Objective

To evaluate the bioequivalence of the test preparation and the reference preparation in a single dose of vortioxetine hydrobromide tablets under fasting and fed conditions in healthy volunteers.

Methods

A randomized, open-ended, single-dose, two-cycle, double-cross bioequivalence trial design was adopted, and 28 subjects were enrolled in the fasting group and the fed group, respectively, and 1 tablet of the test preparation and the reference preparation were taken in the fasting or fed state each cycle. The concentration of vortioxetine in plasma was determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS) method. The main pharmacokinetic parameters were calculated by Phoenix WinNonlin 8.1, and the bioequivalence was evaluated.

Results

The t1/2 for the fasting single oral administration of the test preparation and the reference preparation were (61.74±23.90) and (58.22±18.61)h, the median Tmax were (7.33±2.15) and (7.61±3.89) h, the Cmax were (7.32±1.90) and (7.46±1.98) ng·mL-1, and the AUC0-72 h were (312.61±92.95) and (310.00±93.84) h·ng·mL-1, respectively. The statistical results of the 90% confidence intervals of the main pharmacokinetic parameters Cmax and AUC0-72 h were 92.75%-103.71% and 97.47%-104.43%, respectively, all of which were within the range of 80.00%-125.00%, and the safety of the tested preparation and the reference preparation was good when taken orally on an empty stomach. The t1/2 of single oral administration after prandial administration of the tested preparation and the reference preparation were (77.60±33.87) and (81.61±45.24) h, the median Tmax were (8.06±3.02) and (7.77±2.45)h, the Cmax were (7.54±2.08) and (7.76±2.00) ng·mL-1, and the AUC0-72 h were (319.75±87.71) and (326.03±86.64) h·ng ·mL-1, respectively. The 90% confidence intervals of Cmax, AUC0-72 h were 89.00%-105.32% and 92.21%-102.72%, respectively, which were in the range of 80.00%-125.00%.

Conclusion

In the state of fasting and fed single oral administration, the two kinds of vortioxetine hydrobromide tablets have good bioequivalence.

目的

在空腹和餐后条件下,评价健康受试者单剂量口服氢溴酸伏硫西汀片受试制剂和参比制剂的生物等效性。

方法

空腹试验与餐后试验各入组28名受试者,用随机、开放、单剂量、两周期、双交叉生物等效性试验设计,每周期在空腹或餐后状态下服用受试制剂或参比制剂1片。用液相色谱-串联质谱法(LC-MS/MS)测定血浆中的伏硫西汀浓度,用Phoenix WinNonlin 8.1计算主要药代动力学参数,并进行生物等效性评估。

结果

目的

在空腹和餐后条件下,评价健康受试者单剂量口服氢溴酸伏硫西汀片受试制剂和参比制剂的生物等效性。

方法

空腹试验与餐后试验各入组28名受试者,用随机、开放、单剂量、两周期、双交叉生物等效性试验设计,每周期在空腹或餐后状态下服用受试制剂或参比制剂1片。用液相色谱-串联质谱法(LC-MS/MS)测定血浆中的伏硫西汀浓度,用Phoenix WinNonlin 8.1计算主要药代动力学参数,并进行生物等效性评估。

结论

在空腹与餐后的单次口服给药条件下,2种氢溴酸伏硫西汀片具有较好的生物等效性。

Objective

To evaluate the bioequivalence of the test preparation and the reference preparation in a single dose of vortioxetine hydrobromide tablets under fasting and fed conditions in healthy volunteers.

Methods

A randomized, open-ended, single-dose, two-cycle, double-cross bioequivalence trial design was adopted, and 28 subjects were enrolled in the fasting group and the fed group, respectively, and 1 tablet of the test preparation and the reference preparation were taken in the fasting or fed state each cycle. The concentration of vortioxetine in plasma was determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS) method. The main pharmacokinetic parameters were calculated by Phoenix WinNonlin 8.1, and the bioequivalence was evaluated.

