Objective

To analyze the mortality rate and trend of non-melanoma skin cancer (NMSC) in China from 2005 to 2018 and predict its future trend, providing a basis for effective prevention and control.

Methods

NMSC mortality data from 2005 to 2018 were collected from the Chinese Cancer Registry Annual Report. The Joinpoint regression model was used to estimate the average annual percentage change (AAPC) and annual percentage change (APC) to reflect temporal trends. An age-period-cohort model with intrinsic estimator algorithm was applied to analyze age, period, and cohort effects. The Bayesian age-period-cohort (BAPC) analysis with integrated nested Laplace approximation was employed to predict NMSC mortality trends in China from 2019 to 2035.

Results

From 2005 to 2018, the age-standardized mortality rate (ASMR) of NMSC in China was 0.34 per 100 000, showing an upward trend with an AAPC of 5.6% (95%CI: 3.8%-7.4%). The ASMR was higher in males (0.41 per 100 000) than in females (0.26 per 100 000),but the increase was slightly faster in females (AAPC=6.2%). Rural areas had a higher ASMR (0.40 per 100 000) than urban areas (0.30 per 100 000), but the AAPC was only half that of urban areas. The mortality risk of NMSC increased with age. The cohort effect showed a declining trend in ASMR. The predicted ASMR of NMSC in China is expected to continue rising to 0.78 per 100 000(95%CI: 0.10-1.46 per 100 000) during 2019 and 2035.

Conclusion

The ASMR of NMSC in China exhibited an upward trend from 2005 to 2018, with variations by age, sex, and region. NMSC remains a public health concern requiring sustained attention.

目的

分析2005—2018年中国非黑色素瘤皮肤癌(non-melanoma skin cancer,NMSC)死亡率及其变化趋势,并对未来趋势进行预测,为有效预防和控制NMSC提供基础依据。

方法

收集《中国癌症登记年报》中2005—2018年NMSC死亡率数据。采用joinpoint回归模型估计年均变化百分比(average annual percentage change,AAPC)和年度变化百分比(annual percentage change,APC)来反映时间趋势。通过具有内在估计器算法的年龄-时期-队列模型分析年龄、时期和队列效应。采用集成嵌套拉普拉斯近似的贝叶斯年龄-时期-队列分析(Bayesian age-period-cohort analysis,BAPC)方法对2019—2035年中国NMSC的死亡趋势进行预测。

结果

2005—2018年NMSC全国的年龄标准化死亡率(age-standardized mortality rate,ASMR)为0.34/10万,呈上升趋势,AAPC为5.6%(95%CI: 3.8%~7.4%)。NMSC的ASMR男性为0.41/10万,高于女性的0.26/10万,但女性上升速度略快(AAPC=6.2%)。农村地区NMSC的ASMR为0.40/10万,高于城市地区的0.30/10万,但AAPC只有城市地区的一半。NMSC死亡率的风险随年龄增长而增加。队列效应中ASMR呈现下降趋势。预测中国NMSC的ASMR在2019—2035年将持续升高到0.78/10万(95%CI:0.10/10万~1.46/10万)。

结论

2005—2018年中国NMSC的ASMR呈上升趋势,且存在年龄、性别和地区差异。NMSC仍然是一个需要持续关注的公共卫生问题。

目的

分析2005—2018年中国非黑色素瘤皮肤癌(non-melanoma skin cancer,NMSC)死亡率及其变化趋势,并对未来趋势进行预测,为有效预防和控制NMSC提供基础依据。

方法

收集《中国癌症登记年报》中2005—2018年NMSC死亡率数据。采用joinpoint回归模型估计年均变化百分比(average annual percentage change,AAPC)和年度变化百分比(annual percentage change,APC)来反映时间趋势。通过具有内在估计器算法的年龄-时期-队列模型分析年龄、时期和队列效应。采用集成嵌套拉普拉斯近似的贝叶斯年龄-时期-队列分析(Bayesian age-period-cohort analysis,BAPC)方法对2019—2035年中国NMSC的死亡趋势进行预测。

