Objective To explore the application effect of C-reactive protein combined with blood routine and blood culture in the diagnosis of bacteremia. Methods In this study, 69 patients with suspected bacteremia examined from May 2023 to May 2024 were selected as the observation group. In order to more intuitively understand the application value of the three combined detection methods, 69 healthy patients in the same period were selected as the experimental group. Analysis of blood routine indexes, strain distribution in blood culture and C-reactive protein detection data in both participating groups. Results In the blood cultures. Results A total of 8 strains were detected in the blood culture of 69 suspected bacteremia patients in the observation group. Among the two strains with high detection rate, except gram-negative bacteria and gram-positive bacteria, the two strains with the highest detection rate were staphylococcus 25 strains. Escherichia coli 16 granules, and 16 E. coli with detection rate of${23.19}\%$. WBC$\left({{15.45}\pm {2.18}}\right)\times {10}^{9}/\mathrm{L}$, NEU$\left({{81.24}\pm {2.84}}\right)\%$, LYM$\left({{53.24}\pm {2.68}}\right)\%$, C-reactive protein levels (53.24±2.68) mg/L, WBC (6.45±1.28)×10${}^{9}$/L, NEU (62.41±2.34)%, LYM (26.45±2.18)%, and C-reactive protein levels$\left({{6.28}\pm {1.25}}\right)\mathrm{{mg}}/\mathrm{L}$. WBC, NEU, LYM and C-reactive protein levels in patients with suspected bacteremia in the observation group were significantly higher than those of healthy patients in the experimental group$\left({P <{0.05}}\right)$. Of the 69 suspected bacteremia, 66 were positive and 3 were negative. Among them, 63 cases were positive in blood culture, 58 cases were positive in blood routine test, 56 cases were positive in C-reactive protein test, and 66 cases were positive in C-reactive protein combined with blood routine and blood culture test. Among the four detection methods, C-reactive protein combined with blood routine and blood culture test were no different with the confirmed results. Conclusion In clinical diagnosis and detection of suspected bacteremia patients, C-reactive protein combined with blood routine and blood culture can maximize the accuracy of detection data, and the combined diagnosis of the three is of higher clinical value.

目的 探究 C-反应蛋白联合血常规、血培养检测方式在菌血症诊断中的应用效果。方法 本次研究选取时间点为 2023 年 5 月 -2024 年 5 月检查的 69 例疑似菌血症患者作为观察组,为更直观了解三种联合检测方式的应用价值,本研究选取同期 69 例健康人群作为实验组。对两组参与研究人群的血常规指标、血培养中菌株分布以及$\mathrm{C}-$反应蛋白检测数据进行分析。结果 观察组 69 例疑似菌血症患者的血培养检测中,共检出 8 种菌株,检出率较高的两种菌株中除革兰阴性菌和革兰阳性菌外,检出率最高的两种菌株为葡萄球菌 25 株,大肠埃希菌 16 株。观察组疑似菌血症的患者 WBC(15.45±2.18)×10°/L、NEU(81.24±2.84)%、LYM(53.24±2.68)%,以及C-反应蛋白水平(53.24±2.68)mg/L;实验组健康人群 WBC(6.45±1.28)×10${}^{9}$/L、NEU(62.41±2.34)%、LYM(26.45±2.18)%, 以及C-反应蛋白水平(6.28±1.25)mg/L。观察组疑似菌血症的患者WBC、NEU、LYM 以及C-反应蛋白水平, 明显高于实验组的健康人群$\left({P <{0.05}}\right)$。69 例疑似菌血症中阳性 66 例,阴性 3 例。其中血培养中阳性 63 例,血常规检测阳性 58 例, C- 反应蛋白检测中阳性 56 例, C- 反应蛋白联合血常规、血培养检测方式中阳性 66 例,四种检测方式中, C-反应蛋白联合血常规、血培养检测与确诊结果无差别。结论 临床在对疑似菌血症患者进行诊断检测时,使用$\mathrm{C}$- 反应蛋白联合血常规、血培养的方式进行检测,能够最大限度地提高检测数据的准确度,三者联合诊断临床价值更高。

