Precancerous lesions of gastric cancer (PLGC) are considered as a high-risk factor for the development of gastric cancer. Existing studies have confirmed that hypoxia inducible factor-1α (HIF-1α) plays a key role in the progression of PLGC. HIF-1α promotes the malignant transformation of PLGC by regulating various cellular processes, including angiogenesis, aerobic glycolysis, autophagy, cell proliferation, and apoptosis. Traditional Chinese medicine (TCM) has been shown to modulate the HIF-1α signaling pathway and inhibit these pathological processes, thereby intervening in the development of PLGC. This review will focus on the mechanisms by which HIF-1α mediates PLGC and summarize the potential applications of TCM in the prevention and treatment of PLGC, aiming to provide a clear molecular framework for TCM intervention in PLGC and offer theoretical support and research directions for the development of innovative HIF-1α-targeted Chinese medicines.

胃癌前病变(PLGC)被视为胃癌发生的高危因素。现有研究证实缺氧诱导因子-1α(HIF-1α)在PLGC的进展中起关键作用。HIF-1α通过调控多种细胞生理过程,如新生血管生成、有氧糖酵解、细胞自噬、细胞增殖与凋亡等,促进PLGC的恶性转化。中医药被发现能够通过调控HIF-1α及其相关信号通路,抑制这些病理过程,干预PLGC的发展。本文将重点探讨HIF-1α在PLGC中的作用机制,并结合中医药的相关研究,综述其在防治PLGC中的潜在应用,以期为中医药防治PLGC提供明确的分子机制框架,并为后续靶向HIF-1α的中药创新药物研发提供理论依据和研究方向。

Precancerous lesions of gastric cancer (PLGC) are considered as a high-risk factor for the development of gastric cancer. Existing studies have confirmed that hypoxia inducible factor-1α (HIF-1α) plays a key role in the progression of PLGC. HIF-1α promotes the malignant transformation of PLGC by regulating various cellular processes, including angiogenesis, aerobic glycolysis, autophagy, cell proliferation, and apoptosis. Traditional Chinese medicine (TCM) has been shown to modulate the HIF-1α signaling pathway and inhibit these pathological processes, thereby intervening in the development of PLGC. This review will focus on the