Objective To construct an upper limb exercise program after permanent pacemaker implantation,providing reference for early exercise rehabilitation. Methods Literature on upper limb exercise intervention after permanent pacemaker implantation were searched.Through literature research and discussions within our research group,an initial pool of project items was formed.And expert inquiry questionnaire was compiled.Using the Delphi method,15 experts were subjected to 2 rounds of inquiry to screen for project items. Results A total of 22 articles were included.The effective recovery rates of both rounds of inquiry was 100%.And the expert authority coefficient was 0.93.The Kendall harmony coefficients for item importance and feasibility were 0.275⁃0.467 and 0.211⁃0.438,respectively(P<0.05).The upper limb exercise program after permanent pacemaker implantation constructed in this study included 4 primary items,15 secondary items,and 44 tertiary items. Conclusions The upper limb exercise program for permanent pacemaker implantation constructed in this study has strong clinical practicality and could provide reference for clinical practice.

目的 构建永久性心脏起搏器植入术后上肢运动方案,为开展早期运动康复提供参考。 方法 系统检索永久性心脏起搏器植入术后上肢运动干预的文献,经文献研究、课题组讨论,形成初始方案条目池并编制专家函询问卷。采用德尔菲法,对15名专家进行2轮函询筛选方案条目。 结果 共纳入22篇文献,2轮函询问卷有效回收率均为100%,专家权威系数均为0.93。第2轮函询条目重要性、可行性的肯德尔和谐系数分别为0.275~0.467、0.211~0.438(P<0.05)。本研究构建的永久性心脏起搏器植入术后上肢运动方案包括4项一级条目、15项二级条目、44项三级条目。 结论 本研究构建的永久性心脏起搏器植入术后上肢运动方案临床实用性较强,可为临床实践提供参考。

Objective To construct an upper limb exercise program after permanent pacemaker implantation,providing reference for early exercise rehabilitation. Methods Literature on upper limb exercise intervention after permanent pacemaker implantation were searched.Through literature research and discussions within our research group,an initial pool of project items was formed.And expert inquiry questionnaire was compiled.Using the Delphi method,15 experts were subjected to 2 rounds of inquiry to screen for project items. Results A total of 22 articles were included.The effective recovery rates of both rounds of inquiry was 100%.And the expert authority coefficient was 0.93.The Kendall harmony coefficients for item importance and feasibility were 