目的 探究 C-反应蛋白联合血常规、血培养检测方式在菌血症诊断中的应用效果。方法 本次研究选取时间点为 2023 年 5 月 -2024 年 5 月检查的 69 例疑似菌血症患者作为观察组,为更直观了解三种联合检测方式的应用价值,本研究选取同期 69 例健康人群作为实验组。对两组参与研究人群的血常规指标、血培养中菌株分布以及$\mathrm{C}-$反应蛋白检测数据进行分析。结果 观察组 69 例疑似菌血症患者的血培养检测中,共检出 8 种菌株,检出率较高的两种菌株中除革兰阴性菌和革兰阳性菌外,检出率最高的两种菌株为葡萄球菌 25 株,大肠埃希菌 16 株。观察组疑似菌血症的患者 WBC(15.45±2.18)×10°/L、NEU(81.24±2.84)%、LYM(53.24±2.68)%,以及C-反应蛋白水平(53.24±2.68)mg/L;实验组健康人群 WBC(6.45±1.28)×10${}^{9}$/L、NEU(62.41±2.34)%、LYM(26.45±2.18)%, 以及C-反应蛋白水平(6.28±1.25)mg/L。观察组疑似菌血症的患者WBC、NEU、LYM 以及C-反应蛋白水平, 明显高于实验组的健康人群$\left({P <{0.05}}\right)$。69 例疑似菌血症中阳性 66 例,阴性 3 例。其中血培养中阳性 63 例,血常规检测阳性 58 例, C- 反应蛋白检测中阳性 56 例, C- 反应蛋白联合血常规、血培养检测方式中阳性 66 例,四种检测方式中, C-反应蛋白联合血常规、血培养检测与确诊结果无差别。结论 临床在对疑似菌血症患者进行诊断检测时,使用$\mathrm{C}$- 反应蛋白联合血常规、血培养的方式进行检测,能够最大限度地提高检测数据的准确度,三者联合诊断临床价值更高。

Objective To explore the application effect of C-reactive protein combined with blood routine and blood culture in the diagnosis of bacteremia. Methods In this study, 69 patients with suspected bacteremia examined from May 2023 to May 2024 were selected as the observation group. In order to more intuitively understand the application value of the three combined detection methods, 69 healthy patients in the same period were selected as the experimental group. Analysis of blood routine indexes, strain distribution in blood culture and C-reactive protein detection data in both participating groups. Results In the blood cultures. Results A total of 8 strains were detected in the blood culture of 69 suspected bacteremia patients in the observation group. Among the two strains with high detection rate, except gram-negative bacteria and gram-positive bacteria, the two strains with the highest detection rate were staphylococcus 25 strains. Escherichia coli 16 granules, and 16 E. coli with detection rate of${23.19}\%$. WBC$\left({{15.45}\pm {2.18}}\right)\times {10}^{9}/\mathrm{L}$, NEU$\left({{81.24}\pm {2.84}}\right)\%$, LYM$\left({{53.24}\pm {2.68}}\right)\%$, C-reactive protein levels (53.24±2.68) mg/L, WBC (6.45±1.28)×10${}^{9}$/L, NEU (62.41±2.34)%, LYM (26.45±2.18)%, and C-reactive protein levels$\left({{6.28}\pm {1.25}}\right)\mathrm{{mg}}/\mathrm{L}$. WBC, NEU, LYM and C-reactive protein levels in patients with suspected bacteremia in the observation group were significantly higher than those of healthy patients in the experimental group$\left({P <{0.05}}\right)$. Of the 69 suspected bacteremia, 66 were positive and 3 were negative. Among them, 63 cases were positive in blood culture, 58 cases were positive in blood routine test, 56 cases were positive in C-reactive protein test, and 66 cases were positive in C-reactive protein combined with blood routine and blood culture test. Among the four detection methods, C-reactive protein combined with blood routine and blood culture test were no different with the confirmed results. Conclusion In clinical diagnosis and detection of suspected bacteremia patients, C-reactive protein combined with blood routine and blood culture can maximize the accuracy of detection data, and the combined diagnosis of the three is of higher clinical value.

