Objective

To explore the roles of oxidative stress and apoptosis in the renal deficiency and blood stasis type oligoasthenozoospermia(OAS) model, and to investigate the mechanism of the intervention by the Qi-supplementing, Blood-activating and Essence-nourishing formula.

Methods

The rat model of renal deficiency and blood stasis type oligoasthenozoospermia was established by intragastric administration of Gentiana macrophylla polysaccharides (GTW). The rats were randomly divided into the model group, the levocarnitine group, the low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula groups; another 8 rats were randomly selected as the normal control group. The levocarnitine group was intragastrically administered 1.8 mL·kg-1 levocarnitine oral liquid; the low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula groups were given 7.87, 15.75 and 31.50 g·kg-1,respectively; the blank group and the model group were intragastrically administered the same amount of 0.9% NaCl. All 6 groups of rats were administered the drugs once daily and continuously for 28 days. The general conditions of the rats were observed; the testicular and epididymal indices were measured; the sperm quality was detected; the pathological morphology of the testicular tissue was observed by hematoxylin-eosin staining (HE); the activity of reactive oxygen species (ROS), catalase (CAT) and superoxide dismutase (SOD) in the testicular tissue was detected by enzyme-linked immunosorbent assay (ELISA); the mRNA expression levels of Caspase-3, Bcl-2 and Bax in the testicular tissue were detected by real-time fluorescence quantitative polymerase chain reaction (q-PCR).

Results

The testicular indices of the blank group, model group, low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula group, and the levocarnitine group were (0.83±0.09)%, (0.55±0.10)%, (0.55±0.07)%,(0.71±0.12)%,(0.81±0.08)%, and (0.67±0.07)%, respectively; the epididymal indices were (0.36±0.05)%, (0.24±0.03)%, (0.25±0.04)%, (0.28±0.02)%,(0.35±0.06)%, and (0.28±0.03)%,respectively; the sperm concentrations were (24.11±11.64, 4.65±2.48, 6.75±3.81, 11.60±7.78, 21.72±7.81, 23.22±8.80)×106 sperm·mL-1, respectively; the sperm motility was (86.93±12.00)%, (33.46±16.13)%, (53.01±21.71)%, (63.15±24.35)%, (79.97±10.22)%, and (75.83±25.05)%, respectively; the ROS intensity was 597 926.11±87 518.20, 925 239.02±95 539.79, 846 676.84±64 867.76, 784 277.73±81 354.32, 658 228.04±82 768.68, and 725 740.12±87 846.36, respectively; the CAT activity was (1.40±0.11), (0.56±0.09), (0.77±0.11), (0.95±0.13), (1.15±0.12), and (1.03±0.11) U·mgprot-1, respectively; the SOD activity was (2.41±0.07), (1.65±0.05), (1.79±0.33), (1.90±0.04), and (2.21±0.05), and (2.06±0.04) U·mgprot-1, respectively. the relative expression levels of Bcl-2 mRNA were 1.00±0.04, 0.26±0.02, 0.39±0.04, 0.49±0.02, 0.87±0.02, and 0.66±0.05, respectively; the relative expression levels of Bax mRNA were 1.00±0.05, 1.78±0.07, 1.50±0.04, 1.39±0.02, 1.12±0.04, and 1.27±0.04, respectively; the relative expression levels of Caspase-3 mRNA were 1.00±0.03, 1.95±0.06, 1.81±0.03, 1.68±0.03, 1.18±0.07, and 1.49±0.08, respectively. The above-mentioned indicators of the model group compared with the blank group, the high-dose group compared with the model group, and the L-carnitine group except for the epididymal index compared with the model group, all showed statistically significant differences (P<0.05,P<0.01).

Conclusion

Oxidative stress and cell apoptosis play multiple regulatory roles in the sperm quality and testicular damage of OAS rats. The Qi-supplementing, activating blood, and tonifying essence formula may improve the sperm quality and testicular function of rats by inhibiting oxidative stress and cell apoptosis.

