目的

探究我国中老年人睡眠时间及抑郁与慢性肺部疾病（Chronic lung disease，CLD）的相关性。

方法

本研究基于中国健康与养老追踪调查（CHARLS）2011-2020年的公开数据。采用Cox回归分析中老年人睡眠时间及抑郁与CLD的关联，并使用限制性立方样条（Restricted cubic spline，RCS）分析二者的剂量-反应关系。此外，通过亚组分析和交互作用分析评估上述关联在不同人群中的异质性，采用敏感性分析验证结果的稳定性。

结果

在6 011例受访者中短睡眠时间1 662例（27.65%），正常睡眠时间3 838例（63.85%），长睡眠时间511例（8.50%），抑郁2 142例（35.63%）。Cox回归分析结果显示，短睡眠时间（ HR=1.23，95% CI：1.04~1.45，P=0.018）及抑郁（ HR=1.76，95% CI：1.50~2.05，P<0.001）与CLD风险增高有关。RCS分析结果显示，睡眠时间（P-overall=0.009，P-non-linear=0.121）及抑郁（P-overall<0.001, P-non-linear=0.116）与CLD风险呈线性剂量-反应关系，拐点分析结果显示，当睡眠时间<6.97 h或CES-D分值>7时，CLD风险随睡眠减少或CES-D分值增加而升高。亚组分析结果显示，短睡眠时间与CLD风险升高的关联在45~59岁女性、高中及以上受教育程度、低体力活动、参加社交活动以及非糖尿病患者中更为显著。交互作用显示，随着受教育程度的提高，长睡眠与CLD之间的关联强度减弱约29%，抑郁与CLD之间的关联增强约19%。

结论

短睡眠时间与抑郁是中老年CLD的重要危险因素，应将其评估纳入CLD预防体系。

Objective

To explore the association of sleep duration and depression with Chronic lung disease (CLD) in middle-aged and elderly Chinese.

Methods

Using CHARLS public data (2011-2020), Cox regression and restricted cubic spline (RCS) analyses were performed to assess the associations and dose-response relationships of sleep duration and depression with CLD. Subgroup and interaction analyses was further conducted to evaluate effect heterogeneity, and sensitivity analyses to test the robustness of our results.

Results

Among 6 011 respondents, 27.65% had short sleep, 63.85% had normal sleep, 8.50% had long sleep, and 35.63% had depression. Cox regression analysis showed that short sleep duration ( HR=1.23, 95% CI: 1.04-1.45, P=0.018) and depression ( HR=1.76, 95% CI: 1.50-2.05, P<0.001) significantly increased CLD risk. The results of RCS analysis showed that sleep duration (P-overall=0.009, P-non-linear=0.121) and depression (P-overall<0.001, P-non-linear=0.116) had a dose-response and linear relationship with CLD risk. The results of infled-point analysis showed that, when the sleep duration was <6.97 h or CES-D score was >7, the risk of CLD increased with the decrease of sleep or the increase of CES-D score. Subgroup analysis showed that the association between short sleep duration and the risk of CLD was more significant in women aged 45-59 years, with high school education or above, low physical activity, social participation, and non-diabetes. The interaction showed that with the increase of education level, the association between long sleep and CLD decreased by 29%, and the association between depression and CLD increased by 19%.

Conclusion

Short sleep duration and depression are important risk factors for CLD in middle-aged and elderly people, and their evaluation should be included in the CLD prevention system.

据国家统计局最新数据显示，截止2024年我国45岁及以上中老年人口约6.6亿，占总人口的44%，预计到2035年我国中老年人口将达到7亿^[1]。而随着我国老龄化进程的加快，中老年人口的健康问题也成为我国重要的公共卫生问题之一。慢性肺部疾病（Chronic lung disease，CLD）作为中老年人常见的慢性疾病之一，其发病率随着年龄的增长而逐渐升高，对我国经济和社会造成了重大影响^[2]。因此，预防和控制CLD对优化个体健康管理、减轻社会医疗负担意义重大。有研究显示，约10.97%的中老年人每日睡眠时间<6 h^[3]，约24.1%~37.98%的中老年人存在抑郁症状^[4-5]，而越来越多的证据表明每日睡眠时间<6 h及抑郁与中老年人多种不良健康结局有关，包括心血管疾病^[6-7]、老年痴呆^[8]、糖尿病^[8]、自杀死亡^[9]等。然而，目前仍缺乏基于中国中老年人群的纵向研究，来同时探讨睡眠时间、抑郁与CLD风险之间的关联，并进一步揭示其剂量-反应关系。因此，本研究拟基于中国健康与养老追踪调查（China Health and Retirement Longitudinal Study，CHARLS）数据库，探究我国中老年人睡眠时间及抑郁与CLD的相关性，为今后中老年人群卫生政策的制定提供科学依据。

