Objective To study the killing effect of glycine combined with iodophor disinfectant on drug-resistant Staphylococcus aureus. Methods Neutralizing agent identification and suspension quantitative killing experiment were used to observe the effect of glycine compound combined with iodophor disinfectant in killing drug-resistant Staphylococcus aureus. Results Iodophor can kill Staphylococcus aureus, with a dilution sterilization efficiency of 63.7%. At the same time, the addition of 4% glycine and a synergistic iodophor disinfectant found a sterilization rate of 79.6%. When killing Staphylococcus aureus in suspension, there was a significant promotion of sterilization effect, which was significantly different from the group with only iodophor$\left({P <{0.01}}\right)$. Conclusion Glycine compounds can significantly enhance the killing effect of iodine on Staphylococcus aureus and increase the sensitivity of bacteria to disinfectants.

目的 研究甘氨酸协同碘伏消毒液对耐药性金黄色葡萄球菌的杀灭效果。方法 采用悬液定量杀灭实验, 观察甘氨酸化合物协同碘伏消毒液杀灭耐药性金黄色葡萄球菌的效果。结果 单用碘伏对金黄色葡萄球菌的杀灭率为 63.7%,加入 4% 甘氨酸协同碘伏作用其金黄色葡萄球菌的杀灭率增加至 79.6%,能明显提升碘伏对金黄色葡萄球菌的杀灭效果,与单用碘伏组比较显著差异($P <{0.01}$)。结论 甘氨酸协同碘伏能够明显地增强碘伏对金黄色葡萄球菌的杀灭效果,提高细菌对消毒剂的敏感性。

Objective To study the killing effect of glycine combined with iodophor disinfectant on drug-resistant Staphylococcus aureus. Methods Neutralizing agent identification and suspension quantitative killing experiment were used to observe the effect of glycine compound combined with iodophor disinfectant in killing drug-resistant Staphylococcus aureus. Results Iodophor can kill Staphylococcus aureus, with a dilution sterilization efficiency of 63.7%. At the same time, the addition of 4% glycine and a synergistic