Objective

To investigate the knowledge and attitudes of tuberculosis (TB) control workers regarding preventive treatment for latent tuberculosis infection (LTBI) in Xinjiang Uygur Autonomous Region, providing evidence for promoting LTBI preventive treatment.

Methods

A cross-sectional survey was conducted among TB control workers from 14 prefectures in Xinjiang. Data were entered using EpiData 3.1 and analyzed with SPSS 26.0, with statistical significance set at P<0.05.

Results

Among 235 respondents,92.77% (218/235) knew about LTBI, 63.40% (149/235) understood the probability of LTBI progression, 73.62% (173/235) knew screening methods, 29.79% (70/235) recognized high-risk populations, 65.11% (153/235) knew target groups for preventive treatment, and 60.85% (143/235) understood treatment efficacy. Of those aware of LTBI, 71.56% (156/218) believed preventive treatment was necessary. Significant differences (P<0.05) were found in recommending preventive treatment by gender (χ2=4.565, P=0.033), occupation (χ2=10.839, P=0.004), knowledge of treatment efficacy (χ2=16.490, P<0.001), perceived necessity (χ2=122.919, P<0.001), and training experience (χ2=5.200, P=0.023). Main concerns included uncertain efficacy (39.57%), adverse reactions (32.34%), public knowledge gaps (15.32%), and implementation difficulties (12.77%).

Conclusion

TB control workers demonstrated limited knowledge and moderate acceptance of LTBI preventive treatment. Targeted training and education programs are needed to improve acceptance and implementation of preventive treatment among high-risk populations.

目的

探讨新疆维吾尔自治区(简称新疆)结核病防治人员对结核潜伏感染(latent tuberculosis infection, LTBI)者进行结核病预防性治疗的认知态度,旨在为促进结核病预防性治疗工作的推进提供科学依据。

方法

对新疆14个地州疾控机构和结核病定点医疗机构结核病防治人员进行调查。采用EpiData 3.1软件进行问卷数据录入,使用SPSS 26.0软件对收集的数据进行统计分析,以P<0.05为差异有统计学意义。

结果

92.77%(218/235)的防治人员知晓LTBI,63.40%(149/235)知晓LTBI发病概率,73.62%(173/235)对LTBI筛查方法知晓,29.79%(70/235)对LTBI高危人群知晓,65.11%(153/235)的防治人员对LTBI预防性治疗目标人群知晓,60.85%(143/235)清楚预防性治疗的效果。在知晓LTBI的防治人员中,71.56%(156/218)的防治人员认为有必要对LTBI者开展预防性治疗。不同性别(χ2=4.565,P=0.033)、不同职业类别(χ2=10.839,P=0.004)、是否知晓预防性治疗效果(χ2=16.490,P<0.001)、是否认为有必要对LTBI者开展预防性治疗(χ2=122.919,P<0.001)、是否参加过LTBI相关培训(χ2=5.200,P=0.023)在是否对LTBI者推荐预防性治疗之间比较,差异有统计学意义。“担心预防性治疗效果不明确”“担心预防性治疗会产生不良反应”“担心LTBI相关宣传较少,群众知识缺乏”“担心工作难度大,LTBI者不配合,难以开展”是医务人员对LTBI者开展预防性治疗主要担忧的问题。

结论

结核病防治人员对LTBI相关知识知晓情况以及对预防性治疗接受意愿均不高,应开展有针对性的培训和宣传教育活动,提高其对预防性治疗的接受度,以推动重点人群结核病预防性治疗工作的开展。

目的

探讨新疆维吾尔自治区(简称新疆)结核病防治人员对结核潜伏感染(latent tuberculosis infection, LTBI)者进行结核病预防性治疗的认知态度,旨在为促进结核病预防性治疗工作的推进提供科学依据。

方法

对新疆14个地州疾控机构和结核病定点医疗机构结核病防治人员进行调查。采用EpiData 3.1软件进行问卷数据录入,使用SPSS 26.0软件对收集的数据进行统计分析,以P<0.05为差异有统计学意义。

