Objective

To investigate the improving effect of ginseng (GS) on metoprolol(meto) induced bradycardia in mice with chronic heart failure(CHF)and its molecular mechanism.

Methods

The CHF model in C57BL/6J mouse was established through left anterior descending coronary artery ligation. Mice were randomly divided into 6 groups, including sham group(underwent the same surgical procedure without coronary artery ligation), model group, control group(26 mg·kg-1·d-1 metoprolol), experimental-low group(26 mg·kg-1·d-1 metoprolol + 1.3 g·kg-1·d-1 GS), experimental-midium group(26 mg·kg-1·d-1 metoprolol + 2.6 g·kg-1·d-1 GS)and experimental-high group(26 mg·kg-1·d-1 metoprolol + 5.2 g·kg-1·d-1 GS). Each group contained 9 mice. After continuous administration for 8 weeks, heart rate changes were monitored using non-invasive blood pressure monitors in small animals; transcriptome sequencing was employed to analyze differentially expressed genes in cardiac tissues with functional enrichment analysis; calcium ion concentration in myocardial tissue was measured using a calorimetric assay; Western blot analysis was used to detect relative expression levels of sarcoplasmic reticulum calcium ATPase 2a(SERCA2a), phosphorylated phospholamban(p-PLB)and sodium-calcium exchanger 1(NCX1)in myocardial tissue.

Results

The heart rates of sham group, model group, control group and experimental-L, -M, -H groups were(528.61±60.86),(448.67±84.58),(260.07±74.97),(352.84±40.47),(436.27±90.84)and(501.91±43.11)beats·min-1, respectively. Control group was compared with model group, experimental-L, -M, -H groups were compared with control group, the differences showed statistical significance in heart rates(P<0.05,P<0.01). Transcriptome gene ontology(GO)analysis revealed that differentially expressed genes were significantly enriched in pathways related to myocardial contraction and calcium ion transmembrane transport(all P<0.05). The myocardial tissue calcium ion concentrations in sham group, model group, control group and experimental-L, -M, -H groups were(30.09±2.36),(35.97±1.15),(16.15±2.37),(19.59±1.04), (23.64±0.54)and(28.54±2.82)mmol·L-1,respectively. Compared with control group, experimental-L, -M, -H groups all showed significantly increase(P<0.01, P<0.05). The relative expression levels of SERCA2a in sham group, model group, control group and experimental-L, -M, -H groups were 1.00±0.14, 0.83±0.05, 1.23±0.12, 1.00±0.03, 0.98±0.05 and 0.90±0.11, respectively; the relative expression levels of p-PLB were 1.38±0.24, 1.05±0.19, 2.12±0.35, 1.08±0.24, 0.54±0.57,and 0.52±0.13; while the relative expression levels of NCX1 were 1.00±0.13, 1.08±0.20, 1.69±0.34, 1.06±0.35, 1.15±0.22 and 0.81±0.21. Compared with model group, the relative expression levels of all 3 proteins in control group showed significant increases. Except for SERCA2a in experimental -L group and NCX1 in experimental -M group, the relative expression levels of the above three proteins in the experimental group were significantly lower than that in the control group (P<0.01, P<0.05).

Conclusion

Meto may induce bradycardia adverse drug reaction by increasing the p-PLB/PLB ratio and elevating the expression levels of SERCA2a and NCX1 proteins, which reduces intracellular free calcium ion concentration in cardiomyocytes. Ginseng could significantly down regulate the p-PLB/PLB ratio, increase the protein expression levels of SERCA2a and NCX1, and up regulate the concentration of intracellular free calcium ion, so as to improve Meto induced bradycardia, suggesting that it may antagonize the negative frequency effect of Meto by remodeling the calcium cycle homeostasis.

