Objective To review the application and methods of ecological momentary assessment(EMA) on symptom managemet of with breast cancer patients,so as to provide reference for future researchers to use EMA in patients with breast cancer. Methods According to the scope review methodology,CINAHL,Cochrane Library,EMbase,PsycINFO,PubMed,Web of Science,WanFang Database,VIP,CNKI and CBM were searched.The retrieval period spanned from the establishment of the database to July 30,2024.The retrieved documents were screened and summarized. Results A total of 20 papers were included.The research objective focused on the relationship between breast cancer symptoms and physical activity,the relationship between breast cancer patients and their spouses,the relationship between breast cancer symptoms and insomnia,cognitive function and fatigue of breast cancer,and EMA feasibility.The outcome indicators included six categories:emotional symptoms,physical symptoms,psychosocial factors,behavioral factors,cognitive function and feasibility.EMA data collection mainly relied on smartphone and accelerometers,mostly through random or fixed signal prompts,1 to 6 times a day,with an interval of 0.5 to 11.0 hours,lasting for 5 to 28 days.The participation rate were 29.1%⁃100.0%.The response rate was 57.0%⁃100.0%.The churn rate was 0⁃44.9%.The reward was 15⁃130 dollars. Conclusions EMA is feasible in the management of breast cancer symptoms,and can accurately capture changes in symptoms of breast cancer patients at different points in time.In the future,more standardized EMA reporting guidelines can be further developed to explore the potential of EMA in breast cancer clinical care interventions and other areas to provide a scientific basis for the development of personalized treatment.

目的 对生态瞬时评估(EMA)在乳腺癌病人症状管理中的应用和方法进行范围综述,以期为医护人员开展相关研究提供参考。 方法 按照范围综述方法学,检索数据库CINAHL、Cochrane Library、EMbase、PsycINFO、PubMed、Web of Science、万方数据库、维普中文科技期刊数据库、中国知网和中国生物医学文献数据库,检索时限为建库至2024年7月30日,对检索文献进行筛选及汇总。 结果 共纳入20篇文献;研究目的聚焦乳腺癌症状与身体活动的关系、乳腺癌病人与配偶的关系、乳腺癌症状与失眠的关系、乳腺癌认知功能与疲劳、生态瞬时评估可行性分析5个方向;结局指标包括情绪症状、躯体症状、社会心理因素、行为因素、认知功能、可行性;数据收集以智能手机和加速度计为主,多通过随机信号或固定信号提示,每日1~6次,间隔0.5~11.0 h,持续5~28 d;参与率为29.1%~100.0%,应答率为57.0%~100.0%,流失率为0~44.9%,奖励为15~130美元。 结论 生态瞬时评估在乳腺癌症状管理中具有可行性,能够精确捕捉乳腺癌病人在不同时间点上的症状变化。未来可进一步开发更加标准化的生态瞬时评估报告指南,发掘生态瞬时评估在乳腺癌临床护理干预和其他领域的潜力,为制定个性化治疗方案提供科学依据。

