Objective

To explore the coupling coordination relationship and spatial distribution status between the nursing human resources allocation and the economic development level in China,and to provide reference basis for promoting the coordinated development of nursing human resource allocation and economic level.

Methods

The entropy method and coupling coordination degree model were used to study the allocation of nursing human resources,economic development level and coupling coordination degree in China.The spatial autocorrelation method was used to study the spatial layout and changing trend of the coupling coordination degree in 2019 and 2023.

Results

There were significant regional differences in the allocation of nursing human resources and economic development level in China.The average coupling coordination degree of the two systems was in the stage of approaching imbalance.There was a spatial aggregation phenomenon in the coupling coordination degree,and the Moran's I index increased over time,with the degree of aggregation continuously deepening.The local aggregation presented a spatial pattern of "high in the east and low in the west".

Conclusions

The coupling and coordination degree of nursing human resources allocation and economic development in our country is not high and there are obvious regional differences.Therefore,each province should scientifically plan the plan of nursing human resources based on its own economic situation and regional medical needs,and promote the coordinated development of economic level and the nursing human resources allocation in a coordinated manner.

目的

探讨我国护理人力资源配置水平和经济发展水平耦合协调关系及空间分布现状，为促进护理人力资源配置和经济水平协调发展提供参考依据。

方法

利用熵值法和耦合协调度模型研究我国护理人力资源配置和经济发展水平及耦合协调度，运用空间自相关方法研究2019年和2023年耦合协调度空间布局及变化趋势。

结果

我国护理人力资源配置和经济发展水平地区差异明显，两系统耦合协调度均值处于濒临失调阶段，耦合协调度存在空间聚集现象，Moran's I指数随时间增加，集聚程度不断加深，局部集聚呈现“东高-西低”的空间格局。

结论

我国护理人力资源配置和经济发展耦合协调程度不高且存在明显的地区差异，各省要根据自身经济及地区医疗需求科学规划护理人力资源配置计划，统筹推进经济水平和护理人力资源配置协调发展。

目的

探讨我国护理人力资源配置水平和经济发展水平耦合协调关系及空间分布现状，为促进护理人力资源配置和经济水平协调发展提供参考依据。

方法

利用熵值法和耦合协调度模型研究我国护理人力资源配置和经济发展水平及耦合协调度，运用空间自相关方法研究2019年和2023年耦合协调度空间布局及变化趋势。

结果

我国护理人力资源配置和经济发展水平地区差异明显，两系统耦合协调度均值处于濒临失调阶段，耦合协调度存在空间聚集现象，Moran's I指数随时间增加，集聚程度不断加深，局部集聚呈现“东高-西低”的空间格局。

结论

我国护理人力资源配置和经济发展耦合协调程度不高且存在明显的地区差异，各省要根据自身经济及地区医疗需求科学规划护理人力资源配置计划，统筹推进经济水平和护理人力资源配置协调发展。

Objective

To explore the coupling coordination relationship and spatial distribution status between the nursing human resources allocation and the economic development level in China,and to provide reference basis for promoting the coordinated development of nursing human resource allocation and economic level.

Methods

The entropy method and coupling coordination degree model were used to study the allocation of nursing human resources,economic development level and coupling coordination degree in China.The spatial autocorrelation method was used to study the spatial layout and changing trend of the coupling coordination degree in 2019 and 2023.

Results

There were significant regional differences in the allocation of nursing human resources and economic development level in China.The average coupling coordination degree of the two systems was in the stage of approaching imbalance.There was a spatial aggregation phenomenon in the coupling coordination degree,and the Moran's I index increased over time,with the degree of aggregation continuously deepening.The local aggregation presented a spatial pattern of "high in the east and low in the west".

Conclusions

The coupling and coordination degree of nursing human resources allocation and economic development in our country is not high and there are obvious regional differences.Therefore,each province should scientifically plan the plan of nursing human resources based on its own economic situation and regional medical needs,and promote the coordinated development of economic level and the nursing human resources allocation in a coordinated manner.

党的二十大报告明确提出，要把保障人民健康放在优先发展的战略位置，完善促进人民健康的相关政策。将全民健康作为医疗卫生体系的出发点和立足点是健康中国的核心内涵[1]，卫生资源配置与经济之间联系紧密，存在相互作用的关系[2]，卫生资源配置水平反映一个地区或国家的卫生状况和经济水平，高经济发展水平为卫生资源配置提供物质基础，充足、均衡的卫生资源配置带动社会经济的可持续发展，两者之间相互协调、相互促进，这是社会发展的必然要求[3]。护理人员作为卫生人力资源的重要组成部分，其配置水平关系整个医疗活动的运作和发展，均衡的护理人力资源配置不仅能促进护理服务质量的提升，而且对医疗经济增长具有重要贡献[4]。现有的关于护理人力资源的研究多集中于配置公平性、效率等，缺少护理人力资源与经济发展间的相互影响研究。因此，本研究基于我国2019年与2023年的护理人力资源和经济水平相关数据分析护理人力资源配置与经济发展水平的耦合协调程度，并对得出的耦合协调度进行空间聚集分析，为促进护理人力资源配置和经济水平协调发展、实现卫生事业可持续发展提供参考依据。
1　资料与方法