mechanisms by which HIF-1α mediates PLGC and summarize the potential applications of TCM in the prevention and treatment of PLGC, aiming to provide a clear molecular framework for TCM intervention in PLGC and offer theoretical support and research directions for the development of innovative HIF-1α-targeted Chinese medicines.

胃癌(gastric cancer,GC)是全球癌症相关死亡的第3大主要原因,到2040年,GC全球负担预计增加62.00%[1]。中国是世界上胃癌发病率最高的国家,占全球胃癌发病总数的43.90%[2]。胃癌的高发病率与高致死率使其防治面临严峻挑战。研究表明,胃黏膜癌变经历了正常胃黏膜→非萎缩性胃炎→萎缩性胃炎(chronic atrophic gastritis,CAG)→肠上皮化生(intestinal metaplasia,IM)→异型增生(dysplasia,Dys)→胃腺癌(肠型)的多步骤演变过程[3-4]。其中,胃癌前病变(precancerous lesions of gastric cancer,PLGC)被广泛认为是胃癌发生过程中的关键病理阶段,包括在CAG基础上伴发的IM、Dys。尽管已有诸多研究探索胃癌的致病机制,但目前尚未完全明了,导致基于病因的一级预防措施难以有效实施。鉴于PLGC具备实现病理逆转的潜力,针对其早期阶段进行干预被认为是有效阻断胃癌发生的重要策略,已逐步成为研究热点。目前普遍采用的药物联合手术治疗方案虽能提高疗效,但仍存在适应性差、不良反应多及复发率高的问题,难以满足多样化的临床需求[5-6]。因此,亟需探索兼具疗效与安全性的优化治疗策略。最新研究表明,胃微环境失衡是引起PLGC发生发展的关键病理机制,其中缺氧微环境是重要诱因[7]。当胃黏膜细胞处于缺氧状态时,缺氧诱导因子-1α(hypoxia inducible factor-1,HIF-1α)作为重要转录因子,激活下游靶基因的表达,调节血管生成、葡萄糖代谢、细胞自噬与凋亡,并参与调节多条信号通路,推动癌变过程[8]。中医药能够通过调控HIF-1α的表达,改善胃黏膜缺氧、抑制炎症反应和血管新生等,延缓病理进程,在防治PLGC方面展现出独特优势。本文旨在综述HIF-1α在PLGC发生发展中的作用及调控机制,并进一步探讨中医药干预HIF-1α通路防治PLGC的潜在机制及应用前景。
HIF-1是由α和β亚单位组成的异源二聚体,是公认的转录激活调节氧稳态和代谢激活的基因。其中,HIF-1α是HIF-1二聚体的α亚基,是缺氧应答中关键的核转录调控因子[9]。常氧状态下HIF-1α极不稳定,其氧依赖性降解结构域脯氨酸残基(P402、P564)可被脯氨酸羟化酶(prolyl hydroxylase domain-containing protein,PHD)催化羟基化,羟基化后的HIF-1α可被肿瘤抑制蛋白(von Hippel-Lindau tumor suppressor protein,pVHL)识别并结合,随后pVHL形成的复合物被E3泛素连接酶识别,组装成完整的泛素连接酶复合物,从而介导HIF-1α进入泛素蛋白酶体途径降解[10-11]。低氧环境会抑制PHD的催化活性,降低HIF-1α脯氨酸残基的羟基化修饰,从而阻断其与pVHL的结合及后续的泛素依赖性降解,促使HIF-1α在细胞内累积,随后,HIF-1α转移至细胞核,与β亚基形成稳定的异源二聚体,并结合于靶基因启动子上的缺氧反应元件(hypoxia response element,HRE),启动一系列下游基因的转录表达,包括血管内皮生长因子(vascular endothelial growth factor,VEGF)、糖酵解关键酶乳酸脱氢酶A(lactate dehydrogenase A,LDHA)、促红细胞生成素(Erythropoietin,EPO)、葡萄糖转运蛋白(glucose transporters,GLUTs)等,广泛参与癌变细胞的血管生成、能量代谢、细胞自噬与凋亡等关键生物学过程,加速肿瘤的发生与进展。
