Objective

To explore the clinical efficacy of fusidic acid ointment in patients with acne vulgaris who underwent non-ablative fractional 1 565 nm laser treatment.

Methods

Patients with common acne who received non-peeling fractional 1 565 nm laser treatment were divided into treatment group and control group. Treatment group was treated with fusidic acid ointment, 3 times a day, while control group was not treated with additional treatment.The lesion severity, skin sebum secretion, skin stratum corneum water content, clinical efficacy, matrix metalloproteinase 1 (MMP-1), matrix metalloproteinase tissue inhibitor-1 (TIMP-1), MMP-1/TIMP-1 ratio, skin elasticity indicators (R2, R5, R7), facial acne comprehensive grading system (global acne grading system, GAGS) score and acne-specific quality of life questionnaire (Acne-QOL) score were compared between the two groups.

Results

100 patients with acne vulgaris who received non exfoliative dot matrix 1 565 nm laser were enrolled, including 50 cases in treatment group and 50 cases in control group. After treatment, the total clinical effective rate of treatment group was 96.00% (48 cases/50 cases), and that of control group was 74.00% (37 cases/50 cases), the difference was statistically significant (P<0.05). After 3 months treatment, the skin oil secretion of treatment group and control group were (53.79±7.23) and (69.21±10.67) μg·cm-2, respectively; the moisture content of cuticle were (34.21±5.15)% and (29.68±3.92)%, respectively; MMP-1 were (1.02±0.28) and (1.24±0.43) μg·mL-1, respectively; TIMP-1 were (1.62±0.24) and (1.43±0.20) μg·mL-1, respectively; MMP-1/TIMP-1 were 0.63±0.10 and 0.87±0.15, respectively; the R2 were (53.77±8.75)% and (49.11±7.64)%, respectively; the R5 were (53.88±8.58)% and (49.67±7.69)%, respectively; the R7 were (32.55±6.05)% and (28.39±5.44)%, respectively; the GAGS scores were (13.78±2.69) and (17.83±3.35) points, respectively; the Acne-QOL scores were (105.56±5.58) and (90.21±6.32) points, respectively. After treatment, the above indexes in treatment group were significantly lower than those in control group (all P<0.05). The adverse drug reactions in treatment group and control group were dry, tingling, scaling and flushing. The total incidence of adverse drug reactions in treatment group was 10.00% (5 cases/50 cases) and in control group was 16.00% (8 cases/50 cases). There was no significant difference in the incidence of adverse drug reactions between the two groups (P>0.05).

Conclusion

Fusidic acid ointment could significantly improve the dynamic balance of MMP-1/TIMP-1, skin elasticity and skin physiological indexes in patients with acne vulgaris receiving non-ablative fractional 1 565 nm laser, with good safety.

目的

观察夫西地酸软膏对行非剥脱性点阵1 565 nm激光的寻常痤疮患者的临床疗效和安全性。

方法

将接受非剥脱性点阵1 565 nm激光的寻常痤疮患者,用队列法分为试验组与对照组,试验组使用夫西地酸软膏,每日3次进行治疗,对照组不进行额外治疗。比较2组的皮损程度、皮肤油脂分泌量、皮肤角质层含水量、临床疗效、基质金属蛋白酶1(MMP-1)水平、基质金属蛋白酶组织抑制因子-1(TIMP-1)水平、MMP-1/TIMP-1、皮肤弹性指标(R2、R5、R7)、面部痤疮综合分级系统(GAGS)评分和痤疮特异性生活质量量表(Acne-QOL)评分,并进行安全性评价。

