目的：

研究咽缩肌强化训练联合中医康复对咽期吞咽障碍患者进食不同稠度食物的影响。

方法：

选取新疆维吾尔自治区中医医院2022年4月—2024年2月纳入的95例咽期吞咽障碍患者进行观察，按随机数字表法分为研究组与对照组，研究组47例接受咽缩肌强化训练联合中医康复治疗，对照组48例采取咽缩肌强化训练，比较两组中医证候积分、改良容积黏度测试（VVST-CV）阳性率、功能性经口摄食量表分级（FOIS）、饮水试验、吞咽功能临床评估结果。

结果：

治疗前两组FOIS、饮水试验结果差异均无统计学意义（P >0.05），治疗后研究组FOIS等级高于对照组（P <0.01），但饮水试验等级低于对照组（P <0.01）。治疗前两组吞咽功能临床评估结果差异均无统计学意义（P >0.05），治疗后研究组有效咳嗽发生率高于对照组（P <0.01），最长呼气时间、反复唾液吞咽次数相较于对照组更高（P <0.01），但进食所需时间少于对照组（P <0.01）。治疗后研究组对不同稠度食物（1 ~ 3号）的阳性率分别是29.79% （14/47）、14.89% （7/47）、25.53% （12/47），均较对照组64.58%（31/48）、45.83%（22/48）、54.17% （26/48）低，差异均有统计学意义（P <0.01）。治疗前两组中医证候积分差异无统计学意义（P >0.05），治疗后两组积分均降低（P <0.05），但研究组吞咽困难、流涎、饮水呛咳积分低于对照组（P <0.01）。

结论：

对于咽期吞咽障碍患者，经咽缩肌强化训练联合中医康复干预的效果较好，能够改善患者进食不同稠度食物的吞咽功能，并减轻相关症状，增加有效咳嗽、最长呼气时间以及反复唾液吞咽次数，值得临床推广使用。

Objective:

To study the effect of pharyngeal constrictor muscle strengthening training combined with traditional Chinese medicine rehabilitation on the feeding of food of different consistency in patients with swallowing disorders in the pharyngeal stage.

Methods:

Ninety-five patients with swallowing disorders in pharyngeal stage included in our hospital from April 2022 to February 2024 were selected for observation, and grouped according to the randomized numerical table method. 47 cases in the study group received pharyngeal contraction muscle strengthening training combined with traditional Chinese medicine (TCM) rehabilitation, and 48 cases in the control group received pharyngeal contraction muscle strengthening training, and the results of clinical assessment of pharyngeal function were compared with those of the two groups, including the positive rate of the modified volumetric viscosity test (VVST-CV), the classification of the Functional Oral Intake Scale (FOIS), drinking test, and swallowing function clinical assessment results.

Results:

There was no difference in the results of FOIS and drinking test between the two groups before treatment (P >0.05), and after treatment, the FOIS grade of the study group was higher than that of the control group, but the drinking test grade was lower than that of the control group (P <0.01). There was no difference in the results of clinical assessment of swallowing function between the two groups before treatment (P >0.05), and after treatment, the incidence of effective cough was higher in the study group than in the control group, and the maximum respiratory time and the number of repeated salivary swallows were higher in comparison with the control group, but the time required for eating was less than that in the control group (P <0.01). The post-treatment positive rates of the study group for foods of different consistencies (No.1-3) were 29.79%, 14.89%, and 25.53%, respectively, all of which were lower than those of the control group, 64.58%, 45.83%, and 54.17% (P < 0.01).

Conclusion:

For patients with swallowing disorders in the pharyngeal phase, transpharyngeal constrictor muscle strengthening training combined with Chinese medicine rehabilitation intervention is more effective, which can improve the swallowing function of patients eating food of different consistency, increase the effective cough, the longest respiratory time and the number of repeated salivary swallowing, and it is worth to be popularized and used in the clinic.