缺氧微环境是肿瘤生物学中的重要特征,研究发现其在胃癌的发生、发展过程中发挥关键作用。尤其是在PLGC中,缺氧微环境的形成和维持与细胞增殖、凋亡、糖代谢过程、免疫炎症反应以及血管生成等多种生物学过程密切相关,进一步推动了病变进程和免疫逃逸的发生[12]。缺氧微环境通常由于组织局部供氧不足或肿瘤组织内血管异常引起。在PLGC过程中,慢性炎症、细胞外基质重塑、血管生成不良等均会导致组织氧供应减少,进而产生缺氧微环境。此时,异型增生细胞会激活许多复杂的细胞信号通路,如活化HIF-1、磷脂酰肌醇-3-激酶(phosphoinositide 3-kinase,PI3K)、核因子-κB(nuclear factor kappa-B,NF-κB)等以适应缺氧环境。细胞对缺氧的反应主要由HIF-1介导,缺氧条件下HIF-1α被激活,参与肿瘤血管生成、能量代谢、自噬、凋亡及多种基因转录[13]。鉴于缺氧微环境在PLGC中的重要作用,靶向缺氧相关信号通路的治疗策略成为当前研究的重点。
PLGC细胞团在早期阶段处于无血管状态,随后因缺氧刺激,在HIF-1α的调控下大量分泌血管生成相关因子,促使新的血管生成,进而加速细胞增殖[14]。HIF-1α高表达导致其下游靶基因VEGF激活,其作为主要的血管生成因子,使内皮细胞活化并诱导新生血管生成,同时再次加剧组织缺氧。一项关注VEGF和PLGC进展程度之间相关性的研究发现,VEGF在PLGC中过表达,随着胃黏膜病变程度的加重,其表达水平呈逐步升高趋势[15]。VEGF与其特异性受体结合后,不仅促进血管内皮细胞的存活、增殖、迁移与侵袭,还可增强血管通透性并诱发血管炎症,这些过程的协同作用共同推动异常血管的生成与建立[16]。新生的畸形血管由于存在管腔不规则、局部渗漏和压迫、基底膜缺损及缺乏周细胞包裹等结构和形态异常,致使管腔结构受压塌陷,血管通透性增加,造成血管灌注不良及氧输送受限,最终引发缺氧[17-18]。这种持续的缺氧状态可进一步激活HIF-1α高表达,形成恶性循环,推动病理性血管生成,加速疾病进程。
相关研究证实,糖酵解作为细胞缺氧状态下的葡萄糖代谢方式,能够加快细胞能量供给,使异型细胞增殖加快,进而加速PLGC恶化[19-21]。HIF-1α在缺氧条件下稳定性增加,并在细胞内聚集,表达水平增高。同时,HIF-1α通过与其下游靶基因启动子区域的HRE结合,转录激活多种糖酵解相关基因的表达,包括己糖激酶2(hexokinase 2,HK2)、丙酮酸激酶M2型(pyruvate kinase M2,PKM2)、LDHA等关键酶。这些基因的表达增强,加速了葡萄糖摄取与糖酵解活性,使异型增生细胞在缺氧环境中依然能够获得能量和代谢中间产物,从而快速生成ATP,支持其生存和增殖[22]。在促进糖酵解的同时,HIF-1α还诱导丙酮酸脱氢酶激酶1(pyruvate dehydrogenase kinase 1,PDK1)上调,抑制丙酮酸脱氢酶复合物(pyruvate dehydrogenase complex,PDH)活性,使丙酮酸无法进入线粒体参与三羧酸循环(tricarboxylic acid cycle,TCA),从而抑制有氧呼吸。此外,HIF-1α还能减少线粒体数量,进一步降低氧化磷酸化水平,减少活性氧的产生。这一机制促使细胞代谢从有氧氧化磷酸化转向无氧糖酵解,构成“Warburg效应”的重要分子基础[23]
在胃“炎-癌”转化的不同阶段,自噬可能发挥双重作用,在CAG阶段,细胞自噬受抑制以保护胃黏膜免受恶性转化。然而,当胃黏膜病变进展至IM及Dys阶段时,细胞可激活自噬过程以减轻炎症,并清除有害代谢产物,从而防止其恶性转化[24-26]。当胃黏膜处于低氧状态时,HIF-1α被激活以诱导自噬发生,其通过与Bcl-2-腺病毒E1B 19 kD相互作用蛋白3(Bcl-2-adenovirus E1B 19 kD-interacting protein 3,Bnip3)启动子区域HRE2结合,刺激Bnip3过表达。Bnip3的二聚体结构通过其(LC3-interacting region,LIR)结构域与微管相关蛋白1轻链3(microtubule-associated protein 1-light chain 3,LC3)结合,进一步诱导自噬[27]。此外,HIF-1α还可激活Bcl-2相互作用蛋白(Bcl-2 interacting coiled-coil protein 1,Beclin-1)等自噬关键蛋白,促进自噬发生。