结果

100例接受非剥脱性点阵1 565 nm激光的寻常痤疮患者入组为研究对象,试验组50例、对照组50例。治疗后,试验组的临床总有效率为96.00%(48例/50例),对照组为74.00%(37例/50例),在统计学上差异有统计学意义(P<0.05)。治疗3个月后,试验组和对照组的皮肤油脂分泌量分别为(53.79±7.23)和(69.21±10.67)μg·cm-2;皮肤角质层含水量分别为(34.21±5.15)%和(29.68±3.92)%;MMP-1分别为(1.02±0.28)和(1.24±0.43)μg·mL-1;TIMP-1分别为(1.62±0.24)和(1.43±0.20)μg·mL-1;MMP-1/TIMP-1分别为0.63±0.10和0.87±0.15;R2分别为(53.77±8.75)%和(49.11±7.64)%;R5分别为(53.88±8.58)%和(49.67±7.69)%;R7分别为(32.55±6.05)%和(28.39±5.44)%; GAGS评分分别为(13.78±2.69)和(17.83±3.35)分;Acne-QOL评分分别为(105.56±5.58)和(90.21±6.32)分,治疗后,试验组上述指标与对照组比较,在统计学上差异均有统计学意义(均P<0.05)。试验组和对照组的药物不良反应均为干燥、刺痛、脱屑和潮红,试验组的总药物不良反应发生率为10.00%(5例/50例),对照组为16.00%(8例/50例),2组药物不良反应发生率在统计学上差异无统计学意义(P>0.05)。

结论

夫西地酸软膏治疗接受非剥脱性点阵1 565 nm激光的寻常痤疮患者可以显著改善MMP-1/TIMP-1动态平衡、皮肤弹性及皮肤生理指标,且安全性良好。

目的

观察夫西地酸软膏对行非剥脱性点阵1 565 nm激光的寻常痤疮患者的临床疗效和安全性。

方法

将接受非剥脱性点阵1 565 nm激光的寻常痤疮患者,用队列法分为试验组与对照组,试验组使用夫西地酸软膏,每日3次进行治疗,对照组不进行额外治疗。比较2组的皮损程度、皮肤油脂分泌量、皮肤角质层含水量、临床疗效、基质金属蛋白酶1(MMP-1)水平、基质金属蛋白酶组织抑制因子-1(TIMP-1)水平、MMP-1/TIMP-1、皮肤弹性指标(R2、R5、R7)、面部痤疮综合分级系统(GAGS)评分和痤疮特异性生活质量量表(Acne-QOL)评分,并进行安全性评价。

结果

100例接受非剥脱性点阵1 565 nm激光的寻常痤疮患者入组为研究对象,试验组50例、对照组50例。治疗后,试验组的临床总有效率为96.00%(48例/50例),对照组为74.00%(37例/50例),在统计学上差异有统计学意义(P<0.05)。治疗3个月后,试验组和对照组的皮肤油脂分泌量分别为(53.79±7.23)和(69.21±10.67)μg·cm-2;皮肤角质层含水量分别为(34.21±5.15)%和(29.68±3.92)%;MMP-1分别为(1.02±0.28)和(1.24±0.43)μg·mL-1;TIMP-1分别为(1.62±0.24)和(1.43±0.20)μg·mL-1;MMP-1/TIMP-1分别为0.63±0.10和0.87±0.15;R2分别为(53.77±8.75)%和(49.11±7.64)%;R5分别为(53.88±8.58)%和(49.67±7.69)%;R7分别为(32.55±6.05)%和(28.39±5.44)%; GAGS评分分别为(13.78±2.69)和(17.83±3.35)分;Acne-QOL评分分别为(105.56±5.58)和(90.21±6.32)分,治疗后,试验组上述指标与对照组比较,在统计学上差异均有统计学意义(均P<0.05)。试验组和对照组的药物不良反应均为干燥、刺痛、脱屑和潮红,试验组的总药物不良反应发生率为10.00%(5例/50例),对照组为16.00%(8例/50例),2组药物不良反应发生率在统计学上差异无统计学意义(P>0.05)。

结论

夫西地酸软膏治疗接受非剥脱性点阵1 565 nm激光的寻常痤疮患者可以显著改善MMP-1/TIMP-1动态平衡、皮肤弹性及皮肤生理指标,且安全性良好。

Objective

To explore the clinical efficacy of fusidic acid ointment in patients with acne vulgaris who underwent non-ablative fractional 1 565 nm laser treatment.

