Objective

To understand the willingness to use pre-exposure prophylaxis (PrEP) among MSM in Guiyang, explore related factors, so as to provide reference for promoting PrEP in Guiyang and Guizhou Province.

Methods

Two social organizations in Guiyang were recruited among MSM from gay bars, WeChat groups and other social venues and the Internet by convenient sampling method from April to June, 2022. Questionnaires were collected using the “Wen Juan Xing” platform, the contents of the survey included demographic information, behavioral characteristics, knowledge of PrEP, the willingness to use PrEP and so on. Univariate analysis of the willingness to use PrEP was used Chi-square test, logistics regression was carried out to analyze the multiple-factors related to the willingness to use PrEP. Taking the willingness to use PrEP as the dependent variable, univariate and multivariate logistics regression was carried out to analyze the factors related to the willingness to use PrEP.

Results

608 valid questionnaires were collected. A total of 146 people (24.0%) knew about PrEP, 216 people (35.5%) had used PrEP before, and 525 people (86.3%) were willing to use PrEP if they had high-risk behaviors. The results of multivariate analysis showed that non-Guiyang resident (OR=3.09, 95%CI:1.42-6.73), living in Guiyang for 7-12 months (OR=5.12, 95%CI:2.25-11.65) and more than 1 year (OR=4.19, 95%CI:1.85-9.49), employed (OR=17.05, 95%CI:4.14-70.26), unemployed (OR=6.29, 95%CI:1.18-33.59), tested for HIV in the last 1 year (OR=3.77, 95%CI:1.70-8.38), those who had previously used PrEP (OR=15.85, 95%CI: 5.12-49.11) and aware of PrEP (OR=4.82, 95%CI: 1.88-12.38) were more likely to use PrEP.

Conclusion

The MSM population surveyed in Guiyang has a low awareness rate of PrEP, higher willingness to use PrEP, and relative factors, develop targeted measures to promote knowledge about PrEP according to relative factors, so as to improve the knowledge of PrEP, confidence and willingness to use PrEP. Promote the use of PrEP in the context of comprehensive AIDS prevention and control, such as the use of condoms, to reduce the risk of HIV infection.

目的

了解贵阳市男男性行为者（Men who have sex wih men, MSM）HIV暴露前预防（PrEP）使用意愿，探索相关因素，为贵阳市及贵州省推广PrEP工作提供参考依据。

方法

2022年4—6月，通过贵阳市两家社会组织，采用方便抽样的方法在MSM酒吧、微信群等社交场所及互联网招募调查对象，问卷使用“问卷星”平台收集。调查内容主要包括人口学信息、行为学特征、PrEP知识知晓及使用意愿等情况，PrEP使用意愿的单因素分析运用卡方检验，二分类logistic回归模型进行PrEP使用意愿的多因素分析。

结果

收集有效问卷608份，共有146人(24.0%)知晓PrEP，216人（35.5%）既往使用过PrEP，如果发生感染艾滋病的高危行为，525人（86.3%）愿意采用暴露前预防。多因素分析结果显示，非贵阳市户籍（OR=3.09，95%CI:1.42～6.73）、在贵阳市居住7～12个月（OR=5.12，95%CI:2.25～11.65）和12个月以上（OR=4.19，95%CI:1.85～9.49），在职（OR=17.05，95%CI:4.14～70.26）、待业或无业（OR=6.29，95%CI:1.18～33.59），最近1年检测过HIV（OR=3.77，95%CI:1.70～8.38），既往使用过PrEP（OR=15.85，95%CI:5.12～49.11）和知晓PrEP（OR=4.82，95%CI:1.88～12.38）的调查对象使用PrEP的愿意较高。

结论

贵阳市接受调查的MSM人群PrEP知晓率较低，PrEP使用意愿较高，影响因素较多，应针对影响因素，制定针对性的PrEP用药知识宣传措施，进一步提高MSM人群的PrEP知识知晓、对使用PrEP效果的信心和使用意愿，在使用安全套等艾滋病综合防治的前提下推广使用PrEP，降低感染HIV的风险。

