目的

系统评价中国男男性行为（men who have sex with men, MSM）者HIV暴露后预防用药接受意愿的影响因素。

方法

在PubMed、Web of Science、The Cochrane Library、EMbase、中国知网、维普、万方和CBM数据库中，检索2024年9月5日前MSM HIV暴露后预防用药接受意愿相关因素的研究，采用RevMan 5.3软件分别对纳入的影响因素进行meta分析。

结论

我国MSM者HIV暴露后预防用药接受意愿的影响因素较多且复杂，未来应加强对HIV和PEP相关知识的宣传和推广，降低MSM人群中HIV发病率。

Objective

To systematically evaluate the factors influencing the willingness to accept HIV post-exposure prophylaxis (PEP) among men who have sex with men (MSM) in China.

Methods

Studies related to the willingness to accept HIV PEP among MSM published before September 5, 2024, were searched in PubMed, Web of Science, The Cochrane Library, EM base, China National Knowledge Infrastructure (CNKI), Wan fang, VIP, and CBM databases. Meta-analysis of the included factors was conducted using Rev Man 5.3 software.

Results

A total of 26 studies were included, with 31 600 participants, and 20 influencing factors were extracted. The results of the meta-analysis indicated that factors promoting the willingness to accept HIV PEP included: age ≤ 30 years (OR=1.65, 95%CI:1.31-2.07), monthly income of 5 000-10 000 yuan (OR=1.96, 95%CI: 1.51-2.55), monthly income > 10 000 yuan (OR=1.98, 95%CI:1.03-3.80), having an HIV-positive sexual partner (OR=2.47, 95%CI: 1.21-5.05), multiple sexual partners (OR=2.51, 95%CI: 1.90-3.32), online sexual partners (OR=2.03, 95%CI: 1.55-2.65), awareness of HIV-related knowledge (OR=2.33, 95%CI: 1.81-3.00), awareness of PEP-related knowledge (OR=2.17, 95%CI: 1.33-3.54), having consulted about PEP (OR=26.09, 95%CI: 5.34-127.41), use of recreational drugs (OR=2.01, 95%CI: 1.18-3.42), condom use (OR=2.10, 95%CI: 1.66-2.66), HIV testing at least twice a year (OR=2.33, 95%CI: 1.80-3.02), sexually transmitted infections (OR=1.93, 95%CI: 1.52-2.45), commercial sex work (OR=3.21, 95%CI:1.95-5.30), and peer education (OR=1.60, 95%CI: 1.23-2.08). The factor hindering willingness was identified as being gay (OR=0.34,95%CI: 0.23-0.49).

Conclusion

The factors influencing the willingness to accept HIV PEP among MSM in China are various and complex. Future efforts should focus on enhancing the promotion and dissemination of knowledge related to HIV and PEP to reduce the incidence of HIV within the MSM population.

2021年研究报道，在我国新报告的HIV感染者中有97.9%为性传播感染，其中通过男男性行为（men who have sex with men, MSM）性传播占26.5%^[1]。目前，针对MSM人群采取的预防策略为安全套使用、健康教育、HIV咨询检测等，但该人群无保护性行为发生率仍旧较高^[2]。暴露后预防（post-exposure prevention，PEP）是一种新型生物医学预防措施，在个体发生暴露后72h内开始并连续服用抗病毒药物28 d，具有良好的预防效果^[3]。WHO表明在发生暴露2h内服用PEP药物可有效降低80%感染HIV的风险^[4]，而一篇系统评价表明，我国MSM人群PEP接受率为43%，整体接受意愿不高^[5]。近年来，国内外学者对我国MSM人群PEP使用意愿影响因素开展了大量研究，但由于研究时间、地区等不同，各研究结果的影响因素存在较大差异。因此，本研究旨在对我国MSM者HIV暴露后预防用药的影响因素进行meta分析，以期为促进PEP在MSM人群中推广、降低HIV发病率提供循证依据。

1　资料与方法

1.1　文献检索

在PubMed、Web of Science、The Cochrane Library、EMbase、中国知网、维普、万方和CBM数据库中，检索2024年9月5日前MSM者HIV暴露后预防用药接受意愿相关因素的文献，中文检索词为：男男性行为、男同性恋、MSM、男男性行为人群；暴露后预防、暴露后预防用药、PEP；相关因素、影响因素；英文检索词为：Sexual and Gender Minorities，Homosexual men，MSM，men who have sex with men；Post-Exposure Prophylaxis，PEP，Post Exposure Prevention，biomedical HIV prevention，HIV prophylaxis；willingness, adherence, compliance，acceptability，medical obedience。文献检索以PubMed数据库为例，见表1。

1.2　纳入与排除标准

纳入标准：（1）研究对象为中国的MSM者；（2）研究类型为横断面研究、队列研究和病例对照研究；（3）结局指标为多因素回归分析所得的MSM者HIV暴露后预防用药的相关因素，提供OR值和95%CI。排除标准：（1）无法获取全文或数据错误的文献；（2）综述、学术会议、系统评价等；（3）文献质量评分<4分。

1.3　筛选文献和资料提取

采用Endnote X9软件剔除重复文献后，两名研究者分别阅读标题摘要后核对剩余文献，再进行全文阅读复筛和资料提取，分歧由第三名研究员评估后决定。提取文献内容包括：作者、发表年份、地区、调查时间、年龄、样本量、影响因素。

1.4　文献质量评价

本研究采用美国卫生保健质量和研究机构（Agency for Healthcare Research and Quality，AHRQ）推荐的质量评价标准^[6]，总分11分，8～11分、4～7分、0～3分，分别对应高、中、低质量文献。

