Objective

To study the relationship between the cardiometabolic index (CMI) and hyperuricemia (HUA) in Guizhou minorities.

Methods

Based on the "China Multi-ethnic Cohort Study", 16 630 Miao, Dong and Bouyei population aged 30 to 79 years were included in the study. Logistic regression model was used to analyze the association between CMI and HUA, restricted cubic spline was used to analyze the dose-response relationship between CMI and HUA, and ROC curve was used to analyze the predictive value of CMI for HUA.

Results

The overall detection rate of HUA was 22.8%, and those of Miao, Dong and Bouyei nationalities were 24.4%, 25.2% and 18.5%, respectively. In the total population, Miao, Dong and Bouyei populations, after adjusting for covariates, compared with Q1, the ORs for HUA were 4.39 (95%CI: 3.76-5.12), 3.96 (95%CI: 3.03-5.18), 4.28 (95%CI: 3.36-5.46), and 5.55 (95%CI: 4.08-7.54) in CMI Q4 group, respectively. There was a non-linear dose-response relationship between CMI and risk of HUA (P for nonlinear < 0.001). ROC curve analysis showed that, the area under the curve (the best cut-off value, sensitivity and specificity) of CMI for predicting the risk of HUA in the total population, Miao, Dong and Bouyei populations were 0.742 (0.641, 68.3%,68.8%), 0.724 (0.651, 63.8%, 70.8%) 0.741 (0.654, 68.5%, 68.1%), 0.775 (0.555, 68.8%, 73.0%).

Conclusion

CMI is associated with the increased risk of HUA in different ethnic populations, and can be used as an indicator for predicting HUA in ethnic minority populations in Guizhou province.

目的

研究贵州省少数民族心脏代谢指数(CMI)对高尿酸血症(HUA)的关联关系。

方法

基于“中国多民族队列研究”,共纳入16 630名30~79岁苗族、侗族、布依族人群作为研究对象。 Logistic回归模型分析CMI与HUA的关系,限制性立方样条分析CMI与HUA的剂量-反应关系,采用ROC曲线分析CMI对HUA的预测价值。

结果

HUA总体检出率为22.8%,苗族、侗族、布依族人群分别为22.8%、24.4%、25.2%、18.5%。在总人群、苗、侗、布依族人群中,调整协变量后,与Q1相比,CMI Q4组患HUA的OR分别为4.39(95%CI:3.76~5.12)、3.96(95%CI:3.03~5.18)、4.28(95%CI:3.36~5.46)、5.55(95%CI:4.08~7.54),CMI与HUA风险之间呈非线性剂量-反应关系(P for nonlinear <0.001)。ROC曲线分析显示,CMI在总人群、苗、侗、布依族人群中预测HUA患病风险的曲线下面积(最佳截断值、灵敏度、特异度)分别为0.742(0.641、68.3%、68.8%), 0.724(0.651、63.8%、70.8%)0.741(0.654、68.5%、68.1%)、0.775(0.555、68.8%、73.0%)。

结论

CMI对不同民族人群HUA患病风险增加有关,可作为预测贵州省少数民族人群HUA的指标。

目的

研究贵州省少数民族心脏代谢指数(CMI)对高尿酸血症(HUA)的关联关系。

方法

基于“中国多民族队列研究”,共纳入16 630名30~79岁苗族、侗族、布依族人群作为研究对象。 Logistic回归模型分析CMI与HUA的关系,限制性立方样条分析CMI与HUA的剂量-反应关系,采用ROC曲线分析CMI对HUA的预测价值。

结果

HUA总体检出率为22.8%,苗族、侗族、布依族人群分别为22.8%、24.4%、25.2%、18.5%。在总人群、苗、侗、布依族人群中,调整协变量后,与Q1相比,CMI Q4组患HUA的OR分别为4.39(95%CI:3.76~5.12)、3.96(95%CI:3.03~5.18)、4.28(95%CI:3.36~5.46)、5.55(95%CI:4.08~7.54),CMI与HUA风险之间呈非线性剂量-反应关系(P for nonlinear <0.001)。ROC曲线分析显示,CMI在总人群、苗、侗、布依族人群中预测HUA患病风险的曲线下面积(最佳截断值、灵敏度、特异度)分别为0.742(0.641、68.3%、68.8%), 0.724(0.651、63.8%、70.8%)0.741(0.654、68.5%、68.1%)、0.775(0.555、68.8%、73.0%)。

结论

CMI对不同民族人群HUA患病风险增加有关,可作为预测贵州省少数民族人群HUA的指标。

Objective

To study the relationship between the cardiometabolic index (CMI) and hyperuricemia (HUA) in Guizhou minorities.

