The clinical isolation rate of carbapenem-resistant Acinetobacter baumannii (CRAB) has been increasing year by year, yet effective antimicrobial agents against this pathogen remain severely limited. Sulbactam-durlobactam, a novel β-lactam combination agent, has demonstrated promising in vitro antibacterial activity against CRAB. In May 2023, the U.S. Food and Drug Administration approved its use for patients aged 18 and older to treat hospital-acquired bacterial pneumonia and ventilator-associated bacterial pneumonia caused by susceptible isolates of the Acinetobacter baumannii-calcoaceticus complex, providing a new therapeutic option. This article reviews the mechanism of action, pharmacokinetics, pharmacodynamics, and clinical studies of sulbactam-durlobactam based on literature, aiming to offer insights for clinical practice.

碳青霉烯耐药鲍曼不动杆菌的临床分离率逐年增高,而针对这一病原体的有效抗菌药物却十分有限。舒巴坦-度洛巴坦是新型的β-内酰胺复方制剂,对碳青霉烯耐药鲍曼不动杆菌显示了较好的体外抗菌活性。美国食品药品监督管理局于2023年5月批准该药用于18岁及以上患者,治疗由鲍曼-醋酸钙不动杆菌复合体敏感分离株引起的医院获得性细菌性肺炎和呼吸机相关细菌性肺炎,为患者提供了新的治疗手段。本文通过文献回顾,对舒巴坦-度洛巴坦的作用机制、药代动力学、药效学及临床研究等进行学习,以期为临床实践提供参考。

碳青霉烯耐药鲍曼不动杆菌的临床分离率逐年增高,而针对这一病原体的有效抗菌药物却十分有限。舒巴坦-度洛巴坦是新型的β-内酰胺复方制剂,对碳青霉烯耐药鲍曼不动杆菌显示了较好的体外抗菌活性。美国食品药品监督管理局于2023年5月批准该药用于18岁及以上患者,治疗由鲍曼-醋酸钙不动杆菌复合体敏感分离株引起的医院获得性细菌性肺炎和呼吸机相关细菌性肺炎,为患者提供了新的治疗手段。本文通过文献回顾,对舒巴坦-度洛巴坦的作用机制、药代动力学、药效学及临床研究等进行学习,以期为临床实践提供参考。

The clinical isolation rate of carbapenem-resistant Acinetobacter baumannii (CRAB) has been increasing year by year, yet effective antimicrobial agents against this pathogen remain severely limited. Sulbactam-durlobactam, a novel β-lactam combination agent, has demonstrated promising in vitro antibacterial activity against CRAB. In May 2023, the U.S. Food and Drug Administration approved its use for patients aged 18 and older to treat hospital-acquired bacterial pneumonia and ventilator-associated bacterial pneumonia caused by susceptible isolates of the Acinetobacter baumannii-calcoaceticus complex, providing a new therapeutic option. This article reviews the mechanism of action, pharmacokinetics, pharmacodynamics, and clinical studies of sulbactam-durlobactam based on literature, aiming to offer insights for clinical practice.