目的

了解贵阳市男男性行为者（Men who have sex wih men, MSM）HIV暴露前预防（PrEP）使用意愿，探索相关因素，为贵阳市及贵州省推广PrEP工作提供参考依据。

方法

2022年4—6月，通过贵阳市两家社会组织，采用方便抽样的方法在MSM酒吧、微信群等社交场所及互联网招募调查对象，问卷使用“问卷星”平台收集。调查内容主要包括人口学信息、行为学特征、PrEP知识知晓及使用意愿等情况，PrEP使用意愿的单因素分析运用卡方检验，二分类logistic回归模型进行PrEP使用意愿的多因素分析。

结果

收集有效问卷608份，共有146人(24.0%)知晓PrEP，216人（35.5%）既往使用过PrEP，如果发生感染艾滋病的高危行为，525人（86.3%）愿意采用暴露前预防。多因素分析结果显示，非贵阳市户籍（OR=3.09，95%CI:1.42～6.73）、在贵阳市居住7～12个月（OR=5.12，95%CI:2.25～11.65）和12个月以上（OR=4.19，95%CI:1.85～9.49），在职（OR=17.05，95%CI:4.14～70.26）、待业或无业（OR=6.29，95%CI:1.18～33.59），最近1年检测过HIV（OR=3.77，95%CI:1.70～8.38），既往使用过PrEP（OR=15.85，95%CI:5.12～49.11）和知晓PrEP（OR=4.82，95%CI:1.88～12.38）的调查对象使用PrEP的愿意较高。

结论

贵阳市接受调查的MSM人群PrEP知晓率较低，PrEP使用意愿较高，影响因素较多，应针对影响因素，制定针对性的PrEP用药知识宣传措施，进一步提高MSM人群的PrEP知识知晓、对使用PrEP效果的信心和使用意愿，在使用安全套等艾滋病综合防治的前提下推广使用PrEP，降低感染HIV的风险。

Objective

To understand the willingness to use pre-exposure prophylaxis (PrEP) among MSM in Guiyang, explore related factors, so as to provide reference for promoting PrEP in Guiyang and Guizhou Province.

Methods

Two social organizations in Guiyang were recruited among MSM from gay bars, WeChat groups and other social venues and the Internet by convenient sampling method from April to June, 2022. Questionnaires were collected using the “Wen Juan Xing” platform, the contents of the survey included demographic information, behavioral characteristics, knowledge of PrEP, the willingness to use PrEP and so on. Univariate analysis of the willingness to use PrEP was used Chi-square test, logistics regression was carried out to analyze the multiple-factors related to the willingness to use PrEP. Taking the willingness to use PrEP as the dependent variable, univariate and multivariate logistics regression was carried out to analyze the factors related to the willingness to use PrEP.

Results

608 valid questionnaires were collected. A total of 146 people (24.0%) knew about PrEP, 216 people (35.5%) had used PrEP before, and 525 people (86.3%) were willing to use PrEP if they had high-risk behaviors. The results of multivariate analysis showed that non-Guiyang resident (OR=3.09, 95%CI:1.42-6.73), living in Guiyang for 7-12 months (OR=5.12, 95%CI:2.25-11.65) and more than 1 year (OR=4.19, 95%CI:1.85-9.49), employed (OR=17.05, 95%CI:4.14-70.26), unemployed (OR=6.29, 95%CI:1.18-33.59), tested for HIV in the last 1 year (OR=3.77, 95%CI:1.70-8.38), those who had previously used PrEP (OR=15.85, 95%CI: 5.12-49.11) and aware of PrEP (OR=4.82, 95%CI: 1.88-12.38) were more likely to use PrEP.

