Objective To explore the application effect of multi-component exercise combined with nutritional intervention based on interactive achievement theory in elderly diabetes patients complicated with sarcopenia. Methods A total of 124 elderly diabetes patients complicated with sarcopenia treated in a tertiary grade A hospital in Shanxi province from August to December,2023 were selected as the study subjects.They were randomly divided into test group and control group by random number table method.62 cases each.The control group received routine nursing measures.The test group received the multi⁃component exercise combined with nutrition intervention constructed by the research group.Calf circumference,grip strength,physical function,muscle mass,and quality of life between two groups were compared before intervention and at 3 and 6 months after intervention. Results After intervention,the test group had better grip strength,calf circumference,physical function,muscle mass,and quality of life than the control group(P<0.05). Conclusions The multi-component exercise combined nutrition intervention based on interactive achievement theory was scientific and reliable.The application of this program could effectively improve the related indicators of sarcopenia in elderly diabetes patients complicated with sarcopenia,improve the quality of life of patients,and provide useful reference for clinical nursing practice.

目的 探讨基于互动达标理论的多组分运动联合营养干预在老年糖尿病合并肌少症病人中的应用效果。 方法 选取2023年8月—12月在山西省某三级甲等医院就诊的124例老年糖尿病合并肌少症病人为研究对象,采用随机数字表法将其分为试验组和对照组,各62例。对照组实施常规护理措施,试验组实施课题组构建的多组分运动联合营养干预。比较干预前及干预后3、6个月时两组病人小腿围、握力、躯体功能、肌肉质量和生活质量。 结果 干预后,试验组握力、小腿围、躯体功能、肌肉质量和生活质量均优于对照组(P<0.05)。 结论 基于互动达标理论的多组分运动联合营养干预方案科学、可靠,应用该方案能有效改善老年糖尿病合并肌少症病人肌少症相关指标,提高病人生活质量,为临床护理实践提供有益参考。

运动形式抗阻运动;有氧运动
运动时间每周锻炼3次:每周一、周三、周五08:00-10:00或每周二、周四、周六08:00-10:00