鲍曼-醋酸钙不动杆菌复合体(Acinetobacter baumannii-calcoaceticus complex,ABC)是常见的院内感染病原体,包括4个菌种[1]:醋酸钙不动杆菌(A. calcoaceticus)、鲍曼不动杆菌(A. baumannii)、皮特不动杆菌(A. pittii)和医院不动杆菌(A. nosocomialis),临床分离菌株绝大部分为鲍曼不动杆菌[2],可引起肺炎、败血症、中枢神经系统感染和尿路感染等多种感染。2023年全国细菌耐药监测报告[3]显示鲍曼不动杆菌占所有临床分离菌株约6.7%,其对碳青霉烯类药物的耐药率约为55.5%,部分医院耐药率可达80%以上,较2022年上升约2.1%,碳青霉烯耐药鲍曼不动杆菌(carbapenem-resistant Acinetobacter baumannii,CRAB)对临床多数抗菌药物耐药,给临床抗感染治疗带来巨大挑战。舒巴坦-度洛巴坦(sulbactam-durlobactam,SUL-DUR)是一种新型β-内酰胺酶抑制剂复合制剂,于2023年5月在美国获批上市,用于18岁及以上患者ABC敏感分离株引起的医院获得性细菌性肺炎(hospital-acquired bacterial pneumonia,HABP)和呼吸机相关细菌性肺炎(ventilator-associated bacterial pneumonia,VABP)[4],2024年5月通过优先评审审批程序在我国获批上市[5]。现将SUL-DUR的临床药学研究综述如下。
DUR(ETX2514),是由美国Entasis Therapeutics Inc.研发的一种非β-内酰胺类二氮杂双环辛烷β-内酰胺酶抑制剂[6],其化学结构[7]为(2S,5R)-2-氨基甲酰基-3-甲基-7-氧代-1,6-二氮杂双环[3.2.1]辛-3-烯-6-基硫酸钠,分子式C8H10N3NaO6S,分子量299.23;其分子结构与阿维巴坦相似,见图1,主要区别在于其C3与C4位间增加的双键及C3位引入的甲基[8]。DUR能够通过革兰阴性菌外膜孔蛋白(OmpA)渗透进入细菌内,其活性位点的亲核丝氨酸残基可与β-内酰胺酶发生可逆的氨基甲酰化反应,形成可逆的共价键结合,从而抑制酶活性;这种分子特性赋予该药良好的结合动力学,并将其抑酶作用扩展到更加广泛的β-内酰胺酶,包括A类、C类和D类丝氨酸-β-内酰胺酶[9];特别是对D类里OXA酶的抑酶活性,这也是鲍曼不动杆菌对碳青霉烯类耐药的常见机制[10]。DUR不能抑制B类金属β-内酰胺酶。DUR对ABC没有抗菌活性,但通过抑制β-内酰胺酶,恢复了SUL对碳青霉烯类耐药ABC的活性。
目前研究显示,SUL-DUR对ABC菌株尤其是CRAB,具有较好的体外抗菌活性。
KARLOWSKY等[2]对2016至2021年全球33个国家264个医疗中心分离的5 032株ABC菌株进行了药敏分析,结果显示:ABC菌株对SUL-DUR的总体敏感率达98.3%。在该研究中,鲍曼不动杆菌为ABC主要菌株(80.2%),对美罗培南和亚胺培南的耐药率分别高达62.3%和61.6%[90%最低抑菌浓度(90% minimum inhibitory concentration,MIC90)均>64 μg·mL-1],而对SUL-DUR的耐药率仅为2%(敏感折点MIC≤4 μg·mL-1),MIC90为2 μg·mL-1,表现出显著的抗菌活性,见表1
舒巴坦常用于治疗鲍曼不动杆菌感染,在耐药鲍曼不动菌株ARC5081的单室模型中,舒巴坦的MIC为16 μg·mL-1,在同时存在度洛巴坦4 μg·mL-1的情况下,舒巴坦的MIC为4 μg·mL-1[11]
O’DONNELL等[12]研究了小鼠大腿和肺对鲍曼不动菌株感染模型下的PK/PD终点,舒巴坦-度洛巴坦对小鼠大腿和肺部感染模型的CRAB分离株表现出强大的杀菌活性,在单独使用舒巴坦与鲍曼不动杆菌ARC2058的大腿研究中,与 1-log10菌落形或单位(colonly forming units,CFU)较基线减少值、2-log10 CFU较基线减少值和 EC80 相关的 %fT>MIC 大小分别为 20.5、31.5 和 47.0;在肺模型中,分别为 37.8、50.1 和 68.5。表2总结了舒巴坦固定剂量和舒巴坦-度洛巴坦(4:1)给药的PK/PD暴露终点,fAUC/MIC在模型中幅度一致。当舒巴坦fT>MIC>50%且度洛巴坦fAUC/MIC为10时,可以在24小时内实现1-log10 CFU的降低,确定了舒巴坦fT>MIC>50%且度洛巴坦fAUC/MIC为10作为临床疗效相关的PK/PD目标。
