Objective To explore the pathogenic factors causing outbreaks of bacterial foodborne diseases. Methods Collecting anal swabs, food retention samples, and food processing tool application samples from patients affected by this epidemic, refer to "Microbiological Testing of Food Hygiene" (GB/T 4789.4-2016、GB/T 4789.5-2012、GB/T 4789.6-2016, GB/T 4789.7-2013, GB/T 4789.10-2016) and "National Clinical Testing Operating Procedures" (Fourth Edition). Results Aeromonas sobria was detected in food samples from little yellow croaker and anal swabs from two patients. Aeromonas hydrophila was detected in a kitchen appliance application and one patient's anal swab. Conclusion This was a rare outbreak of foodborne disease caused by mixed Aeromonas, In the diagnosis and treatment of intestinal infectious diarrhea, in addition to considering common intestinal pathogenic bacteria, rare pathogenic bacteria such as Aeromonas should not be ignored to avoid clinical diagnosis and treatment difficulties.

目的 探究引起细菌性食源性疾病暴发的病原学原因,了解气单胞菌污染食品引起聚集性事件的特征,为类似事件的调查处置提供参考。方法 采集本起疫情患者肛拭子、食品留样、食品加工工具涂抹样,参照《食品卫生微生物学检验》《全国临床检验操作规程》(第四版)进行检验。结果 在小黄鱼食物留样和两名患者肛拭子中,检出温和气单胞菌。在一份厨房用具涂抹液和其中一名患者肛拭子中,检出嗜水气单孢菌。结论 这是一起罕见的混合性气单胞菌引起的食源性疾病暴发事件,提示我们在肠道感染性腹泻的诊治过程中,除了考虑常见肠道致病菌外,气单胞菌等罕见致病菌也不容忽视,以免给临床诊断和治疗带来困扰。

目的 探究引起细菌性食源性疾病暴发的病原学原因,了解气单胞菌污染食品引起聚集性事件的特征,为类似事件的调查处置提供参考。方法 采集本起疫情患者肛拭子、食品留样、食品加工工具涂抹样,参照《食品卫生微生物学检验》《全国临床检验操作规程》(第四版)进行检验。结果 在小黄鱼食物留样和两名患者肛拭子中,检出温和气单胞菌。在一份厨房用具涂抹液和其中一名患者肛拭子中,检出嗜水气单孢菌。结论 这是一起罕见的混合性气单胞菌引起的食源性疾病暴发事件,提示我们在肠道感染性腹泻的诊治过程中,除了考虑常见肠道致病菌外,气单胞菌等罕见致病菌也不容忽视,以免给临床诊断和治疗带来困扰。

Objective To explore the pathogenic factors causing outbreaks of bacterial foodborne diseases. Methods Collecting anal swabs, food retention samples, and food processing tool application samples from patients affected by this epidemic, refer to "Microbiological Testing of Food Hygiene" (GB/T 4789.4-2016、GB/T 4789.5-2012、GB/T 4789.6-2016, GB/T 4789.7-2013, GB/T 4789.10-2016) and "National Clinical Testing Operating Procedures" (Fourth Edition). Results Aeromonas sobria was detected in food samples from little yellow croaker and anal swabs from two patients. Aeromonas hydrophila was detected in a kitchen appliance application and one patient's anal swab. Conclusion This was a rare outbreak of foodborne disease caused by mixed Aeromonas, In the diagnosis and treatment of intestinal infectious diarrhea, in addition to considering common intestinal pathogenic bacteria, rare pathogenic bacteria such as Aeromonas should not be ignored to avoid clinical diagnosis and treatment difficulties.