0.275⁃0.467 and 0.211⁃0.438,respectively(P<0.05).The upper limb exercise program after permanent pacemaker implantation constructed in this study included 4 primary items,15 secondary items,and 44 tertiary items. Conclusions The upper limb exercise program for permanent pacemaker implantation constructed in this study has strong clinical practicality and could provide reference for clinical practice.

永久性心脏起搏器植入术(permanent pacemaker implantation,PPI)已成为治疗缓慢性及快速性心律失常、晚期心力衰竭及心肌病等疾病的重要手段。2021年,全球开展的心脏起搏器植入手术约130万台;同年,我国起搏器植入99 306例,较2020年增加了15.2%[1]。为提升永久性心脏起搏器植入术后的护理效果,临床上遵循术侧肩关节制动的原则。然而,过度的术肢制动是导致术后术肢运动功能障碍的影响因素之一,同时也影响病人术后的生活质量[2]。肩痛和肩关节活动范围受限是常见的上肢运动功能障碍[3]。目前虽有研究证实术后早期术肢功能锻炼有利于改善永久性心脏起搏器植入术后病人肩关节功能活动度、提高病人术后生活质量[45],但不同运动方案中运动方式、运动强度和评价指标各不相同。相关指南和专家共识给出了制定运动方案所要遵循的原则,但缺少具体可实行的运动内容[6]。因此,本研究采用文献回顾和德尔菲专家函询法构建安全、科学、病人可接受的永久性心脏起搏器植入术后上肢运动方案,以期为临床提供实践指导。
课题小组包括5名成员:主任护师1人,副主任护师2人,护理专业硕士研究生2人。组员全程参与拟定运动方案条目池、编制专家函询问卷、遴选函询专家、发放和回收问卷、统计分析数据、整理专家意见。
检索中国知网、万方数据库、维普数据库、医脉通、PubMed、Web of Science、Cochrane Library、MedSci、英国国家卫生与服务优化研究院(National Institute for Health and Care Excellence,NICE)、苏格兰院际指南网(Scottish Intercollegiate Guidelines Network,SIGN)中英文数据库以及相关指南网站的文献。中文主题词为“起搏器”“运动康复”“心脏康复”“运动疗法”;英文主题词为“pacemaker”“exercise”“cardiac rehabilitation”“exercises therapies”,采用主题词与自由词相结合的方式检索,检索时限为建库至2023年2月。最终纳入22篇[2,727]文献,其中随机对照研究2篇[11,13],类试验研究11篇[2,7,910,12,14,1620],专家共识5篇[2125],指南3篇[8,1526],证据总结1篇[27]。其中3篇[8,1526]指南和3篇[2122,25]专家共识推荐级别均为“A级”;2篇[2324]专家共识推荐级别为“B级”;11篇[2,7,910,12,14,1620]原始性研究质量评价均为“B级”。由2名研究者通过文献回顾,共同归纳、汇总,经课题组讨论形成永久性心脏起搏器植入术后上肢运动方案初稿。
从浙江省三级甲等医院选取介入治疗、护理、康复领域具有丰富临床经验的专家。专家纳入标准:1)所在医院为三级甲等医院;2)具有本科及以上学历;3)有10年及以上介入治疗、护理、康复领域工作经验;4)自愿参与本研究,并能够积极配合完成至少2轮函询。最终共纳入15名专家,其中男3人,女12人;年龄32~53(41.87±6.47)岁;工作年限10~36(19.60±9.08)年,其中≥20年者5人;正高级职称6人,副高级职称4人,中级职称5人;博士研究生3人,硕士研究生3人,本科9人。
专家函询问卷包括4部分内容:1)致专家信,向专家介绍本研究的背景及意义;2)专家咨询正文,包括咨询问卷填写说明及要求、问卷主体,问卷主体为永久性心脏起搏器植入术后上肢运动方案内容,采用Likert 5级评分法对各条目的重要性、可行性赋值,赋值1~5分,分数越高说明重要程度或可行性越高,在各条目后设置补充、修改、删除栏,专家可根据自身临床经验对其进行删除、修改和补充;3)专家基本信息表,包括专家的年龄、职称、学历、工作年限等;4)专家权威程度自评表,包括对指标的判断依据(Ca)及熟悉程度(Cs)等。
根据函询专家对一级指标、二级指标重要性、可行性评分及变异系数进行筛选。筛选标准:重要性、可行性均分>3.5分,变异系数<0.25。并结合专家意见进行修改或者删减。
2名研究者借助Excel 2010和SPSS 26.0工具分别进行录入数据和统计分析。对条目重要性、可行性、变异系数等数据进行描述性分析;计算专家积极系数、权威系数;采用肯德尔和谐系数和变异系数表示专家意见协调程度。变异系数越小,表明专家意见协调程度越好,以P<0.05为有统计学意义。