结果

92.77%(218/235)的防治人员知晓LTBI,63.40%(149/235)知晓LTBI发病概率,73.62%(173/235)对LTBI筛查方法知晓,29.79%(70/235)对LTBI高危人群知晓,65.11%(153/235)的防治人员对LTBI预防性治疗目标人群知晓,60.85%(143/235)清楚预防性治疗的效果。在知晓LTBI的防治人员中,71.56%(156/218)的防治人员认为有必要对LTBI者开展预防性治疗。不同性别(χ2=4.565,P=0.033)、不同职业类别(χ2=10.839,P=0.004)、是否知晓预防性治疗效果(χ2=16.490,P<0.001)、是否认为有必要对LTBI者开展预防性治疗(χ2=122.919,P<0.001)、是否参加过LTBI相关培训(χ2=5.200,P=0.023)在是否对LTBI者推荐预防性治疗之间比较,差异有统计学意义。“担心预防性治疗效果不明确”“担心预防性治疗会产生不良反应”“担心LTBI相关宣传较少,群众知识缺乏”“担心工作难度大,LTBI者不配合,难以开展”是医务人员对LTBI者开展预防性治疗主要担忧的问题。

结论

结核病防治人员对LTBI相关知识知晓情况以及对预防性治疗接受意愿均不高,应开展有针对性的培训和宣传教育活动,提高其对预防性治疗的接受度,以推动重点人群结核病预防性治疗工作的开展。

Objective

To investigate the knowledge and attitudes of tuberculosis (TB) control workers regarding preventive treatment for latent tuberculosis infection (LTBI) in Xinjiang Uygur Autonomous Region, providing evidence for promoting LTBI preventive treatment.

Methods

A cross-sectional survey was conducted among TB control workers from 14 prefectures in Xinjiang. Data were entered using EpiData 3.1 and analyzed with SPSS 26.0, with statistical significance set at P<0.05.

Results

Among 235 respondents,92.77% (218/235) knew about LTBI, 63.40% (149/235) understood the probability of LTBI progression, 73.62% (173/235) knew screening methods, 29.79% (70/235) recognized high-risk populations, 65.11% (153/235) knew target groups for preventive treatment, and 60.85% (143/235) understood treatment efficacy. Of those aware of LTBI, 71.56% (156/218) believed preventive treatment was necessary. Significant differences (P<0.05) were found in recommending preventive treatment by gender (χ2=4.565, P=0.033), occupation (χ2=10.839, P=0.004), knowledge of treatment efficacy (χ2=16.490, P<0.001), perceived necessity (χ2=122.919, P<0.001), and training experience (χ2=5.200, P=0.023). Main concerns included uncertain efficacy (39.57%), adverse reactions (32.34%), public knowledge gaps (15.32%), and implementation difficulties (12.77%).

Conclusion

TB control workers demonstrated limited knowledge and moderate acceptance of LTBI preventive treatment. Targeted training and education programs are needed to improve acceptance and implementation of preventive treatment among high-risk populations.

