Objective

To explore the development trajectories of pain catastrophizing in elderly lung cancer patients with chronic pain undergoing chemotherapy,to analyze influencing factors,and to compare differences in physical activity among patients in different trajectory categories.

Methods

A total of 213 elderly lung cancer patients with chronic pain undergoing chemotherapy in Jiangsu Provincial People's Hospital were selected as research subjects from January 2022 to February 2024.Pain catastrophizing levels of patients were surveyed before chemotherapy initiation,during the mid-phase of chemotherapy,at the end of chemotherapy,and one month after chemotherapy completion.Group-based trajectory modeling was used to identify categories of pain catastrophizing trajectories.Logistic regression analysis was employed to explore influencing factors for different trajectory categories.Physical activity levels were compared among patients in different trajectory categories.

Results

Model fitting ultimately identified three categories of pain catastrophizing trajectories,namely the low pain catastrophizing group,the moderate pain catastrophizing stable group,and the high pain catastrophizing increasing group.Logistic regression analysis showed that age,pain intensity,duration of persistent pain,personality traits, anxiety or depression,and family support were influencing factors for pain catastrophizing trajectory categories in elderly lung cancer patients with chronic pain undergoing chemotherapy (P<0.05). Comparisons of physical activity scores among patients in different pain catastrophizing trajectory categories showed statistically significant differences(P<0.05).

Conclusions

Elderly lung cancer patients with chronic pain undergoing chemotherapy exhibit three categories of pain catastrophizing trajectories.Healthcare professionals can develop and implement intervention plans based on the influencing factors of pain catastrophizing trajectories of patients to reduce their levels of pain catastrophizing.

目的

探究接受化疗的老年肺癌慢性疼痛病人疼痛灾难化发展轨迹，分析其影响因素，并比较不同轨迹类别病人的体力活动差异。

方法

选取2022年1月—2024年2月在江苏省人民医院接受化疗的213例老年肺癌慢性疼痛病人为研究对象，于病人化疗开始前、化疗中期、化疗结束时、化疗结束后1个月对病人的疼痛灾难化水平进行调查，通过组基轨迹模型识别病人的疼痛灾难化变化轨迹类别，采用Logistic回归分析探讨不同轨迹类别的影响因素，并比较不同轨迹类别病人的体力活动水平。

结果

经模型拟合最终得到低疼痛灾难化组、中疼痛灾难化稳定组、高疼痛灾难化上升组3类疼痛灾难化轨迹。Logistic回归分析结果显示，年龄、疼痛强度、疼痛持续时间、人格特征、焦虑或抑郁、家庭关怀度均是接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别的影响因素（P<0.05）。不同类别疼痛灾难化轨迹病人的体力活动得分比较，差异有统计学意义（P<0.05）。

结论

接受化疗的老年肺癌慢性疼痛病人存在3类疼痛灾难化轨迹，医务人员可根据病人疼痛灾难化轨迹的影响因素制定及实施干预方案，以降低病人的疼痛灾难化水平。

目的

探究接受化疗的老年肺癌慢性疼痛病人疼痛灾难化发展轨迹，分析其影响因素，并比较不同轨迹类别病人的体力活动差异。

方法

选取2022年1月—2024年2月在江苏省人民医院接受化疗的213例老年肺癌慢性疼痛病人为研究对象，于病人化疗开始前、化疗中期、化疗结束时、化疗结束后1个月对病人的疼痛灾难化水平进行调查，通过组基轨迹模型识别病人的疼痛灾难化变化轨迹类别，采用Logistic回归分析探讨不同轨迹类别的影响因素，并比较不同轨迹类别病人的体力活动水平。

结果

经模型拟合最终得到低疼痛灾难化组、中疼痛灾难化稳定组、高疼痛灾难化上升组3类疼痛灾难化轨迹。Logistic回归分析结果显示，年龄、疼痛强度、疼痛持续时间、人格特征、焦虑或抑郁、家庭关怀度均是接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别的影响因素（P<0.05）。不同类别疼痛灾难化轨迹病人的体力活动得分比较，差异有统计学意义（P<0.05）。

结论

接受化疗的老年肺癌慢性疼痛病人存在3类疼痛灾难化轨迹，医务人员可根据病人疼痛灾难化轨迹的影响因素制定及实施干预方案，以降低病人的疼痛灾难化水平。

Objective

To explore the development trajectories of pain catastrophizing in elderly lung cancer patients with chronic pain undergoing chemotherapy,to analyze influencing factors,and to compare differences in physical activity among patients in different trajectory categories.

