Objective

To study the efficacy and safety of allisartan isoproxil tablet combined with indapamide tablet in the treatment of patients with mild to moderate essential hypertension and coronary heart disease.

Methods

Patients with mild to moderate essential hypertension and coronary heart disease were divided into treatment group and control group using the cohort method. The control group was given oral indapamide tablets 2.5 mg once a day based on the conventional treatment regimen. The treatment group was given allisartan isoproxil tablets 240 mg once a day in addition to the control group’s regimen for a total of 12 weeks. The clinical efficacy, 24-hour blood pressure variability, cardiac function, vascular endothelial function and safety evaluation of the two groups were compared.

Results

A total of 105 patients were enrolled, including 54 patients in treatment group and 51 patients in control group. After treatment, the total clinical effective rate of the treatment group was 90.74% (49 cases/54 cases), and that of control group was 72.55% (37 cases/51 cases), which was significantly higher in treatment group than in control group (P<0.05). After treatment, the daytime (d) systolic blood pressure variability (SBPV) levels in treatment group and control group were (11.32±2.13) and (12.48±2.26) mmHg, respectively; the nighttime (n) SBPV levels were (10.03±1.79) and (10.82±2.10) mmHg, respectively; the d diastolic blood pressure variability (DBPV) levels were (8.66±1.51) and (9.36±1.57) mmHg, respectively; the nDBPV levels were (8.05±1.32) and (8.68±1.62) mmHg, respectively; the 24 h SBPV levels were (10.85±2.20) and (11.96±2.05) mmHg, respectively; the 24 h DBPV levels were (9.67±1.93) and (10.66±1.92) mmHg, respectively; the brain natriuretic peptide (BNP) levels were (83.47±10.53) and (89.41±13.19) ng·L-1, respectively; the endothelin-1 (ET-1) levels were (55.44±9.27) and (60.36±10.86) ng·L-1, respectively; and the Apelin levels were (36.44±6.41) and (34.22±4.37) ng·mL-1, respectively. The above metrics showed significant differences between the two groups (P<0.05,P<0.01). The adverse drug reactions in treatment group included diarrhea, fever, fatigue, palpitations, soreness in both knee joints, cough, insomnia, decreased appetite and orthostatic hypotension. The adverse drug reactions in control group included diarrhea, headache, decreased appetite, insomnia and orthostatic hypotension. The total incidence of adverse drug reactions in treatment group was 22.22% (12 cases /54 cases), and that in control group was 17.65% (9 cases /51 cases). There was no statistically significant difference (P>0.05).

Conclusion

The application of allisartan isoproxil combined with indapamide in treatment of patients with mild to moderate essential hypertension and coronary heart disease can achieve significant therapeutic effects, regulate 24-hour blood pressure variability, improve cardiac function, vascular endothelial function, and quality of life, also demonstrate good safety.

目的

研究阿利沙坦酯片联合吲达帕胺片治疗世居青海轻中度原发性高血压合并冠心病患者的临床疗效及安全性。

方法

将轻中度原发性高血压合并冠心病患者用队列法分为试验组与对照组。对照组在常规治疗方案基础上口服吲达帕胺片,每次2.5 mg,每天1次,试验组在对照组基础上加用阿利沙坦酯片,每次240 mg,每天1次,共用药12周。比较2组患者的临床疗效、24 h血压变异性、心功能和血管内皮功能,并进行安全性评价。

结果

结论

轻中度原发性高血压合并冠心病患者应用阿利沙坦酯联合吲达帕胺治疗,能取得显著的治疗效果,调节24 h血压变异性,改善心功能、血管内皮功能及生活质量,并且安全性方面也表现良好。

目的

研究阿利沙坦酯片联合吲达帕胺片治疗世居青海轻中度原发性高血压合并冠心病患者的临床疗效及安全性。

方法

将轻中度原发性高血压合并冠心病患者用队列法分为试验组与对照组。对照组在常规治疗方案基础上口服吲达帕胺片,每次2.5 mg,每天1次,试验组在对照组基础上加用阿利沙坦酯片,每次240 mg,每天1次,共用药12周。比较2组患者的临床疗效、24 h血压变异性、心功能和血管内皮功能,并进行安全性评价。

