Objective

To investigate the knowledge of monkeypox among men who have sex with men (MSM) in Central Jiangsu Province in 2023, and to provide evidence for the prevention and treatment of monkeypox in this population.

Methods

The monkeypox questionnaire designed by Jiangsu Provincial Center for Disease Control and Prevention was used to investigate HIV Voluntary Counseling & Testing (VCT) clinic and social organizations in central Jiangsu Province. Excel 2010 and SPSS19.0 software were used to organize and analyze the data. Also, univariate and multivariate Logistic regressions were used to analyze the factors affecting the awareness rate of monkeypox knowledge. χ2 test was used to compare the difference of concern degree of monkeypox among people with different characteristics.

Results

A total of 1 290 valid questionnaires were collected. The majority were aged from 26 to 50 years, accounting for 65.04% (839/1 290). The majority of the students were college students, bachelor students or above, accounting for 64.26% (829/1 290). The awareness rate of monkeypox was 26.12%, and the complete awareness rate was 15.27%. The results of multivariate Logistic regression analysis showed that the related factors with higher awareness rate of monkeypox included Taizhou (OR=6.710, 95%CI: 4.309-10.447), high school/technical school (OR=2.461, 95%CI: 1.270-4.769), college (OR=2.515, 95%CI: 1.311-4.825), bachelor’s degree or above (OR=2.253, 95%CI: 1.184-4.288), concerned about monkeypox epidemic and related situation (OR=1.684, 95%CI: 1.188-2.387) (all P < 0.05). The correct rates of monkeypox symptoms, preventive measures and transmission routes were 30.78%, 42.25% and 27.75%, respectively. 1 186 people (91.94%) paid attention to monkeypox epidemic and prevention knowledge from WeChat, Weibo and other network new media. MSM with different regions (χ2=65.348, P<0.001), age groups (χ2=19.422, P=0.001), educational levels (χ2=31.447, P<0.001), marital status (χ2=11.162, P=0.025), results of the latest HIV test (χ2=37.793, P<0.001), sexual orientations (χ2=39.441, P<0.001), whether they had sexual contact with the same sex in the last 6 months (χ2=29.329, P<0.001), and whether they had sexual contact with overseas personnel (χ2=29.199, P<0.001), paid different attention to monkeypox information, and the differences were statistically significant.

Conclusion

The awareness rate of MSM monkeypox knowledge in central Jiangsu Province is low, and the accuracy rate of monkeypox symptoms, transmission routes and prevention and control measures are low. Therefore, it is necessary to publicize monkeypox symptoms and transmission routes through new media such as WeChat, Weibo, Douyin and Baidu in the later stage.

目的

了解2023年江苏省苏中地区男男性行为者(Men who have sex with men,MSM)猴痘知识知晓情况,为制定该人群猴痘防治措施提供依据。

方法

利用江苏省疾控中心设计的猴痘调查表,对苏中地区艾滋病自愿咨询检测(HIV Voluntary Counseling & Testing,VCT)门诊和社会组织MSM进行问卷调查,采用单因素和多因素logistic回归分析影响猴痘知识知晓率的因素。采用χ2检验比较不同特征人群对猴痘关注程度的差异。

结果

本次调查共回收有效问卷1 290份。年龄以26~50岁为主,占比65.04%(839/1 290);文化程度以大学专科、本科及以上为主,占比64.26%(829/1 290)。猴痘知识知晓率26.12%,完全知晓率15.27%。多因素logistic回归分析结果显示:猴痘知晓率较高的相关因素包括地区为泰州市(OR=6.710,95%CI:4.309~10.447),文化程度为高中/中专/技校(OR=2.461,95%CI:1.270~4.769)、大学专科(OR=2.515,95%CI:1.311~4.825)、本科及以上(OR=2.253,95%CI:1.184~4.288),对于猴痘疫情及相关情况比较关注(OR=1.684,95%CI:1.188~2.387)(均P<0.05)。 猴痘症状、预防措施和传播途径正确率分别为30.78%、42.25%和27.75%。1 186人(91.94%)从微信、微博等网络新媒体关注猴痘的疫情和防治知识。不同地区(χ2=65.348,P<0.001)、年龄(χ2=19.422,P=0.001)、文化程度(χ2=31.447,P<0.001)、婚姻状况(χ2=11.162,P=0.025)、最近一次HIV检测结果(χ2=37.793,P<0.001)、性取向(χ2=39.441,P<0.001)、最近6个月是否与同性有过性接触(χ2=29.329,P< 0.001)、是否与境外人员有过性接触(χ2=29.199,P<0.001)的MSM,对猴痘信息关注程度不同,差异有统计学意义。