临床全身血液感染性疾病中,发病率较高的一种就是菌血症,这种疾病出现的原因主要是由于血液内有病菌侵入,从而导致病菌随血液流入全身并在全身繁殖。患者在发病后会出现呼吸困难、心率加快以及高烧不止等多种不良症状。菌血症的发病速度较快,容易威胁其他身体器官及组织的正常运行,患者在发病后死亡率较高。为降低菌血症患者死亡率,促进预后改善最主要的工作就是做好菌血症的早期诊断[1]。在菌血症诊断中, 血培养需要对患者进行多次采血, 受检查时间长, 采血次数多的影响, 容易对血液标本质量检验结果产生影响, 即使最后结果为阴性也不能确定患者不属于血流感染[2]。在此基础上, 毒血症、脓血症等疾病需要与菌血症进行有效区分和鉴别, 在临床诊断检测中单纯依靠血培养检查无法进行正常判断,需要与其他方式联合使用, 才能够达到提高菌血症临床诊断效率的目的。基于此,本文将深入分析菌血症诊断中$\mathrm{C}-$反应蛋白联合血常规、血培养检测的应用,希望通过本次研究活动的开展, 提高临床菌血症的诊断效率与诊断质量,让患者能够及时接受治疗。具体研究内容如下。
本次研究选取时间点为 2023 年 5 月 -2024 年 5 月检查的 69 例疑似菌血症患者作为观察组,采用 C-反应蛋白联合血常规、 血培养检测, 为更直观了解三种联合检测方式的应用价值, 选取同期 69 例健康人群作为实验组。年龄在 32~76 岁,平均年龄为(53.46±2.45)岁;男 35 例,女 34 例。观察组,年龄在 33~77 岁, 平均年龄为$\left({{54.55}\pm {2.63}}\right)$ 岁。两组参与研究的对象基本资料无统计学价值 ($P >{0.05}$ )。符合医学伦理学标准。
纳入标准:(1)疑似菌血症观察组研究对象,体温$\geq {38}^{\circ }\mathrm{C}$ , 心率超过 90 次$/\mathrm{{min}}$ ,呼吸频率大于 20 次$/\mathrm{{min}}$ ;(2) 临床资料完整;(3)两组研究对象知晓并同意参与本次研究。
排除标准:(1)有传染性疾病;(2)合并多种感染病患者;(3)治疗前半年内使用过抗菌药物。
参与本次研究的 69 疑似菌血症患者与 69 例健康人群均需要开展 C- 反应蛋白、血常规、血培养检测。
进行$\mathrm{C}-$ 反应蛋白检测中,医务人员需要选择免疫比浊法检测血清$\mathrm{C}-$ 反应蛋白值。血培养中,所有研究对象需要进行两次血培养, 这两次血培养的时间需要间隔一小时, 并且在医务人员进行静脉穿刺中需要从不同位置进行静脉穿刺取血,将血液注入血培养瓶中,由专人在两小时内将血液样品送至微生物实验室进行血培养。血常规 (WBC), 在进行检测时需要对患者静脉血进行抽取,并采用全自动血细胞分析仪(BC-5000 Vet,迈瑞)完成血常规检测。
在临床对菌血症的判断中,判断标准为:(1) 血清$\mathrm{C}-$ 反应蛋白值大于$8\mathrm{{mg}}/\mathrm{L}$ ;(2)血培养显示未阳性;(3)血常规超过${10}\times {10}^{9}/\mathrm{L}$ 。
(1)观察组 69 例中不同菌株检出率。了解 69 例疑似病例中患者出现菌株类型、例数,了解其检出率。
(2)两组研究对象血常规及 C- 反应蛋白水平对比。其中血常规检查包含:白细胞计数 (WBC)、中性粒细胞比例 (NEU)、 淋巴细胞比例 (LYM)。
(3)不同检测方式与诊断结果对比。诊断结果中 66 例为阳性,3 例为阴性,以此标准判断 C- 反应蛋白、血常规、血培养检测及 C- 反应蛋白联合血常规、血培养检测阳性、阴性检出率。
在本次研究中,$\%$ 用以表示计数资料,采用${\chi }^{2}$ 进行检验; 计量资料采用$\left({\bar{x}\pm s}\right)$ 表示,进行$t$ 检验; 数据指标具有统计学价值采用$P <{0.05}$ 表示。
对观察组 C- 反应蛋白联合血常规、血培养检测的 69 例疑似菌血症患者的血培养检测中,共检出 8 种菌株。在八个菌种类型中, 检出率较高的两种菌株中除革兰阴性菌和革兰阳性菌外。检出率最高的两种菌株为葡萄球菌 25 株,检出率 36.23%; 大肠埃希菌 16 粒,检出率 23.19%。如 表 1