iodophor disinfectant found a sterilization rate of 79.6%. When killing Staphylococcus aureus in suspension, there was a significant promotion of sterilization effect, which was significantly different from the group with only iodophor$\left({P <{0.01}}\right)$. Conclusion Glycine compounds can significantly enhance the killing effect of iodine on Staphylococcus aureus and increase the sensitivity of bacteria to disinfectants.

现代生活中, 消毒剂的应用非常广泛, 尤其在医疗机构中使用消毒剂来控制医院环境中的细菌感染。目前, 市场上用于医疗机构的消毒剂种类较多,消毒效果不一致,如何选择合适的消毒剂,合理使用以及管理对控制医院感染尤为关键[1]。碘伏, 又称聚维酮碘溶液, 有广谱抗微生物作用, 对多种细菌、病毒、 真菌有杀灭作用, 因其有广谱杀菌作用, 常用作外伤伤口的杀菌消毒, 能起到预防感染, 保护创口的作用, 相比于酒精, 碘伏作用更加温和,用途更广,因此应用广泛,但碘伏也存在明显的不足,相较于酒精,碘伏可引起皮肤过敏反应。寻找替代或减少碘伏用量的产品达到同样的消毒效果在消毒学领域中有重要的应用价值。甘氨酸不仅来源广泛, 而且价格低廉易获得, 是一种简单的氨基酸代谢物, 在医药、食品和农药等领域广泛应用[2]。甘氨酸自身也有抑菌作用,在微生物学和医药上也有较大用处, 目前在公开发表的文献中还未见有甘氨酸与碘伏协同作用的报道。本研究结合前期的研究基础, 率先开展甘氨酸与消毒剂协同作用的研究, 观察二者协同作用效果, 以期在未来为医疗机构感染防控实践提供重要实验数据支撑。
金黄色葡萄球菌 M1 为本实验室保存的临床耐药菌、胰蛋白大豆胨琼脂培养基、标准硬水、D/E 中和肉汤、4% 甘氨酸溶液、磷酸盐缓冲剂、0.3%碘伏消毒液、0.3%牛血清蛋白溶液, 均由广东环凯微生物科技有限公司提供。
将金黄色葡萄球菌经过复苏、纯化培养、分离得到单个菌落的培养物, 选取单个典型菌落将其接种于胰蛋白大豆胨琼脂斜面培养基(TSA)中。在${37}^{\circ }\mathrm{C}$恒温箱中培养${24}\mathrm{\;h}$,置${4}^{\circ }\mathrm{C}$恒温箱中保存备用。用磷酸盐缓冲液 (PBS) 将斜面培养基上的培养物冲洗到锥形瓶中, 进一步配制成含菌量为$1 \times {10}^{7}\sim 5 \times {10}^{8}\mathrm{{CFU}}/\mathrm{{mL}}$的菌悬液。
中和剂鉴定实验方法如下[3]:依照 GB/T 38502-2020《消毒剂实验室杀菌效果检验方法》中的中和剂鉴定试验方法, 将菌悬液用等量的 0.3% BSA 稀释成 2.5$\times {10}^{3}\sim {1.5}\times {10}^{4}\mathrm{{CFU}}/\mathrm{{mL}}$, 作为试验菌悬液。鉴定试验分为 4 组,第 1 组:${4.5}\mathrm{\;{mL}}$中和剂$+{0.4}\mathrm{\;{mL}}$标准硬水$+{0.1}\mathrm{\;{mL}}$试验菌悬液; 第 2 组:${4.5}\mathrm{\;{mL}}$中和剂$+{0.4}\mathrm{\;{mL}}$消毒剂$+{0.1}\mathrm{\;{mL}}$试验菌悬液;第 3 组:(${4.5}\mathrm{\;{mL}}$稀释液 +0.4 mL 标准硬水)+0.1 mL 试验菌悬液;以上 3 组均作用${10}\mathrm{\;{min}}$。第 4 组分别吸取稀释液、标准硬水与中和剂各${0.5}\mathrm{\;{mL}}$于无菌平皿内,培养观察。