Methods

Patients with common acne who received non-peeling fractional 1 565 nm laser treatment were divided into treatment group and control group. Treatment group was treated with fusidic acid ointment, 3 times a day, while control group was not treated with additional treatment.The lesion severity, skin sebum secretion, skin stratum corneum water content, clinical efficacy, matrix metalloproteinase 1 (MMP-1), matrix metalloproteinase tissue inhibitor-1 (TIMP-1), MMP-1/TIMP-1 ratio, skin elasticity indicators (R2, R5, R7), facial acne comprehensive grading system (global acne grading system, GAGS) score and acne-specific quality of life questionnaire (Acne-QOL) score were compared between the two groups.

Results

100 patients with acne vulgaris who received non exfoliative dot matrix 1 565 nm laser were enrolled, including 50 cases in treatment group and 50 cases in control group. After treatment, the total clinical effective rate of treatment group was 96.00% (48 cases/50 cases), and that of control group was 74.00% (37 cases/50 cases), the difference was statistically significant (P<0.05). After 3 months treatment, the skin oil secretion of treatment group and control group were (53.79±7.23) and (69.21±10.67) μg·cm-2, respectively; the moisture content of cuticle were (34.21±5.15)% and (29.68±3.92)%, respectively; MMP-1 were (1.02±0.28) and (1.24±0.43) μg·mL-1, respectively; TIMP-1 were (1.62±0.24) and (1.43±0.20) μg·mL-1, respectively; MMP-1/TIMP-1 were 0.63±0.10 and 0.87±0.15, respectively; the R2 were (53.77±8.75)% and (49.11±7.64)%, respectively; the R5 were (53.88±8.58)% and (49.67±7.69)%, respectively; the R7 were (32.55±6.05)% and (28.39±5.44)%, respectively; the GAGS scores were (13.78±2.69) and (17.83±3.35) points, respectively; the Acne-QOL scores were (105.56±5.58) and (90.21±6.32) points, respectively. After treatment, the above indexes in treatment group were significantly lower than those in control group (all P<0.05). The adverse drug reactions in treatment group and control group were dry, tingling, scaling and flushing. The total incidence of adverse drug reactions in treatment group was 10.00% (5 cases/50 cases) and in control group was 16.00% (8 cases/50 cases). There was no significant difference in the incidence of adverse drug reactions between the two groups (P>0.05).

Conclusion

Fusidic acid ointment could significantly improve the dynamic balance of MMP-1/TIMP-1, skin elasticity and skin physiological indexes in patients with acne vulgaris receiving non-ablative fractional 1 565 nm laser, with good safety.