1　对象与方法

1.1　研究对象

本研究是一项回顾性队列研究，所有数据均来源于CHARLS数据库的五轮调查数据（2011、2013、2015、2018、2020）。纳入标准：1）年龄≥45岁的中国中老年人；2）2011-2020年CLD诊断明确的受访者；排除标准：1）缺乏睡眠时间及抑郁状况的数据；2）缺乏CLD诊断数据；3）其他关键协变量如性别、受教育程度等数据缺失的受访者。

2011年基线调查时纳入17 708名研究对象，排除关键协变量缺失的个体后获得7 177名有效样本，进一步排除基线时已患CLD者735人，纳入6 442人随访，随访截止到2020年共排除431名失访者后，最终分析样本为6 011人。CHARLS调查经北京大学生物医学伦理委员会批准（IRB00001052-11015），所有受访者均知情同意并自愿参与本研究。

1.2　研究方法

1.2.1　慢性肺部疾病

慢性肺部疾病判定问题为“是否有医生曾经告诉过您有慢性肺部疾患如慢性支气管炎或肺气肿、肺心病（不包括肿瘤或癌）”，若回答“是”则判定为慢性肺部疾患。此外，CHARLS额外询问了受访者“是否知道自己患有慢性肺部疾患”，若受访者回答“知道自己患有”同样判定为慢性肺部疾患^[10-11]。

1.2.2　睡眠时间

CHARLS通过询问“过去一个月内，平均每天晚上真正睡着的时间大约是几小时？（可能短于在床上躺着的时间）”来调查受访者的睡眠时间。此外，通过查阅文献本研究将短睡眠时间定义为“<6 h”，正常睡眠时间定义为“6~8 h”，长睡眠时间定义为“>8 h”^[7]。

1.2.3　抑郁症状

CHARLS采用流调中心抑郁量表^[7]（Center for Epidemiologic Studies Depression Scale，CES-D）评估中老年人在过去一周内的抑郁症状频率和严重程度。该量表共10个条目，很少或没有时间为0分，不太多为1分，有时或一半的时间为2分，大多数时间为3分。量表总分范围0~30分，≥10分代表有抑郁症状^[12-13]。该量表中文版在中国老年人群中有良好的信效度，Cronbach α=0.798^[14]。

1.2.4　协变量

本研究选取年龄、性别、受教育程度、婚姻状况、居住地、体力活动、吸烟、饮酒、自评健康状况、参加社交活动、慢性病作为协变量进行分析，其中年龄分为45~59、60~69、70~79、≥80；受教育程度分为文盲、小学、初中、高中及以上；婚姻状况分为未婚、已婚、离异、丧偶；居住地分为城镇、农村；吸烟分为从不吸烟、现在吸烟、过去吸烟；饮酒分为从不喝酒、每月超过一次、每月少于一次；自评健康状况分为很好、好、一般、不好及很不好；慢性病包括高血压、糖尿病及血脂异常。

1.3　统计学处理

使用Stata 16.0及R语言4.1.2版本统计学软件进行数据整理和分析，计数资料以频数表示，组间比较采用χ²检验。采用Cox回归模型分析睡眠时间及抑郁与CLD之间的关联。采用限制性立方样条（Restricted cubic spline，RCS）评估睡眠时间及CES-D分值与CLD之间的非线性关系，选择4个节点P₅，P₃₅，P₆₅，P₉₅进行限制性立方样条分析，取睡眠时间的P₅₀作为参考值。检验水准α=0.05。

