Objective Comparing two detection methods for chlamydial trachomatis in the genital tract (GeneXpertⓇ CT/NG and Roche® Cobas 4800 CT/NG), we summarize their respective advantages and disadvantages to provide references for the promotion and application of these two methods. Methods During a field survey conducted in a sample hospital, a convenience sampling method was employed to select 60 patients. Vaginal swabs were collected from these patients for testing using the GeneXpert® CT/NG system, while urine samples were collected for testing with the Roche® Cobas 4800 CT/NG system. Staff members were tasked with recording and compiling data related to the testing process, including the time taken for testing, the cost of testing, and the positive detection rates. This data was then used to compare the two testing methods. Results The testing time for Roche® Cobas 4800 CT/NG is 293 minutes, while the testing time for GeneXpertⓇ CT/NG is 92.95 minutes. In other words, the testing time for Roche® Cobas 4800 CT/NG is 3.15 times longer than that of GeneXpert® CT/NG; When the number of tests conducted in the sample hospital reaches 3000 cases, the cost of Roche® Cobas 4800 CT/NG testing is approximately 1/5 of that of GeneXpert® CT/NG testing. As the number of tests increases, when the volume reaches 200,000 cases, the unit cost of Roche® Cobas 4800 CT/NG testing is approximately 1/3 of the unit cost of GeneXpert® CT/NG testing; Among the 60 participants in this study, 11 cases of CT positive patients were detected by GeneXpert® CT/NG, with a CT positive rate of 0.183. Roche®Cobas 4800 CT/NG detected 10 cases, with a CT positive rate of 0.167. Conclusion The GeneXpert® CT/NG test has a significant time advantage, as it takes less time and provides faster results; The Roche® Cobas 4800 CT/NG test has a cost advantage, with lower detection costs; The GeneXpert® CT/ NG test has a higher positive detection rate.

Objective Comparing two detection methods for chlamydial trachomatis in the genital tract (GeneXpertⓇ CT/NG and Roche® Cobas 4800 CT/NG), we summarize their respective advantages and disadvantages to provide references for the promotion and application of these two methods. Methods During a field survey conducted in a sample hospital, a convenience sampling method was employed to select 60 patients. Vaginal swabs were collected from these patients for testing using the GeneXpert® CT/NG system, while urine samples were collected for testing with the Roche® Cobas 4800 CT/NG system. Staff members were tasked with recording and compiling data related to the testing process, including the time taken for testing, the cost of testing, and the positive detection rates. This data was then used to compare the two testing methods. Results The testing time for Roche® Cobas 4800 CT/NG is 293 minutes, while the testing time for GeneXpertⓇ CT/NG is 92.95 minutes. In other words, the testing time for Roche® Cobas 4800 CT/NG is 3.15 times longer than that of GeneXpert® CT/NG; When the number of tests conducted in the sample hospital reaches 3000 cases, the cost of Roche® Cobas 4800 CT/NG testing is approximately 1/5 of that of GeneXpert® CT/NG testing. As the number of tests increases, when the volume reaches 200,000 cases, the unit cost of Roche® Cobas 4800 CT/NG testing is approximately 1/3 of the unit cost of GeneXpert® CT/NG testing; Among the 60 participants in this study, 11 cases of CT positive patients were detected by GeneXpert® CT/NG, with a CT positive rate of 0.183. Roche®Cobas 4800 CT/NG detected 10 cases, with a CT positive rate of 0.167. Conclusion The GeneXpert® CT/NG test has a significant time advantage, as it takes less time and provides faster results; The Roche® Cobas 4800 CT/NG test has a cost advantage, with lower detection costs; The GeneXpert® CT/ NG test has a higher positive detection rate.