悬液定量杀菌实验步骤如下[4]。将菌悬液与有机干扰剂牛血清蛋白溶液作$1 : 1$稀释,取$4\mathrm{\;{mL}}$碘伏消毒液于试管中,将$1\mathrm{\;{mL}}$菌悬液和牛血清蛋白溶液混合液置于该试管中,混匀。取菌药反应液 0.5 mL 于含 4.5 mL 中和剂的试管中反应 10 min。 取${0.5}\mathrm{\;{mL}}$反应液用浓度梯度稀释法进行稀释,并接种于胰蛋白大豆胨琼脂培养基上置${37}^{\circ }\mathrm{C}$恒温箱培养${48}\mathrm{\;h}$。用活菌计数法计算其杀灭对数值,各组的活菌浓度 (CFU/mL) 换算为对数值(N) 后计算杀灭对数值。
首先将碘伏用浓度梯度稀释法进行稀释, 得到不同浓度的碘伏。将不同浓度的碘伏和菌悬液按悬液定量杀菌的方法进行实验, 得到金黄色葡萄球菌在不同浓度碘伏的作用下不一样的杀菌效率。用标准硬水将碘伏稀释,无菌试管内加入${4.5}\mathrm{\;{mL}}$稀释后的碘伏与${0.5}\mathrm{\;{mL}}$菌悬液立即混匀并计时。作用$1\mathrm{\;{min}}$,取${0.5}\mathrm{\;{mL}}$混合液加入装有${4.5}\mathrm{\;{mL}}$中和剂的试管内混匀,中和作用${10}\mathrm{\;{min}}$。取$1\mathrm{\;{mL}}$样液接种到胰蛋白大豆胨琼脂上,置${37}^{\circ }\mathrm{C}$恒温箱培养${48}\mathrm{\;h}$,观察结果碘伏浓度确定后,拟设定碘伏与菌悬液的作用时间分别为 1、5、10 min,观察杀灭效果。
取${0.5}\mathrm{\;{mL}}$菌悬液于试管中,加入${4.5}\mathrm{\;{mL}}$稀释后的碘伏以及甘氨酸溶液,充分振荡反应$1\mathrm{\;{min}}$后孵育。取${0.5}\mathrm{\;{mL}}$溶液加入${4.5}\mathrm{\;{mL}}\mathrm{D}/\mathrm{E}$中和肉汤,充分振荡,反应${10}\mathrm{\;{min}}$。取$1\mathrm{\;{mL}}$反应溶液,用磷酸盐缓冲液稀释后倒板。在${37}^{\circ }\mathrm{C}$恒温箱中培养${48}\mathrm{\;h}$后,观察结果。
甘氨酸的浓度:2%、4%、8% 的甘氨酸溶液与碘伏反应, 按照甘氨酸协同杀灭效果实验的步骤进行实验, 观察最佳甘氨酸浓度实验结果。
结果表明, D/E 中和肉汤可以作为本次实验的中和剂。D/E 中和肉汤能够有效地把残留在金黄色葡萄球菌中的碘伏中和。 同时,$\mathrm{D}/\mathrm{E}$ 中和肉汤和$\mathrm{D}/\mathrm{E}$ 中和肉汤的产物均对金黄色葡萄球菌以及胰蛋白大豆胨琼脂培养基无不良影响,见 表 1
用含碘${0.3}\%$ 的碘伏消毒液作用$1\mathrm{\;{min}}$ ,对悬液内的金黄色葡萄球菌的平均杀灭菌对数值$> 5$ 。结果表明,碘伏的杀菌效果明显。
对$1 \times {10}^{8}\mathrm{{CFU}}/\mathrm{{mL}}$ 的金黄色葡萄球菌进行杀菌实验,结果表明随着碘伏消毒液的稀释,碘伏的杀菌效率不断下降,金黄色葡萄球菌的生存率逐渐升高,见表 2。为了更好地观察协同消毒效果,消毒剂浓度在碘伏为${4.8}\times {10}^{-4}\%$ 时杀灭效率为${64}\%$ , 容易观察协同杀灭效果的显著性,且碘伏浓度为${4.8}\times {10}^{-4}\%$ 时, 随着消毒时间的延长, 消毒剂的杀灭效率没有改善 (见表 3), 需有更好的方法提高杀灭效率, 为后续添加物质后可以增加协同效果提供了更充分的依据。用标准硬水稀释了 500 倍的碘伏的杀菌效率为 64%,可以作为后续实验选取的浓度。