目的

探究氧化应激及细胞凋亡在肾虚血瘀型少弱精子症(OAS)模型中的作用及补气活血益精方干预的机制。

方法

用雷公藤多苷(GTW)灌胃的方法建立肾虚血瘀型OAS大鼠模型,将其随机分为模型组、左卡尼汀组、补气活血益精方低、中、高剂量实验组;另随机取8只大鼠为正常对照组。左卡尼汀组灌胃1.8 mL·kg-1左卡尼汀口服液;补气活血益精方低、中、高剂量组分别灌胃予以7.87、15.75、31.50 g·kg-1中药汤剂;空白组和模型组均灌胃等量0.9%NaCl,6组大鼠每天定时给药1次,连续给药28 d。观察大鼠一般情况,测定大鼠睾丸及附睾指数;检测大鼠精子质量,苏木精-伊红染色法(HE)观察大鼠睾丸组织病理形态学变化,酶联免疫吸附测定(ELISA)检测睾丸组织活性氧(ROS)强度、过氧化氢酶(CAT)及超氧化物歧化酶(SOD)活力,实时荧光定量聚合酶链式反应(q-PCR)检测大鼠睾丸组织中Caspase-3、Bcl-2、Bax mRNA表达水平。

结果

结论

氧化应激及细胞凋亡在OAS大鼠精子质量及睾丸损伤中发挥多重调控作用,补气活血益精方可能通过抑制氧化应激及细胞凋亡,改善大鼠精子质量及睾丸功能。

目的

探究氧化应激及细胞凋亡在肾虚血瘀型少弱精子症(OAS)模型中的作用及补气活血益精方干预的机制。

方法

用雷公藤多苷(GTW)灌胃的方法建立肾虚血瘀型OAS大鼠模型,将其随机分为模型组、左卡尼汀组、补气活血益精方低、中、高剂量实验组;另随机取8只大鼠为正常对照组。左卡尼汀组灌胃1.8 mL·kg-1左卡尼汀口服液;补气活血益精方低、中、高剂量组分别灌胃予以7.87、15.75、31.50 g·kg-1中药汤剂;空白组和模型组均灌胃等量0.9%NaCl,6组大鼠每天定时给药1次,连续给药28 d。观察大鼠一般情况,测定大鼠睾丸及附睾指数;检测大鼠精子质量,苏木精-伊红染色法(HE)观察大鼠睾丸组织病理形态学变化,酶联免疫吸附测定(ELISA)检测睾丸组织活性氧(ROS)强度、过氧化氢酶(CAT)及超氧化物歧化酶(SOD)活力,实时荧光定量聚合酶链式反应(q-PCR)检测大鼠睾丸组织中Caspase-3、Bcl-2、Bax mRNA表达水平。

结果

空白组、模型组、补气活血益精方低、中、高剂量组和左卡尼汀组睾丸指数分别为(0.83±0.09)%、(0.55±0.10)%、(0.55±0.07)%、(0.71±0.12)%、(0.81±0.08)%和(0.67±0.07)%,附睾指数分别为(0.36±0.05)%、(0.24±0.03)%、(0.25±0.04)%、(0.28±0.02)%、(0.35±0.06)%和(0.28±0.03)%,精子浓度分别为(24.11±11.64)、(4.65±2.48)、(6.75±3.81)、(11.60±7.78)、(21.72±7.81)和(23.22±8.80)×106 sperm·mL-1,精子活力分别为(86.93±12.00)%、(33.46±16.13)%、(53.01±21.71)%、(63.15±24.35)%、(79.97±10.22)%和(75.83±25.05)%,ROS强度分别为597 926.11±87 518.20、925 239.02±95 539.79、846 676.84±64 867.76、784 277.73±81 354.32、658 228.04±82 768.68和725 740.12±87 846.36,CAT活力分别为(1.40±0.11)、(0.56±0.09)、(0.77±0.11)、(0.95±0.13)、(1.15±0.12)和(1.03±0.11)U·mg prot-1,SOD活力分别为(2.41±0.07)、(1.65±0.05)、(1.79±0.33)、(1.90±0.04)、(2.21±0.05)和(2.06±0.04)U·mg prot-1,Bcl-2 mRNA相对表达量分别为1.00±0.04、0.26±0.02、0.39±0.04、0.49±0.02、0.87±0.02、0.66±0.05,Bax mRNA相对表达量分别为1.00±0.05、1.78±0.07、1.50±0.04、1.39±0.02、1.12±0.04和1.27±0.04,Caspase-3 mRNA相对表达量分别为1.00±0.03、1.95±0.06、1.81±0.03、1.68±0.03、1.18±0.07和1.49±0.08。模型组的上述指标与空白组比较、高剂量组的上述指标与模型组比较、除附睾指数外左卡尼汀组的上述指标与模型组比较,在统计学上差异均有统计学意义(P<0.05,P<0.01)。

结论

氧化应激及细胞凋亡在OAS大鼠精子质量及睾丸损伤中发挥多重调控作用,补气活血益精方可能通过抑制氧化应激及细胞凋亡,改善大鼠精子质量及睾丸功能。

Objective

To explore the roles of oxidative stress and apoptosis in the renal deficiency and blood stasis type oligoasthenozoospermia(OAS) model, and to investigate the mechanism of the intervention by the Qi-supplementing, Blood-activating and Essence-nourishing formula.

