Objective

To translate the Quality of Life Questionnaire for Aphasics(QLQA) into Chinese and test its reliability and validity in stroke patients with aphasia.

Methods

The Brislin translation model was adopted for the translation of QLQA.The Chinese version of QLQA was developed after cultural adaptation and a preliminary survey.A total of 222 hospitalized stroke patients with aphasia from our hospital between January 1,2024 and May 1,2025 were selected for the questionnaire survey.The reliability and validity of the Chinese version of QLQA were tested.

Results

The Chinese version of QLQA comprised 3 dimensions and 37 items in total.The item-level content validity index ranged from 0.80 to 0.97.The scale-level content validity index based on universal agreement was 0.939.The average content validity index was 0.958.The Stroke and Aphasia Quality of Life Scale(SAQOL-39g) was used as the criterion.The correlation coefficient between the Chinese version of QLQA and the SAQOL-39g scores was 0.566(P<0.05).Exploratory factor analysis extracted 3 common factors with eigenvalues greater than 1 with the cumulative variance contribution rate of 66.0%.Confirmatory factor analysis indicated that the three-factor structural equation model demonstrated a good fit.The overall Cronbach's α coefficient of the questionnaire was 0.952,and the test-retest reliability was 0.969.

Conclusions

The Chinese version of QLQA exhibits good reliability and validity.It could be used to assess the quality of life of stroke patients with aphasia.

目的

对失语症生活质量问卷（QLQA）进行汉化并检验其在脑卒中失语症病人中的信度和效度。

方法

采用Brislin翻译模型对QLQA进行翻译，随后进行文化调适、预调查形成中文版QLQA。选取2024年1月1日—2025年5月1日就诊于我院的222例脑卒中失语症住院病人进行问卷调查，检验中文版QLQA的信效度。

结果

中文版QLQA包括3个维度，共37个条目。问卷条目水平的内容效度指数为0.80~0.97，全体一致量表水平内容效度指数为0.939，平均量表水平内容效度指数为0.958。以脑卒中失语症生活质量量表（SAQOL-39g）作为效标，中文版QLQA得分与SAQOL-39g得分的相关系数为0.566（P<0.05）。探索性因子分析提取3个特征值>1的公因子，累计方差贡献率为66.0%。验证性因子分析结果显示，三因子结构方程模型拟合效果良好。问卷总体的Cronbach's α系数为0.952，重测信度为0.969。

结论

中文版QLQA具有良好的信效度，可用于测评脑卒中失语症病人的生活质量。

目的

对失语症生活质量问卷（QLQA）进行汉化并检验其在脑卒中失语症病人中的信度和效度。

方法

采用Brislin翻译模型对QLQA进行翻译，随后进行文化调适、预调查形成中文版QLQA。选取2024年1月1日—2025年5月1日就诊于我院的222例脑卒中失语症住院病人进行问卷调查，检验中文版QLQA的信效度。

结果

中文版QLQA包括3个维度，共37个条目。问卷条目水平的内容效度指数为0.80~0.97，全体一致量表水平内容效度指数为0.939，平均量表水平内容效度指数为0.958。以脑卒中失语症生活质量量表（SAQOL-39g）作为效标，中文版QLQA得分与SAQOL-39g得分的相关系数为0.566（P<0.05）。探索性因子分析提取3个特征值>1的公因子，累计方差贡献率为66.0%。验证性因子分析结果显示，三因子结构方程模型拟合效果良好。问卷总体的Cronbach's α系数为0.952，重测信度为0.969。

结论

中文版QLQA具有良好的信效度，可用于测评脑卒中失语症病人的生活质量。

Objective

To translate the Quality of Life Questionnaire for Aphasics(QLQA) into Chinese and test its reliability and validity in stroke patients with aphasia.

Methods

The Brislin translation model was adopted for the translation of QLQA.The Chinese version of QLQA was developed after cultural adaptation and a preliminary survey.A total of 222 hospitalized stroke patients with aphasia from our hospital between January 1,2024 and May 1,2025 were selected for the questionnaire survey.The reliability and validity of the Chinese version of QLQA were tested.