结果

2005—2018年NMSC全国的年龄标准化死亡率(age-standardized mortality rate,ASMR)为0.34/10万,呈上升趋势,AAPC为5.6%(95%CI: 3.8%~7.4%)。NMSC的ASMR男性为0.41/10万,高于女性的0.26/10万,但女性上升速度略快(AAPC=6.2%)。农村地区NMSC的ASMR为0.40/10万,高于城市地区的0.30/10万,但AAPC只有城市地区的一半。NMSC死亡率的风险随年龄增长而增加。队列效应中ASMR呈现下降趋势。预测中国NMSC的ASMR在2019—2035年将持续升高到0.78/10万(95%CI:0.10/10万~1.46/10万)。

结论

2005—2018年中国NMSC的ASMR呈上升趋势,且存在年龄、性别和地区差异。NMSC仍然是一个需要持续关注的公共卫生问题。

Objective

To analyze the mortality rate and trend of non-melanoma skin cancer (NMSC) in China from 2005 to 2018 and predict its future trend, providing a basis for effective prevention and control.

Methods

NMSC mortality data from 2005 to 2018 were collected from the Chinese Cancer Registry Annual Report. The Joinpoint regression model was used to estimate the average annual percentage change (AAPC) and annual percentage change (APC) to reflect temporal trends. An age-period-cohort model with intrinsic estimator algorithm was applied to analyze age, period, and cohort effects. The Bayesian age-period-cohort (BAPC) analysis with integrated nested Laplace approximation was employed to predict NMSC mortality trends in China from 2019 to 2035.

Results

From 2005 to 2018, the age-standardized mortality rate (ASMR) of NMSC in China was 0.34 per 100 000, showing an upward trend with an AAPC of 5.6% (95%CI: 3.8%-7.4%). The ASMR was higher in males (0.41 per 100 000) than in females (0.26 per 100 000),but the increase was slightly faster in females (AAPC=6.2%). Rural areas had a higher ASMR (0.40 per 100 000) than urban areas (0.30 per 100 000), but the AAPC was only half that of urban areas. The mortality risk of NMSC increased with age. The cohort effect showed a declining trend in ASMR. The predicted ASMR of NMSC in China is expected to continue rising to 0.78 per 100 000(95%CI: 0.10-1.46 per 100 000) during 2019 and 2035.

Conclusion

The ASMR of NMSC in China exhibited an upward trend from 2005 to 2018, with variations by age, sex, and region. NMSC remains a public health concern requiring sustained attention.

非黑色素瘤皮肤癌(non-melanoma skin cancer,NMSC)是全球范围内常见的一类恶性肿瘤,主要包括基底细胞癌(basal cell carcinoma,BCC)和鳞状细胞癌(squamous cell carcinoma,SCC)[1]。近年来,随着环境污染、紫外线暴露增加以及人口老龄化等因素的影响,NMSC的死亡率在全球范围内不断上升,不论是在发展中国家,还是在发达国家,已成为重大的公共卫生问题之一[2-3],高昂的治疗费用给患者、家庭、社会及医疗系统带来了沉重负担[4-6]。我国皮肤恶性黑色素瘤的死亡率和DALY率在保持稳定中略有下降,但是NMSC的死亡率和DALY率却远高于全球水平[5]。本研究利用2005—2018年中国肿瘤登记年报数据,分析中国NMSC的死亡现状和未来趋势,旨在为制定有效预防措施提供基础依据。
2005—2018年中国NMSC死亡率数据来源于2008—2021年中国肿瘤登记年报。原始数据来自全国31个省(自治区和直辖市)和新疆生产建设兵团肿瘤登记处。根据国际疾病分类第10次修订版诊断NMSC(ICD10,C44)。本研究的城乡分类依据《中国肿瘤登记年报》的分类标准:城市地区归为地级以上城市,农村地区归为县及县级市。根据中国肿瘤登记指南的要求和国际癌症研究机构/国际肿瘤登记协会的标准,对数据进行审查、评价、整理和分析。
NMSC的死亡趋势采用joinpoint回归模型计算年均变化百分比(average annual percentage change,AAPC)及其95 %置信区间(confidence interval,CI)。该模型根据疾病分布的时间特征创建分段回归,通过多个连接点将研究时间划分为不同的区间,并对每个区间内的趋势进行拟合和优化,以评估不同区间内特定疾病变化的特征。采用蒙特卡罗置换检验确定连接点个数、每个连接点的位置以及对应的P值,双侧检验,检验水准α=0.05。Joinpoint回归模型是线性和对数线性模型,在分析基于人群的癌症发病和死亡趋势时,一般选择对数线性模型,公式如下:

其中,是回归系数,ωi是每个分割函数的区间跨度(即区间所包含的年数)的宽度,βi是每个区间对应的回归系数。
采用年龄-时期-队列模型分析年龄、时期、队列对NMSC死亡率的影响。采用泊松对数线性模型,通过求解内在估计量对该模型进行评价,并采用红池信息准则对模型拟合优度进行综合评价[7]。在该模型中,将特定年龄的比率适当地重新编码为连续的5年年龄组(0~4、5~9、…、85~89),2005—2018年的连续5年时期组,以及相应的连续5年出生队列(1920—1924、1925—1929、…、2010—2014、2015—2019),以估计NMSC死亡率的净年龄、时期以及队列效应,公式如下:
其中,Yj为第j组的响应变量,表示对NMSC死亡率的净影响,αβγ分别为年龄、时期和队列的系数,μ为截距,ε为残差。为了避免扩大出生队列和降低描述发病和死亡风险的时间精度,本研究使用2005、2010和2015年的年龄特定数据进行年龄-时期-队列模型的模拟。
使用BAPC模型预测中国未来15年NMSC的死亡风险。在该模型中,二阶随机游走模型用于平滑先验的年龄、时期和队列效应,以预测后验死亡率。我们的方法结合了集成嵌套拉普拉斯近似,防止了马尔科夫链蒙特卡洛采样引起的混合和收敛问题,从而得到更稳定和可靠的数据[8]
采用美国国立癌症研究所开发的joinpoint回归Program 4.9. 1.0软件进行joinpoint回归分析,计算2005—2018年的AAPC及其95%CI,当AAPC及其95%CI均>0、均<0、包含0时,分别代表相应疾病指标呈现上升趋势、下降趋势、保持平稳[9]。年龄-时期-队列模型使用Rosenberg和Check等人开发的在线网络分析工具和Stata 17.0软件(StataCorp,College Station,TX,USA)进行。使用R软件的BAPC和INLA包进行统计预测,“ggplot2”包绘制图形。检验水准α=0.05。
2005—2018年,中国NMSC死亡总数为21 557例,其中男性12 002例(55.68%),女性9 555例(44.32%),城市10 818例(50.18%),农村10 739例(49.82%);粗死亡率为0.72/10万,其中男性0.80/10万,女性0.65/10万,城市0.69/10万,农村0.76/10万;ASMR为0.34/10万,其中男性0.41/10万,女性0.26/10万,城市0.30/10万,农村0.40/10万。男性和农村NMSC的粗死亡率和ASMR分别高于女性和城市。见表1
中国NMSC的ASMR从2005年的0.23/10万增加到2018年的0.41/10万,AAPC=5.6%(95%CI: 3.8%~7.4%)。男性NMSC的ASMR从2005年的0.31/10万增加到2018年的0.50/10万,AAPC=5.0%(95%CI:3.2%~6.8%),女性的ASMR从2005年的0.16/10万增加到2018年的0.33/10万,AAPC=6.2%(95%CI: 4.2%~8.1%)。城市NMSC的ASMR由2005年的0.20/10万上升至2018年的0.36/10万,AAPC= 5.8%(95%CI:3.9%~7.7%),农村ASMR由2005年的0.33/10万上升至2018年的0.45/10万,AAPC=2.6%(95%CI: 0.85%~4.3%)。见表1 2图1
中国NMSC死亡风险随着年龄的增长而增加,至85~89岁达到高峰。
在时期效应中,中国NMSC的死亡风险随着时间的推移逐渐增加。除农村人群外,2005—2018年中国NMSC死亡率呈缓慢上升趋势。农村NMSC死亡率风险呈先下降后上升的趋势,以2010年为拐点。