Methods

A total of 213 elderly lung cancer patients with chronic pain undergoing chemotherapy in Jiangsu Provincial People's Hospital were selected as research subjects from January 2022 to February 2024.Pain catastrophizing levels of patients were surveyed before chemotherapy initiation,during the mid-phase of chemotherapy,at the end of chemotherapy,and one month after chemotherapy completion.Group-based trajectory modeling was used to identify categories of pain catastrophizing trajectories.Logistic regression analysis was employed to explore influencing factors for different trajectory categories.Physical activity levels were compared among patients in different trajectory categories.

Results

Model fitting ultimately identified three categories of pain catastrophizing trajectories,namely the low pain catastrophizing group,the moderate pain catastrophizing stable group,and the high pain catastrophizing increasing group.Logistic regression analysis showed that age,pain intensity,duration of persistent pain,personality traits, anxiety or depression,and family support were influencing factors for pain catastrophizing trajectory categories in elderly lung cancer patients with chronic pain undergoing chemotherapy (P<0.05). Comparisons of physical activity scores among patients in different pain catastrophizing trajectory categories showed statistically significant differences(P<0.05).

Conclusions

疼痛被列为除呼吸、血压、脉搏、体温以外的第五项生命体征[1]。国际疼痛研究协会将疼痛定义为一种与实际或潜在组织损伤相关的不愉快感觉和情绪体验[2]。已有研究结果显示，癌症相关疼痛发生率约为70%，尤其是晚期癌症病人疼痛最为突出[3]。肺癌是威胁人类生命健康的恶性肿瘤性疾病，其患病率和死亡率在我国以及全世界恶性肿瘤疾病中均居于前列，且新发病例和死亡病例呈持续上升趋势[4]。肺癌病人常存在癌痛症状，其可严重影响病人体力活动和生存质量[5]。阿片类药物的规范使用在一定程度上可以控制癌痛，但仍有部分病人因服药依从性或认知不足等导致癌痛频发[6]。统计数据显示，肺癌病人疼痛发生率约为47%，晚期肺癌病人的疼痛发生率为90%[7]。慢性癌痛是指持续时间超过3个月的疼痛[8]。全球20%~30%的人口受慢性疼痛影响，不仅可降低肺癌病人体力活动和生活质量，还可引发多种心理问题[9]。恐惧回避模型指出，对疼痛的负面评价可诱发多种与疼痛相关的灾难性想法，即疼痛灾难化[10]。疼痛灾难化是指个体经历疼痛时表现的负面心理定势，与病人癌因性疲乏、生活质量、负性心理及药物滥用相关[11-13]。老年肺癌慢性疼痛病人抗癌期间会经历治疗阶段的转换和环境变动，进而可能引起病人疼痛灾难化出现动态变化。目前有关老年肺癌慢性疼痛病人化疗期间的疼痛灾难化动态变化追踪报道较少，本研究运用组基轨迹模型识别老年肺癌慢性疼痛病人疼痛灾难化的轨迹类别，并分析其影响因素，不仅有利于医务人员制定更具针对性的疼痛灾难化指导方案，还可通过精准干预提高医疗资源利用率。
1　对象与方法