同时,自噬过程能够通过抑制caspase介导的凋亡途径抑制细胞凋亡,发生自噬与凋亡失衡,出现“选择性细胞增殖”现象,即正常细胞递增殖以及恶化细胞的高增殖以逃避凋亡,逐渐形成肿瘤细胞[28]。此外,HIF-1α在缺氧条件下参与细胞凋亡调控,并表现出既抑制凋亡,又可促进凋亡的双重生物学效应。缺氧环境下HIF-1α被激活后,可能抑制正常细胞的增殖并诱导其凋亡,而对于上皮异常增生或癌变细胞,HIF-1α可介导一系列适应性机制,促进其在低氧环境中的生存与持续增殖[29]。在胃癌前病变中,PI3K/Akt信号通路作为HIF-1α的重要上游调节因子,常处于异常激活状态。这种激活增强了细胞对低氧及营养匮乏环境的适应能力,阻碍细胞凋亡,进一步促进细胞恶性转化[30]。见图1
多种中药及其化学成分有良好的抗肿瘤特性,可通过调控PLGC血管生成、糖酵解、细胞自噬与凋亡等过程抑制HIF-1α表达,最终达到防治胃癌前病变的目的。
胃痞消具有健脾益气,化瘀解毒之功。ZENG等[31]针对该方剂开展的药效学研究表明,在PLGC模型大鼠中,胃痞消通过多靶点调控显著改善胃黏膜病理状态。实验数据显示,经胃痞消干预后,以白细胞分化抗原34(cluster of differentiation 34,CD34)标记的胃黏膜微血管密度(microvessel density,MVD)较模型组显著降低。分子机制研究表明,与模型组相比,胃痞消高、中、低剂量(15.00、7.50、3.75 g·kg-1·d-1)VEGF蛋白表达水平分别降低50.62%、53.09%和67.90%;中、低剂量组大鼠胃黏膜HIF-1α mRNA表达水平分别较模型组下降27.50%和41.67%(P<0.05)。此外,超微电镜观察进一步证实,给药组大鼠胃黏膜血管腔形态结构趋于正常,基底层完整性及内皮细胞超微结构得到明显修复。综合研究结果表明,胃痞消可能通过调节HIF-1α/VEGF/ERK信号轴,改善胃黏膜缺氧微环境,从而有效抑制PLGC病理进程中的异常血管生成,进而阻断癌变发生。
莪蚕健胃方具有益气健脾、化痰祛瘀之功。黄柳向等[32]研究发现,PLGC模型大鼠经高、中、低剂量(32.20、16.10、8.05 g·kg-1·d-1)莪蚕健胃方干预后,MVD水平分别为19.53±1.65、22.83±2.44和31.48±2.54,显著低于模型组的32.23±2.78,在统计学上差异均具有统计学意义(均P<0.05)。同时,检测PLGC大鼠胃黏膜组织中HIF-1α、VEGF mRNA及蛋白的表达情况,结果显示,经莪蚕健胃方高、中剂量干预后的HIF-1α、VEGF mRNA表达水平分别为1.56±0.14、1.72±0.18和1.50±0.14、1.85±0.16,均显著低于模型组的2.46±0.20和2.23±0.23(均P<0.05),免疫组化及蛋白表现出相同趋势。这表明莪蚕健胃方的作用机制可能通过下调HIF-1α、VEGF蛋白和mRNA表达,抑制新生血管生成,从而有效逆转胃癌前病变。
五丹胃福颗粒以理气通降,活血化瘀为法。祁向争等[33-34]用五丹胃福颗粒干预PLGC模型大鼠,结果发现,N-甲基-N′-硝基-N-亚硝基胍(N-methyl-N′-nitro-N-nitrosoguanidine,MNNG)复合因素诱导的大鼠经五丹胃福颗粒高剂量组(1.44 g·mL-1)治疗后,大鼠胃体部黏膜HIF-1α、VEGF蛋白表达水平分别为0.26±0.03和0.23±0.06,均显著低于模型组的0.49±0.10和0.53±0.04(均P<0.05)。同时,采用免疫组化法检测HIF-1α上游PI3K、蛋白激酶B(protein kinase B,Akt)蛋白阳性表达情况及内皮型一氧化氮合酶(endothelial nitric oxide synthase,eNOS)、NF-κB,结果表明与模型组相比,五丹胃福颗粒组PI3K、Akt蛋白阳性率均下降50.00%(均P<0.05);eNOS、NF-κB蛋白表达水平分别为0.47±0.11、0.32±0.03,均显著低于模型组的0.77±0.19和0.65±0.11(均P<0.05)。这表明五丹胃福颗粒在改善PLGC炎症与缺氧微环境的同时抑制大鼠胃黏膜血管新生,延缓癌变进程。