目的 对生态瞬时评估(EMA)在乳腺癌病人症状管理中的应用和方法进行范围综述,以期为医护人员开展相关研究提供参考。 方法 按照范围综述方法学,检索数据库CINAHL、Cochrane Library、EMbase、PsycINFO、PubMed、Web of Science、万方数据库、维普中文科技期刊数据库、中国知网和中国生物医学文献数据库,检索时限为建库至2024年7月30日,对检索文献进行筛选及汇总。 结果 共纳入20篇文献;研究目的聚焦乳腺癌症状与身体活动的关系、乳腺癌病人与配偶的关系、乳腺癌症状与失眠的关系、乳腺癌认知功能与疲劳、生态瞬时评估可行性分析5个方向;结局指标包括情绪症状、躯体症状、社会心理因素、行为因素、认知功能、可行性;数据收集以智能手机和加速度计为主,多通过随机信号或固定信号提示,每日1~6次,间隔0.5~11.0 h,持续5~28 d;参与率为29.1%~100.0%,应答率为57.0%~100.0%,流失率为0~44.9%,奖励为15~130美元。 结论 生态瞬时评估在乳腺癌症状管理中具有可行性,能够精确捕捉乳腺癌病人在不同时间点上的症状变化。未来可进一步开发更加标准化的生态瞬时评估报告指南,发掘生态瞬时评估在乳腺癌临床护理干预和其他领域的潜力,为制定个性化治疗方案提供科学依据。

Objective To review the application and methods of ecological momentary assessment(EMA) on symptom managemet of with breast cancer patients,so as to provide reference for future researchers to use EMA in patients with breast cancer. Methods According to the scope review methodology,CINAHL,Cochrane Library,EMbase,PsycINFO,PubMed,Web of Science,WanFang Database,VIP,CNKI and CBM were searched.The retrieval period spanned from the establishment of the database to July 30,2024.The retrieved documents were screened and summarized. Results A total of 20 papers were included.The research objective focused on the relationship between breast cancer symptoms and physical activity,the relationship between breast cancer patients and their spouses,the relationship between breast cancer symptoms and insomnia,cognitive function and fatigue of breast cancer,and EMA feasibility.The outcome indicators included six categories:emotional symptoms,physical symptoms,psychosocial factors,behavioral factors,cognitive function and feasibility.EMA data collection mainly relied on smartphone and accelerometers,mostly through random or fixed signal prompts,1 to 6 times a day,with an interval of 0.5 to 11.0 hours,lasting for 5 to 28 days.The participation rate were 29.1%⁃100.0%.The response rate was 57.0%⁃100.0%.The churn rate was 0⁃44.9%.The reward was 15⁃130 dollars. Conclusions EMA is feasible in the management of breast cancer symptoms,and can accurately capture changes in symptoms of breast cancer patients at different points in time.In the future,more standardized EMA reporting guidelines can be further developed to explore the potential of EMA in breast cancer clinical care interventions and other areas to provide a scientific basis for the development of personalized treatment.