1.5　统计学分析

采用RevMan 5.3软件对MSM者HIV暴露后预防用药接受意愿相关因素进行meta分析。异质性检验若P≤0.1且I²>50%，采用随机效应模型；反之，采用固定效应模型。转变模型法评估结果稳定性；Egger检验进行发表偏倚检验，若存在发表偏倚（P<0.05），采用剪补法对结果进行判断。

2　结果

2.1　文献筛选流程及结果

数据库共检索出2 005篇文献，按流程剔除重复文献后进行初筛、复筛，最终纳入文献26篇^[7-32]，见图1。

2.2　纳入文献基本特征及质量评分结果

纳入26篇^[7-32]文献均为横断面研究，合计样本量31 600例，共20个影响因素，文献质量评分为5～9分，文献质量中高等，见表1。

2.3　MSM者HIV暴露后预防用药接受意愿相关因素的meta分析结果

本研究将纳入文献数≥2篇的影响因素进行meta分析，结果显示，年龄≤30岁、月收入5 000～10 000元、月收入>10 000元、HIV阳性性伴侣、多个性伴侣、网约性伴侣、知晓HIV相关知识、知晓PEP相关知识、咨询过PEP、使用性毒品、使用安全套、HIV检测≥2次/年、性传播感染、商业性行为、同伴性教育是MSM者暴露后预防用药的促进因素，同性恋是其阻碍因素。Meta分析结果见表2。

2.4　敏感性分析

将20个影响因素分别转换随机和固定效应模型进行敏感性分析。结果显示，两种效应模型合并后的OR值、95%CI、P值较为接近，提示本研究meta分析结果具有稳定性，见表3。

2.5　发表偏倚检验

采用Egger检验进行发表偏倚检验，结果显示，年龄≤30岁、知晓PEP相关知识、性传播感染史存在发表偏倚（P<0.05），其余结果无发表偏倚，见表3。采用剪补法进行校正，剪补前后研究结果均未发生逆转，说明发表偏倚对此次研究结果无显著影响，剪补图以知晓PEP相关知识为例，见图2。

3　讨论

3.1　年龄对MSM者HIV暴露后预防用药接受意愿的影响

研究表明年龄≤30岁的MSM者更愿意在暴露后采取预防用药，与刘思辰等^[11]研究结果一致，可能与年轻人性生活较为频繁，长期活跃于社交网络平台更加容易接受新事物和拥有更多渠道获取药物有关。

3.2　月收入水平对MSM者HIV暴露后预防用药接受意愿的影响

Hess等^[33]研究指出经济负担较重是MSM者使用PEP的主要阻碍因素之一，本研究表明月收入5 000～10 000元、月收入>10 000元的MSM者均愿意接受PEP，且月收入水平高者对PEP接受程度更高，可能与HIV暴露后预防用药所需费用较高，而月收入较高者更能负担得起该费用有关。

3.3　性伴侣情况对MSM者HIV暴露后预防用药意愿的影响

HIV阳性性伴侣的MSM者更愿意接受PEP，可能是MSM者与HIV阳性者发生性关系后，认为自身感染HIV的风险较大，促进其使用PEP相关药物^[34]。Zou等^[35]研究表明多个性伴侣或网约性伴侣的MSM者发生无套肛交等危险性行为的可能性更大，更愿意使用PEP药物，与本研究结果一致。

3.4　疾病相关知识对MSM者HIV暴露后预防用药意愿的影响

知晓HIV相关知识、知晓PEP相关知识、咨询过PEP的MSM者更愿意采取暴露后预防用药措施，可能与MSM者知晓较多的HIV相关知识，预防感染意识增强有关，且研究表明咨询PEP能知晓更多PEP相关知识，认识到该措施在预防HIV方面的有效性和重要性，提高了MSM者对HIV暴露后预防用药使用意愿^[36]。

3.5　个人因素对MSM者HIV暴露后预防用药意愿的影响

研究表明使用性毒品、商业性行为史是MSM者使用PEP的促进因素，可能与性毒品使用与无保护性行为密切相关，而商业性行为发生群交的可能性增大^[37]，这些高危性行为均增大了HIV感染风险，反向提高了预防HIV的意识，故寻求阻断药物预防HIV感染。使用安全套和HIV检测≥2次/年的MSM者使用PEP意愿更大，可能与该类人群对HIV相关知识更为了解，比较关注自身身体健康且有较强的防护意识有关^[38]。有性传播感染史的MSM者更愿意使用PEP，可能与有性传播感染史的MSM者曾有过高危性行为有关，具体原因还需更多研究予以证明。Turpin等^[39]指出接受过同伴性教育的MSM者能积极参加预防HIV相关活动，知晓预防感染疾病的相关知识，对PEP的接受程度有所提升，与本研究结果一致。

本研究结果表明同性恋接受PEP的可能性较低，与MSM人群相比，异性恋或双性恋人群的性伴侣关系较为复杂，且我国有大量MSM人群与异性长期维持婚姻关系^[40]，有婚姻与家庭的异性恋或双性恋人群更担心HIV感染，更愿意接受PEP服务。Garnett等^[38]研究也指出同性恋MSM人群定期做HIV检测，知晓更多PEP相关副作用知识和对HIV的羞耻感较低，相继对PEP接受程度越低，与本研究结果一致。而Leshin等^[41]研究指出，MSM人群对PEP接受意愿高于异性恋人群，与本研究结果相反，可能与教育水平、地域文化、社会包容度不同等因素有关。

研究局限性：（1）由于部分高危性行为属于敏感性话题，数据收集可能存在披露偏差影响研究结果；（2）部分影响因素合并效应量异质性较大；（3）个别研究结果不稳定，可信度有待验证；（4）纳入研究均为横断面研究，因果论证强度较低，未来尚需更多大样本、前瞻性队列研究予以验证。