2轮专家函询问卷的有效回收率皆为100%,分别有13、1名专家提出具有建设性的意见,专家积极性高。2轮函询专家的熟悉程度系数均为0.91,判断系数为均0.95,权威系数均为0.93,专家的权威度高。
第2轮专家函询条目重要性和可行性的肯德尔和谐系数为0.275~0.467,0.211~0.438,详见表1
根据专家意见,经课题组讨论后,对方案条目做出如下修改:将运动“运动类型”改为“运动方式”,并将其分为“手部运动”“肘关节运动”“肩关节运动”三部分;将“出院后”改为“出院当天”和“居家”,并修改相关三级条目;将“手术日至出院前1 d”的健康宣教内容细分为“运动前”“运动中”“运动后”,并修改相关三级条目;“团队准备”中建议增加“心脏康复护士经过统一培训并考核通过”三级条目;补充一般资料相关内容;删除“窦房结功能正常的病人,进行康复运动时由主治医生进行上限心率的设定”相关内容;“基础心率”修改为“低限频率”;“心率应答功能”修改为“频率应答功能”;建议将弹力带推胸运动、弹力带肩关节外旋运动的运动频率改为每天10~20次,每周3 d;运动强度中“心率增加20/min左右”修改为“最大心率的70%~80%”;增加“抗阻运动1次最大重复肌肉力量(1RM)的30%~40%”三级条目;终止指标中补充起搏器相关参数;“院内监测”补充监测运动后生命体征、经皮血氧饱和度、疼痛内容;结局指标建议增加“握力”;“住院天数”改为“本次住院天数”;“中文版肩关节疼痛和功能障碍指数(SPADI)”首次测评时间改为术后当天。
共收获1条专家意见,将条目“教会病人设置手机运动提醒服务,鼓励病人利用互联网、微信群进行自我监控。”修改为“教会病人或家属设置手机运动提醒服务,鼓励病人利用微信群进行自我监控。”,经讨论,予修改。
通过2轮专家问卷函询后,确定了永久性心脏起搏器植入术后上肢运动方案终稿,包括4项一级条目、15项二级条目、44项三级条目,详见表2
本研究检索了各数据库自建库至2023年2月的起搏器/心脏电子设备植入装置病人术后活动文献,最终纳入3篇指南[8,1526]和5篇专家共识[2125]。指南和专家共识均为近5年内发表的文献,证据更新较及时。本研究纳入的指南及专家共识整体质量较好,3篇指南[8,1526]和3篇[2122,25]专家共识推荐级别均为“A级”,2篇[2324]专家共识推荐级别为“B级”。本研究纳入的11篇原始性研究质量评价均为“B级”,整体质量尚可,能够保证方案构建的科学性。本研究邀请的专家皆为本科及以上学历、中级及以上职称,涵盖了介入治疗、护理、康复领域的专家,且专家权威系数为0.93,说明函询可信度高。2轮专家咨询的问卷回收率均为100%,其中第1轮13名专家、第2轮1名专家提出了建设性意见,表明专家重视并积极参与本研究。第2轮函询后,条目重要性和可行性的肯德尔和谐协调系较第1轮均增加,且差异均有统计学意义(P<0.05),进一步验证了专家们对条目的一致性认同程度较高,确保了研究结果的可靠性与科学性。
以运动为基础的心脏康复可促进心血管疾病的二级预防[28]。目前,大约有18个国家,如美国、英国、加拿大、澳大利亚、新西兰等发表了心脏康复指导方针或联合声明[6]。永久性心脏起搏器植入术后病人往往过于担心治疗效果,减少了体力活动,部分病人身体部分(术侧上肢)处于失能状态[29]。因此,永久性心脏起搏器植入术后病人非常需要优质的康复护理。然而,目前心脏康复的对象主要为冠心病、介入术后和心力衰竭病人[30],针对永久性心脏起搏器植入术后病人康复内容较少。2020年,我国《心血管植入型电子器械术后随访的专家共识(2020)》[31]详细规范了心血管植入型电子器械的术后随访,但未涉及病人术后运动康复方面。2021年,欧洲《心脏电子设备植入病人心脏综合康复共识》[22]归纳了术后中等强度连续训练、高等强度间歇训练和抗阻/力量训练的运动频率、运动效果和安全性,但对运动开始时间、类型选择等具体实施流程等未进行明确阐述。近几年,国内学者尝试构建永久性心脏起搏器植入术后病人肩关节活动方案,但未系统性地涵盖运动前评估、运动处方、出院指导及随访要求,或者对于上肢运动处方的证据不具体、不详尽[32]。在永久性心脏起搏器植入术后运动康复的临床研究中,虽有很多国内外学者进行运动方案探索,但未有针对永久性心脏起搏器植入术术肢全面的、详细的、科学的运动方案。因此,本研究在全面检索和评价国内外相关循证资源的基础上,通过德尔菲函询法构建适合我国病人的永久性心脏起搏器植入术后上肢运动方案,有利于了解永久性心脏起搏器植入术后病人实际需求,改善术后运动康复安全。
本研究对相关文献进行系统归纳、分析,形成的方案初稿通过2轮专家函询,最终形成永久性心脏起搏器植入术后上肢运动方案。该方案构建过程严谨,方法科学,专家参与积极性高,意见集中度好,专家协调系数及权威性好,能够为今后提高永久性心脏起搏器植入术后病人上肢运动依从性,助推早期运动康复,改善永久性心脏起搏器植入术后肩关节功能障碍等情况提供有力的支持。完整的方案构建还应包括临床实证研究,故今后将进一步完成临床大样本、多中心的调研,以完成对方案的调试和完善,使其更适用于临床。