乳腺癌是女性人群中发病率最高的癌症[1]。早期乳腺癌病人经规范化治疗后5年生存率可达90%以上[2]。尽管生存率显著提高,但乳腺癌病人在疾病治疗过程中,通常会伴随脱发、食欲减退等治疗相关副作用,不仅导致病人自我形象受损,还会引发睡眠障碍、疲乏、焦虑、抑郁等一系列生理和心理症状,显著降低病人的生活质量[3]。由此可见,对乳腺癌相关症状及时识别及尽早干预至关重要。然而,既往研究对乳腺癌病人症状的评估主要是基于传统的问卷调查方法,病人通过回顾过去某一时间段内的症状填写问卷[4],此方法可能因病人存在记忆偏差,降低结果的准确性。此外,乳腺癌病人的症状变化迅速,传统的症状评估方法无法实时反映症状的动态变化,以至于医护人员难以及时制定或调整干预策略,严重影响治疗效果。生态瞬时评估(ecological momentary assessment,EMA)法通过反复收集病人在自然环境中行为和体验的实时数据,对乳腺癌病人症状进行动态评估,可有效提高评估结果的准确性和生态学效度[5]。目前,国内外关于生态瞬时评估在乳腺癌病人群体中的应用模式和研究方法存在较大异质性,其结局指标类型及应用效果尚不清晰。因此,本研究依据Joanna Briggs Institute(JBI)在2019年12月发布的范围综述指南作为方法学的指导原则[6],对生态瞬时评估在乳腺癌研究领域的应用进行全方位总结与分析,以期为医护人员开展相关研究提供参考。

根据PCC原则制定纳入标准。1)研究对象(population):乳腺癌病人,年龄≥18岁;2)概念(concept):研究内容涉及生态瞬时评估方法,每天进行1次或多次评估,持续时间超过24 h;3)情境(context):在自然情景中进行生态瞬时评估。排除标准:1)研究内容仅有设计方案未提及研究结果;2)无法获取全文的会议文件。
使用EndNote X9软件对检索到的文献进行去重处理后,再由2名研究人员依据纳入与排除标准独立开展初步筛选,全文阅读后进行二次筛选。在筛选过程中,如遇分歧,由第3名研究人员介入协商,确定最终纳入研究的文献。
在Excel表格中进行数据内容提取。1)研究设计特征:研究目的、样本特征(样本量、乳腺癌分期、乳腺癌治疗方式)、作者姓名、发表年份、国家、结局指标;2)生态瞬时评估数据收集方法:应用设备、操作方式、时间表特征(监测周期、持续时间、数据采样方法、每天提示频率及间隔时间);3)生态瞬时评估应答相关结果:参与率、应答率、流失率、奖励措施;4)主要研究结果。
初步检索获得4 012篇相关文献,根据文献的纳入与排除标准进行筛选,最终纳入20篇文献[726]。其中英文19篇,中文1篇,文献筛选的流程详见图1
纳入文献的研究设计特征、生态瞬时评估数据收集方法及应答相关结果见表1
纳入的20项研究的研究目的上具有差异,主要围绕5个方向开展研究。
1)乳腺癌症状与身体活动的关系[8,14,2325]:症状评分与中高强度体力活动(MVPA)和低强度体力活动(LPA)[8,14,25]、久坐行为纵向趋势[2324];2)乳腺癌病人与配偶关系[10,15,19,21,26]:回避与约束[10]、亲密关系与支持互动[15]、支持缓冲与复发恐惧[19]、疼痛影响[21,26];3)乳腺癌症状与失眠的关系[12,18]:症状对睡眠障碍的影响[12]、睡眠⁃症状⁃情绪关联[18];4)乳腺癌认知功能与疲劳[11,1617,22]:疲劳与认知的关联[7,11]、认知功能影响因素[16]、疲劳的病因机制[17]、个性化认知行为疗法对疲劳的干预[22];5)生态瞬时评估可行性分析[13,16,20,22]:移动健康干预[13]、生态瞬时评估数据收集[20]、自动化个体时间序列[22]、认知、环境与睡眠移动评估(MACES)[16]
20篇文献分别在美国(n=17)[78,1020,2326]、荷兰(n=1)[22]、加拿大(n=1)[21]、中国(n=1)[9]开展研究。纳入文献样本量为3~74例;病人平均年龄为52岁。研究对象乳腺癌分期为0~Ⅳ期,乳腺癌治疗方式包括化疗、放疗、内分泌治疗。
研究设计的结局指标主要包括情绪症状、躯体症状、社会心理因素、行为因素、认知功能、可行性6个方面。14篇文献[712,14,1619,2324,26]涉及情绪症状,包括焦虑、抑郁、快乐等;14篇[79,1112,14,1618,2124,26]涉及躯体症状,包括疼痛、疲劳、恶心和呕吐等;5篇文献[10,15,19,2526]涉及社会心理因素,包括乳腺癌病人回避、病人满意度及亲密度等;11篇文献[89,1214,1618,2325]涉及行为因素,包括睡眠行为、久坐行为、体力活动、饮食行为;4篇文献[7,11,16,23]涉及认知功能;4篇文献[13,16,20,22]涉及可行性评估。