本研究在数据收集中出现部分数据缺失，主要原因为问卷条目未作答，我们对缺失的关键变量进行了分析，结果显示睡眠时间缺失率为8.79%，抑郁缺失率为7.54%，CLD缺失率为1.95%，以上关键变量缺失率均小于10%，一般认为产生偏倚的可能性较小^[15]。因此，本研究采用完整病例分析法处理缺失数据。

2　结果

2.1　一般人口学特征

本研究共纳入CHARLS数据库6 011例受访者，其中短睡眠时间1 662例（27.65%），正常睡眠时间3 838例（63.85%），长睡眠时间511例（8.50%），抑郁2 142例（35.63%）。睡眠时间组在年龄、性别、受教育程度、婚姻状况、居住地、饮酒、自评健康状况、参加社交活动差异具有统计学意义。抑郁组在年龄、性别、受教育程度、婚姻状况、居住地、吸烟、饮酒、自评健康状况、参加社交活动、高血压、糖尿病及血脂异常差异具有统计学意义，具体见表1。

2.2　睡眠时间及抑郁与CLD风险的Cox回归分析

Cox回归分析结果显示，在模型3中与正常睡眠时间相比，短睡眠时间与CLD风险增高有关（ HR=1.23，95% CI：1.04~1.45，P=0.018），长睡眠时间与CLD风险无关（ HR=1.05，95% CI：0.79~1.39，P=0.761）。抑郁症状与CLD风险增高有关（ HR=1.76，95% CI：1.50~2.05，P<0.001）。具体见表2。

2.3　睡眠时间及CES-D分值与CLD关联的剂量-反应分析

RCS分析结果显示，睡眠时间（P-overall=0.009, P-non-linear=0.121）及CES-D分值（P-overall<0.001, P-non-linear=0.116）与CLD发生风险呈线性剂量-反应关系。拐点分析结果显示，当睡眠时间<6.97 h时，随着睡眠时间的减少，CLD发生风险逐渐增加，具体见图1A。当CES-D分值>7时，随着CES-D分值的增加，CLD发生风险也随之增加，具体见图1B。

2.4　敏感性分析

本研究将睡眠时间及CES-D分值在前0.5%和后99.5%数据进行缩尾处理，并基于剩余样本重新评估睡眠时间及抑郁与CLD之间的关联，以此来减轻极端值对分析结果的影响。结果显示，在调整了混杂因素的模型3中，短睡眠时间（ HR=1.21，95% CI：1.02~1.43，P=0.031）及抑郁（ HR=1.75，95% CI：1.50~2.05，P<0.001）与CLD风险增高有关，与本研究主要结果保持一致。具体见表3。

2.5　睡眠时间及抑郁与CLD风险的分层分析

本研究通过亚组分析发现，短睡眠时间在45~59岁女性、高中及以上受教育程度、低体力活动、参加社交活动及非糖尿病患者中，与CLD风险增高显著相关。进一步的交互作用分析发现，受教育程度对长睡眠存在显著的修饰作用（ HR=0.71，95% CI：0.52~0.97，P=0.032），即随着受教育程度的提高，长睡眠与CLD之间的关联强度减弱约29%，具体见表4。此外，受教育程度也显著修饰了抑郁与CLD之间的关联（ HR=1.19，95% CI：1.03-1.38，P=0.019），这表明随着受教育程度的提高，抑郁与CLD之间的关联增强了约19%，具体见图2。

3　讨论

基于CHARLS数据库2011-2020年的调查数据，本研究探讨了我国中老年人群睡眠时间及抑郁与CLD之间的关联。结果表明，短睡眠时间（尤其<6.97 h）和抑郁（尤其CES-D分值>7）均与CLD风险升高相关，且该关联在不同亚组人群中均普遍存在。这些发现深化了对睡眠、心理因素与呼吸健康之间复杂机制的理解，凸显其在慢性病防控中的整体性作用。

