Objective To study the application of cell morphology inblood smear and automatic blood cell analyzer detection in blood routineexamination. Methods A analys was was conducted on 180 patients withsuspected Macrocellular anemia at Yishui County People’s Hospital fromFebruary 2020 to February 2023. The positive detection rates of diseasewere statwastically analyzed for individual and combined tests, and thepositive detection rates of toxic particles/vacuolar degeneration,nucleated red blood cells, and atypical lymphocytes were alsostatwastically analyzed for individual and combined tests, statwasticalanalyswas of diagnostic efficacy for individual and joint testing. Results The positive detection rates of monocytes, neutrophils,lymphocytes, basophils, and eosinophils detected by the combination ofblood smear cell morphology and fully automated blood cell analyzer werehigher than those detected alone $\left({P<{0.05}}\right)$ . The positive detection rates of toxicparticle/vacuolar degeneration, nucleated red blood cells, and atypicallymphocytes detected by the combination of blood smear cell morphologyand fully automated blood cell analyzer were higher than those detectedalone $\left({P <{0.05}}\right)$ .The sensitivity, specificity, accuracy, and negative predictive value ofthe combined detection of blood smear cell morphology and fullyautomated blood cell analyzer were higher than those of individualdetection $\left({P<{0.05}}\right)$ . Conclusion The value of combined detectionof blood smear cell morphology and fully automated blood cell analyzerin blood routine examination is higher, so is worthy of promotion.

目的 研究全自动血细胞分析仪、血涂片细胞形态学检测在血常规检验中的应用。方法 选取 2020 年 2 月 - 2023 年 2 月沂水县人民医院疑似大细胞性贫血患者 180例,统计分析单独与联合检测疾病阳性检出情况,并统计分析单独与联合检测中毒颗粒/空泡变性、异型淋巴细胞、有核红细胞阳性检出情况,统计分析单独与联合检测诊断效能。结果 联合检测单核细胞、中性粒细胞、淋巴细胞、嗜酸性细胞、嗜碱性细胞阳性检出率、中毒颗粒给泡变性、异型淋巴细胞、有核红细胞阳性检出率均高于单独检测( $P<{0.05}$ )。联合检测的灵敏性、特异性、准确性、阴性预测值均高于单独检测( $P<{0.05}$ )。结论 全自动血细胞分析仪、血涂片细胞形态学联合检测在血常规检验中的价值较高,值得推广。

目的 研究全自动血细胞分析仪、血涂片细胞形态学检测在血常规检验中的应用。方法 选取 2020 年 2 月 - 2023 年 2 月沂水县人民医院疑似大细胞性贫血患者 180例,统计分析单独与联合检测疾病阳性检出情况,并统计分析单独与联合检测中毒颗粒/空泡变性、异型淋巴细胞、有核红细胞阳性检出情况,统计分析单独与联合检测诊断效能。结果 联合检测单核细胞、中性粒细胞、淋巴细胞、嗜酸性细胞、嗜碱性细胞阳性检出率、中毒颗粒给泡变性、异型淋巴细胞、有核红细胞阳性检出率均高于单独检测( $P<{0.05}$ )。联合检测的灵敏性、特异性、准确性、阴性预测值均高于单独检测( $P<{0.05}$ )。结论 全自动血细胞分析仪、血涂片细胞形态学联合检测在血常规检验中的价值较高,值得推广。

Objective To study the application of cell morphology inblood smear and automatic blood cell analyzer detection in blood routineexamination. Methods A analys was was conducted on 180 patients withsuspected Macrocellular anemia at Yishui County People’s Hospital fromFebruary 2020 to February 2023. The positive detection rates of diseasewere statwastically analyzed for individual and combined tests, and thepositive detection rates of toxic particles/vacuolar degeneration,nucleated red blood cells, and atypical lymphocytes were alsostatwastically analyzed for individual and combined tests, statwasticalanalyswas of diagnostic efficacy for individual and joint testing. Results The positive detection rates of monocytes, neutrophils,lymphocytes, basophils, and eosinophils detected by the combination ofblood smear cell morphology and fully automated blood cell analyzer werehigher than those detected alone $\left({P<{0.05}}\right)$ . The positive detection rates of toxicparticle/vacuolar degeneration, nucleated red blood cells, and atypicallymphocytes detected by the combination of blood smear cell morphologyand fully automated blood cell analyzer were higher than those detectedalone $\left({P <{0.05}}\right)$ .The sensitivity, specificity, accuracy, and negative predictive value ofthe combined detection of blood smear cell morphology and fullyautomated blood cell analyzer were higher than those of individualdetection $\left({P<{0.05}}\right)$ . Conclusion The value of combined detectionof blood smear cell morphology and fully automated blood cell analyzerin blood routine examination is higher, so is worthy of promotion.