制萎扶胃丸具有益气健脾、活血化瘀、通络止痛之功。豆鹏程等[35]研究发现,MNNG复合法诱导的SD大鼠经中、高剂量(0.84、1.67 g·kg-1)制萎扶胃丸治疗后,与模型组相比,胃黏膜组织中HIF-1α蛋白(0.66±0.04、0.43±0.03 vs 0.80±0.02)、VEGF蛋白表达水平(1.21±0.06、0.97±0.06 vs 1.54±0.05)均显著降低(均P<0.01)。大鼠胃黏膜HIF-1α mRNA表达水平分别为1.60±0.06、1.28±0.06,VEGF mRNA表达水平分别为1.69±0.12、1.29±0.04,均显著低于模型组的2.09±0.16和2.08±0.15(均P<0.01)。此外,与模型组相比,中、高剂量组血管内皮生长因子受体-2(vascular endothelial growth factor receptor-2,VEGFR-2)(1.18±0.04、1.03±0.06 vs 1.45±0.02)、血管生成素-2(Ang-2)蛋白表达水平(0.61±0.02、0.51±0.03 vs 0.78±0.03)均显著降低(均P<0.01),mRNA表现出相同趋势。此外,陈心怡等[36]通过免疫荧光检测显示,与模型组比较,高剂量组HIF-1α、VEGF阳性密度值为0.01±0.00、0.01±0.00,均显著低于模型组的0.03±0.01和0.02±0.00(P<0.05,P<0.01)。以上研究说明制萎扶胃丸对PLGC的抑制效应可能与其对HIF-1α/VEGF信号通路的调节作用有关。该方剂通过干预该通路中关键生物标志物的基因转录及蛋白合成,有效改善胃黏膜组织缺氧状态,抑制新生血管生成,同时减轻胃黏膜炎症反应,从而延缓胃黏膜病变的恶性转化进程。
健脾化瘀解毒方以益气健脾,化瘀解毒为法。潘华峰等[37]研究表明,经15.00 g·kg-1健脾化瘀解毒方组干预后,PLGC模型小鼠胃黏膜PI3K、Akt、HIF-1α蛋白相对表达水平分别较模型组降低约61.11%、64.29%和52.38%(P<0.05,P<0.01);糖酵解关键酶LDHA蛋白表达水平较模型组下降约50.00%,在统计学上差异具有统计学意义(P<0.01)。同时,行免疫组化测量细胞恶性增殖标志物粘蛋白2(mucin 2,MUC2)和细胞增殖核抗原Ki-67(cell proliferation antigen Ki-67,Ki-67)蛋白的平均光密度值和累积面积百分率,研究结果表明,健脾化瘀解毒方高剂量组MUC2和Ki-67蛋白平均光密度值分别为120.32±8.36、63.96±6.10,均显著低于模型组的187.65±13.35、92.00±8.23,MUC2和Ki-67蛋白累积面积百分率分别为(13.05±2.61)%、(15.40±6.45)%,均显著低于模型组的(54.65±3.94)%、(27.90±3.85)%(均P<0.01)。以上研究结果表明,健脾化瘀解毒方可能通过调节PI3K/Akt/HIF-1α轴改善胃黏膜局部缺氧状态,调节LDHA参与的糖代谢重编程,改善细胞能量代谢方式,从而保护胃黏膜。
左金丸具有清泻肝火,降逆止呕之功。LIU等[38]研究表明,左金丸加味通过HIF-1α途径抑制PLGC大鼠糖酵解。经中、高剂量(1.26、2.51 g·kg·d-1)左金丸加味干预后,PLGC大鼠胃黏膜HIF-1α蛋白表达水平分别较模型组下降约82.14%、78.57%,HIF-1α免疫荧光强度较模型组下降约40.00%、36.00%;糖酵解关键蛋白6-磷酸果糖-2-激酶/果糖-2,6-二磷酸酶3型(6-Phosphofructo-2-kinase/Fructose-2,6-bisphosphatase 3,PFKFB3)相对表达水平较模型组下降约64.29%、57.14%。低(0.63 g·kg·d-1)、中剂量HK2蛋白表达水平较模型组下降约22.22%,20.00%。以上指标在统计学上差异均具有统计学意义(P<0.05,P<0.01)。同时,体外实验研究发现,PLGC细胞模型经1.26 g·kg·d-1左金丸加味含药血清干预后,HIF-1α及糖酵解关键蛋白表达呈现相同趋势。这表明左金丸加味通过靶向HIF-1α信号通路,干预PLGC糖酵解过程中相关蛋白表达水平,从而抑制癌变进程。