运动强度1)从低强度适应性阻力训练开始,每个动作重复8~10次为1组,每次进行2组或3组训练动作,组间休息3~5 min,遵循循序渐进原则;对运动中和运动后的病人劳累程度进行评分[14],达到Borg自觉劳累程度(RPE)量表12~14分,可逐渐增加阻力,逐步从20%~30%增加到60%~80%
2)初始有氧运动从2~3个代谢当量(METs)开始,运动中应监测心率的变化,维持运动时的心率在中等强度(极限心率的50%~80%)
运动方案1)双上肢外展:双手持500 mL矿泉水瓶,双臂伸直外展,保持3 s后缓慢放。重复10次为1组,共2组,组间休息1~2 min
2)双上肢屈肘:双手持矿泉水瓶,固定双上臂,屈肘,将矿泉水瓶从胸腹部平举到肩关节水平位置,保持3 s后缓慢放。重复10次为1组,共2组,组间休息1~2 min
3)推墙“俯卧撑”:双手伸直,与肩同宽置于墙上,保持躯干竖直,缓慢弯曲手臂,让上身尽可能靠近墙壁,然后缓慢伸直手,回到起点位置。重复10次为1组,共2组,组间休息1~2 min
4)坐位大腿外展:双手身后支撑,挺直腰背,弹力带固定踝关节处,两腿伸直,左腿固定不动,右腿尽量外展至不能为止,保持3 s后缓慢收回。左右交替各3组,组间休息1~2 min
5)坐位抬腿:将弹力带固定于座椅腿,坐正、膝盖弯曲,单腿伸直保持3 s缓慢收回。左右交替各3组,组间休息1~2 min
6)靠墙静蹲:背靠墙,目视前方,双足分开,与肩同宽,足跟距离墙面的距离约是足部长度,身体逐渐下蹲,使小腿长轴与地面垂直,保持3 s缓慢收回。共3~5组,组间休息1~2 min
运动注意事项运动过程中需密切关注病人身体状况和肌肉状态[15],包括血压、心率、血氧饱和度以及疲劳情况等,一旦出现不适,应及时停下休息,监测血压、心率以及血氧饱和度等生命体征,根据不良反应的具体情况作出紧急处理,必要时前往医院就诊
营养处方营养筛查与评估1)使用微型营养评定简表(MNA⁃SF)和营养风险筛查2002(NRS 2002)筛查工具对肌少症老年人进行营养风险筛查[1617]
2)从肌少症严重程度、进食与活动、人体成分测量、老年综合评估等方面,对病人营养状况实施全面评估
蛋白质摄入1)对糖尿病合并肌少症病人,若其肾功能各项指标正常,蛋白质供给量为 1.2~1.5 g/(kg·d)[18],但不超过2.0 g/(kg·d),其中优质蛋白摄入量达到1/2及以上,并均衡分配到1 d的3~5餐中
2)注重补充亮氨酸,最低摄入量为55 mg/(kg·d)[19]
3)适度增加食物中氨基酸和肌酸摄入量[20],增加病人的肌肉量和肌肉质量
碳水化合物对糖尿病合并肌少症病人,糖类占总能量50%~55%[21],基于血糖指数和血糖符合确定糖类摄入质量和重量,且糖类来自蔬菜、全谷类、豆类和乳类等食物,避免富含添加脂肪、糖类或钠等
膳食纤维摄入可溶性及不溶性膳食纤维应控制在25~35 g/d[22]
补充抗氧化糖尿病合并肌少症病人可适当补充维生素C、维生素E、类胡萝卜和硒等抗氧化营养素(如深色蔬菜、水果以及豆类等)[23],减轻体内氧化应激和减少肌肉损伤
补充维生素D和钙1)补充15~20 μg/d(600~800 U/d)维生素D[24],维生素D2与维生素D3可以替换使用
2)每日钙摄入量应达到1 000~1 200 mg[25],建议糖尿病合并肌少症病人饮用300~500 mL/d的牛奶或摄入等量奶
口服营养补充1)当病人进食量不足日标量80%时,推荐口服营养补充(ONS);ONS制剂摄入量400~600 kcal/d,应在两餐间服用或50~100 mL/h饮用[26]
2)每日在餐间或锻炼后额外补充2次ONS制剂,每次摄入的ONS制剂需包含约836.8 kJ能量和15~20 g富含必需氨基酸或亮氨酸的蛋白质[26]
3)当ONS不能满足病人维生素D、ω⁃3脂肪酸等需求时,可额外增加相关营养素补充[27]

运动形式抗阻运动;有氧运动
运动时间每周锻炼3次:每周一、周三、周五08:00-10:00或每周二、周四、周六08:00-10:00