Results

The t1/2 for the fasting single oral administration of the test preparation and the reference preparation were (61.74±23.90) and (58.22±18.61)h, the median Tmax were (7.33±2.15) and (7.61±3.89) h, the Cmax were (7.32±1.90) and (7.46±1.98) ng·mL-1, and the AUC0-72 h were (312.61±92.95) and (310.00±93.84) h·ng·mL-1, respectively. The statistical results of the 90% confidence intervals of the main pharmacokinetic parameters Cmax and AUC0-72 h were 92.75%-103.71% and 97.47%-104.43%, respectively, all of which were within the range of 80.00%-125.00%, and the safety of the tested preparation and the reference preparation was good when taken orally on an empty stomach. The t1/2 of single oral administration after prandial administration of the tested preparation and the reference preparation were (77.60±33.87) and (81.61±45.24) h, the median Tmax were (8.06±3.02) and (7.77±2.45)h, the Cmax were (7.54±2.08) and (7.76±2.00) ng·mL-1, and the AUC0-72 h were (319.75±87.71) and (326.03±86.64) h·ng ·mL-1, respectively. The 90% confidence intervals of Cmax, AUC0-72 h were 89.00%-105.32% and 92.21%-102.72%, respectively, which were in the range of 80.00%-125.00%.

Conclusion

In the state of fasting and fed single oral administration, the two kinds of vortioxetine hydrobromide tablets have good bioequivalence.