胃痞灵具有健脾益气、化瘀解毒之功。刘伟等[39]用氢-钾离子交换ATP酶α亚基基因敲除(Atp4a-/-)小鼠模拟PLGC,经高、低剂量(15.00、7.50 g·kg-1)胃痞灵干预后,HIF-1α蛋白表达水平分别为0.61±0.28、0.35±0.04,mTOR蛋白表达水平为0.24±0.05、0.08±0.04,均显著低于模型组的0.88±0.03和0.66±0.08,高、低剂量组SIRT6蛋白表达水平分别为0.36±0.04、0.47±0.03,均显著高于模型组的0.26±0.04,在统计学上差异均有统计学意义(P<0.01)。参与直接催化糖酵解反应的相关酶HK2蛋白表达水平分别为0.52±0.03、0.34±0.03,PKM2蛋白表达水平分别为0.57±0.05、0.54±0.04,α-烯醇化酶(Enolase 1, ENO1)蛋白表达水平分别为0.66±0.04、0.65±0.13,均显著低于模型组的0.73±0.03、0.93±0.02和1.06±0.12,在统计学上差异均有统计学意义(P<0.01)。这表明,胃痞灵能够通过调控mTOR/HIF-1α/SIRT6信号通路,调节葡萄糖代谢相关关键分子,从而抑制糖酵解过程,进而有效阻断或改善PLGC的发生发展。
南蛇藤具有解毒消肿、祛风除湿等功效,张效泽等[40]用南蛇藤提取物干预MNNG多因素诱导的PLGC模型大鼠,研究发现,经中、高剂量(25.00、50.00 mg·kg-1)南蛇藤提取物治疗后,大鼠胃黏膜组织HIF-1α mRNA表达分别较模型组降低16.00%/32.00%,叉头转录因子4 (forkhead boxO4,FoxO4)mRNA表达水平分别较模型组升高16.67%/34.78%,糖酵解关键酶HK2、PKM2、LDHA、GLUT1 mRNA表达分别较模型组降低20.45%/28.79%、38.46%/46.15%、26.23%/33.33%、29.17%/33.33%(均P<0.01)。免疫组化染色结果表明,与模型组相比,南蛇藤提取物各剂量组HK2、PKM2、LDHA、GLUT1、HIF-1α蛋白阳性染色颗粒明显减少,染色变浅,FOXO4阳性染色颗粒增加,染色较深,免疫组化和mRNA表现出相同趋势。此外,采用化学比色法检测PLGC病变大鼠胃黏膜乳酸含量,结果显示,南蛇藤提取物各剂量组大鼠胃黏膜乳酸含量分别为(0.33±0.01)、(0.28±0.00)mmol·g-1 ,均显著低于模型组的(0.52±0.01)mmol·g-1,在统计学上差异均具有统计学意义(均P<0.01)。这表明南蛇藤提取物能够缓解PLGC大鼠胃黏膜病变,通过调控HIF-1α/FOXO4轴,降低糖酵解相关标志物蛋白及mRNA表达,有效降低大鼠胃组织乳酸蓄积量以纠正酸性微环境,进而缓解胃癌前病变,预防癌变。
张家祥等[41]认为抑制线粒体自噬过度激活可能是阻止PLGC进展及GC发生的关键,并通过动物实验研究证实,经不同剂量小建中汤干预后,大鼠胃黏膜组织HIF-1α、线粒体自噬诱导因子Bnip3蛋白表达水平均不同程度降低,其中高剂量组(0.85 g·mL-1)HIF-1α蛋白表达水平为0.26±0.05,显著低于模型组的0.47±0.02,中(0.425 g·mL-1)、高剂量组Bnip3蛋白表达水平分别为0.45±0.04、0.38±0.038,显著低于模型组的0.63±0.05(均P<0.01)。此外,小建中汤抑制其下游参与自噬过程的Beclin-1、LC3Ⅱ蛋白表达水平,高剂量组Beclin-1蛋白及mRNA表达水平分别为0.53±0.03、1.64±0.18,显著低于模型组的0.80±0.04、2.54±0.24,LC3Ⅱ蛋白及mRNA表达水平分别为0.34±0.06、1.82±0.28,显著低于模型组的0.59±0.02、3.19±0.37(均P<0.01)。同时,超微电镜下检测大鼠胃黏膜自噬情况,研究结果显示,与模型组相比,小建中汤各剂量组大鼠胃黏膜上皮细胞自噬泡数量不同程度减少。这提示小建中汤有效改善PLGC模型大鼠胃黏膜的病理损伤,其作用机制可能是通过抑制HIF-1α/Bnip3通路活化,降低Beclin-1、LC3Ⅱ表达,从而调节线粒体自噬水平,维持细胞内环境稳态。