目的

探讨人参(GS)对美托洛尔(Meto)在慢性心力衰竭(CHF)小鼠中导致心动过缓的改善作用及其分子机制。

方法

通过冠状动脉左前降支结扎术建立C57BL/6J小鼠CHF模型。造模成功的小鼠随机分为:sham组(仅接受穿线操作,不进行结扎)、模型组、对照组(26 mg·kg-1·d-1 Meto处理)、低剂量实验组(26 mg·kg-1·d-1 Meto +1.3 g·kg-1·d-1 GS处理)、中剂量实验组(26 mg·kg-1·d-1 Meto +2.6 g·kg-1·d-1 GS处理)和高剂量实验组(26 mg·kg-1·d-1 Meto +5.2 g·kg-1·d-1 GS处理),每组9只。连续给药8周后,用小动物无创血压仪监测心率变化;用转录组测序法分析心脏组织差异基因并进行功能富集分析;用钙含量显色检测试剂盒法测定心肌组织中钙离子浓度;用蛋白质印迹法检测心肌组织肌浆网钙三磷酸腺苷酶2a(SERCA2a)、磷酸化受磷蛋白(p-PLB)和钠钙交换体1(NCX1)的相对表达水平。

结果

Sham组、模型组、对照组和低、中、高剂量实验组的心率分别为(528.61±60.86)、(448.67±84.58)、(260.07±74.97)、(352.84±40.47)、(436.27±90.84)和(501.91±43.11)beats·min-1,模型组与sham组相比,对照组与模型组相比,低、中、高剂量实验组与对照组相比,小鼠的心率均显著上升(P<0.05,P<0.01)。转录组基因本体论富集(GO)分析显示差异基因显著富集于心肌收缩和钙离子跨膜转运等通路(均P<0.05)。钙含量显色检测试剂盒测定发现,Sham组、模型组、对照组和低、中、高剂量试验组的心肌组织钙离子浓度分别为(30.09±2.36)、(35.97±1.15)、(16.15±2.37)、(19.59±1.04)、(23.64±0.54)和(28.54±2.82)mmol·L-1,与模型组相比,对照组的钙离子浓度显著降低;与对照组相比,低、中、高剂量试验组钙离子浓度均显著升高(P<0.01,P<0.05)。sham组、模型组、对照组和低、中、高剂量实验组心肌组织SERCA2a蛋白相对表达水平分别为1.00±0.14、0.83±0.05、1.23±0.12、1.00±0.03、0.98±0.05和0.90±0.11;p-PLB蛋白相对表达水平分别为1.38±0.24、1.05±0.19、2.12±0.35、1.08±0.24、0.54±0.57和0.52±0.13;NCX1蛋白相对表达水平分别为1.00±0.13、1.08±0.20、1.69±0.34、1.06±0.35、1.15±0.22和0.81±0.21,对照组的上述3种蛋白相对表达水平与模型组相比均显著升高;除SERCA2a低剂量组、NCX1中剂量组外,实验组上述3种蛋白相对表达水平与对照组相比均显著下降(P<0.01,P<0.05)。

结论

Meto可能通过上调p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平,使心肌细胞内游离钙离子浓度下降,从而导致心动过缓药物不良反应发生。人参能够显著下调Meto引起的p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平的增加,上调细胞内游离钙离子浓度,从而发挥改善Meto引起的心动过缓,提示其可能通过重塑钙循环稳态从而拮抗Meto的负性频率作用。

