The pathogenesis of this disease has not been clarified, which is also a major challenge in clinical treatment. Cangfu Daotan Decoction, derived from the secret recipe for diagnosis and treatment of Ye Tianshi Nvke Zhenzhi Mifang, is particularly effective in the treatment of PCOS. Studies have found that Cangfu Daotan Decoction exerts therapeutic effects by regulating phosphatidylinositol 3 - kinase/protein kinase (PI3K/Akt), nuclear transcription factor - κB(NF - κB), adenosine monophosphate - activated protein kinase (AMPK), mitogen- activated protein kinase (MAPK), and transforming growth factor - β1/SMAD family member 3 (TGF - β1/Smad3)signaling pathways. In clinical studies, Cangfu Daotan Decoction alone or modified Cangfu Daotan Decoction or in combination with western medicines or external therapies not only can alleviate the clinical symptoms of PCOS,but also are safe and effective, which are worthy of clinical application. 多囊卵巢综合征（PCOS）是一种常见的生殖内分泌和代谢障碍疾病，其患病率逐年增高，发病机制尚未明确，是临床治疗中的一大挑战。《叶天士女科诊治秘方》所载苍附导痰汤治疗PCOS效果显著。苍附导痰汤可通过干预磷脂酰肌醇3 -激酶/蛋白激酶B（PI3K/Akt）、核转录因子 - κB（NF - κB）、腺苷酸活化蛋白激酶（AMPK）、丝裂原活化蛋白激酶（MAPK）、转化生长因子- β1/Smad同源物3 （TGF- β1/Smad3）等信号通路，发挥治疗作用。在临床应用上，单独使用苍附导痰汤或加减方或联合西药或联合外治法，均可改善PCOS患者的临床症状，安全有效，值得临床推广应用。本文从苍附导痰汤的作用机制以及临床研究现状两个方面，系统总结了其治疗PCOS的研究进展，以期为其进一步实验研究及临床应用推广提供参考。