抑郁症(major depressive disorder,MDD)已被列为全球范围内主要的非致命性健康损害疾病之一,中国MDD患者超过5 400万例,终身患病率约为6.90%[1];抑郁症发病率呈逐年增高的趋势[2],目前药物治疗是主要手段。伏硫西汀是一种研究性抗抑郁药,目前被批准用于治疗重度MDD。氢溴酸伏硫西汀于2013年美国食品药品监督管理局(Food and Drug Administration, FDA)和欧洲药品管理局(European Medicines Agenly, EMA)获批上市[3],2018年在中国正式上市。本研究以国产氢溴酸伏硫西汀片(规格:20 mg)为受试制剂,以原研公司生产的氢溴酸伏硫西汀片(规格:20 mg,商品名:TRINTELLIX)为参比制剂,评价国产制剂和参比制剂之间等效性和安全性。
根据FDA《氢溴酸伏硫西汀片生物等效性草案》、原研说明书及文献报道[3],氢溴酸伏硫西汀属于长半衰期药物,且个体内消除变异性较小,故用截取至72 h的AUC0-72 h来替代AUC,评估2制剂在空腹和餐后条件下的生物等效性。
药品与试剂 受试制剂(test,T):氢溴酸伏硫西汀片,规格:每片20 mg,批号:2212261,安徽金太阳生化药业有限公司生产;参比制剂(reference,R):氢溴酸伏硫西汀片,规格:每片20 mg,批号:12295662,意大利Takeda Manufacturing SPA生产;对照品:伏硫西汀(vortioxetine),含量:99.4%,批号:1744-018A1,加拿大TLC公司生产;内标:伏硫西汀-d8(vortioxetine-d8),含量:98.5%,批号:1898-087A6,加拿大TLC公司生产。
仪器 液相色谱系统(包括LC-20ADXR液相色谱泵,CTO-20A柱温箱,SIL-30ACMP自动进样器)、LCMS-8060 质谱仪,均由日本岛津公司生产。
根据预试验结果及调研资料,空腹和餐后试验的变异系数均设定为10%,显著水平系数为0.05。几何均值比在空腹试验中设置为87.15%,在餐后试验中设置为114.98%,把握度定为80%。考虑到20%的脱落率,计算结果显示,空腹和餐后试验分别需要24例和26例受试者。综合临床试验登记平台的信息,最终确定正式试验的例数为空腹和餐后试验各28例。
本研究经蚌埠医学院第一附属医院临床医学研究伦理委员会批准,伦理批号:蚌医一附院临床医学研究伦理审[2023]106号,临床试验登记号:CTR20233479。共筛选出112例受试者,最终纳入56例受试者,均自愿签署知情同意书。空腹组共纳入28例受试者,其中男性15例,女性13例,年龄(34.29±8.64)周岁;体重(64.27±8.00) kg;身体质量指数(body mass index, BMI)(23.09±1.77) kg·m-2。餐后组同样纳入28例受试者,其中男性19例,女性9例,年龄(33.96±8.92)岁;体质量(66.57±8.09)kg;BMI(23.72±1.76) kg·m-2
入选标准 ①健康男/女性志愿受试者,年龄18~55岁(包含18和55岁);②男性体质量不低于50.0 kg,女性体重不低于45.0 kg,BMI在19.0~26.0 kg·m-2范围内(包括边界值);③受试者已充分知情,并自愿签署知情同意书。
排除标准 ①既往或现有癫痫发作病史或有不稳定型癫痫者;②筛选前4周内使用过与伏硫西汀有相互作用的药物;③既往或现有精神疾病史、青光眼史、异常出血史(如胃肠道出血、皮肤异常淤血)者;④筛选前2周内服用过任何处方药或非处方药者;⑤筛选前1个月内接种过疫苗者;⑥有过敏病史或已知对试验用药及其辅料过敏者等。
本试验用随机、开放、单剂量、两周期、双交叉生物等效性试验设计,每周期在空腹或餐后状态下服用受试制剂或参比制剂1片。
入选合格的56名受试者服用受试制剂或参比制剂的顺序,将根据受试者随机编码表确定,由随机化专员用SAS 9.4软件产生随机表。随机方法为区组随机分配,不同给药序列组受试者分配比为1:1。
受试者在服药前1 d入住Ⅰ期病房,接受统一管理。空腹组和餐后组按照随机表服用T/ R。在早上约08:00第1例受试者开始服药,受试者在空腹或餐后状态下用240 mL温水送服口服T或R 1片。服药后要检查受试者双手、口腔和盛药容器,以确保药物的正确及完全服用。服药前后1 h禁水,服药至少4 h后进食标准餐。洗脱期30 d,第二周期服药时间同第一周期一致。
空腹组和餐后组采血点:给药前0 h(给药前1 h内)和给药后2、3、4、5、5.5、6、6.5、7、7.5、8、8.5、9、10、11、12、15、24、36、48、72 h共21个采血点,每个采血点采集静脉血4 mL至含K2EDTA抗凝剂的真空采血管中。
色谱柱为GL Sciences Inc.;InertSustain C18(3 μm, 2.10 mm×50 mm);流动相A和流动相B分别为0.1% 甲酸(formic acid,FA)水溶液和100% 乙腈(acetonitrile,ACN)溶液(含0.1% FA);洗液R0和洗液R3分别为50% ACN溶液和ACN/甲醇 (1:1, v:v) 溶液;流速为0.40 mL·min-1;进样量为5.0 μL;柱温为40 ℃;初始流动相B比例为40.0%。
离子化使用电喷雾电离(electrospray ionization,ESI)模式;离子源温度和喷射电压分别为500 ℃和5 500 v;雾化气(Gas 1,N2)、辅助气(Gas 2,N2)和干燥气流量分别为3.00、10.00和10.00 L·min-1;加热块温度为400 ℃;正离子模式,多重反应监测(multiple reaction monitoring,MRM)。用于检测的分析物和内标离子对分别为m/z=299.10/150.00与m/z=307.15/153.05。
取样品50.0 μL转移至96孔板,加入50.0 μL 内标溶液,涡旋5 min;加入1%氨水50.0 μL,涡旋5 min;再加入100%乙酸乙酯600 μL,涡旋10 min,在4 ℃条件下4 822 r·min-1离心10 min。将96孔板剧烈振荡5 min(1 200 r·pm),4 ℃ 离心5 min(4 000 r·pm)。在新的96孔板内转移上清溶液300.0 μL,氮气吹干后加入40% ACN 200.0 μL复溶,涡旋5 min,待进样。
将来自不同供体的 6 批空白血浆与标准曲线样本一同进样操作,以确认空白血浆中不存在明显干扰成分。伏硫西汀和伏硫西汀- d8的保留时间为1.028 min和1.022 min,表明空白血浆中的内源性物质对伏硫西汀及内标测定无干扰作用,见图1
以氢溴酸伏硫西汀与氢溴酸伏硫西汀-d8的浓度比作为横坐标,2者的峰面积比值作为纵坐标,运用最小二乘法(权重因子为1/C2)进行回归分析,从而得到标准曲线回归方程为y=2 509.02×10-3x-7 213.26×10-7R2=0.997 1。样品浓度通过标准曲线进行定量确定,氢溴酸伏硫西汀的标准曲线线性范围为0.05~20.00 ng·mL-1,定量下限为0.05 ng·mL-1,表明具有良好的线性关系。
配制质量浓度为0.05、0.15、1.00、8.00、15.00 ng·mL-1伏硫西汀的质控样品。按照“血浆样品处理”方法,将每个浓度水平的样品均分为6份,分别在3个独立分析批次中进行测定,用于评估批内精密度、批间精密度以及绝对提取回收率。在每个浓度水平下,质控样品的批内平均偏差(%RE)应控制在15.0%以内。伏硫西汀在血浆中的精密度和回收率如表1
考察6个不同来源的空白基质配制的高、中、低浓度水平的分析物,经过内标归一化分析结果显示,伏硫西汀的内标归一化因子分别为1.00、1.00和0.99,且所有批次的内标归一化因子的变异系数(%CV)≤15.0%。这表明所用方法中基质效应对氢溴酸伏硫西汀的测定无影响。
氢溴酸伏硫西汀的储存液和工作液在室温放置23 h以及-20 ℃存放93 d均保持稳定。此外,2个质控浓度水平的样品在经过5次冻融循环(-20 ℃冷冻冰箱或-60 ℃以下冰箱储存,室温白光下融化)后仍稳定。同时,血浆中的待测物在-20 ℃存放95 d也表现出良好的稳定性。
用WinNonlin 8.1软件进行非房室模型药代动力学参数的估算分析,用SAS 9.4软件进行统计分析。将主要药代动力学参数(Cmax、AUC0-72 h)经对数转换后进行方差分析、双单侧t检验和90%置信区间分析,Tmax进行非参数检验法分析,计算受试制剂与参比制剂中伏硫西汀Cmax和AUC0-72 h的几何均值比及其90%置信区间。若该置信区间落在等效范围(80.00%~125.00%)内,则判定受试制剂与参比制剂具有生物等效性。
空腹试验共纳入28例受试者,1例脱落,最终27例受试者完成试验。餐后试验同样纳入28例受试者,2例脱落,26例受试者完成试验。健康受试者在空腹和餐后状态下单次口服20 mg氢溴酸伏硫西汀片(受试制剂T/参比制剂R)后,2制剂的血药浓度-时间曲线分别如图2所示。

