Objective

To investigate the correlation between handgrip strength and quality of life in cancer patients,and to analyze gender differences.

Methods

A total of 291 cancer patients hospitalized in the oncology department of a hospital in Guangxi Zhuang Autonomous Region from October 2023 to January 2024 were selected.A survey was conducted using a general information questionnaire and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30(EORTC QLQ-C30).Patients' handgrip strength was measured,and nutritional risk was assessed using the Nutritional Risk Screening 2002 (NRS2002).Spearman correlation analysis was used to analyze the correlation between handgrip strength and quality of life in cancer patients of different genders,and linear regression analysis was further used to explore the impact of handgrip strength on patients' quality of life.

Results

In male cancer patients,handgrip strength was positively correlated with functional domain scores and global health status scores(P<0.05),and negatively correlated with fatigue and pain scores in the symptom domain(P<0.05).In female cancer patients, handgrip strength was negatively correlated with the single-item scores for appetite loss and diarrhea(P<0.05),and positively correlated with financial difficulties scores(P<0.05).The multivariate regression model after adjusting for confounding factors showed that in male patients, greater handgrip strength was associated with higher scores in functional domains and global health status(P<0.05),and lower fatigue scores in the symptom domain(P<0.05).In female patients,greater handgrip strength was associated with lower scores for appetite loss and higher scores for financial difficulties(P<0.05).

Conclusions

Handgrip strength has an impact on function,fatigue,and global health status in male cancer patients,and on appetite loss and financial difficulties in female cancer patients.As a simple and objective measurement indicator,handgrip strength can be used for the auxiliary assessment of quality of life in cancer patients.

目的

探讨肿瘤病人握力与其生活质量的相关性，并分析性别差异。

方法

选取2023年10月—2024年1月在广西壮族自治区某医院肿瘤科住院的肿瘤病人291例，采用一般资料调查表、癌症病人生活质量评价量表（EORTC QLQ-C30）进行问卷调查，同时测量病人的握力，采用营养风险筛查2002（NRS2002）进行营养风险评分，采用Spearman相关分析对不同性别肿瘤病人的握力与生活质量进行相关性分析，线性回归分析进一步探讨握力对病人生活质量的影响。

结果

男性肿瘤病人握力与生活质量功能领域评分及总体健康评分呈正相关（P<0.05），与症状领域中的疲倦、疼痛评分呈负相关（P<0.05）；女性肿瘤病人握力与单项条目的食欲丧失、腹泻评分呈负相关（P<0.05），与经济困难评分呈正相关（P<0.05）；调整混杂因素后的多因素回归模型显示，男性病人握力越大，生活质量中功能领域及总体健康状况得分越高（P<0.05），症状领域中疲倦得分越低（P<0.05）；女性病人握力越大，单项条目中食欲丧失得分越低、经济困难得分越高（P<0.05）。

结论

握力对男性肿瘤病人功能、疲倦、总体健康状况以及女性肿瘤病人食欲丧失和经济困难有影响。握力作为一种简便、客观的测量指标，可用于肿瘤病人生活质量的辅助评估。

目的

探讨肿瘤病人握力与其生活质量的相关性，并分析性别差异。

方法

选取2023年10月—2024年1月在广西壮族自治区某医院肿瘤科住院的肿瘤病人291例，采用一般资料调查表、癌症病人生活质量评价量表（EORTC QLQ-C30）进行问卷调查，同时测量病人的握力，采用营养风险筛查2002（NRS2002）进行营养风险评分，采用Spearman相关分析对不同性别肿瘤病人的握力与生活质量进行相关性分析，线性回归分析进一步探讨握力对病人生活质量的影响。

结果

男性肿瘤病人握力与生活质量功能领域评分及总体健康评分呈正相关（P<0.05），与症状领域中的疲倦、疼痛评分呈负相关（P<0.05）；女性肿瘤病人握力与单项条目的食欲丧失、腹泻评分呈负相关（P<0.05），与经济困难评分呈正相关（P<0.05）；调整混杂因素后的多因素回归模型显示，男性病人握力越大，生活质量中功能领域及总体健康状况得分越高（P<0.05），症状领域中疲倦得分越低（P<0.05）；女性病人握力越大，单项条目中食欲丧失得分越低、经济困难得分越高（P<0.05）。

结论

握力对男性肿瘤病人功能、疲倦、总体健康状况以及女性肿瘤病人食欲丧失和经济困难有影响。握力作为一种简便、客观的测量指标，可用于肿瘤病人生活质量的辅助评估。

Objective

To investigate the correlation between handgrip strength and quality of life in cancer patients,and to analyze gender differences.