Methods

Based on the "China Multi-ethnic Cohort Study", 16 630 Miao, Dong and Bouyei population aged 30 to 79 years were included in the study. Logistic regression model was used to analyze the association between CMI and HUA, restricted cubic spline was used to analyze the dose-response relationship between CMI and HUA, and ROC curve was used to analyze the predictive value of CMI for HUA.

Results

The overall detection rate of HUA was 22.8%, and those of Miao, Dong and Bouyei nationalities were 24.4%, 25.2% and 18.5%, respectively. In the total population, Miao, Dong and Bouyei populations, after adjusting for covariates, compared with Q1, the ORs for HUA were 4.39 (95%CI: 3.76-5.12), 3.96 (95%CI: 3.03-5.18), 4.28 (95%CI: 3.36-5.46), and 5.55 (95%CI: 4.08-7.54) in CMI Q4 group, respectively. There was a non-linear dose-response relationship between CMI and risk of HUA (P for nonlinear < 0.001). ROC curve analysis showed that, the area under the curve (the best cut-off value, sensitivity and specificity) of CMI for predicting the risk of HUA in the total population, Miao, Dong and Bouyei populations were 0.742 (0.641, 68.3%,68.8%), 0.724 (0.651, 63.8%, 70.8%) 0.741 (0.654, 68.5%, 68.1%), 0.775 (0.555, 68.8%, 73.0%).

Conclusion

CMI is associated with the increased risk of HUA in different ethnic populations, and can be used as an indicator for predicting HUA in ethnic minority populations in Guizhou province.