在各科疾病的诊断中,血常规检验是常规项目 [ 1 ] 。血涂片细胞形态学能够将血细胞形态变化信息准确提供给临床, 全自动血细胞分析仪分类检测白细胞时能够依据光电信号鉴别白细胞种类,二者是两种血常规检验常用手段,但是在血液系统疾病、 感染性疾病诊断中,前者较后者具有较低的效能,会有假阳性或假阴性结果出现 [ 2 ] 。二者联合检测能够对单独检测的不足进行弥补 [ 3 ] 。本文统计分析了 2020 年 2 月 -2023 年 2 月沂水县人民医院疑似大细胞性贫血患者 180 例的临床资料, 研究了全自动血细胞分析仪、血涂片细胞形态学检测在血常规检验中的应用, 以期为临床诊疗提供更有效的依据, 现将结果报道如下。
选取 2020 年 2 月 -2023 年 2 月沂水县人民医院疑似大细胞性贫血患者 180 例,年龄 4~71 岁,平均 (33.25±5.20) 岁, 女性 82 例,男性 98 例。体质量 15~75 kg,平均 (38.26±6.34) kg。本研究经本院医学伦理委员会批准 ( 2020 0112)。
纳入标准:①检验样本均合格;②均具有正常的言语沟通能力。排除标准:①有视力及听力障碍;②合并恶性肿瘤。
采用全自动血细胞分析仪检测血液标本中各细胞阳性率, 白细胞计数、中性粒细胞计数、单核细胞计数、嗜酸性粒细胞占白细胞总数百分比、血小板计数、有核红细胞计数分别在 ${3.0}\times {10}^{9}/\mathrm{L}$${2.0}\times {10}^{9}/\mathrm{L}$${0.3}\times {10}^{9}/\mathrm{L}$${0.004}\%$${60.0}\times {10}^{9}/\mathrm{L}$${3.5}\times {10}^{12}/\mathrm{L}$ 以下或 ${28.0}\times {10}^{9}/\mathrm{L}$${7.5}\times {10}^{9}/\mathrm{L}$${0.8}\times {10}^{9}/\mathrm{L}$${0.08}\%$${300.0}\times {10}^{9}/\mathrm{L}$${5.5}\times$ ${10}^{12}/\mathrm{L}$ 以上,异型淋巴细胞百分比、空泡变性粒细胞、有中毒颗粒的中性粒细胞占中性粒细胞总数百分比分别在 3%、10%、 10% 以上, 评定为阳性 [ 4 ]
对有核细胞进行复检,包括形态学、血液寄生虫镜检等。 有异型淋巴细胞出现; 嗜中性粒细胞颗粒缺乏均匀的分布、统一的大小, 具有较深的着色; 空泡胞体肿大, 胞浆清亮, 呈空泡状;中性粒细胞缺乏均匀的大小,颗粒增粗、空泡变性,核固缩, 核左移; 红细胞呈靶形或球形或椭圆形, 细胞缺乏统一的大小, 中央苍白区扩增, 评定为阳性 [ 5 ]
①单独与联合检测疾病阳性检出情况;②单独与联合检测中毒颗粒 / 空泡变性、有核红细胞、异型淋巴细胞阳性检出情况; ③单独与联合检测诊断效能。
采用 SPSS28.0,计数资料用率表示,用 ${\chi }^{2}$ 检验,检验水准 $\alpha ={0.05}$
联合检测疾病阳性检出率均高于单独检测( $P <{0.05}$ )。见 表 1 -2
联合检测中毒颗粒/空泡变性、有核红细胞、异型淋巴细胞阳性检出率均高于单独检测( $P <{0.05}$ )。见 表 3