Methods

A total of 291 cancer patients hospitalized in the oncology department of a hospital in Guangxi Zhuang Autonomous Region from October 2023 to January 2024 were selected.A survey was conducted using a general information questionnaire and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30(EORTC QLQ-C30).Patients' handgrip strength was measured,and nutritional risk was assessed using the Nutritional Risk Screening 2002 (NRS2002).Spearman correlation analysis was used to analyze the correlation between handgrip strength and quality of life in cancer patients of different genders,and linear regression analysis was further used to explore the impact of handgrip strength on patients' quality of life.

Results

In male cancer patients,handgrip strength was positively correlated with functional domain scores and global health status scores(P<0.05),and negatively correlated with fatigue and pain scores in the symptom domain(P<0.05).In female cancer patients, handgrip strength was negatively correlated with the single-item scores for appetite loss and diarrhea(P<0.05),and positively correlated with financial difficulties scores(P<0.05).The multivariate regression model after adjusting for confounding factors showed that in male patients, greater handgrip strength was associated with higher scores in functional domains and global health status(P<0.05),and lower fatigue scores in the symptom domain(P<0.05).In female patients,greater handgrip strength was associated with lower scores for appetite loss and higher scores for financial difficulties(P<0.05).

Conclusions

Handgrip strength has an impact on function,fatigue,and global health status in male cancer patients,and on appetite loss and financial difficulties in female cancer patients.As a simple and objective measurement indicator,handgrip strength can be used for the auxiliary assessment of quality of life in cancer patients.

肿瘤是一种严重的致命性疾病，目前已经成为严重威胁人类生命健康的重大公共卫生问题[1]。虽然随着诊断和治疗技术的进步，世界范围内肿瘤病人的生存率在不断提高[2]，但大多数肿瘤尚不能完全治愈，生活质量成为衡量肿瘤病人生存质量的一个重要指标[3-4]。生活质量是一个多维概念，涵盖了个体与健康相关的社会、功能、情感和心理方面[3]。由于肿瘤本身以及化疗产生的副作用，肿瘤病人生活质量普遍较差[5-6]。因此，如何提高肿瘤病人生活质量已经引起了社会各界的广泛关注[7]。握力是衡量手部肌肉力量的有效指标，已经被证明可以有效反映全身肌肉力量[8]。有研究显示，握力不仅与病人的预后及营养状况密切相关[9-10]，在多项老年人的研究中还发现握力与生活质量也存在相关性[11-13]，但大多局限于国外的研究。关于握力与生活质量之间的关系，我国研究鲜有报道。因此，本研究旨在探讨肿瘤病人握力与生活质量之间的关系，为临床进一步提高肿瘤病人的生活质量提供参考依据。
1　对象与方法