结论

苏中地区MSM猴痘知识知晓率较低,猴痘症状、传播途径和预防控制措施正确率均偏低,所以后期需要在微信、微博、抖音、百度等新媒体采用投放广告、信息推送等形式开展猴痘症状和传播途径等知识的宣传。

目的

了解2023年江苏省苏中地区男男性行为者(Men who have sex with men,MSM)猴痘知识知晓情况,为制定该人群猴痘防治措施提供依据。

方法

利用江苏省疾控中心设计的猴痘调查表,对苏中地区艾滋病自愿咨询检测(HIV Voluntary Counseling & Testing,VCT)门诊和社会组织MSM进行问卷调查,采用单因素和多因素logistic回归分析影响猴痘知识知晓率的因素。采用χ2检验比较不同特征人群对猴痘关注程度的差异。

结果

本次调查共回收有效问卷1 290份。年龄以26~50岁为主,占比65.04%(839/1 290);文化程度以大学专科、本科及以上为主,占比64.26%(829/1 290)。猴痘知识知晓率26.12%,完全知晓率15.27%。多因素logistic回归分析结果显示:猴痘知晓率较高的相关因素包括地区为泰州市(OR=6.710,95%CI:4.309~10.447),文化程度为高中/中专/技校(OR=2.461,95%CI:1.270~4.769)、大学专科(OR=2.515,95%CI:1.311~4.825)、本科及以上(OR=2.253,95%CI:1.184~4.288),对于猴痘疫情及相关情况比较关注(OR=1.684,95%CI:1.188~2.387)(均P<0.05)。 猴痘症状、预防措施和传播途径正确率分别为30.78%、42.25%和27.75%。1 186人(91.94%)从微信、微博等网络新媒体关注猴痘的疫情和防治知识。不同地区(χ2=65.348,P<0.001)、年龄(χ2=19.422,P=0.001)、文化程度(χ2=31.447,P<0.001)、婚姻状况(χ2=11.162,P=0.025)、最近一次HIV检测结果(χ2=37.793,P<0.001)、性取向(χ2=39.441,P<0.001)、最近6个月是否与同性有过性接触(χ2=29.329,P< 0.001)、是否与境外人员有过性接触(χ2=29.199,P<0.001)的MSM,对猴痘信息关注程度不同,差异有统计学意义。

结论

苏中地区MSM猴痘知识知晓率较低,猴痘症状、传播途径和预防控制措施正确率均偏低,所以后期需要在微信、微博、抖音、百度等新媒体采用投放广告、信息推送等形式开展猴痘症状和传播途径等知识的宣传。

Objective

To investigate the knowledge of monkeypox among men who have sex with men (MSM) in Central Jiangsu Province in 2023, and to provide evidence for the prevention and treatment of monkeypox in this population.

Methods

The monkeypox questionnaire designed by Jiangsu Provincial Center for Disease Control and Prevention was used to investigate HIV Voluntary Counseling & Testing (VCT) clinic and social organizations in central Jiangsu Province. Excel 2010 and SPSS19.0 software were used to organize and analyze the data. Also, univariate and multivariate Logistic regressions were used to analyze the factors affecting the awareness rate of monkeypox knowledge. χ2 test was used to compare the difference of concern degree of monkeypox among people with different characteristics.

Results

A total of 1 290 valid questionnaires were collected. The majority were aged from 26 to 50 years, accounting for 65.04% (839/1 290). The majority of the students were college students, bachelor students or above, accounting for 64.26% (829/1 290). The awareness rate of monkeypox was 26.12%, and the complete awareness rate was 15.27%. The results of multivariate Logistic regression analysis showed that the related factors with higher awareness rate of monkeypox included Taizhou (OR=6.710, 95%CI: 4.309-10.447), high school/technical school (OR=2.461, 95%CI: 1.270-4.769), college (OR=2.515, 95%CI: 1.311-4.825), bachelor’s degree or above (OR=2.253, 95%CI: 1.184-4.288), concerned about monkeypox epidemic and related situation (OR=1.684, 95%CI: 1.188-2.387) (all P < 0.05). The correct rates of monkeypox symptoms, preventive measures and transmission routes were 30.78%, 42.25% and 27.75%, respectively. 1 186 people (91.94%) paid attention to monkeypox epidemic and prevention knowledge from WeChat, Weibo and other network new media. MSM with different regions (χ2=65.348, P<0.001), age groups (χ2=19.422, P=0.001), educational levels (χ2=31.447, P<0.001), marital status (χ2=11.162, P=0.025), results of the latest HIV test (χ2=37.793, P<0.001), sexual orientations (χ2=39.441, P<0.001), whether they had sexual contact with the same sex in the last 6 months (χ2=29.329, P<0.001), and whether they had sexual contact with overseas personnel (χ2=29.199, P<0.001), paid different attention to monkeypox information, and the differences were statistically significant.