运动强度1)从低强度适应性阻力训练开始,每个动作重复8~10次为1组,每次进行2组或3组训练动作,组间休息3~5 min,遵循循序渐进原则;对运动中和运动后的病人劳累程度进行评分[14],达到Borg自觉劳累程度(RPE)量表12~14分,可逐渐增加阻力,逐步从20%~30%增加到60%~80%
2)初始有氧运动从2~3个代谢当量(METs)开始,运动中应监测心率的变化,维持运动时的心率在中等强度(极限心率的50%~80%)
运动方案1)双上肢外展:双手持500 mL矿泉水瓶,双臂伸直外展,保持3 s后缓慢放。重复10次为1组,共2组,组间休息1~2 min
2)双上肢屈肘:双手持矿泉水瓶,固定双上臂,屈肘,将矿泉水瓶从胸腹部平举到肩关节水平位置,保持3 s后缓慢放。重复10次为1组,共2组,组间休息1~2 min
3)推墙“俯卧撑”:双手伸直,与肩同宽置于墙上,保持躯干竖直,缓慢弯曲手臂,让上身尽可能靠近墙壁,然后缓慢伸直手,回到起点位置。重复10次为1组,共2组,组间休息1~2 min
4)坐位大腿外展:双手身后支撑,挺直腰背,弹力带固定踝关节处,两腿伸直,左腿固定不动,右腿尽量外展至不能为止,保持3 s后缓慢收回。左右交替各3组,组间休息1~2 min
5)坐位抬腿:将弹力带固定于座椅腿,坐正、膝盖弯曲,单腿伸直保持3 s缓慢收回。左右交替各3组,组间休息1~2 min
6)靠墙静蹲:背靠墙,目视前方,双足分开,与肩同宽,足跟距离墙面的距离约是足部长度,身体逐渐下蹲,使小腿长轴与地面垂直,保持3 s缓慢收回。共3~5组,组间休息1~2 min
运动注意事项运动过程中需密切关注病人身体状况和肌肉状态[15],包括血压、心率、血氧饱和度以及疲劳情况等,一旦出现不适,应及时停下休息,监测血压、心率以及血氧饱和度等生命体征,根据不良反应的具体情况作出紧急处理,必要时前往医院就诊
营养处方营养筛查与评估1)使用微型营养评定简表(MNA⁃SF)和营养风险筛查2002(NRS 2002)筛查工具对肌少症老年人进行营养风险筛查[1617]
2)从肌少症严重程度、进食与活动、人体成分测量、老年综合评估等方面,对病人营养状况实施全面评估
蛋白质摄入1)对糖尿病合并肌少症病人,若其肾功能各项指标正常,蛋白质供给量为 1.2~1.5 g/(kg·d)[18],但不超过2.0 g/(kg·d),其中优质蛋白摄入量达到1/2及以上,并均衡分配到1 d的3~5餐中
2)注重补充亮氨酸,最低摄入量为55 mg/(kg·d)[19]
3)适度增加食物中氨基酸和肌酸摄入量[20],增加病人的肌肉量和肌肉质量
碳水化合物对糖尿病合并肌少症病人,糖类占总能量50%~55%[21],基于血糖指数和血糖符合确定糖类摄入质量和重量,且糖类来自蔬菜、全谷类、豆类和乳类等食物,避免富含添加脂肪、糖类或钠等
膳食纤维摄入可溶性及不溶性膳食纤维应控制在25~35 g/d[22]
补充抗氧化糖尿病合并肌少症病人可适当补充维生素C、维生素E、类胡萝卜和硒等抗氧化营养素(如深色蔬菜、水果以及豆类等)[23],减轻体内氧化应激和减少肌肉损伤
补充维生素D和钙1)补充15~20 μg/d(600~800 U/d)维生素D[24],维生素D2与维生素D3可以替换使用
2)每日钙摄入量应达到1 000~1 200 mg[25],建议糖尿病合并肌少症病人饮用300~500 mL/d的牛奶或摄入等量奶
口服营养补充1)当病人进食量不足日标量80%时,推荐口服营养补充(ONS);ONS制剂摄入量400~600 kcal/d,应在两餐间服用或50~100 mL/h饮用[26]
2)每日在餐间或锻炼后额外补充2次ONS制剂,每次摄入的ONS制剂需包含约836.8 kJ能量和15~20 g富含必需氨基酸或亮氨酸的蛋白质[26]
3)当ONS不能满足病人维生素D、ω⁃3脂肪酸等需求时,可额外增加相关营养素补充[27]

目的 探讨基于互动达标理论的多组分运动联合营养干预在老年糖尿病合并肌少症病人中的应用效果。 方法 选取2023年8月—12月在山西省某三级甲等医院就诊的124例老年糖尿病合并肌少症病人为研究对象,采用随机数字表法将其分为试验组和对照组,各62例。对照组实施常规护理措施,试验组实施课题组构建的多组分运动联合营养干预。比较干预前及干预后3、6个月时两组病人小腿围、握力、躯体功能、肌肉质量和生活质量。 结果 干预后,试验组握力、小腿围、躯体功能、肌肉质量和生活质量均优于对照组(P<0.05)。 结论 基于互动达标理论的多组分运动联合营养干预方案科学、可靠,应用该方案能有效改善老年糖尿病合并肌少症病人肌少症相关指标,提高病人生活质量,为临床护理实践提供有益参考。