NMSC死亡风险的队列效应在1920—1924年达到顶峰(RR=1.98,95%CI: 1.3~2.66),然后在1995—1999年下降并达到最低点(RR=-1.99,95%CI: -3.06~-0.91),随后在2000—2019年呈波浪式升高。见图2
2019—2035年,中国NMSC的死亡预测显示其ASMR逐渐升高,从全国范围来看,ASMR预计将从2019年的0.57/10万(95%CI:0.52/10万~0.62/10万)逐渐增加到2035年的0.78/10万(95%CI:0.10/10万~1.46/10万),特别是男性,ASMR预计升至1.13/10万(95% CI: 0.13/10万~2.12/10万)。
相比之下,女性的ASMR预计将从2019年的0.45/10万(95%CI:0.40/10万~0.50/10万)上升至2034年的0.54/10万(95%CI:0.03/10万~1.05/10万),在2035年预计为0.55/10万(95%CI:-0.01/10万~1.10/10万);在城市地区,ASMR预计将从2019年的0.53/10万(95%CI:0.47/10万~0.59/10万)上升到2034年的0.76/10万(95%CI:0.04/10万~1.49/10万),在2035年预计为0.79/10万(95%CI:-0.02/10万~1.59/10万);在农村地区,ASMR预计将从2019年的0.60/10万(95%CI:0.52/10万~0.67/10万)上升到2032年的0.67/10万(95%CI:0.05/10万~1.30/10万),在2035年预计为0.70/10万(95%CI:-0.13/10万~1.53/10万)。见表3
中国NMSC死亡率从2005—2018年持续上升,全国ASMR从0.23/10万增至0.41/10万,预计到2035年将达到0.78/10万。这一趋势在所有亚组中均有体现,但增长速度有所不同。
中国NMSC死亡率增长速度呈现显著的性别和城乡差异,特别是城市地区和女性人群。与农村相比,城市地区的NMSC死亡率增长较快,可能与城市地区人口密集污染较重有关。在城市地区,影响居民健康状况的环境因素主要是空气污染和工业垃圾污染[10]。皮肤作为机体和环境之间的屏障,是直接暴露在环境污染物中的最大器官,污染因子可以通过损害或降解角蛋白使皮肤丧失保护功能而进入组织致病[11]。暴露于高浓度的许多室外空气污染物(多环芳烃氢燃料碳含量、挥发性有机化合物、颗粒物)和室内空气污染物(固体燃料消耗),可能会中断皮肤的正常屏障功能[12]。Augustin等[13]的研究结果表明区域社会经济因素与NMSC相关。如高教育水平和高收入与较高的NMSC发病率呈正相关,这可能是因为经济状况较好的人群外出活动增加,导致接触紫外线的概率增加。因此,针对不同人群采取差异化的预防和干预措施显得尤为重要。
在NMSC死亡率上中国男性和农村人群显现出相对劣势,这可能与接受紫外线辐射更多有关[12,14]。全球GBD数据分析同样显示男性在NMSC死亡率上的劣势[3]。有研究表明美国南半部NMSC的死亡率高于北半部,这些差异可能是由各州之间的紫外线指数差异来解释的,因为南部各州的紫外线指数通常高于北部各州[15]。在一些阳光照射充足的地区和国家,居民患NMSC的风险明显更高[16]。男性更可能从事户外工作,暴露于紫外线辐射和工业污染物的机会更多。此外,男性在阳光防护措施上的重视程度可能低于女性,一项针对男性涂抹防晒霜的研究表面大多数男性表明没有经常使用防晒霜的习惯[17]。因此,未来的公共卫生策略应特别关注男性群体,尤其是户外工作者,加强防晒和防护措施教育。中国农村人群长期的户外农业劳作也可能导致紫外线接触过多,同时农村地区的经济水平较低也会影响农村人群对皮肤的防护。此外,农村居民的健康教育程度较低,防晒意识和防护措施不如城市居民。这些认知不佳,如教育水平低和缺乏健康意识,会增加皮肤癌的发病率和死亡率[18]。公共卫生政策应着重改善农村地区的医疗服务水平,增加早期筛查和诊断的可及性,并加强健康教育,提高农村居民的防晒意识。