对两组研究对象血常规及$\mathrm{C}-$ 反应蛋白水平比较得出(见表 2), 观察组疑似菌血症的患者 WBC、NEU、LYM 以及 C- 反应蛋白水平, 明显高于实验组的健康人群, 数据差异存在统计学价值$\left({P <{0.05}}\right)$ ,表明$\mathrm{C}-$ 反应蛋白联合血常规、血培养检测联合诊断方式,能够提高诊断的准确率。如表 2
临床确诊结果中,69 例疑似菌血症中阳性 66 例,阴性 3 例。 其中血培养中阳性 63 例,血常规检测阳性 58 例, C- 反应蛋白检测中阳性 56 例, C- 反应蛋白联合血常规、血培养检测方式中阳性 66 例。四种检测方式中, C- 反应蛋白联合血常规、血培养检测与确诊结果无差别。如表 3
临床危急病症中,常见的病症就是菌血症,这种病症若不及时诊断治疗严重的甚至会诱发脓毒血症以及败血症等病症。 临床诊断菌血症中,医务人员需要重视菌血症与毒血症以及脓血症诊断之间的区别。在毒血症中,细菌以及毒素通常会通过局部感染的方式进入血液循环中,从而对患者身体造成严重影响, 甚至部分患者还会出现休克。患者机体内血细胞遭到破坏从而引起贫血。对于血管栓塞情况, 受大面积组织受损的影响, 毒血症出现,临床诊断检测中毒菌主要为革兰阳性菌。对于脓血症在检测中出现的病菌株为革兰阳性杆菌与金黄色葡萄球菌, 这种病菌的出现会随血液传播到机体其他部分, 从而形成新化脓病灶引发全身感染的出现。
菌血症患者发病后主要表现为,体内多个器官出现急性转移性感染[3]。体表手术造口或导尿管造口这些问题均可能会导致菌血症的出现,此外,短暂性菌血症的出现主要受静脉注射或脓肿切开、心内导管等多种操作的影响。相关资料显示,约有 30% 左右菌血症患者会出现血流动力学紊乱,严重的甚至会造成脓毒性休克,影响患者的生命健康[4]。因此,患者在出现菌血症后需要及时将体内细菌进行清除,否则将会造成较为严重的后果[5]。血培养检测中,需要对被检测者的血液进行多次采血与送检, 整个检测时间较长, 检测结果往往在一周内, 这种检测容易影响菌血症患者的最佳治疗时期。患者在发生急性感染后,对于疑似菌血症,需要立即将其送检,达到确定病原菌种类的目的[6]。在本次研究中,在八个菌种类型中, C-反应蛋白联合血常规、血培养检测模式下检出率较高的两个菌种分别为革兰阴性菌和革兰阳性菌。
血常规是一种较为常见的检查项目, 在进行检查时所涉及的指标多, WBC、NEU 偏高高度怀疑患者存在细菌感染。C- 反应蛋白主要用于判断感染性疾病及验证反应指标。本次研究通过 C-反应蛋白联合血常规、血培养检测开展菌血症诊断[7]。 C- 反应蛋白是一种非特异性的炎性标志物, 属于急性时相反应蛋白, 对于健康人群来说检测时其水平较低, 只有在被检测人员身体出现感染或炎症反应时, C- 反应蛋白水平才会有所提升。 正常状态下, WBC、NEU 以及淋巴细胞比例较为稳定, 若机体发生炎症后,这些数值就会发生变化。对两组研究对象血常规及$\mathrm{C}-$ 反应蛋白水平比较得出,$\mathrm{C}-$ 反应蛋白联合血常规、血培养检测模式下对疑似菌血症患者进行 WBC、NEU、LYM,以及 C- 反应蛋白水平检测,数据准确性更高。
临床对菌血症进行诊断中,采用 C- 反应蛋白联合血常规、 血培养这种联合诊断方式, 能够提高诊断的准确率, 避免假阳性结果的出现, 帮助医务人员更快确定患者的感染源及致病菌, 达到改善患者预后效果的目的。此外,采用$\mathrm{C}-$ 反应蛋白联合血常规、血培养这种联合诊断能够实现优势互补,血培养诊断时间长, 而血常规检查与 CRP 检测能够弥补诊断时间过长的问题, 为医务人员临床治疗方案的制定提供参考, 提高临床用药和治疗的科学性、合理性[8]
革兰阴性菌型、革兰阳性菌型、真菌型是检查菌血症菌型中主要类型[9]。对于健康人群来说,革兰阴性菌型的菌血症患者属于间歇性和机会性,并不会对机体产生影响。而对于化疗后、 免疫力受损严重等患者容易造成不良后果[10]
在临床对菌血症进行诊断中,采用 C-反应蛋白联合血常规、 血培养检测方式能够更快做出结论, 减少单一检测方式中时间长、数据准确性低等多种问题的出现, 为医务工作者制定临床治疗计划提供可靠的数据支持,让患者更快接受治疗。由此可见, C- 反应蛋白联合血常规、血培养检测方式临床应用价值高,值得广泛推广。