1.1　研究对象
采用便利抽样法，选取2022年1月—2024年2月在江苏省人民医院接受化疗的213例老年肺癌慢性疼痛病人为研究对象。纳入标准：1）年龄≥60岁；2）符合《中华医学会肺癌临床诊疗指南（2022版）》[14]中的肺癌诊断标准，且经病理检查确诊；3）接受化疗；4）疼痛持续时间≥3个月；5）意识清楚，可正常沟通交流，可理解问卷调查内容；6）知晓自身病情与诊断；7）临床资料完整；8）自愿参与本研究。排除标准：1）伴有认知障碍或精神异常；2）合并其他恶性肿瘤；3）近期有丧偶、离异等其他创伤性事件；4）处于谵妄状态或有痴呆等神经系统疾病。剔除标准：中途退出研究。本研究已通过我院伦理委员会批准（编号：2021-KY268-016）。
1.2　调查工具
1.2.1　一般资料调查表
自制一般资料调查表，内容包括性别、年龄、体质指数（BMI）、文化程度、居住地、婚姻状况、家庭人均月收入、吸烟史、饮酒史、癌症分型、病理分期、疼痛持续时间、肺癌家族史。
1.2.2　中文版疼痛灾难化量表（Chinese Pain Catastrophizing Scale）
该量表由Yap等[15]汉化，包括反刍、夸大、无助3个维度，共13个条目，采用Likert 5级评分法，0分为从来没有，4分为总是如此，总分0~52分，得分越高提示疼痛灾难化越严重。量表Cronhach's α系数为0.927[15]。
1.2.3　疼痛强度数值评定量表（11-Point Numerical Pain Rating Scale）
该量表由0~10个数字组成，0分表示无疼痛，10分表示极度疼痛，总分1~3分表示轻度疼痛，即病人可能感觉到疼痛，但不会明显影响日常生活；4~6分表示中度疼痛，即疼痛影响日常生活，病人可能需要采取措施缓解疼痛；7~10分表示重度疼痛，即疼痛显著影响日常生活，病人可能难以执行日常任务，需要采取积极的疼痛管理措施[16]。
1.2.4　焦虑抑郁量表
该量表包括焦虑、抑郁2个分量表，每个量表均由7个条目组成，采用Likert 3级评分法，2个分量表总分均为0~21分，得分越高提示受试者焦虑或抑郁症状越严重，得分≥8分表明受试者存在焦虑或抑郁症状[17]。量表具有较高信效度。
1.2.5　中国版大五人格量表简式版(CBF-PI-B)
该量表包括神经质、宜人性、严谨性、开放性和外向性5个分量表，采用Likert 6级评分法，1分为完全不符合，6分为完全符合，得分越高提示人格特质越明显[18]。5个分量表的Cronbach's α系数均>0.75。
1.2.6　家庭关怀度量表
该量表包括成长、适应性、亲密关系、伙伴关系、合作性5个条目，采用Likert 3级评分法，0分表示几乎很少，2分表示经常这样，总分0~10分，≥7分提示家庭关怀度良好，<7分提示家庭关怀度一般[19]。
1.2.7　老年人体力活动量表（PASE）
该量表包括坐位活动、家务性体力活动、休闲性体力活动、职业性体力活动4个维度，共26个条目。总分为0~400分，得分越高提示身体活动水平越高[20]。
1.3　资料收集方法
于化疗开始前、化疗中期、化疗结束时、化疗结束后1个月对接受化疗的老年肺癌慢性疼痛病人疼痛灾难化水平进行调查。研究者调查前向病人说明本次研究的目的、意义以及随访时间等，以确保所有病人在知情且同意的情况下参与研究。问卷调查时间为10~15 min，病人完成问卷填写后当场检查，如有规律作答或遗漏予以完善或剔除。本研究共发放230份问卷，回收213份有效问卷，问卷有效回收率为92.61%。
1.4　统计学方法
采用SPSS 26.0和Stata MP17软件进行统计分析。采用组基轨迹模型识别疼痛灾难化变化轨迹。模型检验指标包括赤池信息准则（AIC）、贝叶斯信息准则（BIC）和样本矫正的BIC（aBIC）、熵（Entropy）、Bootstrap的似然比检验（BLRT）和罗-梦戴尔-鲁本校正似然比检验（LMRT），其中，AIC、BIC、aBIC用于检验拟合度；Entropy用于辨别分模型标准度；LMRT和BLRT用于检测拟合差异。正态分布的定量资料以均数±标准差（x±s）表示，定性资料以频数、百分比（%）表示，组间比较采用方差分析、t检验、χ2检验；采用Logistic回归分析探讨疼痛灾难化轨迹类别的影响因素。检验水准α=0.05。
2　结果