目的

探讨人参(GS)对美托洛尔(Meto)在慢性心力衰竭(CHF)小鼠中导致心动过缓的改善作用及其分子机制。

方法

通过冠状动脉左前降支结扎术建立C57BL/6J小鼠CHF模型。造模成功的小鼠随机分为:sham组(仅接受穿线操作,不进行结扎)、模型组、对照组(26 mg·kg-1·d-1 Meto处理)、低剂量实验组(26 mg·kg-1·d-1 Meto +1.3 g·kg-1·d-1 GS处理)、中剂量实验组(26 mg·kg-1·d-1 Meto +2.6 g·kg-1·d-1 GS处理)和高剂量实验组(26 mg·kg-1·d-1 Meto +5.2 g·kg-1·d-1 GS处理),每组9只。连续给药8周后,用小动物无创血压仪监测心率变化;用转录组测序法分析心脏组织差异基因并进行功能富集分析;用钙含量显色检测试剂盒法测定心肌组织中钙离子浓度;用蛋白质印迹法检测心肌组织肌浆网钙三磷酸腺苷酶2a(SERCA2a)、磷酸化受磷蛋白(p-PLB)和钠钙交换体1(NCX1)的相对表达水平。

结果

Sham组、模型组、对照组和低、中、高剂量实验组的心率分别为(528.61±60.86)、(448.67±84.58)、(260.07±74.97)、(352.84±40.47)、(436.27±90.84)和(501.91±43.11)beats·min-1,模型组与sham组相比,对照组与模型组相比,低、中、高剂量实验组与对照组相比,小鼠的心率均显著上升(P<0.05,P<0.01)。转录组基因本体论富集(GO)分析显示差异基因显著富集于心肌收缩和钙离子跨膜转运等通路(均P<0.05)。钙含量显色检测试剂盒测定发现,Sham组、模型组、对照组和低、中、高剂量试验组的心肌组织钙离子浓度分别为(30.09±2.36)、(35.97±1.15)、(16.15±2.37)、(19.59±1.04)、(23.64±0.54)和(28.54±2.82)mmol·L-1,与模型组相比,对照组的钙离子浓度显著降低;与对照组相比,低、中、高剂量试验组钙离子浓度均显著升高(P<0.01,P<0.05)。sham组、模型组、对照组和低、中、高剂量实验组心肌组织SERCA2a蛋白相对表达水平分别为1.00±0.14、0.83±0.05、1.23±0.12、1.00±0.03、0.98±0.05和0.90±0.11;p-PLB蛋白相对表达水平分别为1.38±0.24、1.05±0.19、2.12±0.35、1.08±0.24、0.54±0.57和0.52±0.13;NCX1蛋白相对表达水平分别为1.00±0.13、1.08±0.20、1.69±0.34、1.06±0.35、1.15±0.22和0.81±0.21,对照组的上述3种蛋白相对表达水平与模型组相比均显著升高;除SERCA2a低剂量组、NCX1中剂量组外,实验组上述3种蛋白相对表达水平与对照组相比均显著下降(P<0.01,P<0.05)。

结论

Meto可能通过上调p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平,使心肌细胞内游离钙离子浓度下降,从而导致心动过缓药物不良反应发生。人参能够显著下调Meto引起的p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平的增加,上调细胞内游离钙离子浓度,从而发挥改善Meto引起的心动过缓,提示其可能通过重塑钙循环稳态从而拮抗Meto的负性频率作用。

Objective

To investigate the improving effect of ginseng (GS) on metoprolol(meto) induced bradycardia in mice with chronic heart failure(CHF)and its molecular mechanism.

Methods

The CHF model in C57BL/6J mouse was established through left anterior descending coronary artery ligation. Mice were randomly divided into 6 groups, including sham group(underwent the same surgical procedure without coronary artery ligation), model group, control group(26 mg·kg-1·d-1 metoprolol), experimental-low group(26 mg·kg-1·d-1 metoprolol + 1.3 g·kg-1·d-1 GS), experimental-midium group(26 mg·kg-1·d-1 metoprolol + 2.6 g·kg-1·d-1 GS)and experimental-high group(26 mg·kg-1·d-1 metoprolol + 5.2 g·kg-1·d-1 GS). Each group contained 9 mice. After continuous administration for 8 weeks, heart rate changes were monitored using non-invasive blood pressure monitors in small animals; transcriptome sequencing was employed to analyze differentially expressed genes in cardiac tissues with functional enrichment analysis; calcium ion concentration in myocardial tissue was measured using a calorimetric assay; Western blot analysis was used to detect relative expression levels of sarcoplasmic reticulum calcium ATPase 2a(SERCA2a), phosphorylated phospholamban(p-PLB)and sodium-calcium exchanger 1(NCX1)in myocardial tissue.