Methods

The rat model of renal deficiency and blood stasis type oligoasthenozoospermia was established by intragastric administration of Gentiana macrophylla polysaccharides (GTW). The rats were randomly divided into the model group, the levocarnitine group, the low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula groups; another 8 rats were randomly selected as the normal control group. The levocarnitine group was intragastrically administered 1.8 mL·kg-1 levocarnitine oral liquid; the low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula groups were given 7.87, 15.75 and 31.50 g·kg-1,respectively; the blank group and the model group were intragastrically administered the same amount of 0.9% NaCl. All 6 groups of rats were administered the drugs once daily and continuously for 28 days. The general conditions of the rats were observed; the testicular and epididymal indices were measured; the sperm quality was detected; the pathological morphology of the testicular tissue was observed by hematoxylin-eosin staining (HE); the activity of reactive oxygen species (ROS), catalase (CAT) and superoxide dismutase (SOD) in the testicular tissue was detected by enzyme-linked immunosorbent assay (ELISA); the mRNA expression levels of Caspase-3, Bcl-2 and Bax in the testicular tissue were detected by real-time fluorescence quantitative polymerase chain reaction (q-PCR).

The testicular indices of the blank group, model group, low, medium and high doses of the Qi-supplementing, Blood-activating and Essence-nourishing formula group, and the levocarnitine group were (0.83±0.09)%, (0.55±0.10)%, (0.55±0.07)%,(0.71±0.12)%,(0.81±0.08)%, and (0.67±0.07)%, respectively; the epididymal indices were (0.36±0.05)%, (0.24±0.03)%, (0.25±0.04)%, (0.28±0.02)%,(0.35±0.06)%, and (0.28±0.03)%,respectively; the sperm concentrations were (24.11±11.64, 4.65±2.48, 6.75±3.81, 11.60±7.78, 21.72±7.81, 23.22±8.80)×106 sperm·mL-1, respectively; the sperm motility was (86.93±12.00)%, (33.46±16.13)%, (53.01±21.71)%, (63.15±24.35)%, (79.97±10.22)%, and (75.83±25.05)%, respectively; the ROS intensity was 597 926.11±87 518.20, 925 239.02±95 539.79, 846 676.84±64 867.76, 784 277.73±81 354.32, 658 228.04±82 768.68, and 725 740.12±87 846.36, respectively; the CAT activity was (1.40±0.11), (0.56±0.09), (0.77±0.11), (0.95±0.13), (1.15±0.12), and (1.03±0.11) U·mgprot-1, respectively; the SOD activity was (2.41±0.07), (1.65±0.05), (1.79±0.33), (1.90±0.04), and (2.21±0.05), and (2.06±0.04) U·mgprot-1, respectively. the relative expression levels of Bcl-2 mRNA were 1.00±0.04, 0.26±0.02, 0.39±0.04, 0.49±0.02, 0.87±0.02, and 0.66±0.05, respectively; the relative expression levels of Bax mRNA were 1.00±0.05, 1.78±0.07, 1.50±0.04, 1.39±0.02, 1.12±0.04, and 1.27±0.04, respectively; the relative expression levels of Caspase-3 mRNA were 1.00±0.03, 1.95±0.06, 1.81±0.03, 1.68±0.03, 1.18±0.07, and 1.49±0.08, respectively. The above-mentioned indicators of the model group compared with the blank group, the high-dose group compared with the model group, and the L-carnitine group except for the epididymal index compared with the model group, all showed statistically significant differences (P<0.05,P<0.01).

Conclusion

Oxidative stress and cell apoptosis play multiple regulatory roles in the sperm quality and testicular damage of OAS rats. The Qi-supplementing, activating blood, and tonifying essence formula may improve the sperm quality and testicular function of rats by inhibiting oxidative stress and cell apoptosis.