表 4中列出来用不同浓度甘氨酸进行协同实验的结果发现, 在 4% 甘氨酸协同效果做好, 故用 4% 甘氨酸浓度进行下一步验证实验
结果表明 (见图 1), 单用 4% 甘氨酸发现甘氨酸对悬液中的金黄色葡萄球菌无明显不良影响;碘伏适量稀释后对金黄色葡萄球菌进行杀菌,杀菌效率为 63.7%,符合实验要求。
但加入 4% 甘氨酸协同 0.3% 碘伏消毒剂对悬液中的金黄色葡萄球菌进行杀灭时能明显的起到协同效果, 杀菌率为 79.6%, 与单加碘伏组比较可以明显提高杀菌效率,有显著性差异$\left({P <{0.01}}\right)$ ,说明甘氨酸和碘伏对金黄色葡萄球菌有协同杀灭作用, 甘氨酸可以有效提高碘伏的杀菌效率。
近年来, 在医疗卫生机构、社区和家庭、畜牧和食品生产企业中消毒产品越来越普及,消毒剂有效杀灭病原微生物和致病菌的同时, 会给环境带来压力以及生物的毒副作用, 各种消毒剂被过量及频繁地不合理使用甚至会使病原微生物对消毒剂产生耐药性[5-7]。碘伏具有广谱杀菌作用,可杀灭细菌繁殖体、 真菌、原虫和部分病毒,对环境中的细菌进行杀菌,也可以用于皮肤、黏膜的消毒及感染,器械浸泡消毒等。因此,碘伏一直是用来控制医疗环境中细菌感染的重要手段[8-9]
金黄色葡萄球菌是一种革兰氏阳性共生菌, 虽然金黄色葡萄球菌主要是一种共生微生物, 但在医疗机构中金黄色葡萄球菌作为一种条件致病菌可能会引起多种疾病, 甚至会引起破坏性强和治疗难度大的感染[10-11]。医院中长期使用消毒剂对金黄色葡萄球菌进行杀菌导致了其耐药性,临床的分离率日渐攀升, 而且其多重耐药性导致住院患者的感染难以控制,病程延长, 死亡率和住院费用随之增加[12]。因此生产低毒、高效、安全且作用范围广的消毒剂产品或者生产可以提高细菌对消毒剂敏感性的制剂在医疗健康领域有广阔的应用前景, 同时在日常生活中也有较大的消费需求[13]
甘氨酸是人体中最简单的氨基酸,它不仅安全低毒,绿色环保, 成本低而且也容易制备, 容易获得。甘氨酸能抑制枯草杆菌、大肠杆菌的生长,尤其是对枯草杆菌有专一性的抑菌作用, 甘氨酸与低级脂肪酸甘油酯(乳甘油辛酸酯、甘油月桂酸酯)合并使用时, 对引起豆腐腐败的微生物 (如枯草芽孢杆菌、假单孢菌、大肠杆菌、乳杆菌、微球菌)有显著的抑菌作用[14]。甘氨酸具有抑菌、抗氧化、护肝和细胞保护等多种药理作用,尤其是对日益增多的耐药菌和真菌感染有着重要意义, 还可作为重要的医药中间体,可用于制备各种有效药剂,也因具有抗氧化作用而被添加到奶油、干酪、人造奶油中, 能延长保质期, 还用于配制运动员的营养饮料及其氨基酸食品, 食用添加甘氨酸的食品对人体胃液酸度的调节、肌肉活力的提高有一定作用[15]。 在农药方面, 甘氨酸可以与其他化学物质生成新型绿色环保的除草剂草甘膦, 可以有效除草。此外, 甘氨酸也有明显的抑菌作用, 石春芝等对甘氨酸的抑菌作用研究表明, 甘氨酸对坚强芽孢杆菌、枯草芽孢杆菌抑菌的作用[16]。甘氨酸是人体中最简单的氨基酸,它不仅安全低毒,绿色环保,成本低而且也容易制备, 容易获得。有研究发现甘氨酸作为提高细菌对抗生素敏感的小分子物质可以达到抑制耐药菌的作用, 甘氨酸能降低抗生素导致的耐药性[17]。目前,虽然公开发表的文献中有关于甘氨酸抑制病原微生物的研究, 但并未见甘氨酸在消毒学领域的研究。
本研究就通过甘氨酸联合常用消毒剂对医院环境内分离的金黄色葡萄球菌的作用效果进行研究, 发现甘氨酸有协同杀菌效果明显,通过代谢物促进细菌对消毒剂的敏感性,建立了一种新的策略来防治细菌, 既可以杀灭医院环境内分离的金黄色葡萄球菌, 又可以减少消毒剂的使用, 降低耐药性。这种策略是通过使用无毒性的化合物来调节消毒剂耐药菌的代谢状态, 增强细菌对消毒剂的摄取, 从而达到杀死细菌的目的。此研究为医疗机构消毒剂的使用提供参考,为未来消毒剂的研发应用提供实验数据和技术支撑。