SUL和DUR在单剂量和多剂量给药的药代动力学参数相似[7]。SAGAN等[13]研究显示,肾功能正常人群使用SUL 1 g和DUR 1 g,每6 h给药1次,每次输注3 h,给药4 d的药代动力学参数基本相当,见表3。各参数在不同年龄(18~91岁)、性别、体重(35~150 kg)和种族(白人、黑人、亚洲人、美洲印第安人/阿拉斯加原住民等)没有显著差异。
一项评估肾功能损害患者SUL-DUR药代动力学的临床试验[14],健康受试者和轻、中度肾功能损害患者静脉给予SUL和DUR各1 000 mg,而重度肾功能损害患者剂量调整为各500 mg,血液透析患者则在透析前后各给予500 mg。结果显示,SUL和DUR的半衰期(t1/2)随肾功能损害程度加重而延长,分别为1.8~10 h和2.3~7.1 h,且血浆峰值浓度(Cmax)和药时曲线下面积(AUC)随肾功能下降呈线性增加;在健康受试者和轻、中度肾损害患者中,约65%~93%的SUL和70%的DUR经尿液排泄(主要发生在给药后24 h内),而重度肾损害患者SUL和DUR的肾脏清除率(CLr)分别降至总清除率的42%和38%;终末期肾病患者血液透析可清除41%的SUL和33%的DUR。
肝功能损害对SUL-DUR药代动力学的影响尚未进行评价。因SUL-DUR主要经过肾脏代谢、排泄,肝功能受损的患者不需要调整剂量。
目前完成的一项Ⅲ期随机对照、非劣效性临床试验(NCT03894046)[15],对静脉注射SUL-DUR治疗ABC(包括多重耐药菌株和碳青霉烯类耐药菌株)感染患者的安全性和有效性进行评估。本研究纳入181例18岁以上ABC感染患者,包括HABP、VABP和血流感染,随机分为SUL-DUR组(1 g/1 g,iv,q6 h,输注3 h)和粘菌素组(2.5 mg·kg-1,iv,q12 h,输注30 min)治疗7~14 d,两组同时给与亚胺培南/西司他丁(1 g/1 g,iv,q6 h,输注1 h)治疗。研究结果显示125例碳青霉烯耐药ABC感染患者中,SUL-DUR组在临床治疗方面和微生物学均优于粘菌素组,见表4。该研究安全性分析显示,不良事件包括过敏反应、新发感染、腹泻、恶心等;SUL-DUR组(91例)和粘菌素组(86例)分别有40%(36例/91例)和49%(42例/86例)报告严重不良事件,11%(10例/91例)和16%(14例/86例)报告导致停药的治疗相关不良事件,SUL-DUR组不良事件的发生率低于粘菌素组。
MCLEOD等[16]对该研究进一步分析,比较2种治疗方案对单一或混合微生物感染的28天全因死亡率、临床治愈率和微生物学结果的影响,见表5。接受SUL-DUR联合亚胺培南/西司他丁治疗的患者,无论是单一微生物还是混合微生物感染均表现出相似的临床结局,且其临床治愈率和微生物清除率均高于黏菌素组。相比之下,黏菌素组中单一微生物ABC感染患者的28天全因死亡率更高,临床治愈率和微生物清除率低于混合感染患者。两组基线中,SUL-DUR组合并亚胺培南敏感革兰阴性菌感染的发生率为17.5%(11/63),高于黏菌素组的8.1%(5/62),而两组合并亚胺培南耐药革兰阴性菌感染的患者比例相近。本研究中,虽然所有患者在基线时均分离出碳青霉烯耐药ABC菌株,但对于混合感染病例,无法确定病原体是否为无症状定植菌。
一项针对80例复杂性尿路感染(cUTI)患者的Ⅱ期临床试验(NCT03445195)[13],受试者按2:1比例分组,分别接受SUL-DUR(1 g/1 g,iv,q6 h)或安慰剂治疗7~14 d,并联合亚胺培南/西司他丁(0.5 g,iv,q6 h)治疗,评估患者对SUL-DUR的耐受性及PK参数。结果显示,SUL-DUR耐受性良好,未报告严重不良事件,常见药物相关症状为头痛(5.7%)、恶心(3.8%)、腹泻(3.8%)和血管疼痛(3.8%),且多为轻中度。疗效分析显示,在微生物学改良意向治疗人群中,SUL-DUR组和安慰剂组的成功率分别为76.6%和81.0%;在微生物学可评估人群中,SUL-DUR组和安慰剂组的成功率分别为80.0%和81.0%,2组总体成功率相近。本研究未纳入ABC感染患者,基线存在亚胺培南耐药革兰阴性菌患者7例,SUL-DUR组3例(100%)在治愈试验(test of cure,TOC)访视时达到治疗成功,安慰剂组4例中3例(75%)成功。
综上,SUL-DUR通过对更广泛的β-内酰胺酶的抑制作用,解决了常见ABC耐药的问题,为相关肺炎患者提供了新的治疗手段,减少了对氨基糖苷类、粘菌素、替加环素等效果较差或毒性较大的药物依赖,提供了基于β-内酰胺类药物治疗的新选择,该药物主要通过肾脏排泄,肝功能受损患者不需要调整剂量的,依据肾功能调整用药剂量更方便临床应用。尽管已批准上市,但还需要进一步进行临床研究,提供更为广泛的药代动力学、药效学、不良反应及耐药性等数据,为临床合理用药提供帮助。