结果

共纳入105例患者,试验组54例、对照组51例。治疗后试验组总有效率为90.74%(49例/54例),对照组为72.55%(37例/51例),试验组显著高于对照组(P<0.05)。治疗后,试验组和对照组白昼(d)收缩压变异性(SBPV)水平分别为(11.32±2.13)和(12.48±2.26)mmHg,夜间(n)SBPV水平分别为(10.03±1.79)和(10.82±2.10)mmHg,d舒张压变异性(DBPV)水平分别为(8.66±1.51)和(9.36±1.57)mmHg,nDBPV水平分别为(8.05±1.32)和(8.68±1.62)mmHg,24 h SBPV水平分别为(10.85±2.20)和(11.96±2.05)mmHg,24 h DBPV水平分别为(9.67±1.93)和(10.66±1.92)mmHg,B型脑钠肽(BNP)水平分别为(83.47±10.53)和(89.41±13.19)ng·L-1,内皮素-1(ET-1)水平分别为(55.44±9.27)和(60.36±10.86)ng·L-1,血管活性肽(Apelin)水平分别为(36.44±6.41)和(34.22±4.37)ng·mL-1,上述指标在2组间比较,在统计学上差异均有统计学意义(P<0.05,P<0.01)。试验组药物不良反应有失眠、心悸、体位性低血压、咳嗽、食欲降低、腹泻、双膝关节酸痛、发热和乏力,对照组药物不良反应有头痛、失眠、直立性低血压、腹泻和食欲降低,试验组药物不良反应总发生率为22.22%(12例/54例),对照组为17.65%(9例/51例),在统计学上差异无统计学意义(P>0.05)。

结论

轻中度原发性高血压合并冠心病患者应用阿利沙坦酯联合吲达帕胺治疗,能取得显著的治疗效果,调节24 h血压变异性,改善心功能、血管内皮功能及生活质量,并且安全性方面也表现良好。

Objective

To study the efficacy and safety of allisartan isoproxil tablet combined with indapamide tablet in the treatment of patients with mild to moderate essential hypertension and coronary heart disease.

Methods

Patients with mild to moderate essential hypertension and coronary heart disease were divided into treatment group and control group using the cohort method. The control group was given oral indapamide tablets 2.5 mg once a day based on the conventional treatment regimen. The treatment group was given allisartan isoproxil tablets 240 mg once a day in addition to the control group’s regimen for a total of 12 weeks. The clinical efficacy, 24-hour blood pressure variability, cardiac function, vascular endothelial function and safety evaluation of the two groups were compared.

Results

A total of 105 patients were enrolled, including 54 patients in treatment group and 51 patients in control group. After treatment, the total clinical effective rate of the treatment group was 90.74% (49 cases/54 cases), and that of control group was 72.55% (37 cases/51 cases), which was significantly higher in treatment group than in control group (P<0.05). After treatment, the daytime (d) systolic blood pressure variability (SBPV) levels in treatment group and control group were (11.32±2.13) and (12.48±2.26) mmHg, respectively; the nighttime (n) SBPV levels were (10.03±1.79) and (10.82±2.10) mmHg, respectively; the d diastolic blood pressure variability (DBPV) levels were (8.66±1.51) and (9.36±1.57) mmHg, respectively; the nDBPV levels were (8.05±1.32) and (8.68±1.62) mmHg, respectively; the 24 h SBPV levels were (10.85±2.20) and (11.96±2.05) mmHg, respectively; the 24 h DBPV levels were (9.67±1.93) and (10.66±1.92) mmHg, respectively; the brain natriuretic peptide (BNP) levels were (83.47±10.53) and (89.41±13.19) ng·L-1, respectively; the endothelin-1 (ET-1) levels were (55.44±9.27) and (60.36±10.86) ng·L-1, respectively; and the Apelin levels were (36.44±6.41) and (34.22±4.37) ng·mL-1, respectively. The above metrics showed significant differences between the two groups (P<0.05,P<0.01). The adverse drug reactions in treatment group included diarrhea, fever, fatigue, palpitations, soreness in both knee joints, cough, insomnia, decreased appetite and orthostatic hypotension. The adverse drug reactions in control group included diarrhea, headache, decreased appetite, insomnia and orthostatic hypotension. The total incidence of adverse drug reactions in treatment group was 22.22% (12 cases /54 cases), and that in control group was 17.65% (9 cases /51 cases). There was no statistically significant difference (P>0.05).

Conclusion

The application of allisartan isoproxil combined with indapamide in treatment of patients with mild to moderate essential hypertension and coronary heart disease can achieve significant therapeutic effects, regulate 24-hour blood pressure variability, improve cardiac function, vascular endothelial function, and quality of life, also demonstrate good safety.