Conclusion

The awareness rate of MSM monkeypox knowledge in central Jiangsu Province is low, and the accuracy rate of monkeypox symptoms, transmission routes and prevention and control measures are low. Therefore, it is necessary to publicize monkeypox symptoms and transmission routes through new media such as WeChat, Weibo, Douyin and Baidu in the later stage.

猴痘是由猴痘病毒(Monkeypox virus,MPXV)感染所致的一种病毒性人兽共患病[1-2],可通过接触感染的动物、与猴痘感染者密切接触、母婴传播和性接触等途径传播[3-4],临床表现主要为发热、皮疹和淋巴结肿大[5-7]。2022年1月以来,全球共有111个国家和地区向世界卫生组织报告了超过8.7万例猴痘病例,死亡病例达到140例[8]。国内猴痘疫情监测显示,病例均为男性,绝大部分明确为男男性行为者(Men who have sex with men,MSM)[9],MSM已成为猴痘防控的重点人群。为了解苏中地区MSM猴痘防治知识知晓情况,为做好高危人群猴痘干预和后续开展猴痘健康宣传提供科学依据,特开展此次调查。
2023年8月份在苏中地区(包括南通市、扬州市和泰州市)共74家艾滋病自愿咨询检测(HIV Voluntary Counseling & Testing,VCT)门诊和4个社会组织,利用社会组织推送+各地VCT门诊求诊者的形式对最近12个月发生过男男性行为(肛交或口交)的MSM开展调查,研究对象自愿参加本调查并签署知情同意书。本研究已通过扬州市疾病预防控制中心伦理审查委员会批准(编号:KY2024010)。
在VCT门诊和4个社会组织利用方便抽样的方法,招募MSM一对一开展猴痘知晓率的问卷调查,内容包括人口学特征、猴痘防治知识要点和后续开展猴痘健康宣教方向,本研究共8道题目,知晓率=知晓人数/调查人数,答对7道题目及以上视为知晓,答对全部题目视为完全知晓。同时统计调查人群对每道题目的回答正确率,正确率=回答正确人数/调查人数。
采用Excel 2010、SPSS 19.0软件对数据进行整理分析。计数资料采用构成比、率等指标进行描述。利用单因素logistic进行影响因素分析,将有统计学意义的自变量,全部纳入进行多因素logistic回归分析影响因素。采用χ2检验比较不同特征人群对猴痘关注程度的差异,检验水准α = 0.05。自变量赋值见表1
本次调查共回收问卷1 323份,其中有效问卷1 290份,回收率97.51%。其中,经社会组织推荐837人,占比64.88%,经VCT门诊推荐453人,占比35.12%;本南通市215人,扬州市674人,泰州市401人;年龄以26~50岁为主,占比65.04%(839/1 290);文化程度以大学专科、本科及以上为主,占比64.26%(829/1 290)人;婚姻状况以未婚为主,占比53.57%(691/1 290);最近一次艾滋病病毒(Human Immunodeficiency Virus,HIV)检测结果为阳性占比20.39%(263/1 290);394人(30.54%)对于猴痘疫情及相关情况非常关注,586人(45.43%)比较关注,310人(24.03%)不太关注或完全没有关注过;631人(48.91%)最近6个月与同性有过性接触。
调查对象答对7道题及以上337人,猴痘知识知晓率26.12%;答对8道题197人,完全知晓率15.27%。见表2
分别将年龄、文化程度、婚姻状况、最近一次HIV检测结果、对猴痘疫情及相关情况关注程度、是否感染过猴痘、性取向、最近6个月是否与同性有过性接触、是否与异性有过性接触和是否与境外人员有过性接触作为自变量,以是否知晓猴痘防治知识为因变量(0=否;1=是),进行单因素logistic回归分析,其后将有统计学意义(检验水准α = 0.05)的自变量,包括地区、文化程度、婚姻状况、对于猴痘疫情及相关情况关注程度和最近6个月是否与同性有过性接触,全部纳入进行多因素logistic回归分析,结果显示:猴痘知晓率较高的相关因素包括地区为泰州市(OR=6.710,95%CI:4.309~10.447),文化程度为高中/中专/技校(OR=2.461,95%CI:1.270~4.769)、大学专科(OR=2.515,95%CI:1.311~4.825)、本科及以上(OR=2.253,95%CI:1.184~4.288),对于猴痘疫情及相关情况比较关注(OR=1.684,95%CI:1.188~2.387)。见表2