全球男性精子质量呈下降趋势[1]。少弱精子症(oligoasthenozoospermia,OAS)是男性不育的主要病因,约占男性不育的50%[2]。肾虚血瘀证是OAS常见的中医证型,前期临床实践及研究表明,补气活血益精方可显著提高肾虚血瘀型OAS患者的精子浓度及活力[3]。近年研究认为OAS与氧化应激、细胞凋亡密切相关[4]。氧化应激(oxidative stress,OS)是体内氧化和抗氧化平衡的失调状态,可通过精子细胞膜脂质过氧化、蛋白质氧化、影响下丘脑-垂体-性腺轴和DNA链断裂损伤精子细胞[5-9]。细胞凋亡[5]是一种程序性死亡过程,外源性途径或受体凋亡和内源性或线粒体凋亡是凋亡启动的2种不同机制[10]。细胞凋亡增加可能会对精子产生不利影响,最终损害男性的生育能力[11]。本研究旨在探究氧化应激及细胞凋亡在肾虚血瘀型OAS大鼠精子质量及睾丸组织损伤中的作用及补气活血益精方的药理作用。
动物 SPF级健康雄性斯普拉格-道利(Sprag-Dawley,SD)大鼠,鼠龄6~8周,体质量180~220 g,购自北京斯贝福生物技术有限公司。动物生产许可证号:SCXK(京)2019-0010。本实验经甘肃中医药大学动物伦理委员会批准(伦理批号:SY2024-316)。
药品与试剂 雷公藤多苷片:规格:每片10 mg,批号:20231104,批准文号:Z43020138,湖南千金协力药业有限公司生产。左卡尼汀口服液:规格:每支10 mL,批号:240219,批准文号:H19990372,东北制药集团沈阳第一制药有限公司生产。补气活血益精方由熟地黄 15 g:批号:16091307、麸炒山药 15 g:批号:2402007、酒萸肉 15 g:批号:YP02-2309013、红花 5 g:批号:180707、鸡血藤 20 g:批号:1801137、丹参 15 g:批号:170301、当归 15 g:批号:20180101、桑寄生 15 g:批号:202404002、桑葚 15 g:批号:24011602、炙淫羊藿 15 g:批号:202204002、炙黄芪 30 g:批号:24060602组成,中药饮片均购于甘肃中医药大学附属医院中药房,经甘肃中医药大学附属医院药学部靳子明教授鉴定为正品;反转录试剂盒,批号:11141ES60(H9305270),上海翊圣生物科技有限公司生产;荧光定量试剂盒,上海翊圣生物科技有限公司生产;TrizolReagent,ambion生产;活性氧(reactive oxygen species,ROS)、过氧化氢酶(catalase,CAT)、超氧化物歧化酶(superoxide dismutase,SOD)检测试剂盒,南京建成生物工程研究所生产。
仪器 SAS-DM6精液分析仪,上海朗逸医疗器械有限公司产品;D-315病理切片机、YD-AB3组织摊片机、YD-6L生物组织冷冻包埋机,金华市益迪医疗设备有限公司产品;SN100680221显微拍照系统,山东志盈医学科技有限公司产品;K5800微量核酸测定仪,北京凯奥科技发展有限公司产品;A300聚合酶链式反应(polymerase chain reaction, PCR)扩增仪,杭州朗基科学仪器有限公司产品;荧光定量PCR仪,ROCGENE产品。
补气活血益精方由熟地黄15 g、麸炒山药15 g、酒萸肉15 g、红花5 g、鸡血藤20 g、丹参15 g、当归15 g、桑寄生15 g、桑葚15 g、炙淫羊藿15 g和炙黄芪30 g组成,将以上药材浸泡30 min,大火煮沸后转为文火继续煎煮40 min,滤出药液后再次加水煎煮40 min,将2次药液充分混匀,浓缩至含生药量2 g·mL-1,置于4 ℃冰箱冷藏备用。
将SD大鼠随机分为空白组(10只),肾虚血瘀型OAS组(50只)。根据文献[12]的方法,肾虚血瘀型OAS组用雷公藤多苷(glycosides of Tripterygium Wilfordii,GTW)灌胃4周,4周后随机抽取空白组2只,造模组6只,通过精子质量检测及大鼠一般情况观察验证肾虚血瘀型OAS大鼠模型。正常对照组给予等量生理盐水灌胃4周。