青海地区海拔较高、氧气含量较少、大气压力较低,在这种氧低气压环境下,血管内皮细胞易受损,进而引发功能失调,增加原发性高血压风险[1]。作为冠心病的重要危险因素,长期高血压可损害血管内膜功能,加速血管粥样硬化,引发冠心病[2]。单一降压药物疗效受限,如β受体阻滞剂可降低运动耐量,肾素-血管紧张素-醛固酮系统阻滞剂在极高海拔地区的降压效果有限等,且高原低氧状态会加剧血压波动[3]。吲达帕胺是磺胺类利尿药,对高血压合并冠心病患者的疗效较好[4]。阿利沙坦酯能全面、持续性地实现高效降压[5]。本研究以世居青海的轻中度原发性高血压合并冠心病患者为对象,旨在探讨阿利沙坦酯联合吲达帕胺治疗的临床疗效及安全性,以期为该地区患者的药物治疗提供临床数据支持。
2021年1月至2024年1月以本院收治的世居青海的轻中度原发性高血压合并冠心病患者为研究对象。本研究经青海大学附属医院伦理委员会批准(伦理批号:P-SL-202408)。所有患者均签署知情同意书。
纳入标准 符合《中国高血压防治指南2018年修订版》[6]中关于轻中度原发性高血压的诊断标准者:没有实施降压药治疗,分别在不同的日期完成3次血压测量,收缩压(systolic blood pressure,SBP)处于140~179 mmHg(含140 mmHg和179 mmHg)和(或)舒张压(diastolic blood pressure,DBP)处于90~109 mmHg(含90 mmHg和109 mmHg);符合《2019年ESC慢性冠状动脉综合征诊断和管理指南》[7]中关于冠心病的诊断标准者:患者出现胸痛、胸闷、放射痛、呼吸困难、乏力、恶心等临床症状,心电图显示ST段抬高或压低、T波倒置,冠脉造影显示冠状动脉呈现不同程度狭窄;年龄≥18岁者;高血压分级1~2级者;纽约心脏病协会心功能分级Ⅰ~Ⅲ级[8]者;世居青海者;临床资料完整者。
排除标准 继发性高血压者,严重心律失常、心房颤动者,糖尿病血糖控制不佳者,入组前6个月内有急性冠脉综合征、脑血管出血者,既往行心脏相关手术(心脏支架植入术、心脏起搏器置入术等)者,妊娠期或哺乳期妇女,肝、肾功能严重损害者,合并慢性消耗性疾病或恶性肿瘤者,患有精神类疾病者。
吲达帕胺片,规格:每片2.5 mg,批号:20200508、20200604、20220310、20230505、2311081,批准文号:国药准字H37022368,烟台巨先药业有限公司生产;阿利沙坦酯片,规格:每片240 mg,批号:C190523、C200295、C210571、B240834,批准文号:H20138002,深圳信立泰药业股份有限公司生产。一氧化氮(nitric oxide,NO)试剂盒,上海科艾博生物技术有限公司生产;B型脑钠肽(brain natriuretic peptide,BNP)、内皮素-1(endothelin-1,ET-1)、同型半胱氨酸(homocysteine,Hcy)和血管活性肽(Apelin)试剂盒,均由上海酶研生物科技有限公司生产。
DS-25动态血压监测仪,日本尼士公司产品;Vivid E9彩色多普勒超声诊断仪,美国GE公司产品;Multiskan skyhigh酶标仪,美国赛默飞世尔公司产品。
将轻中度原发性高血压合并冠心病患者按队列法分为试验组与对照组。2组均进行控糖、降脂等基础治疗;并予以冠心病常规治疗药物(包括β受体阻滞剂、钙通道阻滞剂、抗血小板或抗凝药物等)治疗。对照组在常规治疗方案基础上口服吲达帕胺片,每次2.5 mg,每天1次,共用药12 周。试验组在对照组基础上加用阿利沙坦酯片,每次240 mg,每天1次,共用药12 周。
24 h血压变异参数[9] 在治疗前和治疗12周后,用动态血压监测仪记录白昼(daytime, d)、夜间(nighttime, n)及24 h的SBP和DBP水平,白昼的SBP和DBP在6:00~22:00间监测,每间隔30 min测量;夜间的SBP和DBP在22:00~6:00间监测,每间隔60 min测量,随后依据Parati法计算血压变异参数[10],收缩压变异参数(systolic blood pressure variability,SBPV)和舒张压变异参数(diastolic blood pressure variability,DBPV)在不同时段(d、n、24 h)的水平分别按照式(1)和式(2)计算。