气单胞菌 (Aeromonas) 隶属于弧菌科, 革兰氏染色阴性, 绝大多数有单鞭毛,动力阳性,兼性厌氧,在 $0 \sim {45}^{\circ }\mathrm{C}$ 范围内均可生长,最适生长温度为 ${30}^{\circ }{35}^{\circ }{\mathrm{C}}^{\left\lbrack 1\right\rbrack }$ 。该菌属广泛存在于湖水、 河水、土壤中,也可存在于禽畜肉等食品中 [ 2 ] ,是一种人 - 畜 - 鱼共患病病原菌 [ 3 ] ,可引起鱼、虾等水生动物患病,给养殖产业造成较大的经济损失 [ 4 ] 。人类因食用加热不彻底的食物,或饮用被气单胞菌污染的水源而患病, 可引起肠道内感染和肠道外感染 [ 5 - 6 ] 。肠道内感染主要表现为感染性腹泻、急性胃肠炎, 肠道外感染可引起肺炎、胆囊炎、尿路感染、败血症等 [ 7 - 9 ]
目前发现的气单胞菌共有 30 余种, 其中与人类疾病高度相关的主要有嗜水气单胞菌(A. hydrophila)、豚鼠气单胞菌(A. caviae)、维隆气单胞菌温和生物型 ( $A$ . veronii biovar. sobria) 和达卡气单胞菌(A. dhakensis) [ 10 ] 。气单胞菌携带有肠毒素(act)、 气溶素(aerA)、溶血素(hlyA)、不耐热肠毒素(alt)、耐热肠毒素(ast)五种毒力因子,其中以嗜水气单胞菌毒力基因的携带率最高 [ 11 - 12 ] 。本研究对一起食物中毒中的样品进行病原学检测, 通过分离培养及生化鉴定分析, 为一起食源性暴发事件溯源提供了技术支持, 为后续的治疗处置工作提供了依据。
接报疑似食源性疾病暴发后, 现场处置过程中采集到的食物留样 7 份, 厨具、餐具涂抹液 13 份, 患者肛拭子 11 份。
碱性蛋白胨水、肉浸液肉汤、志贺氏沙门氏菌增菌液、7.5% 氯化钠肉汤、SS 平板、TCBS 平板、麦康凯平板、血平板、B-P 平板、克氏双糖铁培养基均购自广东环凯微生物科技有限公司。 V-P 试剂购自北京陆桥技术有限公司。革兰氏阴性细菌 GN 鉴定卡片和革兰氏阳性细菌GP 鉴定卡片购自生物梅里埃中国有限公司。质控菌株鼠伤寒沙门氏菌(ATCC14028)和副溶血性弧菌 ( ATCC17802 ) 购自广东环凯微生物科技有限公司。所有培养基和试剂均鉴定合格,在有效期内使用。
VITEK 2 Compact 细菌自动鉴定与药敏系统为法国生物梅里埃公司生产。
病原菌分离与鉴定参照 GB 4789.4-2016《食品安全国家标准 食品微生物学检验 沙门氏菌检验》、GB 4789.6-2016《食品安全国家标准 食品微生物学检验 致泻大肠埃希氏菌检验》 GB 4789.5-2012《食品安全国家标准 食品微生物学检验 志贺氏菌检验》、GB 4789.7-2013《食品安全国家标准 食品微生物学检验 副溶血性弧菌检验》《全国临床检验操作规程》(第四版)进行。 V-P 试验用于辅助鉴别嗜水气单胞菌和豚鼠气单胞菌, V-P 试验阳性为嗜水气单胞菌, V-P 试验阴性为豚鼠气单胞菌。
于SS平板 ${36}^{\circ }\mathrm{C}$ 培养 ${24}\mathrm{\;h}$ 后,发现 5 份样品出现光滑、湿润、 边缘整齐、中等大小、无色半透明的圆形菌落,疑似肠道致病菌。 5 株分离株均为革兰氏染色阴性短小杆菌, 无芽孢, 无荚膜。转血平板 ${36}^{\circ }\mathrm{C}$ 培养 ${24}\mathrm{\;h}$ ,菌落为灰白色半透明,较大菌落,有 $\beta$ 溶血环。 克氏双糖 $\mathrm{K}/\mathrm{A}$ ,不产气,不产 ${\mathrm{H}}_{2}\mathrm{\;S}$ ,触酶阳性,氧化酶阳性。
其中 3 株 VITEK 2 Compact 鉴定结果为温和气单胞菌, 符合率 97%。具体生化反应结果见 表 1
另 2 株 VITEK 2 Compact 鉴定结果为嗜水气单胞菌 / 豚鼠气单
胞菌,符合率 99%。辅助试验 V-P 阳性。具体生化反应结果见 表 2
在小黄鱼食物留样和两名患者肛拭子中, 检出温和气单胞菌。在一份厨房用具涂抹液和其中一名患者肛拭子中,检出嗜水气单孢菌。所有样品中未检出其他致病菌。
感染性腹泻可见于每个季节,尤以夏、秋季高发,其中食源性细菌感染和诺如病毒感染最为常见 [ 13 ] 。随着生活条件的提高, 我国食源性感染性腹泻的主要病原菌主要为致泻性大肠埃希菌, 其次为沙门菌属、弧菌属、空肠弯曲菌、金黄色葡糖球菌、 气单胞菌 [ 14 ]
气单胞菌感染的主要症状为腹痛、腹泻、恶心、呕吐等 [ 15 ] , 与沙门氏菌、副溶血性弧菌、志贺氏菌感染的症状相似,也有小部分人群为无症状携带者 [ 16 ] ,因此需要准确的病原学检测结果加以鉴别。
常规的食源性疾病检测中, 我们首先考虑的病原菌主要是沙门氏菌、金黄色葡萄球菌、副溶血性弧菌,而气单胞菌引起的食源性疾病暴发报道较少, 症状又与沙门氏菌等引起的疾病类似, 且该菌目前尚没有实验室分离与鉴定的国家标准检测方法, 所以我们并未将该菌列入常规的食源性致病菌进行检测, 容易导致漏检。基于以上原因,现场工作人员需与实验室检验人员要多协调沟通,对于有明显食源性疾病特征的聚集性病例, 如果未检出常见致病菌, 需要把优势菌进行分离鉴定, 以免漏检 [ 17 ] 。由于气单胞菌在水中的高检出率 [ 4 ] ,发达国家(如美国) 已将嗜水气单胞菌列入了水质污染指示菌名录, 公布了饮用水中气单胞菌的标准检测方法 [ 1 ]
随着水生养殖产业的发展, 经营者发现气单胞菌对养殖产业的危害越来越大,于是开始使用一些廉价抗生素来减少损失。 如此一来,气单胞菌对抗生素的耐药性也增强了 [ 6 , 18 ] 。有研究表明部分气单胞菌能产生 3 种 $\beta$ -内酰胺酶,包括 $\mathrm{B}$ 类金属内酰胺酶、C类头孢菌素酶和 D类青霉素酶,分别能水解碳青酶烯类抗菌药物、广谱头孢类抗菌药物以及青霉素类 [ 19 ] ,因此我们除了严格规范临床的抗生素使用外, 对水产养殖业的要求也要规范起来。喹诺酮类药物对气单胞菌的生物被摸具有明显的抑制作用 [ 20 ] ,且成本相对低廉,深受养殖业的欢迎,但是监管机构应当给出明确应用指导, 防止这类药物也被滥用, 使病原菌形成新的耐药机制。
随着经济的发展和人民生活水平的提高, 我们需要不断地更新检测能力和知识水平, 提高不明原因腹泻的检出率并出台应对措施, 同时加强对餐饮行业经营者和从业人员培训, 切实做到 “生熟分开”,对重点人群开展食品安全和个人卫生相关知识宣传, 尽量减少食源性疾病的发生。