NMSC死亡风险随年龄增长显著增加,同时年龄对NMSC死亡率的影响在全国及城乡人群中基本一致。在0~44岁人群中,NMSC死亡风险显著降低,而在45~89岁范围内,NMSC死亡风险增大且增速趋快。研究发现,皮肤癌的发生与年龄相关[19],这与本研究结论一致。特别是在女性和农村人口中,年龄效应显著且变化剧烈,55~89岁人群中死亡率的大幅上升可能与某些外部因素有关,如女性平均寿命高于男性导致高年龄组女性更容易因NMSC死亡,也可能是由于慢性积累暴露和老年人皮肤屏障弱化导致晚年的死亡率大幅上升[20]。流行病学调查显示,50岁以上的人群更易出现皮肤癌[19],老年人群的NMSC负担更大[21]。队列效应分析显示,1920—1924年出生的群体死亡风险最高,1995—1999年出生的群体死亡风险最低,随后2000—2019年出生的群体呈波浪式升高。这一趋势可能反映了不同年代人群在紫外线暴露、生活方式和医疗条件上的差异。1920—1924年出生队列的高风险可能与当时的生活方式、环境条件及医疗水平有关。尽管越早的出生队列所处的环境污染更少,但由于长年累月的紫外线暴露,他们因NMSC死亡的风险也在不断积累[20]。1995—1999年出生的群体死亡风险最低可能说明成年人在青年时期的皮肤屏障处在最佳状态。而2000—2019年出生队列NMSC死亡风险的略有上升,可能是由于青少年的皮肤处于发育阶段,皮肤屏障在逐步完善,此时收到紫外线照射受损的可能更大,且随着年龄增加,受到的紫外线也缓慢积累,导致NMSC风险上升。这提示我们,不同出生队列的个体受外部环境因素和生活方式变化的影响显著,在制定NMSC预防措施时,应结合不同队列的特点,采取有针对性的干预措施。
另外,预测结果显示,到2035年所有亚组的ASMR均有上升,但95%CI较宽,表明未来趋势存在较大不确定性。这可能反映模型对远期预测的局限性,未来还需要结合更多数据进一步验证。同时存在部分年份的预测AMSR的95%CI包含0,同样反映出不确定性增加。预测结果也凸显了明显的性别差异,与女性相比,男性的NMSC死亡率表现出更明显且统计上显著的增加。而城市和农村地区的地理差异在长期预测中显示出类似的不确定性模式。但从全中国NMSC的ASMR预测趋势来看,中国NMSC在未来呈现不断上升的趋势,提醒我们要提高对NMSC的研究和预防。
本研究存在一定的局限性。NMSC数据来源于2005—2018年中国肿瘤登记年报,原始数据来自全国肿瘤登记,而不是随机抽样,因此对整个人群的代表性和外推结果是不足的。本研究在时效性方面也存在局限性,因为最新的肿瘤登记数据一般存在三年的时间延迟。而我国目前最新出版的肿瘤登记年报数据仅更新了2018年的肿瘤统计数据。由于可用数据不足,本研究未对NMSC进行分型研究。
总之,2005—2018年,中国NMSC死亡率在全国范围内呈现持续上升趋势,到2035年也将继续上升。男性NMSC的ASMR高于女性,但女性的增长速度略高于男性。农村ASMR高于城市,但城市增长速度高于农村。为应对这一趋势,制定有效的预防措施和策略,尤为重要,而减少紫外线接触是降低NMSC死亡率的主要方法,环境污染和皮肤癌认知也应作为策略制定的核心内容。因此,我们需要提高公众的健康意识、推广防晒措施和改善环境污染等。
