目的：

基于“髓虚毒损”理论探讨解毒益智方对阿尔茨海默病（AD）的临床疗效。

方法：

纳入2024年6月—2025年3月上海市浦东医院收治的88例AD患者，随机数字表法分为观察组（常规治疗+解毒益智方）和对照组（常规治疗），每组44例，比较两组患者治疗12周后临床疗效，治疗前及治疗12周后认知功能［简易精神量表（MMSE）、蒙特利尔认知评估量表（MoCA）］、日常生活能力［日常生活能力量表（ADL）、Barthel评分］、脑微循环（椎基底动脉血流速度）、氧化应激指标［超氧化物歧化酶（SOD）、丙二醛（MDA）、β淀粉样蛋白（Aβ）］、炎症指标［白细胞介素-1β（IL-1β）、白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）］水平。

结果：

治疗12周后，观察组总有效率高于对照组（P < 0.05）；治疗12周后，观察组MMSE、MoCA、Barthel评分、脑微循环指标（左侧椎动脉、右侧椎动脉、基底动脉）、SOD较治疗前提升（P < 0.05），且高于对照组（P < 0.05），观察组ADL评分、MDA、Aβ、IL-1β、TNF-α水平较治疗前下降（P < 0.05），且低于对照组（P < 0.05）；两组治疗前和治疗12周后IL-6比较无显著差异（P > 0.05）。

结论：

解毒益智方对阿尔茨海默病的临床疗效良好，显著提升认知功能和日常生活能力，其机制可能为改善患者脑微循环、降低氧化应激和炎症水平。

Objective:

To investigate the clinical efficacy of Jiedu Yizhi Formula for Alzheimer's disease (AD) based on the theory of "marrow deficiency and toxin damage. "

Methods:

A total of 88 AD patients admitted to Shanghai Pudong Hospital from June 2024 to March 2025 were enrolled and randomly divided into an observation group (conventional treatment + Jiedu Yizhi Formula) and a control group (conventional treatment), with 44 cases in each group. The therapeutic efficacy was compared between the two groups after 12 weeks of treatment. Cognitive function [Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA)], activities of daily living [Activity of Daily Living Scale (ADL), Barthel Index], cerebral microcirculation (vertebrobasilar artery blood flow velocity), oxidative stress indicators [superoxide dismutase (SOD), malondialdehyde (MDA), β-amyloid (Aβ)], and inflammatory indicators [interleukin (IL)-1β, IL-6, tumor necrosis factor-alpha (TNF-α)] were assessed before treatment and after 12 weeks of treatment.

Results:

After 12 weeks of treatment, the total effective rate in the observation group was higher than that in the control group (P <0.05). After 12 weeks of treatment, the MMSE, MoCA, Barthel Index scores, cerebral microcirculation indicators (left vertebral artery, right vertebral artery, basilar artery), and SOD levels in the observation group were increased compared with before treatment (P < 0.05) and were higher than those in the control group (P < 0.05). The ADL score, MDA, Aβ, IL-1β, and TNF-α levels in the observation group were decreased compared with before treatment (P < 0.05) and were lower than those in the control group (P < 0.05). There was no significant difference in IL-6 levels between the two groups before treatment and after 12 weeks of treatment (P > 0.05).

Conclusion:

Jiedu Yizhi Formula demonstrates good clinical efficacy in treating Alzheimer's disease, significantly improving cognitive function and activities of daily living. The mechanism may be related to improving cerebral microcirculation and reducing oxidative stress and inflammation levels.