2.1　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别
以接受化疗的老年肺癌慢性疼痛病人化疗开始前、化疗中期、化疗结束时、化疗结束后1个月的疼痛灾难化得分作为观测指标，拟合不同数量的疼痛灾难化轨迹类别，当类别数由1个增加至3个时，AIC、BIC、aBIC绝对值逐渐减小，LMRT和BLRT均有统计学意义（P<0.05），3个类别的Entropy值最高（0.875），故确定3个类别为最佳拟合状态。此外，3个类别分配平均验后概率为0.91，0.94，0.92，大于规定Entropy（0.7），模型拟合效果良好，详见表1。
2.2　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别及命名
根据疼痛灾难化得分拟合结果，选择类别数为3的轨迹模型。213例接受化疗的老年肺癌慢性疼痛病人化疗开始前、化疗中期、化疗结束时、化疗结束后1个月的疼痛灾难化得分依次为（33.59±4.23）分、（35.35±4.95）分、（34.69±4.57）分、（35.60±6.14）分，均分为（34.81±4.63）分。类别1病人占26.3%（56/213），该类病人的疼痛灾难化初始得分在3个类别中最低，为（28.14±3.37）分，其在不同随访时段的疼痛灾难化得分处于较低水平，故命名为“低疼痛灾难化组”；类别2病人占54.5%（116/213），该类病人的疼痛灾难化初始得分为（34.88±5.53）分，与疼痛灾难化总均分[（34.81±4.63）分]相近，其在不同随访时间的疼痛灾难化得分变化不显著，故命名为“中疼痛灾难化稳定组”；类别3病人占19.2%（41/213），该类病人的疼痛灾难化初始得分为（38.44±7.42）分，高于疼痛灾难化总均分[（34.81±4.63）分]，且疼痛灾难化得分随时间推移呈上升趋势，故命名为“高疼痛灾难化上升组”，详见图1。
2.3　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别影响因素的单因素分析（见表2）
2.4　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别影响因素的Logistic回归分析
以接受化疗的老年肺癌慢性疼痛病人的疼痛灾难化轨迹类别为因变量（以低疼痛灾难化组为参照），以单因素分析中有统计学意义（P<0.05）的变量作为自变量，进行Logistic回归分析，结果显示，与低疼痛灾难化组相比，年龄>70岁、疼痛强度为中度或重度、疼痛持续时间>6个月、人格特征为神经质、伴有焦虑或抑郁、家庭关怀度一般的老年肺癌慢性疼痛病人更易归为中疼痛灾难化稳定组和高疼痛灾难化上升组（P<0.05）。自变量赋值方式见表3，接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别影响因素的Logistic回归分析结果见表4。