Results

The Chinese version of QLQA comprised 3 dimensions and 37 items in total.The item-level content validity index ranged from 0.80 to 0.97.The scale-level content validity index based on universal agreement was 0.939.The average content validity index was 0.958.The Stroke and Aphasia Quality of Life Scale(SAQOL-39g) was used as the criterion.The correlation coefficient between the Chinese version of QLQA and the SAQOL-39g scores was 0.566(P<0.05).Exploratory factor analysis extracted 3 common factors with eigenvalues greater than 1 with the cumulative variance contribution rate of 66.0%.Confirmatory factor analysis indicated that the three-factor structural equation model demonstrated a good fit.The overall Cronbach's α coefficient of the questionnaire was 0.952,and the test-retest reliability was 0.969.

Conclusions

The Chinese version of QLQA exhibits good reliability and validity.It could be used to assess the quality of life of stroke patients with aphasia.

脑卒中在我国的发病率不断增高，给家庭和社会造成较大的经济压力，严重影响了病人生活质量。调查数据显示，脑卒中幸存者中约80%会有不同程度的神经功能受损，同时15%~38%的病人会伴有失语症[1]。失语症是因大脑局部病变导致神经受损引起的后天语言、理解或表达能力受损或丧失的一种获得性语言障碍综合征[2]，85%的失语症病人由脑卒中导致[3]。失语症病人容易出现焦虑、抑郁等负性心理，导致人际交往受到影响，生活质量降低[4]，Li等[2]研究发现，失语症病人的生活质量仅优于癌症和老年痴呆病人。目前我国关于脑卒中失语症病人生活质量的特异性问卷相对较少，不利于准确、全面地评估脑卒中失语症病人的生活质量。因此，本研究对失语症生活质量问卷（Quality of Life Questionnaire for Aphasics，QLQA）进行汉化并在脑卒中失语症病人中进行初步应用，检验其信度和效度，旨在为临床评估脑卒中失语症病人的生活质量提供可靠工具。
1　研究方法