高尿酸血症(hyperuricemia, HUA)是嘌呤代谢紊乱而导致体内尿酸生成增加和/或排泄减少引起的一种代谢性疾病[1-2]。作为继高血压、高脂血症、高血糖后的“第四高”,高尿酸血症已成为目前重要的公共卫生问题之一。高尿酸血症不仅是引发痛风的基础,而且与高血压、慢性肾病、心血管疾病等发生密切相关[3]。多项研究表明,高尿酸血症是心血管疾病的独立危险因素,在心血管疾病的发生、发展过程中起到重要的作用[4-6]。心脏代谢指数(cardiometabolic index, CMI)一种新的代谢指标,可同时反映个体肥胖情况和血脂水平;也是一种新的心血管风险标志物,可用于预测心血管疾病异常情况。现已有证据表示CMI在高尿酸血症中具有较强的诊断价值[7],目前研究CMI与高尿酸血症的关联研究较少,因此,本研究旨在探讨CMI与贵州省少数民族人群高尿酸血症的关联性,并评估CMI对贵州省少数民族高尿酸血症的预测价值,为贵州省少数民族人群高尿酸血症的预防和控制提供理论依据。
基于“中国多民族队列研究”的基线调查数据,共有18 790人(苗族5 559人、侗族7 239人、布依族5 992人)符合队列研究的纳入标准。进一步排除血清尿酸缺失、空腹时间<8 h、肾小球滤过率(eGFR)<60 ml/(min·1.73 m2)的研究对象,最终进行统计分析的研究对象有16 630名(苗族4 980人、侗族6 222人、布依族5 428人)。本研究经四川大学医学伦理委员会(K2016038)和贵州医科大学附属医院医学伦理委员会[2018(094)]批准。
采用“西南区域少数民族聚集地世居自然人群队列研究”自制的调查问卷,包括人口学特征、吸烟、饮酒、生活方式、体力活动等。
体格检查指标包括:身高(Height, H)、体重、腰围(Waist Cirumference, WC)、收缩压(Systolic Blood Pressure, SBP)、舒张压( Diastolic Blood Pressure, DBP ) 等。采集空腹静脉血后使用全自动生化分析仪(HITACHI 7180, Tokyo, Japan)检测高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、总胆固醇(TC)、甘油三酯(TG)、空腹血糖( Fasting Plasma Glucose, FPG )、血清尿酸( Serum Uric Acid, SUA )、血清肌酐( Serum creatinine, Scr )等生化招标。
吸烟:至今吸烟共计超过100支;(2)饮酒:过去半年每周至少饮酒一次;(3)体力活动:为职业活动、交通出行、家务活动和休闲活动等四者的代谢当量总和,单位为METs(h/d)。(4)体重指数:BMI=体重(kg)/身高的平方(m2)。(5)心脏代谢指数:CMI=(TG/HDL-C)×(WC/H)[8]
《高尿酸血症/痛风患者实践指南》(2020版)[9]的诊断标准为:空腹状态下,男性SUA≥7 mg/dl(420 μmol/L),女性SUA≥6 mg/dl(360 μmol/L),即为高尿酸血症。
使用SPSS 25.0分析数据。不服从正态分布的连续变量采用中位数(P25P75)表示,两组间差异比较采用Mann-Whitney U检验,分类变量采用频数(%)表示,组间比较采用卡方检验。CMI以总人群CMI的四分位数由低到高分为四组,依次为Q1、Q2、Q3、Q4。采用logistic回归模型分析CMI和高尿酸血症的关联性。使用R 4.1.3分析CMI与高尿酸血症的剂量反应关系。通过受试者工作特征曲线(ROC)计算CMI的曲线下面积(AUC),并分析AUC值之间的差异是否有统计学意义。检验水准α=0.05,双侧检验。
本研究共纳入16 630名研究对象,HUA总体患病率为22.8%(3 790/16 630),男性HUA患病率为34.1%(1 915/5 613),女性HUA患病率为17.0%(1 875/11 017)。苗族、侗族、布依族HUA患病率分别为24.4%(1 216/4 980)、25.2%(1 71/6 222)、18.5%(1 003/5 428)。HUA组与非HUA组人群在年龄、性别、民族、吸烟、饮酒、教育程度、体力活动、糖脂代谢指标(FBG、HDL-C、LDL-C、TG、TC)、BMI、CMI等具有统计学意义(P<0.001),详见表1
在总人群中,与Q1相比,不同范围的CMI水平在粗模型、模型1 、模型2中均与高尿酸血症存在正相关(P for trend<0.001)。调整性别、年龄、教育程度、体力活动等协变量后Q2、Q3、Q4的OR分别为1.50(95%CI:1.31~1.72)2.66(95%CI:2.34~3.03)、4.39(95%CI:3.76~5.12),随着CMI增加,HUA患病风险呈上升趋势。对民族进行亚组分析后,CMI与苗族、侗族、布依族人群高尿酸血症仍然存在正向关联(P for trend<0.001),并在布依族人群中发现CMI与HUA的关联强度高于苗族、侗族人群,其CMI最高四分位数组的最终调整OR值为5.55(95%CI:4.08~7.54)见表2