结核潜伏感染(latent tuberculosis infection, LTBI)是对结核分枝杆菌抗原刺激做出持续免疫应答反应的一种状态,但无任何临床病原学或影像学证据证明是活动性结核(active tuberculosis, ATB)。全球约有1/4的LTBI者,若未及时对LTBI者采取相应措施,在其一生中有5%~10%的可能进展为ATB[1]。世界卫生组织(World Health Organization, WHO)推荐对LTBI高危人群开展预防性治疗,并将其作为实现结核病消除目标的关键策略之一[2]。我国作为结核病高负担国家,也将预防性治疗纳入结核病防治的重要内容[3]。新疆维吾尔自治区(简称新疆)结核病发病率远高于全国平均水平[4],施行结核病防治“新疆模式”以来,已取得显著成效,但依旧是新疆重大公共卫生问题之一[5]。结核病防治人员对预防性治疗的认知态度对实施LTBI预防性治疗至关重要[6]。本研究由新疆结核病防治人员对LTBI者开展结核病预防性治疗的认知态度进行调查,旨在为当地制定有效的预防性防治策略提供科学依据。
研究对象涵盖新疆14个地州96个县市结核病防治人员,采用分层随机抽样法,以县区为单位,每个县区抽取疾控机构、定点医疗机构结核病防治人员各1~3名作为调查对象。在知情同意的情况下开展问卷调查,共调查235名结核病防治人员,回收问卷235份,有效问卷235份,问卷有效率100.00%。其中疾病预防控制中心结核病防治专职人员141名,结核病定点医疗机构医务人员94名。对26名结核病防治人员进行访谈,进一步了解其对LTBI的知晓情况及对预防性治疗的认知和态度。
根据研究目的,通过咨询专家并查阅相关文献[1,6-7]制定调查问卷,调查内容包括基本人口学特征(如年龄、性别、文化程度、相关工作从事时间等)、LTBI和结核病预防性治疗相关知识、对预防性治疗的态度及担忧的问题等。自行设计访谈提纲,对从事结核病防治人员进行面对面访谈,了解其对LTBI及预防性治疗认知情况,每名医务人员访谈时间约为20 min。
采用EpiData 3.1软件建立数据库,进行数据双录入,使用SPSS 26.0软件进行数据整理与分析。计数资料采用频数和构成比描述。以是否认为对LTBI者开展预防性治疗有必要作为因变量,单因素分析采用χ2检验,以P<0.05为差异有统计学意义。定性资料分析采用主题框架法,利用分类归纳总结的方法对观点、行为进行描述性分析[8]
本研究已获得新疆维吾尔自治区疾病预防控制中心伦理审查委员会的批准(批准编号:2023-016)。问卷调查和定性访谈均得到了研究对象的知情同意,并已签署纸质版知情同意书。
本研究共纳入结核病防治人员235名,其中男女性别比为1∶3.43,平均年龄为(38.94±9.32)岁。文化程度以本科及以上为主,占62.98%;研究对象包括疾控机构结核病防治人员和定点医疗机构结核门诊医务人员,分别占60.00%(141/235)和40.00%(94/235)。66.81%(157/235)的医务人员参加过LTBI相关培训,其中包括71.63%(101/141)的疾控机构结核病防治人员和59.57%(56/94)定点医疗机构结核门诊医务人员。
92.77%(218/235)的结核病防治人员知晓LTBI,63.40%(149/235)知晓LTBI发病概率,73.62%(173/235)的防治人员知晓LTBI筛查方法是结核菌素皮肤试验、γ-干扰素释放试验、重组结核杆菌融合蛋白,29.79%(70/235)的防治人员知晓LTBI的高危人群,65.11%(153/235)的防治人员知晓LTBI者预防性治疗的目标人群,60.85%(143/235)清楚预防性治疗的效果。在知晓LTBI的防治人员中,71.56%(156/218)的防治人员认为有必要对LTBI者开展预防性治疗。见表1
在知晓LTBI的218名防治人员中,64.68%(141/218)的防治人员推荐对LTBI者开展预防性治疗。女性(χ2=4.565,P=0.033)、知晓预防性治疗效果(χ2=16.490,P<0.001)、认为有必要对LTBI者开展预防性治疗(χ2=122.919,P<0.001)、参加过LTBI相关培训(χ2=5.200,P=0.023)的防治人员推荐开展预防性治疗的比例较男性、不知晓预防性治疗效果、认为没必要/不清楚是否有必要对LTBI者开展预防性治疗以及未参加过LTBI相关培训的防治人员高。不同职业类别的防治人员在是否推荐预防性治疗方面也存在显著差异(χ2=10.839,P=0.004)。见表2