1.1　原问卷介绍
QLQA由意大利学者Spaccavento等[5]针对失语症病人理解和表达困难研制，是衡量失语症病人生活质量的特异性问卷。问卷主要包含37个条目，分为3个维度，其中，沟通维度（22个条目）用于评价病人在现实生活和失语情境中的表达和理解能力；心理维度（6个条目)用于评价语言障碍对情绪状态的影响；自主性维度（9个条目）用于评价失语症病人在日常生活活动中的独立性。问卷采用Likert 5级评分法评分，0分表示个人完全无法成功执行行为；1分表示个人能够成功执行约25%的行为；2分表示个人能够成功执行约50%的行为；3分表示个体能够成功执行约75%的行为；4分表示个体能够成功执行100%的行为[5-6]。
1.2　问卷汉化
1.2.1　问卷翻译
征得问卷原作者同意后，采用Brislin翻译模型进行翻译。1）直译：由2名职称为主管护师、学历为硕士研究生且英语水平均达到六级的护士分别将QLQA翻译为中文，形成翻译版QLQA-1和翻译版QLQA-2。2）整合：研究者比较翻译结果，与2名护士讨论后汇总形成翻译版QLQA-3。3）回译：由1名护理学博士研究生和1名神经内科临床医学博士研究生分别将翻译版QLQA-3回译成英文，形成回译版QLQA-1和回译版QLQA-2。4）综合：2名博士研究生对回译版QLQA-1、回译版QLQA-2和原问卷进行讨论、比较、分析，对翻译版QLQA-3再次进行修订，形成中文版QLQA。
1.2.2　文化调适
邀请5名相关领域专家对中文版QLQA进行测评，包括2名护理学专家、1名心理学专家、2名神经内科医生，均具有副高级及以上职称、硕士研究生及以上学历。5名专家根据临床经验和相关专业知识评估中文版QLQA各条目的难易程度以及内容相关性。
1.2.3　预调查
随机选取我院20例脑卒中失语症病人进行预调查。纳入标准：1）符合中华医学会制定的脑卒中诊断标准[7]及《中国康复研究中心的汉语标准失语症检查表》中的失语症诊断标准；2）年龄>18岁；3）首次诊断为脑卒中；4）波士顿诊断性失语症检查（Boston Diagnostic Aphasia Examination，BDAE）分级≥3级（具备基本语言理解与表达能力，可理解问卷条目含义并完成自主应答）；5）病程超过2周；6）生命体征平稳；7）知情同意并自愿参与本研究。排除标准：1）合并严重的急慢性疾病；2）存在精神或认知障碍。在预调查过程中详细询问脑卒中失语症病人对问卷内容能否理解，请病人用书面形式对不理解的条目进行描述并提出建议。分析预调查过程中的病人意见，对问卷进行再次讨论与修订，使条目内容更加通俗易懂。
1.3　正式调查
1.3.1　调查对象
选取2024年1月1日—2025年5月1日就诊于我院的222例脑卒中失语症住院病人进行问卷调查。纳入与排除标准同预调查。样本量的计算根据问卷条目数的5~10倍原则，同时考虑20%的失访率，最终纳入病人222例。本研究已通过我院伦理委员会审核批准[编号：2025-k(YJ)-067]。
1.3.2　调查工具
1）一般资料调查表：研究者查阅相关中英文文献后自行设计，主要包括性别、年龄、文化程度、工作情况、疾病类型、病程。2）中文版QLQA：基于原问卷（37个条目、3个维度）汉化修订而成，保留沟通（22个条目）、心理（6个条目）、自主性（9个条目）核心维度，采用Likert 5级评分法（0~4分），得分越高表示生活质量越好。3）脑卒中失语症生活质量量表（Stroke and Aphasia Quality of Life Scale，SAQOL-39g）：该量表在我国应用较普遍，具有良好信效度，包括心理社会、沟通、生理3个维度，满分为195分，得分越高表示生存质量越高[7]。
1.3.3　资料收集方法
征得医院领导同意后，对脑卒中失语症病人进行问卷调查。征得病人知情同意后向病人解释本研究的目的和意义，由病人独立填写问卷。问卷填写完成后由研究者检查，无漏项后回收问卷。本研究共发放问卷222份，回收有效问卷222份，问卷有效回收率为100%。2周后随机抽取50例脑卒中失语症病人进行重复调查，用于评估中文版QLQA的重测信度。
1.4　统计学方法
采用SPSS 21.0和AMOS 21.0进行统计分析。定性资料以频数及百分比（%）描述，使用内部一致性信度（Cronbach's α系数）和重测信度检验问卷信度，采用内容效度、结构效度、效标效度检验问卷效度。以P<0.05为有统计学意义。
2　结果

2.1　文化调适
5名专家认为问卷条目简洁、易懂，符合我国人群的语言表达习惯，但部分条目用语需修改，课题组讨论后将条目12“理解他人快速说话”修改为“当他人说话语速快的时候我可以理解”；将条目25“羞与别人在一起”修改为“当你和别人在一起时感到尴尬”。
2.2　预调查
所有病人均能独立、顺利完成问卷填写，无漏填、错填或逻辑矛盾情况，问卷有效回收率为100%，完成时间（13.52±4.23）min。病人反馈问卷条目表述清晰准确、语言通俗规范，均未提出条目修改意见，一致认可问卷具有适用性与可操作性。
2.3　正式调查
2.3.1　病人一般资料
222例脑卒中失语症病人年龄34~79（52.25±15.55）岁，其余一般资料见表1。
2.3.2　效度分析
2.3.2.1　内容效度
经5名专家评定后，计算得出中文版QLQA条目水平的内容效度指数为0.80~0.97，全体一致量表水平内容效度指数为0.939，平均量表水平内容效度指数为0.958。
2.3.2.2　效标效度
选用SAQOL-39g作为效标，结果显示，中文版QLQA得分与SAQOL-39g得分的相关系数为0.566（P<0.05)，表明中文版QLQA具有良好的效标效度。
2.3.2.3　结构效度
使用探索性因子分析和验证性因子分析进行结构效度检验。探索性因子分析结果显示，中文版QLQA的KMO值为0.929，Bartlett's球形检验χ2=642.665（P<0.05)，说明适合做因子分析[8]。采用主成分分析法，经方差正交旋转后提取出3个特征值>1的公因子，各因子条目构成与原问卷一致，3个公因子分别命名为沟通、心理和自主性，其方差贡献率依次为33.6%、12.3%和20.1%，累计方差贡献率为66.0%。各条目因子载荷为0.501~0.856，不存在双载荷现象，见表2。采用极大似然法进行验证性因子分析，得到三因子结构方程模型，见图1。三因子结构方程模型卡方自由度比（χ2/df）=1.160，近似误差均方根（RMSEA）=0.034，均方根误差（RMR）=0.029，拟合优度指数（GFI）=0.950，调整的拟合优度指数（AGFI）=0.939，相对拟合度指数（CFI）=0.979，拟合效果良好。
2.3.3　信度分析（见表3）