纳入的20篇文献[726]中,数据收集设备和操作方式存在较大差异。收集设备具体包括:1)智能手机[79,11,1314,16,20,2225];2)活动记录仪:加速度计[8,1314,16,20,2325]、手表计步器[1617];3)纸质日记[12,17];4)其他设备:个人数字助理(personal digital assistant,PDA)[10,18,21,26]、电话[12]、基于浏览器的调查,如邮件[15]。操作方式包括:1)Likert量表[810,12,1415,1726];2)屏幕滑动[78,11,14,16,20,23,25],即在视觉模拟刻度滑条上进行滑动或基于数字符号进行相关认知测试;3)既定选项[78,12,15,1718,2122,24,26],即提供固定选项供病人填写;4)自定义输入[12],如记录用药情况。
1)监测周期:12篇文献[7913,15,1719,21,26]为1个周期,5篇文献[8,14,20,23,25]为3个周期,其余为2个周期[22]和5个周期[16,24];2)持续时间:5~28 d;3)数据采样方法:包括事件抽样[9,13](运动、饮食评估和睡眠记录)、随机信号[8,10,13141618,2021,2326]、固定信号[7,9,1112,15,19,22];4)每天提示频率:提示频次为每天1~6次,其中4篇文献[10,18,21,26]在固定时间内随机提示病人,仅1篇文献[15]每日进行1次日记记录;5)提示间隔特征:0.5~11.0 h,仅2篇文献[9,17]报告间隔时间不固定。
20篇文献[726]中报告的参与率为29.1%~100.0%。生态瞬时评估应答率范围为57.0%~100.0%,其中5篇文献[8,14,20,2325]同时报告了生态瞬时评估和加速度计的应答率,分别为86.0%和82.3%,2项研究[15,22]未报告生态瞬时评估应答率。此外,流失概率为0~44.9%。同时大多数研究[8,10,1217,2021,2326]为提高应答率给予研究对象奖励,为15~130美元。
研究表明,乳腺癌病人当日身体活动与症状严重程度呈现双向动态关联,即症状缓解期病人身体活动量显著增加,而身体活动水平提升反向降低当日症状强度[8,14]。Welch等[23]纵向研究证实,在化疗期间,病人症状评分越高,久坐时间越长。Pinto等[24]采用生态瞬时评估进行为期12个月的追踪研究,通过5个时间节点(基线、3、6、9、12个月)的7 d连续监测发现,负面情绪与疲劳程度越高,久坐行为发生率越高。Auster⁃Gussman等[25]基于社会认知理论的研究表明,自我效能、目标设定、环境支持等认知行为因素,可显著预测当日中高强度体力活动和低强度体力活动的持续时间[25]
4篇文献[10,15,19,26]同时使用生态瞬时评估调查乳腺癌病人及其配偶,均对乳腺癌夫妇进行电子日记评估。在转移性乳腺癌病人中,病人回避行为与伴侣的社会约束存在时间上的联系,伴侣的社会约束对病人第2天情绪状态产生消极影响[10]。研究表明,疼痛显著影响夫妻关系质量,伴侣通过提供相应社会支持,能够有效缓解病人的痛苦和消极情绪[26]。Belcher等[15]研究发现,病人接受支持和伴侣提供支持之间存在互惠效应,其与亲密关系的积极体验显著相关。乳腺癌病人和配偶的保护性缓冲(如隐藏负面情绪)与亲密关系的减少会提高癌症复发恐惧的风险[19]
化疗期间,乳腺癌病人睡眠质量与躯体症状呈动态关联,表现为治疗初期至中期睡眠质量下降,后期逐步缓解,且症状严重程度与睡眠障碍的关联强度随治疗阶段变化[9]。Rumble等[12]通过生态瞬时评估验证了失眠的因果路径,乳腺癌病人日间和夜间的功能失调性睡眠认知(如过度担忧失眠后果)和睡眠抑制行为(如日间过度补觉)可预测失眠症状发生,疼痛、疲劳、潮热等躯体症状与积极情绪缺失形成负向循环,进一步加剧失眠。Ratcliff等[18]研究表明,化疗前睡眠障碍可加剧疲劳及负性情绪,而化疗前维持良好睡眠是缓解治疗相关焦虑情绪的保护性因素。
乳腺癌病人疲劳程度越高,认知功能越差[11]。认知功能下降可进一步导致当日体力活动减少,形成疲劳⁃认知⁃行为负向循环[14]。乳腺癌病人治疗后疲劳具有长期持续性特征[17]。Harnas等[22]应用自动化个体时间序列分析,制定个性化认知行为疗法,以缓解癌症相关疲劳。内分泌治疗可加速认知功能衰退,生态瞬时评估能够识别主观与客观认知衰退症状[16]。Veal等[7]应用生态瞬时评估发现,主观认知与自我报告疲劳、抑郁情绪相关,为早期筛查癌症治疗相关认知障碍提供依据。
Allicock等[13]研究基于移动健康干预对乳腺癌病人进行为期4周的体育活动和饮食干预,结果表明,干预组久坐时间以及饮食摄入明显少于对照组,而体力活动时间显著延长。乳腺癌化疗病人使用智能手机和加速度计收集生态瞬时评估数据具有可行性[20]。Harnas等[22]通过案例分析证实,病人能够高效完成生态瞬时评估数据报告,且基于生态瞬时评估数据的个性化认知行为疗法在缓解癌症相关疲劳症状中具有临床可行性。Derbes等[16]对接受内分泌治疗的乳腺癌病人利用技术设备(智能手机和多种活动记录仪)进行认知、环境与睡眠移动评估,结果表明,这项纵向移动评估可为未来的研究设计提供参考。