寻常痤疮作为常见的慢性毛囊皮脂腺炎症性疾病,其发病机制与皮脂腺过度分泌、毛囊角化异常及痤疮丙酸杆菌感染相关[1]。非剥脱性点阵1 565 nm激光作为新兴物理治疗手段,通过选择性加热皮脂腺并刺激胶原重塑,在改善毛孔粗大、控油及轻度瘢痕方面效果显著。但该疗法存在局限性,其对深层炎症反应控制不足,术后易出现红肿等急性炎症,对中重度炎症性皮损(如囊肿、结节)的细菌清除效果有限,易致病情反复[2]。夫西地酸软膏可以通过抑制细菌蛋白质合成直接杀灭痤疮丙酸杆菌,同时其脂溶性特性可快速渗透皮损微环境,抑制白细胞趋化性以缓解激光术后炎症。相较于口服抗生素,其局部应用避免了胃肠道刺激及耐药性问题,安全性更高[3]。基质金属蛋白酶1(matrix metalloproteinase-1,MMP-1)的过度表达可破坏细胞外基质平衡,而基质金属蛋白酶组织抑制因子-1(tissue inhibitor of metalloproteinases-1,TIMP-1)通过抑制MMP-1活性维持皮肤弹性稳态,2者的平衡直接影响皮肤弹性[4-5]。因此,本研究旨在通过分析夫西地酸软膏和非剥脱性点阵1 565 nm激光联合治疗对寻常痤疮患者MMP-1/TIMP-1平衡及皮肤弹性的影响,为优化痤疮综合治疗方案、改善患者远期皮肤功能提供临床数据支持。
2022年6月至2024年6月以本院就诊的寻常痤疮患者入选为研究对象。本研究经黄河三门峡医院伦理委员会批准(伦理批号:20230106)。所有患者均签署知情同意书。
诊断与入选标准 符合《中国痤疮治疗指南(2019修订版)》[6]中关于寻常痤疮诊断标准者,年龄≥18岁者,依从性较好者,临床资料完整者,首次接受治疗者,Pillsbury临床分级[7]为Ⅱ~Ⅳ级者。
排除标准 心肝肾功能不全者,近3个月内有抗生素、免疫调节剂、糖皮质激素使用史者,近半年内接受过面部强脉冲光或激光治疗者,近1个月有暴晒史者,妊娠或哺乳期妇女,合并银屑病、活动性白癜风者,合并光敏性疾病者,皮肤破损严重者,合并活动性微生物感染者。
夫西地酸乳膏,规格:每支10 g,批号:6220530,批准文号:国药准字 HC20150044,奥美制药厂有限公司生产;基质金属蛋白酶1(matrix metalloproteinase 1, MMP-1)和基质金属蛋白酶组织抑制因子-1(matrix metalloproteinase tissue inhibitor-1, TIMP-1)酶联免疫试剂盒,均由杭州联科生物技术股份有限公司生产。
1 565 nm非剥脱性点阵激光仪,美国Lumenis公司产品;Cutometer MPA® 580型皮肤弹性测试仪,德国CK公司产品;BGMR-1000酶联免疫分析仪,杭州柏恒科技有限公司产品。
将本院接受非剥脱性点阵1 565 nm激光的寻常痤疮患者,用队列法分为试验组与对照组。2组患者均用1 565 nm非剥脱性点阵激光技术进行治疗,治疗结束后,立即以冰袋对治疗区域进行冷敷处理。此治疗方案每月实施1次,共计3次。试验组在此基础上外用夫西地酸乳膏涂于患处,每日3次,持续治疗3个月,对照组不再接受额外治疗。
皮肤指标[8] 在治疗前(T0)与治疗后1个月(T1)、2个月(T2)、3个月(T3)时,清晨空腹取静脉血5 mL,以3 000 r·min-1离心10 min,待分层后取上层血清,并冷藏于-20 ℃低温冰箱待测。用酶联免疫试剂盒检测MMP-1和TIMP-1水平,并计算MMP-1/TIMP-1比值。用皮肤弹性测试仪评估皮肤弹性,其中,R2代表在无负压状态下皮肤的回弹量Ua相对于施加负压时的最大拉伸量Uf的比例;R5代表在皮肤测试的首个循环中,恢复阶段的弹性成分Ur与拉伸阶段的弹性成分Ue之比;R7代表在首次测试循环里,恢复阶段的弹性成分Ur与该循环中皮肤所能达到的最大拉伸量Uf的比值,其反映了皮肤在形变后能够恢复至原始状态的能力。这些数值越趋近于1,即表示皮肤的弹性状态越佳。每个测试区测量3次取平均值。