将44只肾虚血瘀型OAS大鼠模型随机分为模型组、补气活血益精方低、中、高剂量组、左卡尼汀组,每组8 只,余4 只大鼠备用。参照人与实验动物给药剂量的换算方法[13],左卡尼汀组灌胃左卡尼汀口服液(1.8 mL·kg-1左卡尼汀口服液,qd,连续28 d),补气活血益精方低、中、高剂量组分别予以(7.87、15.75、31.5 g·kg-1中药汤剂灌胃,qd,连续28 d)。空白组和模型组给予等量0.9% NaCl灌胃。
观察大鼠的一般情况,包括毛发、精神状态、活动度、大小便及睾丸血管脉络等方面。
使用精密天平称取大鼠体质量及两侧睾丸及附睾的总质量。按照公式“睾丸/附睾指数=双侧睾丸/附睾质量(mg)/体质量(g)”计算出相应的睾丸/附睾指数。
末次给药24 h后,记录大鼠体质量,用1 %戊巴比妥麻醉后处死大鼠,迅速分离附睾,将双侧附睾置于含3 mL(37 ℃)温水的离心管充分剪碎,用自动精子分析仪检测精子浓度和活力。
将大鼠左侧睾丸组织固定于4%多聚甲醛溶液,用酒精、二甲苯脱水透明后,石蜡包埋,制作3~5 μm厚切片,进行苏木精-伊红(hematoxylin and eosin,HE)染色,脱水封片晾干,显微系统拍照并观察睾丸组织的病理形态变化。
用组织研磨机研磨睾丸组织,用于ROS强度、CAT及SOD活力测量。所有检测均根据所售试剂盒的说明进行,ROS强度检测:开盖前短暂离心试剂,荧光探针标记,用96孔细胞培养板检测;CAT及SOD活力检测:精准称取组织重量,按组织重量(g)与生理盐水体积(mL)1:9 的比例,加入9 倍体积生理盐水,以 4 000 r·min-1离心10 分钟,取上清液,用试剂盒进行测定。
用TRIzol试剂从大鼠睾丸中提取总RNA,cDNA合成通过去除基因组DNA后,利用反转录试剂盒在PCR仪中完成。以甘油醛-3-磷酸脱氢酶(glyceraldehyde-3-phosphatedehydrogenase,GAPDH)为内参,qPCR部分采用SYBRGreenMix、特定引物及两步法扩增程序进行检测。mRNA表达水平使用2-△△Ct法方法计算,见表1
用SPSS 26.0软件进行统计分析,计量资料用表示,当数据同时满足正态分布及方差齐性时,组间比较采用单因素方差分析;不满足正态分布或方差不齐时,组间比较用非参数检验。
与空白组大鼠相比,造模组大鼠进食量与饮水量开始逐渐减少,精神状态愈发萎靡,行动迟缓呆滞,对周围环境的刺激反应迟钝;毛发稀疏且失去光泽,色泽晦暗;从排泄系统的表现来看,大鼠的小便量明显减少,大便干结,排泄过程相对困难;可以清晰地发现大鼠睾丸组织外表血管脉络变呈现充盈胀满的态势,颜色也从原本的正常色泽转变为紫黯之色,这些表现与中医理论中肾虚血瘀证型的症状高度吻合。
与空白组相比,模型组睾丸指数可见显著减低(P<0.01);与模型组相比,左卡尼汀组睾丸指数可见显著上升(P<0.05),补气活血益精方中、高剂量组大鼠睾丸指数均可见显著上升(均P<0.01),补气活血益精方低剂量组睾丸指数在统计学上差异无统计学意义(P>0.05)。
与空白组相比,模型组附睾指数可见明显减低(P<0.01);与模型组相比,补气活血益精方低、中剂量组和左卡尼汀组附睾指数上升不明显(P>0.05),补气活血益精方高剂量组大鼠附睾指数可见显著上升(P<0.01),见表2
与空白组相比,模型组大鼠精子活力显著降低(P<0.01)。与模型组比较,左卡尼汀组、补气活血益精方中、高剂量组大鼠精子活力均显著升高(均P<0.01)。与空白组相比,模型组大鼠精子浓度均显著降低(P<0.01)。与模型组比较,左卡尼汀组、补气活血益精方高剂量组大鼠精子浓度均显著升高(均P<0.01),补气活血益精方低、中剂量组精子浓度在统计学上差异无统计学意义(P>0.05),见表3