1.1　对象
采用便利抽样法，选取2023年10月—2024年1月在广西壮族自治区某医院肿瘤科住院的肿瘤病人为研究对象。纳入标准：经病理学诊断确诊为肿瘤病人；年龄≥18岁；意识清楚，能接受动作性指令，能够进行言语交流；知情同意，并自愿参与本研究。排除标准：有精神或认知障碍性疾病者；长期肢体疼痛、肿胀、关节强直或严重关节炎，影响测试者；因神经功能损害而影响上肢力量者；正在参加其他研究的病人；患有偏瘫、神经肌肉疾病、骨关节病的病人。本研究经我院医学伦理委员会审查批准后实施（伦理审批号：IRB-2023-003），研究过程中充分保护研究对象知情同意权及隐私权。
1.2　方法
1.2.1　调查工具
1.2.1.1　一般资料调查表
一般资料包括病人的性别、年龄，文化程度、职业、体质指数（BMI）等；疾病相关资料通过电子病历系统提取，包括疾病诊断、肿瘤分期、有无合并高血压、糖尿病等。
1.2.1.2　癌症病人生活质量量表（European Organization for the Research and Treatment of Cancer,Quality of Life Questionnaire-Core 30,EORTC QLQ-C30）
量表由欧洲癌症研究与治疗组织开发的用于评价癌症病人的生活质量[14]，共30个条目，其中躯体功能、角色功能、认知功能、情绪功能、社会功能5个功能领域，疲倦、恶心与呕吐、疼痛3个症状领域，气促、失眠、食欲丧失、便秘、腹泻、经济困难6个单一条目以及1个总体健康状况，第29个和第30个条目采用7级评分，从“很差”到“很好”分别计1~7分，其余条目均采用4级评分，“没有”计1分，“有一点”计2分，“有一些”计3分，“非常多”计4分，总体健康状况维度、功能维度得分越高，表示功能状况和生活质量越好，各症状条目得分越高表示症状或问题越严重(生活质量越差)。各领域得分是将各领域的条目得分相加后除以条目数得到。为使各领域得分能相互比较，采用极差化方法将上述得分线性变换转化为在0~100内取值的标准化得分。2005年，万崇华等[15]将其汉化,中文版量表各维度Cronbach's α系数及重测信度均>0.73，表明量表的信效度较好，可用于中国癌症病人生活质量的测量。
1.2.1.3　握力测量
采用广东香山CAMRY:EH101型电子握力计进行测量。指导病人采用优势手进行测量，根据病人手形适当调整握距，握力计勿与身体和衣物接触, 嘱病人竭力握握力计，研究者读取握力计表盘显示数据并记录。握力的界定标准参考国内学者洪忠新等[16]的研究，分为低、中、高握力水平。
1.2.1.4　营养评估
采用营养风险筛查2002（NRS 2002）评价病人营养状况，包括疾病有关评分、营养状况有关评分和年龄评分，以评分≥3分为存在营养风险，分值<3分,表明不存在营养风险[17]。
1.2.2　调查方法
研究者向病人讲解本研究的内容、目的及意义，在征得病人同意后指导其配合测量握力，随后发放调查问卷，不方便填写问卷的病人由研究者提问相关问题并填写后回收问卷。本研究共发放问卷300份，回收有效问卷291份，有效回收率为97.0%。
1.2.3　统计学方法
采用SPSS23.0软件进行数据统计分析，定性资料采用频数、百分比（%）描述，符合正态分布的定量资料采用均数±标准差（x±s）描述，两组比较采用独立样本t检验；定性资料比较采用χ2检验。采用Spearman相关分析探讨握力与生活质量各领域的相关性，线性回归分析探讨握力与生活质量的关系以及进一步分析调整混杂因素之后握力与生活质量各维度之间的关系。以P<0.05为差异有统计学意义。
2　结果

2.1　病人的一般资料
291例肿瘤病人中，男147例，女144例；年龄18~86（52.17±11.81）岁，其中男性病人（53.72±11.92）岁，女性病人（50.58±11.53）岁；乳腺肿瘤66例，消化系统肿瘤58例，呼吸系统肿瘤120例，其他肿瘤47例。具体见表1。
2.2　肿瘤病人握力与生活质量的相关性
由于不同性别病人在年龄、身高、体重等一般资料比较差异有统计学意义，为了减少一般资料差异导致结果产生误差，分别对女性病人及男性病人握力和生活质量进行相关性分析。结果显示，在男性病人中，握力与生活质量功能领域维度得分及总体健康状况得分均呈正相关（P<0.05），与症状领域的疲倦、疼痛得分呈负相关（P<0.05），但与单项条目相关性无统计学意义（P>0.05）；在女性病人中，握力与单项条目的食欲丧失、腹泻得分呈负相关（P<0.05），与经济困难得分呈正相关（P<0.05），但与其他维度相关性无统计学意义（P>0.05），见表2。
2.3　肿瘤病人握力对生活质量的影响
肿瘤病人生活质量本身受多种因素影响，有研究报道，病人的性别、年龄、TNM分期、营养状况等均对生活质量产生影响[18-20]。因此，本研究以生活质量得分为因变量，握力作为自变量进行简单线性回归分析，按照回归分析中差异有统计学意义的变量进一步调整回归模型，在调整其他混杂因素影响的情况下探讨握力对生活质量的影响。
2.3.1　男性肿瘤病人握力对生活质量的影响
结果显示，男性肿瘤病人握力对生活质量中功能领域（躯体功能、角色功能、情绪功能、认知功能、社会功能）有影响（P<0.05）；握力对症状领域中疲倦有影响（P<0.05）；同时，握力对总体健康状况有影响（P<0.05），但是对疼痛的影响无统计学意义（P>0.05），见表3。
2.3.2　女性肿瘤病人握力对生活质量的影响
结果显示，女性病人握力对生活质量单项条目中食欲丧失和经济困难有影响（P<0.05），但是对腹泻的影响无统计学意义（P>0.05），见表4。