1 021人(79.15%)知晓现阶段我国猴痘疫情主要发生在男男性行为人群;790人(61.24%)知晓感染猴痘可能不出现症状;397人(30.78%)知晓猴痘症状;545人(42.25%)知晓正确的预防猴痘措施;358人(27.75%)知晓猴痘的传播途径。见表3。1 290名受调查MSM中,1 186人(91.94%)从微信、微博、抖音、百度等网络新媒体,831人(64.42%)从电视、报纸、广播等传统方式关注猴痘的疫情和防治知识;886人(68.68%)觉得对猴痘的主要传播途径和高危人群宣传最有必要,691人(53.57%)觉得对近期国内外的猴痘感染人数宣传最有必要;1 127人(87.36%)认为最需要在微信、微博、抖音等新媒体网络软件开展猴痘健康宣传,890人(68.99%)认为最需要在聊天交友软件上开展猴痘健康宣传。
不同地区(χ2=65.348,P<0.001)、年龄(χ2=19.422,P=0.001)、文化程度(χ2=31.447,P<0.001)、婚姻状况(χ2=11.162,P=0.025)、最近一次HIV检测结果(χ2=37.793,P<0.001)、性取向(χ2=39.441,P<0.001)、最近6个月是否与同性有过接触(χ2=29.329,P<0.001)、是否与境外人员有过性接触(χ2=29.199,P<0.001)的MSM,对猴痘信息关注程度不同,差异有统计学意义。见表4
本次MSM猴痘知识知晓率调查对象年龄以26~50岁组为主,文化程度以大学专科及以上为主, 20.39%调查对象最近一次HIV检测结果为阳性(感染),所以应该在以上特征人群中开展猴痘知识宣教,并在艾滋病高危人群中开展猴痘监测,及时发现和治疗猴痘病例[10]
本次调查MSM猴痘知识知晓率26.12%,完全知晓率15.27%,知晓率偏低,所以应该在VCT门诊和社会组织加大对MSM的猴痘相关知识宣教,提高猴痘防治知识知晓率,有猴痘相关症状及时到正规的医疗机构进行对症治疗,安全性行为和避免到猴痘疫情地区。
在对2023年苏中地区MSM猴痘知识回答正确率的分析中,猴痘症状、预防措施和传播途径正确率分别为30.78%、42.25%和27.75%,正确率较低,而知晓猴痘症状和预防措施等知识有利于降低MSM的感染风险和便于重点人群的自我健康监测,所以在后期MSM宣传干预工作中,应该侧重猴痘症状、预防措施和传播途径知识的宣教。
91.94%调查对象从微信、微博等网络新媒体获取猴痘知识,而目前有24.03%调查对象对于猴痘疫情及相关情况不太关注或者从来没有关注过,不同地区、年龄、文化程度、婚姻状况、文化程度、最近一次HIV检测结果、性取向、最近6个月是否与同性有过性接触、是否与境外人员有过性接触的MSM,对猴痘信息关注程度不同,所以应该加大对猴痘知识的宣传力度,在网络新媒体开展形式丰富多样、多渠道的猴痘知识宣传[11]
猴痘知晓率影响因素分析结果显示:与南通市相比,扬州市知晓率较低,而泰州市知晓率更高,可能每个地区对猴痘知晓的宣教力度不同,知识的普及程度不同,所以应该在知晓率偏低地区加强猴痘知识宣教;与文化程度初中及以下相比,文化程度为大学专科及以上,知晓率更高,和杭州市拱墅区研究结果一致[12],可能是因为文化程度为初中及以下的人员可能是在校学生或者本身文化程度较低,对猴痘知识的关注度可能不够,随着受教育程度的提高,猴痘知识储备量增加;最近6个月和同性发生性接触知晓率更高,有文献报道在艾滋病知晓率中也存在类似特点[13-14],可能是因为猴痘主要发生MSM,该人群会去主动获取猴痘相关知识。
本研究还存在一定的局限性和不足。一是采用方便抽样,不能代表全人群MSM猴痘认知的整体水平;二是在对苏中地区MSM猴痘知识知晓情况影响因素分析时可能存在混杂因素,在后续研究中需要多方面考虑[15]
综上所述,江苏省中部地区MSM猴痘防治知识认知相对不足,后续需要针对薄弱点,比如猴痘症状、猴痘传播途径和预防控制措施,及时补充相关知识,并且加大宣传力度、增加宣传渠道,多方面开展猴痘防治知识宣传,不断提高猴痘知识知晓率。