Results

The heart rates of sham group, model group, control group and experimental-L, -M, -H groups were(528.61±60.86),(448.67±84.58),(260.07±74.97),(352.84±40.47),(436.27±90.84)and(501.91±43.11)beats·min-1, respectively. Control group was compared with model group, experimental-L, -M, -H groups were compared with control group, the differences showed statistical significance in heart rates(P<0.05,P<0.01). Transcriptome gene ontology(GO)analysis revealed that differentially expressed genes were significantly enriched in pathways related to myocardial contraction and calcium ion transmembrane transport(all P<0.05). The myocardial tissue calcium ion concentrations in sham group, model group, control group and experimental-L, -M, -H groups were(30.09±2.36),(35.97±1.15),(16.15±2.37),(19.59±1.04), (23.64±0.54)and(28.54±2.82)mmol·L-1,respectively. Compared with control group, experimental-L, -M, -H groups all showed significantly increase(P<0.01, P<0.05). The relative expression levels of SERCA2a in sham group, model group, control group and experimental-L, -M, -H groups were 1.00±0.14, 0.83±0.05, 1.23±0.12, 1.00±0.03, 0.98±0.05 and 0.90±0.11, respectively; the relative expression levels of p-PLB were 1.38±0.24, 1.05±0.19, 2.12±0.35, 1.08±0.24, 0.54±0.57,and 0.52±0.13; while the relative expression levels of NCX1 were 1.00±0.13, 1.08±0.20, 1.69±0.34, 1.06±0.35, 1.15±0.22 and 0.81±0.21. Compared with model group, the relative expression levels of all 3 proteins in control group showed significant increases. Except for SERCA2a in experimental -L group and NCX1 in experimental -M group, the relative expression levels of the above three proteins in the experimental group were significantly lower than that in the control group (P<0.01, P<0.05).

Conclusion

Meto may induce bradycardia adverse drug reaction by increasing the p-PLB/PLB ratio and elevating the expression levels of SERCA2a and NCX1 proteins, which reduces intracellular free calcium ion concentration in cardiomyocytes. Ginseng could significantly down regulate the p-PLB/PLB ratio, increase the protein expression levels of SERCA2a and NCX1, and up regulate the concentration of intracellular free calcium ion, so as to improve Meto induced bradycardia, suggesting that it may antagonize the negative frequency effect of Meto by remodeling the calcium cycle homeostasis.