Objective To explore the application effect of multi-component exercise combined with nutritional intervention based on interactive achievement theory in elderly diabetes patients complicated with sarcopenia. Methods A total of 124 elderly diabetes patients complicated with sarcopenia treated in a tertiary grade A hospital in Shanxi province from August to December,2023 were selected as the study subjects.They were randomly divided into test group and control group by random number table method.62 cases each.The control group received routine nursing measures.The test group received the multi⁃component exercise combined with nutrition intervention constructed by the research group.Calf circumference,grip strength,physical function,muscle mass,and quality of life between two groups were compared before intervention and at 3 and 6 months after intervention. Results After intervention,the test group had better grip strength,calf circumference,physical function,muscle mass,and quality of life than the control group(P<0.05). Conclusions The multi-component exercise combined nutrition intervention based on interactive achievement theory was scientific and reliable.The application of this program could effectively improve the related indicators of sarcopenia in elderly diabetes patients complicated with sarcopenia,improve the quality of life of patients,and provide useful reference for clinical nursing practice.

肌少症是指与年龄增长相关的骨骼肌质量和肌肉力量或躯体功能下降的肌肉衰减综合征[1]。国际糖尿病联盟(IDF)统计数据显示,全球约有5.390亿例成年人(20~79岁)患有糖尿病,其中我国约有1.409亿人[2]。由于肌少症与糖尿病危险因素叠加[34],糖尿病病人肌少症患病率明显升高。研究显示,我国糖尿病病人肌少症患病率为14.8%[5]。国内一项关于北京地区糖尿病病人的调查研究显示,糖尿病合并肌少症的发生率为25.5%[6]。糖尿病合并肌少症互为因果,导致病人跌倒、失能、非计划性再入院甚至死亡等不良结局风险增加[78],已对病人健康预期寿命构成严重威胁。现有国内外研究集中于糖尿病合并肌少症现状调查及危险因素分析等,如何有效防控糖尿病合并肌少症亟待解决。互动达标理论由美国专家King[9]提出,倡导“以病人为中心”的服务理念,注重利益相关者之间的持续互动交流,共同实现康复目标。鉴于此,本研究基于互动达标理论的多组分运动联合营养干预应用于老年糖尿病合并肌少症病人并评价其效果,旨在为护理人员开展系统化、规范化糖尿病合并肌少症多组分干预提供参考,现报道如下。