218名知晓LTBI的防治人员对预防性治疗提出了不同程度的担忧,其中“担心预防性治疗会产生不良反应”“担心LTBI相关宣传较少,群众知识缺乏”“担心预防性治疗效果不明确”是不推荐对LTBI者开展预防性治疗主要担忧的问题,分别占92.21%、89.61%、80.52%。在推荐对LTBI者开展预防性治疗的防治人员中,“担心预防性治疗保护效果不明确”则是最为担忧的问题,占46.10%。见表3
在本次访谈中,22名(84.62%)防治人员表示赞成对LTBI者开展预防性治疗,赞成的原因主要可以概括为:当地为结核病高负担地区,开展预防性治疗可以有效降低结核病发病率,同时对学生群体开展预防性治疗更具有价值,无论是对社会、家庭还是个人都是有益的。对预防性治疗持反对的原因主要可概括为以下几点:一是没有相关政策支持,预防性治疗工作难以开展;二是无法确定治疗效果,不能保证治疗后能够100%不发病;三是担心预防性治疗后,LTBI者对结核分枝杆菌产生耐药,更不利于其治疗;四是担心预防性治疗可能会出现的不良反应,特别是对年龄较小、免疫缺陷LTBI者的健康影响。同时防治人员也对提高LTBI者预防性治疗接受度提出了建议,主要包括:加强宣传,提高LTBI者的认知水平;加强对相关防治人员培训,在防治人员中对LTBI预防性治疗态度形成统一认知;给予一定的激励政策,如为接受预防性治疗的人群提供营养早餐、交通补助或其他生活补助;对取药不方便的LTBI者实施线上管理,如电子药盒或视频面视下服药。关于在全疆范围内推广结核病预防性治疗政策的建议,主要包括以下几个方面:一是政策经费保障;二是技术支持;三是加强宣教;四是多部门协作。
对LTBI者进行预防性治疗是降低LTBI者进展为ATB风险的重要措施,也是实现终止结核病流行这一目标的重要策略[9]。我国出台的《中国结核病防治工作技术指南》及《中国结核病预防性治疗指南(2023版)》中,明确要求对结核病发病高危人群包括肺结核患者密切接触者、艾滋病病毒感染者/艾滋病患者以及其他高风险人群开展预防性治疗工作[10-11]。从事相关结核病防治工作的结核病防治人员不仅是结核病防治措施的实施者,同时也是感染结核分枝杆菌高风险人群[12]。因此提高防治人员对LTBI的认知水平,一方面可以积极推进结核潜伏感染预防性治疗工作,另一方面也可以有效提高防治人员自身对预防性治疗的接受度[13]
本研究发现,大部分结核病防治人员知晓LTBI,但对于LTBI高危人群、发病概率、预防性治疗目标人群以及预防性治疗保护效果的知晓程度相对较低,这与重庆市对结核病防治人员的调查结果相同[7]。知晓LTBI的防治人员中,仅64.68%的防治人员推荐对LTBI者开展预防性治疗,推荐占比相对较低。参加过LTBI相关培训、知晓预防性治疗效果的防治人员更有可能推荐开展预防性治疗。以往研究表明,对LTBI及其治疗益处了解不足是防治人员不推荐或不愿意接受预防性治疗的重要原因[14]。因此加强对防治人员开展结核潜伏感染及预防性治疗相关培训,将有效提高其对预防性治疗的推荐程度。在本研究中,担心预防性治疗会产生不良反应、担心LTBI相关宣传较少,群众知识缺乏、担心预防性治疗保护效果不明确是防治人员不推荐对LTBI者开展预防性治疗的主要担心问题。
在本次访谈过程中,大部分防治人员对开展预防性治疗持肯定态度,这与我国先前开展的研究结果一致[6]。少数对开展预防性治疗持反对态度的防治人员则认为,治疗过程中发生不良反应、耐药、不配合、影响身心健康、无法保证治疗效果、无政策支持等是阻碍预防性治疗开展关键因素,这同国内外研究结果相似[15-17]。结核病防治人员对推广预防性治疗也提出了相关建议,包括政策经费保障、技术支持、加强宣教、多部门协作等。推广预防性治疗涉及多个领域,也受到多种因素的影响,包括政府重视程度、医疗卫生机构人员配置、当地卫生资源、相关人群接受预防性治疗的意愿等[18]