Conclusion

The MSM population surveyed in Guiyang has a low awareness rate of PrEP, higher willingness to use PrEP, and relative factors, develop targeted measures to promote knowledge about PrEP according to relative factors, so as to improve the knowledge of PrEP, confidence and willingness to use PrEP. Promote the use of PrEP in the context of comprehensive AIDS prevention and control, such as the use of condoms, to reduce the risk of HIV infection.

截至2022年底，全国（不含港、澳、台地区）报告现存活艾滋病感染者（Human immunodeficiency virus, HIV）/艾滋病患者（Acquired immunodeficiency syndrome, AIDS）122.3万，2022年新发现HIV感染者中，经男男同性传播占比为25.6%[1]，预防男男性行为者（Men who have sex wih men, MSM）感染HIV仍是公共卫生关注的问题。贵阳市近年采取扩大宣传、检测、抗病毒治疗和HIV暴露后预防等综合干预措施后，艾滋病防治取得一定成效，MSM人群HIV检出率由2014年的36.5%下降到2019年的12.2%[2]，但仍高于全国水平[3]，贵阳市近年新报告的HIV病例中，经同性传播的人数和晚发现的比例均增加[4]，MSM人群疫情形势严峻，现有的综合干预措施不能满足该人群的预防需求，亟需新的干预方法预防HIV在MSM人群中的传播，控制我市HIV在MSM人群中的流行。HIV暴露前预防(Pre-exposure prophylaxis, PrEP)是指HIV阴性人群通过使用抗反转录病毒药物来预防HIV感染的生物学预防方法，全球已经开展多项针对MSM等易感染HIV人群的PrEP临床试验等研究，可以降低感染HIV的风险[5]。2017年贵州省呈现艾滋病“高-高”集聚区域，与四川、重庆、云南和广西几大艾滋病高发地区地理位置相接，地区间人口流动、人群交往较为频繁，且少数民族聚居，少数民族占36.4%，有着特殊的历史文化背景，对性行为的态度比较开放和包容，预防意识比较淡薄，增加了HIV感染的可能性[6-7]。贵阳市是贵州省的省会，随着经济的发展，MSM相对较多，流动性大，且贵阳市乃至贵州省从未开展过MSM人群对PrEP服务知识知晓及使用意愿相关调查，没有适合本地的PrEP服务推广的策略和措施，该研究的实施对贵阳市、贵州省MSM人群开展并推广PrEP服务具有重要的指导意义，同时，对西南片区遏制HIV在MSM人群中的区域传播和流行也起到一定的积极作用。
1　对象与方法

1.1　研究对象
纳入标准：①年龄≥18岁；②生理性别为男性；③现住址是贵阳市；④最近一年发生过男男性行为（与男性发生过肛交或口交）；⑤意识清醒，精神正常能够自主决策；⑥自愿参与本次调查并签署电子版知情同意书。排除标准：HIV检测阳性者。所有调查对象签署知情同意书。本研究经贵阳市疾病预防控制中心伦理委员会审批（备案编号：MR-52-24-023451）。

2022年4—6月，通过贵阳市两家社会组织，采用方便抽样的方法在MSM酒吧、微信群等社交场所及互联网招募调查对象。告知MSM对象纳入标准、调查目的及内容等，符合标准并同意调查的对象进行匿名填写问卷。调查对象通过扫描问卷二维码，在线签署知情同意书并填写问卷，后台收集数据，并及时审核数据的完整性及检查逻辑错误，及时联系社会组织工作人员进行回访更正，且每个IP地址只能填写一份问卷。问卷内容包括一般人口学特征、高危行为，PrEP知晓情况及使用意愿、既往HIV检测等问题。“您认为暴露前预防的作用是?”、“ 您认为以下哪些人需要暴露前预防?”、“您认为使用暴露前预防前是否需要HIV检测呈阴性？”、“您认为暴露前预防药物服用方法是？（多选题：每日服药和按需服药）”、“您认为服用暴露前预防期间发生性行为是否需要使用安全套？”，五个问题全部答对定义为知晓。
1.3　统计分析
“问卷星”平台导出Excel 2020格式的问卷，核对Excel 2020数据库信息，SPSS 18.0导入数据库进行统计学处理和分析。PrEP使用意愿的单因素分析运用卡方检验；以是否有使用PrEP意愿作为因变量（0=不愿意，1=愿意），二分类logistic回归模型进行PrEP使用意愿的多因素分析，多因素分析采用backward stepwise (conditional LR)法将单因素分析中P<0.05的变量引入logistic回归模型，检验水准α=0.05。
2　结果