在空腹状态下氢溴酸伏硫西汀受试制剂与参比制剂的Cmax和AUC0-72 h几何均值比的90%置信区间分别为92.75%~103.71%和97.47%~104.43%;在餐后状态下对应的90%置信区间分别为89.00%~105.32%和92.21%~102.72%。两制剂PK参数(Cmax、AUC0-72 h)的几何均值比及其90%置信区间均在80.00%~125.00%范围内,符合生物等效性标准。
试验期间,无受试者发生严重不良事件。空腹组有6例(21.43%)受试者发生10次药物不良反应,餐后组有8例(28.57%)受试者发生9例次药物不良反应。空腹组和餐后组不良事件严重程度均为1级或2级,均随访至不良事件结束。不良事件主要为“各类检查”、“代谢及营养类疾病”、“胃肠系统疾病”和“各类神经系统疾病”等。除空腹组1例受试者自行退出外,无受试者因不良事件退出试验。
2018年伏硫西汀在中国上市,国内相关临床试验文献较少。临床常用高效抗抑郁药艾司西酞普兰[4]治疗抑郁症,但研究表明,伏硫西汀在中国人群中的疗效与艾司西酞普兰相当[5],且与舍曲林、文拉法辛等药物相比,疗效相当或更优,同时具有更低的药物不良反应发生率[6-8]。其治疗有效率和患者接受度均优于多数同类药物。临床实践证实,伏硫西汀治疗重度抑郁症有效、安全且耐受性良好,能显著改善抑郁严重程度、认知功能及功能障碍[9-10]
本试验表明,该制剂为变异度较低(<20%)的长半衰期(t1/2>24h)品种,符合采用AUC0-72 h来代替AUC0-t和AUC0-∞的情况。且主要药代动力学参数与既往研究基本一致[3],微小差异可能源于药物规格、受试者个体差异或饮食因素。日本MATSUNO等[11]研究也显示:伏硫西汀在日本成人中吸收和消除缓慢,无显著性别差异,空腹与进食状态下药代动力学相似,安全性良好,与本试验结论吻合。
本次试验健康受试者在空腹组和餐后组,口服受试制剂(T)氢溴酸伏硫西汀片(规格:20mg)与参比制剂(R)(商品名:TRINTELLIX、规格:20mg)氢溴酸伏硫西汀片后,2制剂的主要药代动力学参数的Cmax、AUC0-72 h几何均值比的90%置信区间均落在80.00%~125.00%,故受试制剂和参比制剂在空腹和餐后条件下均等效。且试验期间,未发生严重不良事件,安全性良好。