慢性心力衰竭(chronic heart failure, CHF)是多种心血管疾病发展的终末阶段,其发病率与死亡率居高不下,已经成为全球公共卫生领域的重要挑战,对家庭及社会经济造成了巨大负担[1]。β受体阻滞剂作为CHF标准治疗的“金三角”药物之一,可通过抑制交感神经过度激活改善患者预后,其中美托洛尔(metoprolol, Meto)因其确切的临床疗效被广泛应用[2]。然而,该药物引发的剂量性心动过缓药物不良反应常导致治疗中断[3-4],严重影响临床疗效[5]。传统中药材人参(Panax ginseng C. A. Mey., GS),具有“大补元气、复脉固脱”之功效,现代药理学研究证实其具有显著的心血管保护作用[6]。本课题组前期研究发现,人参提取物可显著增强Meto对CHF模型动物的治疗作用,并发现其能有效改善Meto所致的心率下降现象[7]。这一发现为解决β受体阻滞剂的临床局限性提供了新思路,但其具体作用机制尚未阐明。因此,本研究通过建立CHF动物模型,系统评价GS对Meto所致心动过缓的改善作用,并探讨其潜在机制,旨在为临床联合用药提供实验依据。
8周龄 C57BL/6J 雄性小鼠54只,购买自北京华阜康生物技术有限公司,生产许可证号:SCXK(京)2019-0008。本研究经天津中医药大学动物伦理委员会批准(伦理批号:TCM-LAEC2023183c7761)。小鼠饲养于天津中医药大学动物中心,普通饲料适应性喂养1周,室温设定为(24±2)℃,相对湿度为40%~60%。
人参,批号:220707801,购于达仁堂中药饮片有限公司,由天津中医药大学中医药研究院常艳旭教授鉴定为正品;酒石酸美托洛尔片,批号:2212075,批准文号:国药准字H32025391,由阿斯利康药业(中国)有限公司生产;钙含量显色检测试剂盒,上海碧云天生物技术有限公司生产;兔抗受磷蛋白(phospholamban,PLB)抗体、兔抗磷酸化受磷蛋白(phosphorylated phospholamban,p-PLB)抗体和肌浆网钙三磷酸腺苷酶2a(sarcoplasmic reticulum calcium ATPase 2a,SERCA2a)抗体,均由武汉爱博泰克生物科技有限公司生产;兔抗钠钙交换体1(sodium-calcium exchanger 1,NCX1)抗体,武汉三鹰生物技术有限公司生产;山羊抗兔二抗,天津优抗生物技术有限公司生产。
小动物便携超声系统,江苏飞依诺公司产品;多通道小动物生理信号记录仪,澳大利亚Powerlab公司产品;小动物麻醉呼吸一体机,美国Kent Scientific公司产品;Spark 多功能酶标仪,瑞士Tecan公司产品;ChemiDoc MP蛋白质免疫印迹系统,美国伯乐Bio-Rad公司产品。
8周龄C57 BL/6J小鼠适应性饲养1周后,手术结扎小鼠冠状动脉左前降支。Sham组小鼠在开胸后,仅将缝合针穿过心脏,不结扎冠状动脉左前降支,术后继续喂养8周。
2020版《中国药典》[8]推荐人参临床生药剂量为3~9 g。根据小鼠与人的体表面积比,计算出对应的小鼠用药剂量为1.3 g·kg-1,低、中、高剂量实验组给药量分别为1.3、2.6与5.2 g·kg-1·d-1。人参饮片采用70%乙醇回流提取2次,每次2 h,合并2次提取液,旋转蒸发浓缩后冷冻干燥机干燥,最终得到冻干粉,提取率为25%。Meto治疗的临床使用剂量为200 mg·d-1 ,根据小鼠与人的体表面积比,计算得到实验使用的Meto剂量为26 mg·kg-1·d-1。手术造模8周后使用小动物便携超声仪器检测小鼠心功能,将造模成功的小鼠随机分为模型组(不进行处理)、对照组(26 mg·kg-1·d-1 Meto处理)、低剂量实验组(1.3 g·kg-1·d-1人参 + 26 mg·kg-1·d-1 Meto处理)、中剂量实验组(2.6 g·kg-1·d-1人参 + 26 mg·kg-1·d-1 Meto处理)和高剂量实验组(5.2 g·kg-1·d-1人参 + 26 mg·kg-1·d-1 Meto处理),未结扎冠状动脉左前降支的小鼠为sham组,每组9只。各组小鼠每天灌胃给药,sham组和模型组给予2%羧甲基纤维素钠(2% carboxymethyl cellulose sodium,2% CMC-Na)溶液,共给药8周。