EORTC QLQ-C30是目前全球使用最广泛的肿瘤病人健康相关生活质量调查表之一[20]，因此，本研究采用EORTC QLQ-C30分别从功能领域、症状和总体健康状况等方面对肿瘤病人进行问卷调查，能够全面、具体地了解病人的生活质量情况。
本研究中，男性病人与女性病人的一般情况存在差异，为了减少由于性别差异导致结果产生偏倚，分别对女性病人及男性病人握力和生活质量进行相关性分析。结果显示，在男性病人中，握力与生活质量功能领域各维度及总体健康状况均存在正相关关系（P<0.05），与症状领域的疲倦、疼痛存在负相关关系（P<0.05），而在女性病人中，握力仅与单项条目中的食欲丧失、腹泻及经济困难存在相关性关系（P<0.05）。握力与肿瘤病人生活质量之间存在相关性，但男性病人与女性病人握力与生活质量各维度之间的相关性各有不同。可能的原因：本研究男性病人握力（33.34±9.24）kg，女性病人握力（22.62±5.84）kg，男性病人握力大于女性病人，与以往研究报道[21]一致；此外，本研究男性病人年龄大于女性病人，有研究指出，随着年龄的增长，握力也在下降[22-23]，因此握力对男性病人的影响可能更大；另外，也有可能是由于性别、体重和激素的差异导致，需要进行进一步研究。
为了进一步明确握力与肿瘤病人生活质量各领域之间的关系，本研究将与握力存在相关性的变量进行回归分析，探讨握力对生活质量的影响。线性回归分析表明，在调整病人性别、年龄、TNM分期、NRS 2002营养筛查评分后，握力是影响肿瘤病人生活质量的重要因素。在男性病人中，握力越大，生活质量中功能领域的得分越高（P<0.05），说明病人的握力越大，功能状况越好，与Kilgour等[24]的研究结果相似。握力的减少会导致活动受限，对日常生活活动产生负面影响[25]。有研究表明，握力每增加10 kg，日常功能活动受损的概率降低39%[26]。Mcnicholl等[27]在一项前瞻性大样本研究中发现，握力对住院病人生活质量的功能领域有较好的预测价值。一项在关节置换术病人中开展的研究表明，握力与病人术后功能状况得分的改善存在正相关关系[28]。可见，握力对病人生活质量的功能领域有重要影响。
病人握力越大，症状领域中疲倦的得分越低（P<0.05），说明握力越小，病人越容易发生疲倦症状，这与Kilgour等[29]在胃肠道与非小细胞肺癌中的研究结果相似。还有研究发现，握力较小的病人疲劳水平明显更高，同时也发现握力较低的病人表现出更高的症状负担[24]。本研究结果显示，握力越大，总体健康状况得分也越高（P<0.05），说明病人的握力越大其健康状况越好；Marques等[30]的研究也发现，握力的降低会对健康状况产生影响。握力是代表机体肌肉力量的一项重要指标[31]，而有研究指出肌肉力量不足可能导致一些影响身体感觉的慢性病的发展，肌肉力量不足也可能是已经存在或目前患有影响视觉功能等生理感觉的慢性疾病(如糖尿病)的标志[32]，因此，握力可间接影响病人的健康状况。在女性病人中，握力越大，单项条目中的食欲丧失得分越小（P<0.05），说明女性病人的握力越小，越容易产生食欲丧失的症状。目前尚未检索到有报道肿瘤病人握力与食欲的相关研究，具体的影响机制有待进一步探讨。Norman等[33]评估了营养缺乏的非癌性胃肠道疾病病人，结果显示补充营养后肌肉力量和生活质量改善。因此，与癌症相关的营养不良可能是握力和生活质量之间存在关系的一个桥梁[24]。此外，经济困难维度与握力仍存在统计学关联（P<0.05），提示女性病人的体力状况与经济困难之间可能存在一定联系，其具体原因仍有待进一步研究。研究表明，保持握力的强度可以改善人们的生活质量[13]，提示医务人员要重视肿瘤病人的握力水平，对于握力较低的肿瘤病人，可考虑给予运动或是营养支持等提高握力的干预措施，以最大限度提高病人的生活质量，从而促进病人整体健康水平。

综上所述，握力可在一定程度上反映肿瘤病人的生活质量，主要体现在男性病人功能领域、症状领域以及女性病人食欲丧失条目中。因此，握力测量在肿瘤病人中具有重要意义。同时，握力是一项便捷且实用的测量指标，测量无需大量的人力、物力，数值方便获取，可用于肿瘤病人生活质量的辅助评估。