1.1　资料来源
以我国31个省级行政区2019年及2023年数据为样本研究护理人力资源配置水平和经济水平耦合协调程度，数据均来自《中国统计年鉴》和《中国卫生健康统计年鉴》。同时，根据卫生健康统计年鉴对我国31个省级行政区划分标准进行划分，东部地区包括北京、天津、河北、辽宁、上海、江苏、浙江、福建、山东、广东、海南；中部地区包括山西、吉林、黑龙江、安徽、江西、河南、湖南、湖北；西部地区包括内蒙古、广西、重庆、四川、贵州、云南、西藏、陕西、甘肃、青海、宁夏、新疆。
1.2　指标选取
在护理人力资源配置评价体系上，参照陈仁昌等[5-8]对护理人力资源配置的研究，选取可及性和配置规模共5个指标对护理人力资源配置水平进行评价。通过医护比量化医师与护士的配置，反映两者之间的协作关系；通过床护比来反映护理服务的人力资源投入情况；每千人口注册护士人数和每平方千米注册护士数则体现护理服务的人口及地理可及性；注册护士占卫生技术人员的比重体现卫生体系中注册护士的地位及队伍体量。在经济发展水平评价体系上，基于现有文献研究[9-10]，从经济规模、经济效益和经济结构3方面选取6个指标，详见表1。
1.3　研究方法
1.3.1　熵值法
1）数据标准化：因各项指标数据的量纲和正负取向存在差异，需对原始数据进行标准化处理，为避免出现无意义数值，将处理后的数值加0.000 1[11]。公式如下：

Xij'=(maxXij-Xij)/(maxXij-minXij)+0.000 1（负向指标）。
2）计算i年j项指标比重（Yij）：Yij=Xij'/∑i=1mXij'。
3）计算j指标的熵值（ej）：ej=-1lnm∑i=1mYij×ln(Yij)。
4）计算j指标差异系数（dj）：dj=1-ej。
5）计算j项指标的权重（wj）：wj=dj/∑j=1ndj。
6）计算综合得分f(x)=∑j=1mwj×Xij'；g(x)=∑j=1mwj×Xij'。
式中：i为地区，j为评价指标，Xij为原始数值，Xij'表示经标准化后数值(i=1,2,3……m，j=1,2,3……n)，其中，f(x)和g(x)分别为护理人力资源配置水平综合得分和经济发展水平综合得分。
1.3.2　耦合协调度模型

式中：C为耦合度；T为协调指数，α和β为权重指数。本研究认为护理人力资源配置和经济水平同等重要，故令α=β=0.5；D为耦合协调度，取值0~1，根据既往研究[12-13]将耦合协调度划分为10个等级，详见表2。
1.3.3　空间自相关分析
运用GeoDa软件对我国护理人力资源配置水平和经济发展水平耦合协调度进行空间自相关分析。全局Moran's I分析整体上是否存在空间关联和聚集，其取值范围为[-1，1]，当数值为正值时说明存在空间聚集性；数值为负值时表示空间负相关，即相异的空间单元值聚集；当数值为0时表示各地市之间不存在空间自相关关系，呈现随机分布[14-15]。局部Moran's I研究空间省份与其邻近省份之间的相关关系[16]，共有“高-高”“低-低”“高-低”“低-高”4种聚集类型。
2　研究结果

2.1　我国护理人力资源配置水平和经济发展水平耦合协调度分析
结果显示，2023年大部分地区的护理人力资源配置和经济发展水平相比2019年都呈现上升趋势。从护理人力资源配置水平得分来看，东部地区均值微降，中部和西部地区均值微涨，2019年与2023年护理人力资源配置得分最高和最低的地区均是上海市和西藏自治区，两者相差由0.876分到0.802分，地区间差异显著；从经济发展水平得分来看，东、中、西部地区得分均值均呈增加趋势，经济发展水平得分2019年最高和最低的地区分别是北京市和西藏自治区，两者相差0.699分，2023年得分最高和最低的地区分别是上海市和甘肃省，两者相差0.688分，地区间经济发展水平同样存在显著差异。详见表3。
2019年及2023年我国护理人力资源配置和经济发展水平的耦合协调等级均处于濒临失调阶段。从区域划分来看，东部地区由2019年的勉强协调升为2023年的初级协调，中部地区处于濒临失调，西部地区处于轻度失调。2019年我国护理人力资源配置和经济发展水平耦合协调度包含极度失调到良好协调9个等级，2023年涉及从严重失调到良好协调8个等级，等级跨度依旧很大。相较2019年，2023年除山西、河北升至濒临失调，广西降至轻度失调，西藏升至严重失调外，其余省份耦合协调等级无变化。从2023年耦合协调度看，北京和上海处于良好协调等级，江苏、浙江和广东属于初级协调，天津、福建和山东属于勉强协调，这些较为协调的地区均处于东部地区，除以上地区和西藏外，其余地区均属于濒临失调和轻度失调阶段。总的来说，我国护理人力资源配置和经济水平耦合协调度5年间小有浮动，总体处于濒临失调阶段，各区域耦合协调度两极分化，区域间差异并未缩小，详见表4。
2.2　我国护理人力资源配置水平和经济发展水平耦合协调度空间自相关分析
对耦合协调度进行空间分析，结果显示2019—2023年Moran's I指数为正显著，Moran's I指数由2019年0.376 5降至2020年的0.370 2之后一直升至2023年0.413 6，说明我国护理人力资源配置水平和经济发展水平耦合协调度存在空间聚集且随着时间推移在空间上聚集程度不断加深，见表5。
在正相关的基础上利用局部空间自相关方法，进一步探讨耦合协调度是否存在空间异质性。结果表明，高-高聚集区为东部地区的江苏、浙江和上海，2023年低-低聚集区除2019年西藏、青海、四川、新疆和云南外新增加内蒙古，集中在西部地区，形成了“东高-西低”的空间格局，详见表6。