一方面本研究结果显示，短睡眠时间与CLD风险增高相关，而长睡眠时间与CLD发生风险无关。RCS分析结果显示，睡眠时间与CLD发生风险呈线性关系，当睡眠时间<6.97 h时，随着睡眠时间的减少，CLD发生风险逐渐增加。该结果与既往研究一致，Chuang LP^[16]与Wang S等^[17]也发现短睡眠时间分别与肺功能下降及COPD风险增加相关，且长睡眠时间均未显示显著关联。然而，既往研究多数样本量有限，且多基于西方人群或聚焦单一肺部疾病（如COPD），并缺乏更精确的剂量-反应分析。而本研究基于更大规模的中老年人群，将研究对象拓展至慢性肺部疾病整体，并采用RCS方法进行剂量-反应分析，揭示了更精确的阈值效应，从而为睡眠时间与CLD风险之间的关联提供了更为可靠的人群证据。从机制的角度看，短睡眠时间可以通过增强TLR4的敏感性，促进单核细胞NF-κB和AP-1核转位，上调促炎细胞因子如IL-6、TNF-α转录^[18-19]。IL-6作为一种多效性促炎细胞因子，通过激活STAT3信号通路，上调趋化因子（如CXCL8/IL-8）的表达，促进中性粒细胞向气道迁移，释放蛋白酶和活性氧（ROS），导致气道黏膜破坏，驱动慢性气道炎症进而导致COPD^[20-21]。

另一方面本研究结果显示，抑郁症状与CLD风险增加呈显著正相关，RCS分析结果显示，CES-D分值与CLD发生风险呈线性关系。这与既往的研究结果一致，如Lotfaliany M^[22]及蔺婧等^[23]的研究均证实抑郁与CLD风险相关。然而，既往研究主要报道了组间风险比较，而本研究通过RCS分析，不仅验证了关联的存在，进一步明确了抑郁（CES-D分值）与CLD风险之间存在严格的线性剂量-反应关系，揭示了CLD风险随CES-D连续变化的形态。此外，流行病学研究数据也表明，特发性肺纤维化（Idiopathic Pulmonary Fibrosis, IPF）作为CLD的重要亚型，其患者群体中抑郁的发病率高达62.5%^[24]，提示抑郁与IPF之间可能存在显著的病理生理学关联。具体而言，抑郁患者下丘脑-垂体-肾上腺（HPA）轴过度活跃，导致皮质醇升高，伴随糖皮质激素受体功能下调，失去对NF-κB的抑制作用，促使IL-6等促炎因子释放，IL-6通过与IL-6R结合，招募gp130共受体，触发下游MAPK信号通路，而MAPK的磷酸化激活促进细胞周期蛋白表达，进一步推动成纤维细胞从G1期进入S期，增强增殖能力，进而导致肺纤维化^[20-21,25]。

既往有研究表明，不同受教育程度人群的CLD发病率存在显著差异^[26]。为探究睡眠时间及抑郁与CLD关联是否存在受教育水平修饰效应，本研究进行交互作用分析。结果显示，受教育程度对长睡眠时间与CLD风险的关联具有显著的修饰效应，较高的受教育程度减弱了长睡眠时间与CLD的关联，这一发现可能与高教育人群更好的健康管理能力有关^[27-28]。此外，受教育程度也显著修饰了抑郁与CLD之间的关联，与预期不同的是，较高的受教育程度并未缓冲抑郁对CLD的不良影响，反而增强了这一关联。这可能提示在高教育群体中，抑郁患者可能面临独特的工作压力或社会期望压力，从而放大了抑郁对CLD的影响。此外，低教育程度人群暴露于更强的CLD危险因素，如缺乏医疗资源、患有多种慢性病、吸烟、营养不良等^[26]，这些都降低了抑郁的独立作用。

4　结论

基于本研究结果，可得出以下重要结论：在我国中老年人群中，短睡眠时间及抑郁与CLD呈线性剂量-反应关系，且该关联在多数亚组中保持一致。当睡眠时间<6.97 h时，随着睡眠时间的减少，CLD发生风险逐渐增加，当CES-D分值>7分时，随着CES-D分值增加，CLD发生风险也随之增加。此外，受教育程度对上述关联具有异质性修饰作用，表现为对长睡眠风险呈减弱效应，而对抑郁风险呈增强效应。因此，关注并改善我国中老年人群的睡眠时间及抑郁状态可能是预防CLD的有效策略。