180 例患者中,临床确诊阳性 160 例,阴性 20 例;血涂片细胞形态学检测阳性 133 例, 阴性 47 例; 全自动血细胞分析仪检测阳性 136 例, 阴性 44 例; 联合检测阳性 156 例, 阴性 24 例。以临床确诊为金标准,全自动血细胞分析仪检测的灵敏性为 82.50%( 132/160 ), 特异性为 80.00%( 16/20 ), 准确性为 82.22%( 148/180 ), 阳性预测值为 97.06%( 132/136 ), 阴性预测值为 36.36%( 16/44 )。血涂片细胞形态学检测的灵敏性为 81.25%( 130/160 ), 特异性为 85.00%( 17/20 ), 准确性为 81.67%( 147/180 ), 阳性预测值为 97.74%( 130/133 ), 阴性预测值为 36.17%( 17/47 )。联合检测的灵敏性为 96.88%( 155/160 ), 特异性为 95.00%(19/20), 准确性为 96.67%(174/180), 阳性预测值为 99.36%( 155/156 ), 阴性预测值为 79.17%( 19/24 )。 联合检测的灵敏性、特异性、准确性、阴性预测值均高于单独检测 ( $P <{0.05}$ ),但单独与联合检测的阳性预测值之间的差异不显著 ( $P >{0.05}$ )。见 表 4 -5
血常规检查, 就是抽取病人的外周血, 通过全自动血液分析仪, 对病人外周血中各种血细胞的数量, 形态, 比例进行检查,
从而对病人身体的健康状况进行诊断的一种检查方法 [ 6 ] 。通过血常规检查, 能够发现人体是否有血液系统疾病, 同时还能够发现是否有某些感染性疾病,对临床做出有用的指导 [ 7 ] 。比如, 如果血常规检查时, 发现了病人外周血中白细胞数量异常增高, 同时以中性粒细胞增高明显,另外, 病人还有感染发热等一系列症状,这时候就提示病人发生了严重感染的情况 [ 8 ] 。另外, 如果血常规检查时, 发现了病人外周血中全血细胞数量不同程度的降低,那么在这种情况下,就不排除再生障碍性贫血,骨髓增生异常综合征, 低增生性白血病等血液系统疾病, 往往需要通过骨髓穿刺检查来进一步确诊 [ 9 ] 。血常规化验检查的项目是比较多的, 一般需要关注三个方面的检查结果。第一个是血红蛋白的检查结果。第二个是白细胞的分类与计数的检查结果。 第三个是血小板计数的检查结果。这三方面能比较直接地反映人体的问题 [ 10 - 11 ]
有研究表明 [ 12 - 14 ] ,全自动血细胞分析仪是一种利用先进的光学、计算机、生物技术等综合手段,对体液中的细胞及其他成分进行快速自动分析、计数和分类的现代化生化分析仪器。 是临床检验、生物科研和医学教育中不可或缺的重要设备。血涂片细胞检查时可发现患者的血细胞,如红细胞、白细胞、血小板等。本研究结果表明, 联合检测疾病阳性检出率、中毒颗粒/空泡变性、异型淋巴细胞、有核红细胞阳性检出率均高于单独检测, 灵敏性、特异性、准确性、阴性预测值均高于单独检测,原因为血涂片细胞形态学检测在光学显微镜下制片、染色等,然后观察患者血细胞形态,一方面能够对患者细胞学形态进行分析, 另一方面还能够将患者病变情况呈现出来。二者联合检测能够优势互补,进一步提升检验符合率 [ 15 ]
综上所述, 联合检测的应用价值较单独检测高, 值得推广。