本研究表明，大部分地区的护理人力资源配置和经济发展水平综合得分均呈上升趋势，但整体的得分均值仍处于偏低水平，且同一年份内地区间得分水平参差不齐。从护理人力资源配置来看，2019年与2023年得分最高与最低的城市差值均高达0.8左右；从经济水平来看，2019年与2023年得分最高与最低的城市差值均将近0.7，说明护理人力资源配置水平和经济水平存在明显的地区发展不均问题。首先，针对护理人力资源配置不均情况，相关部门应适应护理服务的需求，扩大护理专业招生，加强对护理人才的培养，提高护理服务能力，保证护理人才的数量与质量，做到科学公平地配置护理人员[17-18]；其次，提高护理工作人员的薪资待遇，合理安排工作秩序，提高其工作积极性，降低护理工作者的离职率[19]。对于经济水平地区差异较大的问题，政府要加强对落后地区的财政投入，发达地区帮扶落后地区，同时地区要找准自身的发展优势，以优势产业带动区域经济，加强自身经济实力，为护理服务的开展提供支撑条件[20]。
3.2　护理人力资源配置和经济发展水平耦合协调度有待提高
2019—2023年，5年间我国护理人力资源配置和经济发展水平耦合协调度变动较小，处于濒临失调状态，耦合协调度均值东部地区>中部地区>西部地区。2023年位于濒临失调和轻度失调的地区约占全国的67%，大部分地区的护理人力资源和经济水平并未达到协调，总是存在一方滞后的情况，经济水平的不平衡是影响卫生资源配置不均的一种深层次原因，这种现象不利于两者协同发展，互利共赢[21]。因此，需精准针对不同地区发展情况及地区需求采取差异化措施，处于协调的地区应更加注重经济的高质量发展，以绿色和共享作为经济发展理念，同时整合优质的护理人力资源满足群众更高水平的护理需求，推动经济与护理人力配置协调程度进一步提升。对于失调的地区，护理人力资源配置滞后的地区，要提高护理支持力度，兼顾地理与人口需求配置护理人员，完善护理岗位流程秩序，提供高效的护理服务[22]；经济落后的地区，要在政府财政支持和协调发达地区的辐射引领下打造自身特色产业，拉动经济水平，做到经济和护理人力配置的协调发展[23]。
3.3　耦合协调度空间聚集加深，局部存在集聚性
本研究空间分析结果显示，2019—2023年全局Moran's I指数均为正且指数在上升，说明空间聚集程度随时间在加深，局部空间分析结果显示高-高聚集区集中在东部地区，低-低聚集区集中在西部地区，2023年受西部地区低-低聚集区的同质化影响新增内蒙古，使得低-低聚集区扩大。西部低-低聚集区虽然获得一定政策支持，但受其环境、人口等因素影响，经济条件薄弱限制发展而加剧失调。一般来讲，空间上距离越近周边地区的地区相似性与关联性就越强[24]，为缩小地区间差异，国家应继续加强规划引导，以财政投入改善经济落后地区失调情况[25]，同时充分发挥高协调区的辐射和带动作用，对濒临失调和勉强协调等过渡阶段的城市提供学习经验，与高-高聚集区的核心地区建立城市网，逐步扩大辐射区域，带动相邻城市间经济协作与护理服务资源共享[26]。耦合协调度下降及较低地区政府应加大重视程度，分析造成低-低聚集存在的共同原因，综合考虑不同耦合类型地区发展情况，动态调整政策措施，实现对口帮扶[27]，从区域经济的实际需求出发制定针对性政策，实现护理人力资源配置和经济水平的协同发展，促进医疗事业可持续发展。