生殖道沙眼衣原体(chlamydia trachomatis, CT)感染是目前最常见的性传播疾病之一。据世界卫生组织最新估计, 2020 年全球新发感染 1.286 亿和现症感染 1.275 亿,女性人群的发病率和患病率均高于男性 [ 1 ] 。对中国 105 个性病监测点的病例资料进行分析发现, 历年来, CT 感染报告发病率呈上升趋势, 报告发病率由 2015 年的 37.18/10 万上升到 2019 年的 55.32/10 万, 年均增长 10.44% [ 2 ] 。然而,实际感染人数可能更高,因为 50% 的男性和 80% 的女性沙眼衣原体感染通常无症状 [ 3 ] 。如果不加以治疗, 沙眼衣原体可能会导致生殖器官的长期严重损伤, 如慢性疼痛、盆腔炎、异位妊娠,严重可导致女性不孕不育。通过垂直传播还可导致新生儿损伤,如结膜炎、肺炎。对于男性可导致尿道炎、附睾炎、前列腺炎等 [ 4 ] 。因此,生殖道沙眼衣原体感染的防治具有重要的意义。只有选择合适有效的检测方法, 才能更有效地预防和控制生殖道沙眼衣原体的传播。
传统上, 培养方法一直被视为诊断生殖道沙眼衣原体的金标准。然而,这种传统培养方式涉及多个复杂步骤,包括样本的采集、处理、接种和培养, 随后还需要观察与鉴定以及结果判读, 整个流程耗时且繁琐, 培养周期长, 且其敏感性相对较低 [ 5 ] 。因此,目前这种方法在临床上的广泛应用面临挑战。
随着科学技术的发展, 产生了一些相对快速和敏感的检测方法 [ 6 ] 。核酸扩增试验(Nucleic Acid Amplification Test, NAAT) 由于其高度特异性和敏感性, 目前被大多数高收入国家采用, 比如美国、英国、欧盟、加拿大等,都将其推荐为 CT 的诊断方法 [ 7 ] 。 但 NAAT 需要多个步骤和昂贵的设备, 并且需要 1-2 天才能产生结果 [ 8 ] 。比如 Roche® Cobas 4800 CT/NG 检测 ( 图 1 )。该方法主要利用病原体的特异性核酸片段, 通过体外的不断扩增进行检测,灵敏度、特异度高,但是所需的仪器设备和试剂价格比较昂贵 [ 9 ] ,出具结果的时间较长。Roche® Cobas 4800 体积较大, 更适合放置在固定的实验室环境中, 因此在基层医疗机构的应用受到限制,未能广泛普及 [ 10 ]
GeneXpert@CT/NG 检测是一种快速的 NAAT ( 核酸扩增测试)分析技术, 它属于体外定量核酸诊断领域, 并且归类为 POCT (即时检验) 产品 [ 11 ] ,其设计小巧,便于运输,具体流程可见 图 2 。GeneXpert@CT/NG 检测自动化程度高、操作简便、 结果快速可靠。它可以在短时间内完成从样本处理到结果判定的全过程 [ 12 ] ,具备按需上机、随来随做的灵活性,对检测人员技术要求较低,因此满足临床快速诊断的需求 [ 13 ] 。此外,由于该检测方法具有高灵敏度和高特异性 [ 14 ] ,因此能够准确识别出病原体感染,减少误诊和漏诊的情况,但所需的仪器设备和试剂价格也比较昂贵 [ 15 ]
研究对象纳入标准: 18 岁及以上具有阴道分泌物的门诊就诊者女性,知情且同意参加本研究;研究对象排除标准:18 岁以下, 没有阴道分泌物, 拒绝参加本研究者。
目前国内的快速检测方法只有 GeneXpert@CT/NG 和 USTAR, GeneXpert® CT/NG 最早进入国内使用。Roche® Cobas 4800 CT/NG 检测是传统的检测方法,且是国际公认的金标准。
这两种方法都具备高敏感性和特异性,都基于 PCR 技术, 能够提供高敏感性和特异性的检测结果。并且都是高度自动化的系统, 能够减少人工操作, 提高检测效率和准确性。另外, 这两种方法都能同时检测多种病原体, 可以同时检测生殖道沙眼衣原体(Chlamydia Trachomatis, CT)和淋病奈瑟菌(Neisseria Gonorrhoeae, NG), 还都广泛用于临床诊断和治疗中, 可以帮助医生快速准确地诊断感染,指导治疗。因此选择这两种进口试剂具备良好的可比性。
本研究利用方便抽样收集了 60 位患者, 在深圳市某公立医院进行生殖道沙眼衣原体检测, 从每位患者处取阴道拭子和尿液标本。阴道拭子用于 GeneXpert@ CT/NG 检测,尿液标本用于 Roche® CT/NG 检测。项目开展点由 1 名负责协调整个研究工作的主要项目负责人、1 名临床 / 技术监察员以及若干现场和实验室工作人员组成, 由工作人员记录和整理本研究所需的数据, 从而对两种检测方法的检测时间、检测成本以及检测阳性率进行比较。
本研究深入样本医院进行调查, 分别调研了生殖道沙眼衣原体两种检测方法的业务流程 ( 表 1 -2)。
表 1 表 2 可以发现, Roche® Cobas 4800 CT/NG 检测与 GeneXpert@CT/NG 检测采集样本不同且检测的流程不同。 GeneXpert@ CT/NG 检测采集阴道拭子, Roche® Cobas 4800 CT/NG 检测采集尿液标本。Roche® Cobas 4800 CT/NG 检测比 GeneXpert® CT/NG 检测的流程要更加复杂繁琐。
本研究根据上述两种检测方法的步骤, 与医护人员访谈和计量等方法, 结合行业标准得出以下数据 ( 表 3 )。
本研究发现 Roche® Cobas 4800 CT/NG 的检测时间为 293 分钟, GeneXpert@CT/NG 检测时间为 92.95 分钟,即 Roche® Cobas 4800 CT/NG 检测是 GeneXpert@ CT/NG 检测时间的 3.15 倍。其余步骤时间差别不大。
本研究通过询问医院财务、管理人员以及设备提供方, 获得两种检测方法在不同检测量下的检测成本, 具体见 表 4
根据样本医院的实际情况,没有筛查项目时 CT/NG 检测量在 3000 例左右;有筛查项目时检测量在 200000 例左右。经调查, 当样本医院检测量在 3000 例时, Roche®Cobas 4800 CT/NG 检测单位成本约为 GeneXpert@CT/NG 检测的单位成本的 1/5, 当检测量在 200000 例时, Roche®Cobas 4800 CT/NG 检测单位成本约为 GeneXpert@CT/NG 检测单位成本的 1/3。
检测阳性率为本研究的 60 位参与者中 CT 检测阳性人数占总人数的比重,具体结果见 表 5
调查结果显示, GeneXpert@ CT/NG 检测出 11 例 CT 阳性患者,故 CT 阳性率为 0.183。Roche®Cobas4800 CT/NG 检测出 10 例 CT 阳性患者,故 CT 阳性率为 0.167 。
总之,两种检测方法各有优势。GeneXpert@CT/NG 检测时间更短, 出结果更快, 检测阳性检出率更高; Roche®Cobas ${4800}\mathrm{{CT}}/\mathrm{{NG}}$ 检测成本更低。
如何选择取决于医院的需求。若医院条件允许, 可以选择出结果快且阳性率相对较高的 GeneXpert@ CT/NG 检测。若不追求检测速度,可选择价格相对较低的 Roche®Cobas 4800 CT/NG 检测。
受限于研究水平和条件, 本文仅对比分析了这两种检测方法。未来还需要进一步拓展研究范围, 需要对更多的生殖道沙眼衣原体检测方法开展对比研究, 以期为其检测方法的推广应用提供参考。