生活质量评价 治疗前后,分别用痤疮特异性生活质量量表(acne-specific quality of life questionnaire,Acne-QOL)进行评分[9],包括4个方面,其中痤疮症状、自我感知、情感功能,每项分数范围0~30分,社会功能分数范围0~24分,分数越高,生活质量越好。
临床疗效评价 治疗前后,用面部痤疮综合分级系统(global acne grading system,GAGS)[10]评分评价临床疗效。粉刺赋值为1分,丘疹为2分,脓疱为3分,囊肿则为4分。位置加权:鼻部为1,额部、左右颊部及下颏部均为2,其余部位则为3。局部区域总分数=该区域皮损分数×位置加权,分数越高越严重。疗效指数=(治疗前评分-治疗后评分)/治疗前评分×100%,该指数>90%记为痊愈,60%~90%记为显效,20%~59%记为有效,< 20%记为无效。总有效率=(痊愈例数+显效例数)/总例数×100%。
安全性评价 治疗过程中,观察并记录患者的干燥、刺痛、脱屑、红肿等药物不良反应发生情况。
用SPSS 25.0软件进行统计分析。计量资料用表示,2组间比较用独立样本t检验,多组比较用F检验,组内比较用配对样本t检验。计数资料采用率(%)表示,2组间比较用χ2检验。
共100例接受非剥脱性点阵1 565 nm激光的寻常痤疮患者入组为研究对象,试验组50例、对照组50例。2组患者的年龄、性别、体质量指数等一般资料比较,在统计学上差异均无统计学意义(均P>0.05),组间具有可比性,见表1
2组患者治疗后的粉刺、丘疹、脓包与结节个数、皮肤油脂分泌量、MMP-1和MMP-1/TIMP-1较治疗前均显著下降(均P<0.05);皮肤角质层含水量、TIMP-1、R2、R5和R7较治疗前均显著上升(均P<0.05)。试验组治疗1、2、3个月的粉刺、丘疹、脓包与结节个数、皮肤油脂、MMP-1和MMP-1/TIMP-1均显著低于对照组(均P<0.05);试验组治疗1、2、3个月后的皮肤角质层含水量均显著高于对照组(均P<0.05);试验组治疗2、3个月后的TIMP-1,R2、R5和R7均显著高于对照组(均P<0.05)。
治疗后,2组患者的GAGS评分较治疗前显著下降,Acne-QOL评分较治疗前显著上升(均P<0.05)。试验组治疗1、2、3个月的GAGS评分均显著低于同期对照组,Acne-QOL评分均显著高于同期对照组(均P<0.05),见表3
试验组患者的总有效率为96.00%,显著高于对照组的74.00%(P<0.05);在试验组中,不同Pillsbury临床分级患者的临床疗效比较,在统计学上差异均无统计学意义(均P>0.05);在对照组中,不同Pillsbury临床分级患者的临床疗效比较,在统计学上差异均有统计学意义(均P<0.05),见表4
在试验组患者中,出现干燥1例,刺痛1例,脱屑1例,潮红2例,总药物不良反应发生率为10.00%(5例/50例);在对照组患者中,出现干燥2例,刺痛1例,脱屑2例,潮红3例,总药物不良反应发生率为16.00%(8例/50例),2组药物不良反应发生率在统计学上差异无统计学意义(P>0.05)。