选取2023年8月—12月在山西省某三级甲等医院就诊的124例老年糖尿病合并肌少症病人为研究对象。纳入标准:1)符合中国老年糖尿病诊疗指南(2021年版)[10],空腹血清葡萄糖≥7.0 mmol/L或75 g葡萄糖耐量试验(OCTT)后的2 h血清葡萄糖≥11.1 mmol/L。2)符合2019年亚洲肌少症工作组制订的肌少症诊断标准[11]。①四肢骨骼肌含量:双能X线扫描法(DXA)男性<7.0 kg/m2、女性<5.4 kg/m2,或生物电阻抗法(BIA)男性<7.0 kg/m2、女性<5.7 kg/m2;②65岁以上人群握力男性<28 kg、女性<18 kg;③6 m步行速度<1.0 m/s或5次坐起时间≥12 s或简易体能测量表(SPPB)≤9分。上述3项标准符合第1条及第2条或第3条中任意1条即可诊断为肌少症。3)意识清楚且沟通能力良好。4)知情同意且自愿参加本研究。排除标准:1)存在运动禁忌证;2)病情尚未稳定。剔除标准:1)病人突发病情加重无法继续参与研究;2)因各种原因导致的失访;3)病人血糖不稳定且波动幅度大。共纳入124例糖尿病合并肌少症病人,按照随机数字表法将其分为试验组和对照组,各62例。为避免组间沾染,设一病区为试验组,二病区为对照组。本研究已获山西白求恩医院伦理委员会审批(审批号:YXLL⁃2024⁃070)。
实施糖尿病病人住院常规护理措施。主要包括疾病相关知识、常用口服降糖药物及注射类降糖药物相关知识与不良反应、糖尿病专科饮食指导、运动指导、血糖监测等内容,出院前集中指导病人规律用药及定期复诊随访。出院后2、4、12周时电话随访,及时获取病人反馈。
在常规护理基础上实施课题组基于循证和专家函询构建的糖尿病合并肌少症病人多组分运动联合营养干预方案。
以内分泌科为单位,建立老年医学专家主导的多学科团队实施干预管理,包括老年医学专家3人、内分泌代谢专家2人、运动康复师4人、营养管理师3人、药剂学专家1人、专科护士4人。干预前对专科管理小组成员实施规范化培训,以提高团队成员肌少症管理执行效能,考核合格后方纳入管理小组。
首次干预前,由糖尿病专科护士严格按照互动达标理论对病人实施综合评估。1)个人系统:包括病人疾病相关知识知晓情况、运动习惯、饮食偏好及生活行为习惯等;2)人际间系统:包括照护者对病人疾病认知及态度、沟通情况等;3)社会系统:包括病人社会支持体系中来自家庭、社会等多方支持程度等。对纳入病人开展面对面访谈,掌握现存指南要求与临床实践差距,全面分析糖尿病合并肌少症病人多组分运动及营养干预的促进因素与障碍因素,为健康管理目标制订及执行提供参考依据。
针对病人目前现存主要问题,结合病人实际病情与个性化需求由利益相关者共同制订科学、可行的老年糖尿病合并肌少症病人标准化管理清单,详见表1
评价两组肌肉质量、躯体功能、握力、小腿围。1)肌肉质量:采用简易五项评分问卷(SARC⁃F)评价两组躯体功能。SARC⁃F由Malmstrom等[28]于2013年研制,包含肌肉力量、辅助行走、座椅起立、攀爬楼梯、跌倒病史5个条目,各条目评分为0~2分,“没有困难”计0分,“有时有困难”计1分,“有困难或不能做到”计2分。得分越高,肌肉质量越差,得分≥4分即达到肌少症初步诊断标准,该量表Cronbach's α系数为0.803,内部一致性良好[28]。2)躯体功能:采用简易体能状况量表(Short Physical Performance Bettery,SPPB)[29]评价两组躯体功能。该量表是由美国国家老年病研究所研发,包括站立平衡测试、4 m步速测试和5次座椅站立测试3项内容。按照测试耗时作为计分原则,各条目得分为0~4分,满分为12分,得分越高提示功能状态越好。