2.1　调查对象一般人口学特征
共有800名MSM参与PrEP使用意愿调查，经核对，有效调查问卷608份。608名调查对象中，18～24岁占41.0%（249人），25～34岁占39.5%（240人），平均年龄（28.7±8.1）岁；汉族占88.7%（539人）；贵阳市户籍占72.9%（443人）；未婚占90.6%（551人）；大专及以上占80.2%（488人）；在职占56.4%（343人），学生占30.1%（183人），月收入小于1 000元占22.7%（138人），1 000～5 000元占48.5%（295人）；在贵阳市居住12个月以上占58.4%（355人）。见表1。
2.2　调查对象行为特征
608名调查对象中，性取向是同性占83.6%（508人），双性13.8%（84人），异性2.6%（16人）；最近6个月发生同性性行为占48%（292人）；性伴2～5人占24.5%（149人），6个及以上性伴占6.1%（37人）；最近6个月，7.1%（43人）的调查对象发生同性商业性行为，分别有10.7%（65人）和2.8%（17人）的调查对象偶尔发生和经常发生同性群交性行为；性行为中使用新型毒品占17.8%（108人）；45.4%（276人）的调查对象最近1年检测过HIV；既往使用过PrEP占35.5%；最近6个月检测出患有性病占9.7%（59人）。见表1。
2.3　PrEP相关问题知晓情况
608名调查对象中，5道PrEP相关问题回答情况:483人(79.4%)答对“暴露前预防的作用?”；408人(67.1%)答对“哪些人需要暴露前预防?”；514人(84.5%)答对“使用暴露前预防前是否需要HIV检测呈阴性?”；268人(44.1%)答对“暴露前预防药物服用方法”；549人(90.3%)答对“服用暴露前预防药物期间发生性行为是否需要使用安全套?”。共有146人(24.0%)知晓PrEP(答对5道题)。216人（35.5%）既往使用过PrEP，如果发生感染艾滋病的高危行为，525人（86.3%）愿意采用暴露前预防。见表1。
2.4　PrEP使用意愿影响因素分析
2.4.1　单因素分析
结果显示，调查对象的年龄，户籍，在贵阳市居住的时间，职业，月收入，性取向，最近6个月是否发生同性性行为、性伴数和性行为中是否使用新型毒品，最近1年是否检测过HIV，既往是否使用过PrEP和PrEP知晓情况与 PrEP使用意愿相关有统计学意义（P均<0.05）；婚姻状况、文化程度、最近6个月是否发生同性商业性行为和群交性行为、最近6个月是否检测出患有性病与 PrEP使用意愿相关无统计学意义（P均>0.05）。见表2。
2.4.2　多因素logistic回归分析
将单因素有意义的变量纳入二元logistic回归模型进行分析，结果显示，非贵阳市户籍，在贵阳市居住7～12个月和12个月以上，在职、待业或无业，最近1年检测过HIV，既往使用过PrEP和知晓PrEP的调查对象如果发生感染艾滋病的高危行为时使用暴露前预防的愿意较高，差异有统计学意义（P均<0.05）。见表3、4。