基于总人群和民族分层,调整2.2中模型3的相关变量,本研究使用了5个节点的限制性立方样条模型分析了CMI与高尿酸血症患病风险的关联,见图1。总人群、苗族、侗族、布依族人群均显示CMI与高尿酸血症患病风险之间呈非线性剂量反应关系(P for nonlinear <0.001)。我们发现总人群、侗族人群的剂量反应关系类似,HUA患病风险随着CMI增加出现先快后慢趋势,苗族人群HUA患病风险随着CMI增加出现先上升后下降的趋势,而布依族人群随着CMI不断增加HUA患病风险随之增加。
CMI对总人群、苗族、侗族、布依族人群高尿酸血症预测的AUC分别为0.742、0.724、0.741、0.775。CMI对不同民族高尿酸血症的预测能力较好,其中,CMI预测布依族人群高尿酸血症的最佳截断值低于总人群以及苗族、侗族人群,最佳截断值对应的特异度(68.8%)、敏感度(73.0%)均高于总人群、苗族人群和侗族人群,见表3
本研究基于“西南区域少数民族队列”的基线数据,探讨贵州省苗族、侗族、布依族人群CMI与高尿酸血症的关联性,结果发现随着CMI增加,高尿酸血症患病风险随之升高;CMI在预测高尿酸血症患病上具有一定的临床参考价值。
目前我国大陆HUA患病率为16.4%,西南区域HUA患病率为21.2%[10]。本研究发现,贵州省少数民族人群总体HUA患病率为22.4%,高于我国和西南区域HUA患病率。在本研究中发现各民族HUA患病率存在差异,布依族人群HUA患病率(18.5%)低于苗族(24.4%)、侗族人群HUA患病率(25.2%)。与其它少数民族人群相比,本研究人群HUA患病率明显高于新疆地区少数民族人群(5.58%)[11]和广西省毛南族人群(13.9%)[12]。HUA患病率在不同性别中存在差异(男性34.1%,女性17.0%),与先前的研究结果一致[10]。本研究HUA患病率存在民族差异,可能与不同民族的遗传因素、生活行为方式、饮食习惯等有关。
肥胖和胰岛素抵抗被认为是高尿酸血症的独立危险因素[13-16],CMI是一种新型的肥胖相关指标,与传统肥胖指标体重指数(BMI)、腰围(WC)、腰高比(WHtR)相比,CMI可以评估内脏肥胖程度。CMI是TG/HDL-C和WHtR的乘积,其中WHtR是评估腹型肥胖的指标之一,已有研究证明WHtR是HUA独立且比BMI、WC更好的预测因子[17]。CMI中的TG/HDL-C是代谢综合征和心血管疾病的预测因子[18-19],也在胰岛素抵抗方面具有预测价值[20]。已有研究证明多项研究表明CMI是HUA的独立预测因子[7,21-22]。如中国一项随访4年的队列研究显示,在女性和正常体质量人群中,TG/HDL-C与HUA的风险呈正相关[23]。本研究对贵州省少数民族人群进行CMI与HUA的关联研究,在调整了性别、年龄、教育程度、吸烟、饮酒、体力活动、eGFR、糖尿病、高血压、血脂异常等协变量后,结果显示贵州省少数民族人群CMI与HUA呈正相关,高水平CMI会增加HUA患病风险。本研究以民族为分层变量,同样发现随着CMI增加,苗族、侗族、布依族人群HUA患病风险相应增加;并且CMI与不同民族人群HUA患病风险存在非线性剂量反应关系。一项纳入新疆医科大学第一附属医院参加健康体检的9 429名人群的横断面分析得出,CMI与新疆地区人群HUA呈正相关,最高四分位数的OR为6.053(95%CI:4.043~7.189)[11]。另一项在对江苏省73 150例非肥胖人群进行CMI与高尿酸血症的横断面研究结果显示,在非肥胖人群中,CMI与HUA呈正相关;女性CMI最高四分位数的OR值为6.311(95%CI:5.734~6.947),男性为6.785(95%CI:6.092~7.557)[24]。本研究发现CMI对贵州省少数民族人群HUA的预测能力较好,这可能是因为CMI是一个综合了血脂水平和和腹型肥胖的指标,血脂水平和腹型肥胖都是HUA的影响因素,因此,CMI可用于预测贵州省少数民族HUA的风险,为当地HUA的预防和防治提供一定的理论依据。但本研究也存在一定的局限性:首先,本研究为横断面研究,不能确定CMI与HUA之间的因果关系,将在后续随访中验证CMI与HUA的因果关联。第二,虽然本研究调整了与HUA相关的协变量,但不能排除其它未知混杂因素的影响。第三,研究对象均来自贵州省少数民族地区,研究结果可能对其它地区的少数民族的推广性有限。最后,本研究仅测量一次SUA,可能会出现结果误差。
综上所述,贵州省苗族、侗族、布依族人群CMI与HUA患病风险存在关联,且存在非线性剂量反应关系,CMI对贵州省少数民族人群HUA有一定的预测能力。应结合人群生活行为方式和民族差异制定相应的防治措施,以降低和预防因内脏脂肪组织积累所导致的HUA患病风险。