心功能[11] 在治疗前和治疗12 周后,用心脏超声仪检测左心室收缩末期内径(left ventricular end systolic diameter,LVESD)和左室射血分数(left ventricular ejection fraction,LVEF)水平;抽取静脉血分离血清,用酶联免疫吸附实验法检测BNP水平。
血管内皮功能[12] 在治疗前和治疗后,以酶联免疫吸附实验法测定NO、ET-1、Hcy和Apelin水平。
生活质量[13] 在治疗前和治疗后用西雅图心绞痛评价量表(Seattle angina questionnaire,SAQ)评估生活质量,共有5个维度,19个条目,标准积分=(实际得分-该维度最低得分)/(该维度最高分-该维度最低分)×100,分数越高反映越好的生活质量和机体功能状态。
临床疗效评价[14-15] 显效为DBP较治疗前降低>10 mmHg或恢复至正常值,或SBP降低>20 mmHg,心绞痛发作频率降低>80%,或心电图提示心肌缺血征象消失;有效为DBP降低<10 mmHg,或SBP降低范围10~20 mmHg,心绞痛发作频率降低50%~79%;无效为血压不符合以上标准,心绞痛仍发作或心肌缺血症状没得到改善,冠心病发作频率没出现下降。总有效率由显效和有效病例数之和占总病例数的百分比计算得出。
安全性评价[16] 对用药过程中出现的药物不良反应进行记录。
用SPSS 26.0软件进行统计分析,计量资料以表示,组内比较用配对样本t检验,组间比较用独立样本t检验,计数资料以率(%)表示,比较用χ2Fisher检验。
共入组105例,试验组54例、对照组51例。试验组和对照组患者的性别、年龄、民族、身体质量指数、病程、高血压分级、心功能分级、合并疾病等一般资料比较,在统计学上差异均无统计学意义(均P>0.05),见表1
治疗前,试验组和对照组患者血压和24 h血压变异性指标水平在统计学上差异均无统计学意义(均P>0.05);治疗后,试验组和对照组的上述指标均显著下降,且试验组的上述指标较对照组显著降低(均P<0.05,P<0.01,P<0.001),见表2表3
治疗前,试验组和对照组的LVESD水平分别为(60.29±9.95)和(59.71±8.79)mm,LVEF水平分别为(41.51±6.78)%和(41.50±6.20)%,BNP水平分别为(265.61±38.63)和(262.58±37.88)ng·L-1,上述指标在2组间比较,在统计学上差异均无统计学意义(均P>0.05);治疗后,试验组和对照组的LVESD水平分别为(48.80±6.68)和(51.94±5.85)mm,LVEF水平分别为(53.66±10.44)%和(49.75±7.84)%,BNP水平分别为(83.47±10.53)和(89.41±13.19)ng·L-1;与治疗前相比,试验组和对照组的LVEF水平均显著升高,且试验组显著高于对照组(均P<0.05)。与治疗前相比,试验组和对照组的LVESD和BNP水平均显著降低,且试验组均显著低于对照组(均P<0.05)。
治疗前,试验组和对照组患者的血管内皮功能指标水平在统计学上差异均无统计学意义(均P>0.05)。治疗后,试验组和对照组的ET-1和Hcy水平均显著降低,且试验组的上述指标显著低于对照组(P<0.05,P<0.01,P<0.001);试验组和对照组的NO和Apelin水平均显著升高,且试验组的上述指标显著高于对照组(P<0.05,P<0.001),见表4