采用糖尿病病人特异性生活质量量表(the Diabetes Specific Quality of Life Scale,DSQL)评价两组生活质量,该量表由冯正仪等编制,包括心理/精神(8个条目)、生理功能(12个条目)、治疗影响(3个条目)、社会关系(4个条目)4个维度,共27个条目。各条目采用Likert 5级评分法,计0~5分,总分0~135分,得分越低提示病人生活质量越高[30]。该量表Cronbach's α系数为0.945,内部一致性良好[31]
干预实施前,资料收集成员统一接受培训,使用统一指导语和问卷评估方法,向病人及家属介绍本研究的目的、内容及注意事项。对无法完成问卷填写者,由研究者询问后代为填写并再次由病人确认是否填写无误。由2名研究组成员独立进行资料收集,研究组自行编制一般资料调查表,由医院电子病例系统录入研究基线资料。干预3、6个月后,在内分泌科/老年专科门诊病人复诊时或电话随访时进行数据收集。
采用SPSS 23.0软件进行统计分析。服从或近似服从正态分布的定量资料采用均数±标准差($\bar{x}\pm s$)描述,采用独立样本t检验;不符合正态分布的定量资料采用中位数(四分位数)[MP25,P75)]表示,采用非参数秩和检验;重复测量资料采用重复测量方差分析。定性资料采用例数描述,采用χ2检验进行比较。以P<0.05为差异有统计学意义。
研究过程中,对照组脱落1例(1例合并严重感染),试验组脱落2例(1例中途转院,1例死亡),最终对照组61例病人、试验组60例病人完成本研究。两组性别、年龄、受教育程度等一般资料比较,差异无统计学意义(P>0.05),具有可比性,详见表2

运动、营养和药物是预防、延缓和治疗肌少症的关键。然而,目前仍未有针对肌少症有效的治疗药物,非药物干预(科学运动干预和营养管理)已成为糖尿病病人肌少症防控的有效方式[3233]。本研究前期基于互动达标理论和专家函询等整理现有证据内容,严格遵循个体化运动和营养管理原则,充分考虑医院、社区、家庭不同环境转换中的联动管理,遴选省内外老年及运动康复、营养管理领域专家实施方案论证与修订,最终构建了科学、可行的老年糖尿病合并肌少症病人多组分运动联合营养干预方案。研究期间参与病人遵医行为较高,研究过程中均未发生相关不良事件,具有较好的临床实用性,可作为临床糖尿病合并肌少症病人管理的科学工具。
研究证明,低运动量是糖尿病合并肌少症发生的独立危险因素,低运动量容易导致肌肉力量下降,最终导致肌肉量和活动能力下降[3435]。本研究结果表明,以抗阻运动及有氧运动的多组分运动模式联合营养干预可有效改善病人肌肉质量,与Bagheri等[36]研究结果一致。原因可能是:1)本研究根据病人病情和运动偏好情况制订个性化抗阻运动方案,包含6项简易抗阻训练动作,可有效增加肌肉耗氧量,使肌肉毛细血管增加,增加肌肉蛋白质合成、优化肌肉质量及功能[37];2)有氧运动可以增加一磷酸腺苷(AMP)依赖的蛋白酶活性,刺激健康线粒体合成以改善肌细胞代谢能力;3)基于互动达标理论充分评估后设定个性化目标,注重病人积极参与自我健康管理过程,根据病人定期门诊随访动态评估并及时调整干预方案,保障了干预质量和病人较好的依从性。临床医护人员应加强对老年糖尿病病人肌少症前期筛查,充分做到预防关口前移,通过实施多组分干预提高病人疾病认知和自我管理能力,改善躯体功能。
本研究结果显示,多组分运动联合营养干预可提高老年糖尿病合并肌少症病人生存质量,与王磊等[38]研究结果一致。肌少症的发生是导致糖尿病病人发生跌倒、失能及影响病人日常生活活动的重要危险因素。研究组构建的多组分干预方案贯穿肌少症病人全周期健康管理,针对病人个体化病情和日常偏好行为综合制定科学的管理策略,有利于避免病人因肌肉质量和躯体功能下降产生的跌倒风险,有效提高病人生活质量。护理人员根据方案要求,通过开展渐进性抗阻运动并结合有氧运动,科学营养搭配,通过日常生活锻炼与良好行为习惯养成不断提高病人肌肉质量和躯体功能,最终提高糖尿病合并肌少症病人生活质量。
本研究开展的多组分运动联合营养干预在糖尿病合并肌少症病人中的应用方案具有科学性和可行性。但由于现有研究人力物力资源、研究时间等限制,研究样本量较小、研究周期短,今后亟待进一步开展多中心、大样本、长期追踪随访的高质量随机对照试验,以不断验证和完善该方案。