HE染色结果显示:空白组大鼠曲细精管结构完整,管腔呈圆形或椭圆形,生精细胞排列整齐,可见成熟精子;模型组曲细精管形态不规则,局部曲细精管数量减少,管周膜增生,小血管扩张充血,并血栓形成;与模型组比较,经补气活血益精方组和左卡尼汀组治疗后,睾丸组织病变明显改善,精曲小管结构相对完整,生精细胞排列比较整齐,腔内正常精子量增多,补气活血益精方高剂量组与正常组结构相近。说明补气活血益精方能够修复肾虚血瘀型OAS大鼠的睾丸组织损伤,见图1
ELISA检测结果显示:模型组大鼠睾丸组织ROS强度相较空白组明显升高,CAT、SOD活力均显著降低(均P<0.01)。与模型组比较补气活血益精方可以降低ROS强度(P<0.05,P<0.01),升高CAT、SOD活力(均P<0.01);在给予左卡尼汀治疗后,大鼠睾丸组织中ROS强度相较模型组显著降低,CAT、SOD活力升高(均P<0.01),见表4
模型组大鼠睾丸组织中Bax、Caspase-3 mRNA相对表达量增加,Bcl-2 mRNA相对表达量降低,与空白组比较有显著性差异(均P<0.01);与模型组比较,补气活血益精方各剂量组和左卡尼汀组Bax、Caspase-3 mRNA相对表达量下降(P<0.05,P<0.01),Bcl-2 mRNA相对表达量上升(P<0.01)。
补气活血益精方由熟地黄、麸炒山药、酒萸肉、炙黄芪、当归、炙淫羊藿、桑葚、桑寄生、红花、丹参、鸡血藤等11味中药组成,具有益肾填精,化瘀通络之效。补气活血益精方为甘肃中医药大学附属医院邢喜平主任医师自拟方,邢喜平主任医师认为少弱精子症的核心病机为“肾虚血瘀”,主张以“益肾填精,化瘀通络”为其主要治法。方中熟地黄、麸炒山药、酒萸肉补益肝肾,益髓填精;黄芪、当归补气生血,以血化精;淫羊藿温补肾阳,桑葚、桑寄生益阴敛营,增水盈精,三药相伍,阴阳并补,动静相依。此外,红花合当归、鸡血藤活血化瘀,配伍黄芪则补气行血。全方相伍,肾、肝、脾同养,气、血、精并补。
左卡尼汀是一种天然存在的化合物,被认为是治疗男性不育的潜在方法,可能改善精子参数[14]。左卡尼汀可以通过促进脂肪酸氧化为精子运动成熟提供能量,保护精子质膜免受活性氧的损害[15]。因此本研究采用左卡尼汀为阳性对照药物。此外本研究使用肾虚血瘀型OAS经典造模方法(GTW灌胃)。造模后,与空白组大鼠比较,肾虚血瘀型OAS大鼠出现精神萎靡,行动迟缓,反应迟钝,毛发稀疏,色泽晦暗,小便减少,大便干结,睾丸组织外表血管脉络充盈胀满,颜色紫黯,与以往报告相符[12]。表明GTW可造模肾虚血瘀型OAS大鼠模型,而补气活血益精方对症肾虚血瘀证型,并可有效改善症状。诸多研究表明,补肾活血类药物可改善睾丸组织病理损害[16]。补气活血益精方在长期临床及临床研究中取得了良好疗效,证明补气活血益精方可有效改善肾虚血瘀型OAS患者精子浓度及活力,但具体机制目前尚不清楚。
为此,我们构建了肾虚血瘀型OAS大鼠模型,之后给予补气活血益精方。结果显示,补气活血益精方治疗后,大鼠一般情况改善,睾丸及附睾指数上升,在大鼠精子活力方面,补气活血益精方高剂量组治疗效果更优,在精子浓度改善方面,左卡尼汀组优于补气活血益精方各治疗组。补气活血益精方也改善了大鼠精子的凋亡状况,以及大鼠精原细胞、初级精母细胞、成熟精子等生精细胞数量减少,曲细精管形态不规则等现象。这些结果都充分说明补气活血益精方对少弱精症的治疗作用,为进一步探明机制,进行了ELISA和q-PCR实验,结果表明,模型组ROS强度明显升高,Bax、Caspase-3 mRNA相对表达量明显增加,CAT、SOD等活性及Bcl-2 mRNA相对表达量明显降低,而补气活血益精方可以回调这些mRNA及抗氧化酶的改变。以上一系列结果说明,补气活血益精方发挥治疗作用很可能就是通过氧化应激及细胞凋亡途径实现的。
综上所述,本实验结果显示,补气活血益精方能够有效保护肾虚血瘀型OAS大鼠的精子细胞和睾丸组织损伤,其作用机制可能与调控氧化应激及细胞凋亡有关。本研究为少弱精症患者临床治疗提供了一定理论基础。