给药8周后记录各组小鼠心率。将小鼠用固定器进行固定,依次将光纤袖带传感器(optical fiber cuff sensor,O-cuff)和容量压力记录传感器(volume pressure recording sensor,VPR)套至小鼠尾巴上,固定小鼠尾巴,连接仪器主机。等小鼠在固定器中稳定5 min后开始检测,每隔5 s测量1次,连续测量5次后,计算平均值。
脱颈处死小鼠后,打开胸腔,扎破心脏心耳,将注射器插入心尖部,用预冷0.9%NaCl灌流直至口鼻流出0.9%NaCl。灌流后去除心脏周围结缔组织,吸干水分,称质量后用锡纸包裹,存于-80 ℃冻存管中备用。
从-80 ℃的冰箱中取出小鼠心脏组织样本。每只小鼠心脏剪取约10 mg的左心室梗死区域和非梗死区域的组织,将其剪碎后放入1.5 mL EP管中。每组随机选取3只小鼠的样本,在干冰条件下送往上海美吉生物医药科技有限公司进行转录组测序。
将小鼠心脏组织剪成细碎小块,每10 mg心脏组织加入配制的裂解液100 μL裂解后离心,取上清液。取出标准品解冻后,严格按照钙含量显色检测试剂盒说明注书制备梯度浓度标准溶液。将标准溶液与样品溶液分别与显色液1:1混合,孵育后使用酶标仪测量575 nm处的光密度,制作标准曲线,计算得到样品中钙含量。
将小鼠心脏左心室组织10 mg剪成小块,置于EP管中,破碎裂解后离心,取上清液,双辛可宁酸法(bicinchoninic acid assay,BCA) 浓度测定后制备成蛋白样品。进行电泳,结束后将蛋白转移至聚偏二氟乙烯膜(polyvinylidene difluoride,PVDF)上,封闭2 h后,用含0.1% 吐温- 20的三羟甲基氨基甲烷缓冲液(tris buffered saline with tween-20,TBST)在摇床上清洗3次,每次6 min,将PVDF膜放入一抗中孵育过夜后,将膜用TBST清洗3次,至于二抗中孵育2 h,再次用TBST清膜3次,然后进行曝光显影,用Image J软件统计并分析各蛋白条带灰度值。
用SPSS 27.0软件行统计分析,用GraphPad Prism 9软件作统计图,实验数据均用表示,多组数据比较用单因素方差分析。
Sham组、模型组、对照组和低中高剂量试验组的小鼠心率分别为(528.61±60.86)、(448.67±84.58)、(260.07±74.97)、(352.84±40.47)、(436.27±90.84)和(501.91±43.11)beats·min-1。与sham组相比,模型组小鼠的心率著降低(P<0.01)。与模型组相比,对照组小鼠的心率显著降低(P<0.01)。与对照组相比,低、中、高剂量实验组小鼠的心率则均显著升高(P<0.01或0.05)。人参显著改善了Meto导致的小鼠心动过缓。
对小鼠心脏组织转录组分析进行基因本体论富集(gene ontology ,GO)分析,结果显示,差异基因主要与心肌收缩及钙离子跨膜转运的调节等过程相关,其中心肌收缩差异基因个数为13个(GO:0060048);钙离子跨膜转运的调节差异基因个数为21个(GO:1903169);钙离子释放到胞质溶胶的调控差异基因个为数17个(GO:0051279);肌节组织差异基因个数为15个(GO:0045214);心肌组织形态发生差异基因个数为14个(GO:0055008);肌动蛋白丝运动的调控差异基因个数为10个(GO:1903115)(均P<0.01)。
