分析
回归模型结果显示，LE8得分每增加1分，MAFLD的患病风险下降7% （OR=0.93，95% CI：0.92～0.93）。与高水平LE8相比，中水平（OR=4.44，95% CI：4.03～4.89）和低水平LE8（OR=13.63，95% CI：11.67～15.94）显著增加MAFLD的患病风险。
LE8得分每增加一个单位，超重/肥胖型MAFLD的风险降低5%（OR=0.95，95% CI：0.94～0.95），代谢异常/瘦型MAFLD的风险降低2%（OR=0.98，95% CI：0.97～0.99），糖尿病型MAFLD的风险降低10%（OR=0.90，95% CI：0.90～0.91）。其次，低水平和中水平LE8与高水平的LE8相比，显著增加超重/肥胖型MAFLD、代谢异常/瘦型MAFLD患病风险。见表3。
2.4　预测概率曲线
采用logistic回归模型构建预测概率曲线探讨LE8得分与MAFLD及其亚型间的关系。结果显示，MAFLD的预测概率随LE8得分从低到高（0～100）呈现出持续下降的趋势，预测概率由接近100%降至约10%以下，表明LE8得分是MAFLD的保护因素。在超重/肥胖型MAFLD和糖尿病型MAFLD中也呈现类似趋势。而代谢异常/瘦型MAFLD起始预测概率较低（<10%），且随LE8得分变化幅度较小，可能与本研究中该亚型样本量较小有关。见图1。
3　讨论

本研究基于西南职业人群队列，在基线随访中共5 506人患MAFLD，MAFLD患病率为25.50%，这一患病比例略低于国内研究结果[13]。研究结果显示，LE8得分与MAFLD及其不同亚型的患病率均显著相关。
研究发现，LE8得分越高患MALFD的风险越低，中等水平LE8人群患MAFLD的风险是高LE8水平的4.44倍，而低水平LE8人群患MAFLD风险则为高水平LE8人群的13.63倍。既往在中国人群的研究多探讨了LE8评分与卒中风险的关系，例如，一项基于社区的前瞻性研究对41 043和27 842名参与者进行了分析，发现LE8评分能够显著预测未来卒中风险和动脉硬化状态[14]，另一项针对26 719名中国人群的研究也表明，LE8的长期变化模式与卒中风险相关[15]。此外，中国人群的研究中，LE8的研究还涉及特定环境因素（如固体燃料使用）与心血管疾病的关联[16]，LE8评分在低收入人群和少数民族中的分布和影响也有初步探索[17]，但目前较少有研究探讨LE8与中国职业人群MAFLD患病间的关联。
本研究使用LE8得分预测MAFLD患病概率，总体曲线均呈现出随着LE8得分的提高而下降的趋势，表明LE8得分越高，MAFLD的患病风险越低。究其原因，LE8低分者往往存在不良生活方式（如低体力活动、高脂高糖饮食等），这些行为通常会通过不同方式促进MAFLD发生发展，如低体力活动和高糖摄入加剧胰岛素抵抗，促进肝脏脂肪沉积[18]。对于代谢指标而言，LE8中的血糖、血脂和血压组分能反应代谢异常程度，且与MAFLD的病理生理直接相关，如胰岛素抵抗、脂质堆积等直接促进肝脂肪变性[19]，导致MAFLD发生发展。MAFLD亚组分析发现，无论是LE8连续性得分还是不同水平的LE8得分，对糖尿病型MAFLD的保护效应均明显强于其他两类MAFLD亚型。既往研究表明，糖尿病型MAFLD患者表现出最高的全因死亡率和疾病特异性死亡率风险，其风险显著高于单纯肥胖型或瘦型MAFLD[20]，并且糖尿病型MAFLD患者的肝纤维化进展较其他亚型更为严重[21]，表明糖尿病MAFLD群体可能具有更低的LE8水平和更高的并发严重不良结局的可能性，需重点关注这一人群的筛查和预后。本研究中超重/肥胖型MAFLD与LE8的效应次于糖尿病型MAFLD，且超重/肥胖型是MAFLD的主要亚型，占比高达82.73%。主要原因是肥胖者更易发生肝脏脂肪堆积，肥胖导致的脂质代谢紊乱促使游离脂肪酸在肝脏过度沉积形成肝细胞脂肪变性[22]，既往研究也表明约80%的肥胖患者会发展为MAFLD[23]。因此，改善LE8指标可能对超重/肥胖型MAFLD患者的长期预后具有积极意义。
综上所述，LE8得分与MAFLD及其亚型的患病率之间存在显著相关性，LE8得分可作为MAFLD患病风险评价的辅助工具。本研究具有如下局限性：首先，本研究基于西南职业人群队列基线数据，难以进行因果推论。其次，本研究人群队列集中于西南地区，限制了研究结果的外推性。最后，在MAFLD亚型与LE8评分的关联性研究中，代谢异常/瘦型MAFLD与不同LE8水平的关系并未与MAFLD，以及超重/肥胖型和糖尿病型MAFLD呈现出类似的趋势，这可能与本研究人群中代谢异常/瘦型MAFLD样本量较小有关。