黎乐怡等[42]通过动物实验研究认为,健脾化瘀解毒方抑制PLGC的作用机制可能与其通过PI3K/Akt/HIF-1α通路调节肿瘤缺氧微环境以及调控细胞自噬与凋亡相关。该研究观察健脾化瘀解毒方在不同时间点对大鼠胃黏膜组织中PI3K、Akt、HIF-1α、Beclin-1、抗凋亡蛋白(B-cell lymphoma-2,Bcl-2)、促凋亡蛋白(bcl-2-associated X,Bax)蛋白表达水平的影响,结果发现,MNNG+隔日进食法诱导的PLGC大鼠经健脾化瘀解毒方高剂量(9 g·kg-1)干预后,在第18周,大鼠胃黏膜组织PI3K蛋白表达水平较模型组升高,为模型组的2.31倍,Akt蛋白表达水平较模型组升高85.71%,HIF-1α、Bcl-2、Bcl-2/Bax蛋白表达水平分别较模型组降低23.33%、25.00%、46.67%(P<0.05,P<0.01)。第28周,大鼠胃黏膜组织PI3K蛋白表达水平较模型组升高,为模型组的2.50倍,Akt蛋白表达水平较模型组升高50.00%,HIF-1α、Beclin-1、Bcl-2/Bax蛋白表达水平分别较模型组降低12.00%、55.56%、54.54%(P<0.05,P<0.01)。第39周,大鼠胃黏膜组织Akt蛋白表达水平较模型组升高约92.00%,HIF-1α、Beclin-1、Bcl-2/Bax蛋白表达水平分别较模型组降低31.03%、25.71%、33.33%(P<0.05,P<0.01)。上述研究结果表明,健脾化瘀解毒方可能通过激活PI3K/Akt信号通路,降低HIF-1α及Bcl-2/Bax比值,在不同病程阶段调节胃黏膜细胞自噬与凋亡,改善大鼠胃黏膜病变,从而延缓胃癌前病变恶性进展。
胃祺饮具有益气活血化瘀之功。YIN等[43]研究发现,MNNG+高盐复合造模法构建的萎缩性胃炎伴癌前病变大鼠模型经高剂量胃祺饮组(4 g·kg-1)治疗后,蛋白免疫印迹法检测HIF-1α蛋白表达水平较模型组下降约50.00%(P<0.01)。高、中(2 g·kg-1)剂量细胞增殖标志物Ki-67蛋白表达水平分别较模型组下降约40.00%和36.00%(P<0.05,P<0.01)。细胞凋亡标志物Cleaved Caspase 3蛋白表达水平较模型组升高,分别为模型组的2.33和2.00倍。免疫组化染色高、中、低(1 g·kg-1)剂量细胞增殖标志物Ki-67蛋白表达水平分别较模型组下降约79.17%、50.00%和58.33%(P<0.01,P<0.001)。这表明胃祺饮可能通过调控HIF-1α信号通路,纠正胃黏膜细胞增殖和凋亡失衡状态,从而改善细胞稳态,抑制癌变进程。
闫巧等[12]研究发现党参低聚糖可通过调节细胞凋亡的线粒体途径抑制癌变细胞增殖。研究人员用党参低聚糖处理MNNG+40%乙醇+饥饱失常复合造模法诱导的PLGC模型大鼠,结果发现,0.15、0.60 g·kg-1党参低聚糖与模型组相比,大鼠胃黏膜组织Bcl-2/Bax蛋白表达水平(1.21±0.14、1.03±0.08 vs 1.85±0.13)均显著降低(均P<0.01),Caspase-3蛋白表达水平(0.81±0.10、0.94±0.04 vs 0.54±0.10)均显著升高(P<0.05,P<0.01);大鼠胃黏膜PI3K、p-AKT、HIF-1α蛋白表达水平(1.11±0.09、1.06±0.05 vs 1.35±0.07)、(1.24±0.23、1.12±0.09 vs 1.64±0.08)、(1.27±0.23、1.20±0.07 vs 1.87±0.11)均显著降低(P<0.05,P<0.01)。这说明党参低聚糖能够抑制PI3K/AKT/HIF-1α信号通路的异常活化,进而恢复胃黏膜上皮细胞增殖与凋亡的动态平衡,有效逆转PLGC。
HIF-1α在多种以缺氧为背景的疾病发生发展中发挥重要作用。胃癌前病变伴随局部缺氧微环境的形成,这一微环境促进了异型增生细胞对低氧环境的适应,并激活了HIF-1α的表达。研究PLGC中HIF-1α的相关调控机制,有助于深入揭示该病的发病机制,为临床药物研发提供理论支持。中医药通过调节HIF-1α相关信号通路,有望改善缺氧微环境,逆转胃黏膜病理改变。其作用机制包括抑制新生血管生成、糖代谢重编程,调控细胞自噬与凋亡等过程,从而保护胃黏膜,延缓PLGC的进展,展现出巨大的治疗潜力。因此,研究中医药作用于HIF-1α治疗PLGC的具体机制,不仅为中医药在分子层面的治疗提供了新方向,还有助于发现药物治疗新靶点。现有研究表明,中医药能够通过调节HIF-1α信号通路抑制癌变进程。然而,关于中医药调控HIF-1α的研究尚不充分,存在2大问题:一是相关的调控机制尚未完全阐明;二是临床应用证据不足。因此,深入探索中医药通过HIF-1α机制治疗PLGC的有效性与安全性,仍需科研人员进行更深入的研究和临床验证。