使用钙离子检测试剂盒检测各组小鼠心肌组织中钙含量,sham组、模型组、对照组和低、中、高剂量试验组的心脏组织钙离子浓度分别为(30.09±2.36)、(35.97±1.15)、(16.15±2.37)、(19.59±1.04)、(23.64±0.54)和(28.54±2.82)mmol·L-1,模型组与sham组相比,心脏组织钙离子浓度显著上升(P<0.05);对照组与模型组相比,心脏组织钙离子浓度显著下降(P<0.01),低、中、高剂量实验组小鼠与对照组相比,心脏组织钙离子浓度均显著升高(P<0.01,P<0.05),且与剂量呈正比例。
与sham组比较,模型组的SERCA2a、p-PLB和NCX1蛋白相对表达水平均显著下降;与模型组比较,对照组的蛋白相对表达水平均显著上升;除SERCA2a低剂量组、NCX1中剂量组外,实验组上述3种蛋白相对表达水平与对照组相比均显著下降(P<0.01,P<0.05),见表1图1
CHF作为多种心血管疾病的终末期阶段,其高发病率和高死亡率已成为全球性公共卫生问题[9]。根据最新流行病学调查数据,我国CHF患病率呈持续上升趋势,已成为城乡居民死亡的主要原因之一,给医疗卫生系统带来沉重负担。在CHF的药物治疗中,β受体阻滞剂Meto因其确切的临床疗效被列为指南推荐的一线用药[2],可以显著减少CHF患者的住院率和死亡率[10]。然而,临床研究发现有大量患者因药物诱发的心动过缓而被迫减量或停药,严重影响治疗效果[5]
本课题组在前期研究中发现,传统中药人参与Meto联用可产生显著的协同效应[7]。特别值得注意的是,26 mg·kg-1·d-1 Meto灌胃给药的CHF小鼠心率显著下降,人参提取物能有效预防Meto相关心动过缓的发生。这一现象提示人参可能通过特定机制调节心率,但其具体作用靶点尚未明确。
基于转录组学分析,我们发现Meto可显著影响心肌细胞钙调控相关基因的表达谱,其中SERCA2a、p-PLB和NCX1的表达变化尤为突出。这些蛋白共同构成了心肌细胞钙循环的核心调控网络,肌浆网上的SERCA2a蛋白负责肌浆网钙离子重摄取,使胞浆中的Ca2+含量减少[11-12]。PLB是一种由52个氨基酸组成的单基因跨膜蛋白,以五聚体的形式存在于肌浆网中,可以调节SERCA2a的活性[13]。非磷酸化的PLB以单体形式存在,可以与SERCA2a形成复合物,从而抑制SERCA2a活性;磷酸化的PLB与SERCA2a解离,重新形成五聚体,从而减轻对SERCA2a的抑制作用[14-15]。NCX1是位于质膜上具有细胞内外Na+/Ca2+离子交换功能的转运蛋白,可以将Na+转入细胞内,同时将Ca2+转出细胞[16]。既往研究发现[17-18],心衰时SERCA2a功能或含量下调,导致内质网内Ca2+含量减少,心肌细胞收缩功能下降,这与本研究中观察到的CHF小鼠心脏组织中SERCA2a下降一致。但与模型组相比,本研究观察到对照组给药后SERCA2a蛋白相对表达水平显著上升,提示Meto提高内质网Ca2+摄取能力,使胞浆游离Ca2+含量显著下降。此外,本研究还观察到与Sham组小鼠相比,模型小鼠心肌PLB磷酸化水平显著下降,与模型组相比,Meto可以显著上调p-PLB/PLB比值,解除了PLB对SERCA2a的抑制作用,引起细胞内游离钙离子浓度下降。同时NCX1表达显著增加导致钙离子外排,进一步减少细胞内游离钙离子。因此,Meto可能通过上调p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平,最终使心肌细胞内游离钙离子浓度下降,从而导致Meto的心动过缓的药物不良反应发生。值得注意的是,与对照组相比,使用人参进行处理可以逆转Meto引起的p-PLB/PLB比值、SERCA2a及NCX1蛋白相对表达水平的增加,上调细胞内游离钙离子浓度,从而缓解Meto导致的心动过缓,提示其可能通过重塑钙循环稳态从而拮抗Meto的负性频率作用。
本研究提示:人参可能通过重塑钙循环稳态拮抗美托洛尔的负性频率作用,改善美托洛尔导致的心动过缓,为临床解决β受体阻滞剂应用受限问题提供了新思路。但需要指出的是,本研究中心率变化与临床心动过缓的对应关系需进一步验证;其次,人参活性成分对钙调蛋白的直接作用机制仍有待于进一步阐明。