治疗前,试验组和对照组患者的躯体活动受限程度评分分别为(56.57±8.88)和(55.63±10.87)分,心绞痛稳定状态评分分别为(43.24±7.78)和(44.59±7.96)分,心绞痛发作情况评分分别为(52.98±12.93)和(51.98±10.22)分,治疗满意程度评分分别为(52.46±7.51)和(54.25±7.90)分,疾病认知程度评分分别为(30.76±5.02)和(29.18±5.05)分,2组间比较,在统计学上差异均无统计学意义(均P>0.05);治疗后,试验组和对照组躯体活动受限程度评分分别为(77.85±12.04)和(72.10±12.37)分,心绞痛稳定状态评分分别为(79.94±12.63)和(73.69±12.74)分,心绞痛发作情况评分分别为(76.52±13.52)和(71.18±11.79)分,治疗满意程度评分分别为(79.06±11.52)和(73.88±12.30)分,疾病认知程度评分分别为(66.02±10.65)和(59.84±9.98)分;治疗后,试验组各评分均显著升高,且试验组评分均显著高于对照组(均P<0.05)。
试验组的总有效率为90.74%,对照组为72.55%,试验组显著高于对照组(P<0.05),见表5
试验组出现失眠2例、心悸1例、体位性低血压1例、咳嗽2例、食欲降低1例、腹泻1例、双膝关节酸痛1例、发热1例、乏力2例;对照组出现头痛2例、失眠3例、直立性低血压1例、食欲降低1例、腹泻2例;2组的药物不良反应发生率(22.22% vs 17.65%)比较,在统计学上差异无统计学意义(P>0.05)。
青海地区具有高原低氧的特征,再加上居民独特的饮食结构和遗传等因素影响,导致原发性高血压成为该地区面临的重大疾病负担之一。原发性高血压发病率较高,需终身服药,是引起冠心病的重要因素之一。阿利沙坦酯能够阻断血管紧张素Ⅱ与其受体之间的结合,实现血管扩张、血压下降、改善心脏功能的效果。周从良等[17]研究指出,对于轻中度老年高血压患者,应用阿利沙坦酯可有效改善血压及血压变异性,同时保护靶器官。吲达帕胺主要通过抑制远端小管对钠和氯化物的重吸收来发挥降压作用。王鸿懿等[18]研究指出,阿利沙坦酯单药治疗未达标的患者,联用吲达帕胺是合理可行的治疗选择。基于上述研究,本研究将2药联合应用,发现世居青海的轻中度原发性高血压合并冠心病患者治疗总有效率、LVEF和SAQ评分均显著升高,LVESD和BNP水平均显著降低,2组药物不良反应发生率无显著差异,表明阿利沙坦酯联合吲达帕胺的治疗方案效果显著,能改善生活质量和心功能,且安全性良好。
血压变异性,是指一定时间内血压波动的幅度,常用指标包括dSBPV、dDBPV、nSBPV、nDBPV、24 h SBPV和24 h DBPV。血压变异性升高会损害血管调节功能,显著增加心血管疾病死亡、心力衰竭、终末期肾脏疾病、冠状动脉疾病、脑卒中等的发生风险。结果显示,阿利沙坦酯联合吲达帕胺的治疗方案能调节血压和24 h血压变异性。高血压可引起血管内皮损伤,促进动脉粥样硬化。在反映血管内皮细胞功能的众多指标中,ET-1和NO占据着重要地位,作为一种具有舒张血管作用的物质,NO能够增加组织的血流量,ET-1则是一种血管收缩调节因子,2者相互拮抗,共同维持着血管的正常生理状态,当ET-1水平升高且NO水平下降时意味着血管内皮受损[19]。Hcy水平的升高能够通过激活体内的氧化应激和炎症反应,对血管内皮细胞造成损害[20]。Apelin在心血管系统中也发挥着重要作用,能够通过刺激内皮细胞释放NO,实现血管的扩张,同时可提高心肌的收缩力[21]。本研究发现,治疗后,试验组血管内皮功能指标(ET-1和Hcy水平均显著降低,NO和Apelin水平显著升高)均有显著改善,表明阿利沙坦酯联合吲达帕胺治疗方案能改善血管内皮功能,与谢立强等[22]结果类似。可能因为阿利沙坦酯能够与血管紧张素Ⅱ受体结合,抑制血管收缩,从而减轻血管阻力,改善血管内皮功能;吲达帕胺属于磺胺类利尿剂,具有钙通道阻滞及扩血管作用,减少钙离子内流,舒张血管平滑肌,降低血压,减轻血管内皮损伤。
本研究提示:世居青海的轻中度原发性高血压合并冠心病患者应用阿利沙坦酯联合吲达帕胺治疗,效果显著,可调节24 h血压变异性,改善心功能、血管内皮功能及生活质量,安全性良好,为高原地区高血压合并冠心病的诊疗策略提供了重要参考,对降低当地心血管疾病负担具有积极意义。但本研究样本量较少、为单中心回顾性研究,观察周期较短,缺乏对药物长期治疗效果的评估,未来可开展更大规模、多中心、长期随访的研究,以更全面地评估该治疗方案对患者长期预后的影响。