3.1　中文版QLQA具有良好信度
问卷的信度是指其测量结果内在一致性和可靠性，排除随机误差后获取真实信息的水平[9]。内在一致性信度即整个问卷是否在测量同一问题，反映不同条目间的内在一致性。评估问卷内在一致性最常用的指标是Cronbach's α系数，>0.80说明问卷内在一致性良好。重测信度是在不同时间评估问卷后结果的一致性水平，一般认为重测信度>0.75说明问卷跨时间性好[10]。本研究结果显示，中文版QLQA总体及其3个维度的Cronbach's α系数均>0.80，说明中文版QLQA具有良好的内在一致性。同时，本研究结果显示，2周后再次进行问卷调查，中文版QLQA总体及其3个维度的重测信度均>0.75，说明中文版QLQA重测信度较好。QLQA原问卷的Cronbach's α系数为0.962，心理维度的Cronbach's α系数为0.847，自主性维度的Cronbach's α系数为0.893，沟通维度的Cronbach's α系数为0.920；QLQA原问卷在脑卒中失语症病人中的重测信度为0.984，心理维度重测信度为0.872，自主性维度重测信度为0.881，沟通维度重测信度为0.927[5]。中文版QLQA与原问卷信度差距不大，说明QLQA具有良好的时间稳定性和内部一致性。
3.2　中文版QLQA具有良好效度
效度即有效性，指问卷能够准确评估出所需测量事物的水平，测量结果与要考察的内容越吻合，效度越高[10]。结构效度、效标效度、内容效度等是常用的效度评价指标，其中，结构效度评价利用探索性因子分析和验证性因子分析实现，用于检验问卷的结构是否与问卷编制时构想的结构一致[11]。本研究探索性因子分析提取3个公因子，累积方差贡献率>60%。验证性因子分析结果显示，拟合指数均可接受，与原作者的3个维度构想一致。效标效度是以相对准确的测量手段或指标测量结果作为金标准，考察待评指标的测量结果是否与其一致。本研究结果显示，中文版QLQA在脑卒中失语症病人中应用后总分与SAQOL-39g总分呈正相关，表明中文版QLQA效标关联效度较好。内容效度用于评价测量指标的含义是否准确反映真实情况。本研究采用专家评价的方法测评内容效度，结果显示，条目水平的内容效度指数、全体一致量表水平内容效度指数、平均量表水平内容效度指数均≥0.80，说明中文版QLQA的内容效度良好。

中文版QLQA的信度和效度均在合理范围，可用于评估我国脑卒中失语症病人的生活质量。但本研究受取样条件限制，样本局限于三级甲等医院的脑卒中住院病人，门诊和社区病人未涉及，同时未对中文版QLQA的区分效度进行评估，未来可以进一步探讨中文版QLQA在不同地区、不同特征脑卒中病人中的适用性。