PrEP是目前MSM人群中预防HIV感染较为流行的一种新型有效的生物学预防方法。2022年贵阳市启动了HIV PrEP项目，为更好推广和使用PrEP服务，我们对贵阳市MSM人群PrEP使用意愿、相关影响因素进行了调查和分析。
608名MSM中有86.3%愿意在发生感染艾滋病的高危行为前采用暴露前预防，高于其他地区的研究[8-9]；35.5%既往用过PrEP，高于贾雯雯等[10]的调查结果8.4%，潘玲等[11]的结果17.86%，这可能与2018年起，贵阳市疾控与社会组织通过网络、娱乐场所等多途径宣传HIV暴露前预防知识有关。PrEP相关问题均知晓率为24%，低于其他地区调查结果[10]。
通过本次调查，发现非贵阳市户籍与贵阳市户籍相比，使用PrEP的愿意更高，可能与非本地户籍的MSM，性伴更多、更复杂，自我风险意识高有关，而本地户籍的调查对象因家人朋友居住在本地较多，担心到医院购买和服用暴露前预防药物可能会暴露个人隐私，故不愿使用PrEP，Ding等[12]研究结果也显示，外地居民比本地居民更愿意使用PrEP；与在本地居住小于等于6个月的MSM相比，长期居住在本地MSM更愿意使用PrEP，与张祖祥等[13]研究结果一致，究其原因，可能有以下：流动的MSM的人群无法规范使用PrEP和定期随访检测；长期居住在本地的MSM对当地疫情形势更关注，接受暴露前预防服务的宣传较多，知晓获得PrEP途径等信息。目前贵阳市服用PrEP是自费的，调查显示在职人员暴露前预防使用意愿更高，Sineath 等[14]研究表明有稳定工作的MSM对PrEP更了解，接受意愿更高；兼职的调查对象可能收入较低，考虑经济效益，更愿意使用安全套等成本较低的预防措施；薛晖等[15]2013年通过网络调查显示，暴露前预防用药免费或很便宜会使用，低收入人群使用PrEP意愿较低。本次调查待业或无业人员暴露前预防使用意愿更高，可能由于本次调查有部分对象在酒吧招募，待业或无业人员较多，酒吧等场所容易发生多性伴等高危性行为，使用PrEP预防HIV愿意更高。最近1年检测过HIV的MSM使用PrEP的意愿较高，与既往研究研究结果一致[16]，康文婷等[17]研究还发现，近1年HIV检测次数越多，暴露前预防知识知晓率越高。
除此以外，知晓PrEP者使用PrEP意愿更高，薛珲等[15]研究显示，理解暴露前预防用药内容的MSM使用PrEP意愿更高。在本次调查中，虽然听说过的人较多，但PrEP相关知识均知晓的率低，尤其是知晓“暴露前预防药物服用方法”的仅有44.1%，调查对象没有充分了解PrEP，导致使用率不高，郭佳欢等[18]研究也表明MSM人群虽然知晓PrEP知识，但实际使用极低，存在知行分离现象，说明MSM人群对PrEP认识的不足，会影响PrEP的使用。使用过PrEP的MSM使用PrEP意愿更高，因为他们对PrEP相关知识更加了解，体验过PrEP带来的预防效果，对PrEP预防效果更有信心，石安霞等[19]研究也表明，既往使用PrEP的MSM，坚持按时使用暴露前预防药物等PrEP的使用效能更高。
本研究存在局限性，一是采用线上和线下方便抽样的方式招募调查对象，样本的代表性不足；二是问卷涉及了许多隐私问题，如性行为和新型毒品使用等，受试者可能存在报告偏倚；三是本研究是横断面研究，各影响因素与PrEP使用意愿关联无法推断。
综上所述，贵阳市接受调查的MSM人群PrEP知晓率较低，PrEP使用意愿较高，影响因素较多，应针对PrEP知识知晓、HIV检测等影响因素，制定适合贵阳市的PrEP用药知识宣传策略和措施，进一步提高MSM人群的PrEP知识知晓、对使用PrEP效果的信心和使用意愿，在使用安全套等艾滋病综合防治的前提下推广使用PrEP，降低感染HIV的风险。