乳腺癌病人的症状负担具有动态性、多维交互性、异质性[27]。Phillips等[14]研究应用生态瞬时评估发现,乳腺癌病人疲劳、疼痛等躯体症状与体力活动间具有时序特征。研究表明,伴侣支持、亲密关系等社会心理触发因素对改善乳腺癌病人负性情绪症状具有积极效应[10,15]。因此,生态瞬时评估通过高频次的评估,同时基于个体化症状轨迹制定动态护理方案,能够实现早期预警与个性化决策。此外,乳腺癌病人的治疗依从性和健康行为是影响预后的关键因素[28]。研究表明,乳腺癌病人在长期治疗期间,因治疗周期长或药物副作用等多种因素,病人治疗依从性普遍较低,显著提高了复发风险[29]。生态瞬时评估通过实时提醒与行为反馈,有助于提升治疗依从性,优化健康行为指导策略[12]。移动健康技术的发展为生态瞬时评估方法智能化、便捷化提供硬件支持[30]。因此,生态瞬时评估以实时、高密度的数据采集为特点,在乳腺癌病人症状管理中发挥显著优势。
生态瞬时评估在临床中应用仍面临诸多挑战。对于老年乳腺癌病人,由于年龄较大,生态瞬时评估的高频次提示增加病人心理负担;且操作设备复杂,增加病人操作负担;生态瞬时评估数据分析复杂增加研究者难度,这些问题的存在限制了生态瞬时评估的广泛推广和应用。
目前,国内乳腺癌领域的生态瞬时评估研究尚处于起步阶段,仅尚子妹等[9]研究基于微信平台对乳腺癌化疗期间症状进行监测,针对症状动态机制的高质量实证研究较少。因此,未来需构建本土化生态瞬时评估平台,优化生态瞬时评估评估方法,从多方面探讨症状的交互机制,利用机器学习分析时序数据,提高生态瞬时评估的临床应用效果,推动精准护理实践。
本研究结果显示,现有生态瞬时评估研究在数据采集设备、采样方法及评估频率等方面存在显著差异,研究设计需结合研究目的及乳腺癌人群特征进行个性化调整。Perndorfer等[19]研究表明,乳腺癌夫妇因担心投入时间有限以及存在主观上不愿参与心理,导致参与率较低。同时,Derbes等[16]研究因监测周期长,病人退出率较高。Geeraerts等[31]采用定性研究对晚期乳腺癌病人进行访谈,在观察性研究验证前进行试点研究,探索生态瞬时评估方案的实际应用效果。因此,正式研究前,相关人员应首先进行相关方法培训以增强受试者的动机和信心,从而提高参与率;其次,采取适当奖励机制减少流失率;最后,在正式研究前通过预试验进一步优化生态瞬时评估方案,进而提升受试者的应答率。此外,为确保受试者应答结果的准确性,研究者应制定相应措施监测应答质量。然而,本研究纳入的文献在报告方式上缺乏标准化,无法对研究之间的应答相关结果进行比较。Liao等[32]设计的生态瞬时评估研究报告准则可作为报告标准的起点,通过提供完整问卷信息和报告的使用情况,准确地探索研究设计特征对应答相关结果的影响。由于先前生态瞬时评估测量问卷是基于回顾性标准化的患者自我报告测量问卷进行设计,其在生态瞬时评估研究中的有效性尚未得到充分验证。因此,未来学者在生态瞬时评估研究设计和报告研究结果上应遵循最佳实践指南,并通过专家评审或德尔菲法以及质量控制促进生态瞬时评估实施标准化,提高研究的可重复性及研究结果的准确性。
生态瞬时干预是生态瞬时评估的扩展,即在病人1 d中及时提供行为指导或放松刺激等其他干预措施[5]。此外,通过长期观察病人进行算法预测,在不良症状或行为发生之前进行提前干预以应对疾病挑战[5]。生态瞬时干预是实时、实地的干预,且干预形式具有多样性,既可以作为单独生态瞬时干预,又可以将生态瞬时评估与传统干预结合。本研究结果发现,生态瞬时干预已应用于乳腺癌病人中。Harans等[22]用生态瞬时干预评估结果预测乳腺癌病人疲劳因素,进而制订个性化治疗方案,结果发现病人疲劳评分显著降低[22]。Allicock等[13]采用基于生态瞬时评估的移动健康干预为病人定制健康信息,显著改善了乳腺癌病人的身体活动和饮食行为。Merckaert等[33]基于应用程序提示对乳腺癌情绪进行动态调节,研究结果显示,当病人面临触发负面情绪的因素时,通过主动的情绪调节策略,可以产生更多的积极情绪[33]。由此可见,生态瞬时干预在国外乳腺癌治疗领域的应用已取得显著进展。因此,国内学者可基于生态瞬时评估的实时监测结果,为乳腺癌病人制定个性化的动态干预方案以及精准的健康管理服务,提高乳腺癌病人治疗依从性和生存率。
本研究通过对生态瞬时评估在乳腺癌病人中的应用进行范围综述,证实生态瞬时评估在乳腺癌症状管理中具有可行性,但当前生态瞬时评估研究在设计中存在差异,研究结果之间难以进行同质性比较。本研究存在一定的局限性。首先,由于生态瞬时评估术语广泛且仅纳入中英文文献,可能遗漏其他语言发表的高质量文献;其次,本研究未进行方法学质量评估,影响结果的可信度。随着移动医疗技术和智能设备的发展,国内研究者应进一步开发适合我国国情的生态瞬时评估数据采集平台以及标准化的生态瞬时评估报告指南,探索基于生态瞬时评估的生态瞬时干预,以实现病人症状的即时监控和干预,促进精准护理的发展,进一步实现以病人为中心的护理模式。