寻常痤疮可严重影响患者的皮肤健康及生活质量。非剥脱性点阵1 565 nm激光通过特定的波长作用于皮肤深层,刺激胶原纤维和弹力纤维的再生,减少炎症和色素沉着,从而改善皮肤质地和弹性,但仍存在术后易出现红肿、渗出等急性炎症反应,影响愈合[11]。夫西地酸软膏因其卓越的抗菌和抗炎特性,显著缩短炎症消退时间,降低瘢痕形成风险,因此,常被应用于痤疮的系统治疗中。本研究通过比较单用剥脱性点阵1 565 nm激光和联合这2种治疗方式,探讨联合治疗方案对寻常痤疮患者MMP-1/TIMP-1表达、皮肤弹性及皮肤生理指标的影响。
MMP-1是一种胶原酶,主要负责降解皮肤真皮层中的Ⅰ型和Ⅲ型胶原蛋白。这2种胶原蛋白对于维持皮肤的结构和弹性至关重要。在伤口愈合过程中,MMP-1可促进细胞的分离和迁移,以及组织重构和表皮细胞再生。而在痤疮等皮肤病中,MMP-1的异常表达可能会特异性分解上述胶原蛋白,引发皮肤弹性减弱及皮肤老化[12]。在皮损炎症过程中,中性粒细胞等炎性细胞可释放细胞因子,导致血管扩张、成纤维细胞增殖和肥大细胞活化[13]。这些过程可能诱导MMP-1的表达升高,降解胶原蛋白,导致萎缩性瘢痕形成。TIMP-1是MMP-1的天然组织抑制因子,通过与MMP-1形成稳定的分子复合物来阻断其活性,从而减少基质胶原的降解,保持皮肤弹性和紧致度。研究发现,慢性皮肤溃疡在经过治疗后,创面肉芽组织中MMP-9水平下降,TIMP-1水平上升,MMP-9/TIMP-1状态失衡[14]。此外,MMP-9/TIMP-1与烧伤患者瘢痕评分、创面愈合时间也密切相关[15]。本研究显示,2组患者治疗后的MMP-1、MMP-1/TIMP-1均较治疗前下降,TIMP-1水平均上升。联合治疗组的MMP-1、TIMP-1、MMP-1/TIMP-1及皮肤弹性的改善水平显著优于单独激光治疗组。
本研究显示,试验组的总有效率显著高于对照组,安全性在统计学上差异无统计学意义,且试验组治疗后的GAGS评分和Acne-QOL评分均较对照组有显著改善。夫西地酸作为夫西地酸乳膏的核心活性成分,属于甾体类化合物,其抗菌作用机制源于对细菌蛋白质合成的特异性抑制。该药物对多种革兰氏阳性球菌及部分革兰氏阴性菌具有抗菌活性。其高脂溶性特性赋予其快速渗透至皮肤病变部位的能力,可在局部达到有效抑菌浓度,从而在降低全身性药物不良反应风险的同时,实现与口服抗生素相当的治疗效果[16]。研究发现,相较于轻度痤疮患者,中重度痤疮患者的皮脂变化更为显著,且伴随着皮肤水分的严重流失与角质层水分含量的显著降低[17]。对采用1 565 nm非剥脱性点阵激光治疗的患者加用外用抗生素,能有效调控皮脂腺功能,减少皮脂产生,提高皮肤抵抗力,改善患者皮肤生理状况,且安全性良好[18]。本研究显示,试验组的皮损程度、皮肤油脂和角质层含水量改善情况均优于对照组,这提示联合治疗对寻常痤疮患者的临床疗效更高。此外,在试验组中,不同Pillsbury临床分级患者的临床疗效无明显差异;在对照组中,不同Pillsbury临床分级患者的临床疗效在统计学上差异有统计学意义,这提示联合治疗能够覆盖痤疮发病的感染、炎症、瘢痕等多个环节,使不同严重程度的患者均能受益。非剥脱性点阵激光单独使用时,其疗效受限于作用机制单一性,难以全面应对高分级痤疮的复杂病理变化。对于轻度痤疮,可优先考虑非剥脱性点阵激光或联合治疗;对于中重度痤疮,建议用联合治疗方案以控制病情。
本研究提示:夫西地酸软膏治疗非剥脱性点阵1 565 nm激光术后寻常痤疮患者可显著改善MMP-1/TIMP-1动态平衡、皮肤弹性及皮肤生理指标,且安全性良好。但本研究样本量较小且为单中心研究,限制了结果的普适性;观察期为3个月,虽然能够初步评估治疗效果,但长期疗效和安全性仍需进一步观察,未来研究需大样本长期研究进行验证,以全面评估治疗方案的长期影响。