3　讨论

3.1　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹具有群体异质性
本研究结果显示，接受化疗的老年肺癌慢性疼痛病人疼痛灾难化存在低疼痛灾难化、中疼痛灾难化稳定、高疼痛灾难化上升3类，与潘琦[21]报道的老年全膝关节置换病人相似。3类疼痛灾难化轨迹中，低疼痛灾难化组病人占26.3%，说明部分病人从化疗开始前至化疗结束后1个月的疼痛灾难化持续保持较低水平。本研究中疼痛灾难化稳定组和高疼痛灾难化上升组共占73.7%，说明多数接受化疗的老年肺癌慢性疼痛病人的疼痛灾难化水平处于中水平或高水平，这可能是因为病人在化疗过程中面临未知恐惧、疼痛症状、化疗副作用和家庭功能不佳等问题，导致对疼痛的负面评价增加，疼痛灾难化水平提高。提示医务人员需重点关注接受化疗的老年肺癌慢性疼痛病人疼痛灾难化表达，并在化疗期间提供帮助，以消除病人对疼痛的恐惧和负面认知。此外，医务人员也应定期评估病人的疼痛水平，通过提供个性化的支持和干预措施帮助病人有效管理疼痛症状，从而降低疼痛灾难化表达。
3.2　接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别影响因素
本研究结果显示，年龄、疼痛强度、疼痛持续时间、人格特征、焦虑或抑郁、家庭关怀度为接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别的影响因素（P<0.05）。1）年龄较大的老年肺癌慢性疼痛病人疼痛灾难化水平较高，这可能是由于高龄病人身体机能和免疫系统功能相对较弱，对疼痛的耐受能力和恢复能力较差，容易因疼痛产生更高的疼痛灾难化表达。但也有研究发现，随着病人年龄增长，其神经元密度逐渐降低，同时神经传导功能不断减弱，可能导致疼痛阈值升高，疼痛灾难化呈低表达[7]。不同研究结果不一致，原因可能与研究对象年龄、疼痛程度等存在差异有关，后续仍需进一步研究以明确年龄与疼痛灾难化的关系。2）疼痛强度为中度或重度的老年肺癌慢性疼痛病人疼痛灾难化水平较高，与Kanzawa-Lee等[22]研究结果相似。疼痛强度增加可能会加剧病人的不适感和痛苦感，加重病人疼痛体验和不良心理状态，继而导致病人疼痛灾难化水平升高。3）疼痛持续时间较长的老年肺癌慢性疼痛病人疼痛灾难化水平较高。长期的疼痛不仅会引起病人生理不适，还可能影响病人生活质量，限制其日常活动和社交互动，增加孤独感和无助感，促使病人对疼痛的负面评价增加，导致疼痛灾难化表达加剧。Bovbjerg等[23]研究表明，随着癌症病人癌痛持续时间延长，其疼痛灾难化水平呈升高趋势。4）人格特征为神经质的老年肺癌慢性疼痛病人疼痛灾难化水平较高，与常清[24]研究结果相似。原因可能为神经质人格特质病人对疼痛的敏感性较高，更易对疼痛产生负面评价和过度担忧。而具有宜人性、严谨性、开放性和外向性人格特质的病人通常拥有更好的情绪调节能力和应对策略，能够更有效地管理和应对疼痛及负面情绪。5）伴有焦虑或抑郁的老年肺癌慢性疼痛病人疼痛灾难化水平较高，与Wang等[25]调查结果相似。伴有焦虑或抑郁的病人可能对化疗的期望值较高，实际疗效未达到预期时，病人失望和挫败感增加，从而加剧疼痛灾难化。6）家庭关怀度一般的老年肺癌慢性疼痛病人疼痛灾难化水平较高。袁淑莲[26]研究发现，家庭功能障碍会增加骨科慢性疼痛病人疼痛灾难化表达。原因可能为家庭关怀度一般的病人可能缺乏足够的心理支持和情感慰藉，导致其对疼痛的负面评价增加，心理调节能力变差，从而加剧疼痛灾难化表达。
3.3　不同疼痛灾难化轨迹类别病人体力活动评分不同
已有研究结果显示，规律的体力活动在改善心肺功能、减轻疼痛、保护认知功能及降低死亡率方面具有积极作用[27-28]。肺癌病人在疾病的任何阶段均可以进行体力活动，且症状困扰较严重的病人从体力活动中获益较大[29]。澳大利亚临床肿瘤学会也指出，适当的运动锻炼有利于病人对抗癌症治疗及癌症本身带来的不利影响，应作为癌症疾病护理实践的重要内容，并鼓励癌症病人积极参与[30]。本研究结果显示，不同疼痛灾难化轨迹类别病人体力活动得分存在差异，高疼痛灾难化上升组病人体力活动得分低于中疼痛灾难化稳定组和低疼痛灾难化组，而中疼痛灾难化稳定组病人体力活动得分低于低疼痛灾难化组（均P<0.05）。与冠心病病人中的研究结果[31]相似。原因可能为随着疼痛灾难化水平升高，病人的体力活动能力逐渐受限，同时疼痛和负面情绪影响增大，导致病人参与体力活动的意愿和能力降低。医务人员可通过提供心理支持、进行疼痛管理和开展健康教育等干预措施，降低病人的疼痛灾难化表达，从而提高其体力活动水平，使更多病人从运动锻炼中获益。
4　小结

接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹存在群体异质性，少部分病人疼痛灾难化处于低水平。年龄、疼痛强度、疼痛持续时间、人格特征、焦虑或抑郁、家庭关怀度为接受化疗的老年肺癌慢性疼痛病人疼痛灾难化轨迹类别的影响因素。医护人员可针对不同疼痛灾难化轨迹类别病人制定并实施相应的干预方案，以降低病人疼痛灾难化表达。但本研究为单中心研究，仅追踪至病人化疗后1个月，未来需行多中心研究并延长追踪时间，以深入探究病人的疼痛灾难化轨迹变化。
